RESUMO

Trifluridine/tipiracil (TAS-102) is an oral chemotherapy approved for the treatment of metastatic colorectal cancer. The efficacy and tolerability of TAS-102 were shown in phase II-III clinical trials and in several real-life studies. The elderly and other special subgroups are underrepresented in published literature. We conducted a retrospective multicenter study to assess the effectiveness and safety of TAS-102 in consecutive patients with pretreated mCRC. In particular, we estimated the effectiveness and safety of TAS-102 in elderly patients (aged ≥70, ≥75 and ≥80 years) and in special subgroups, e.g., patients with concomitant heart disease. One hundred and sixty patients were enrolled. In particular, 71 patients (44%) were 70 years of age or older, 50 (31%) were 75 years of age or older, and 23 (14%) were 80 years of age or older. 19 patients (12%) had a concomitant chronic heart disease, three (2%) patients were HIV positive, and one (<1%) patient had a DPYD gene polymorphism. In 115 (72%) cases TAS-102 was administered as a third-line treatment. The median overall survival (OS) in the overall population was 8 months (95% confidence interval [CI], 6-9), while the median progression-free survival (PFS) was 3 months (95% CI, 3-4). No significant age-related reduction in effectiveness was observed in the subpopulations of elderly patients included. The toxicity profile was acceptable in both the whole and subgroups' population. Our study confirms the effectiveness and safety of TAS-102 in patients with pretreated mCRC, suggesting a similar risk-benefit profile in the elderly.

BACKGROUND: Polysomnography is the gold standard for measuring and detecting sleep patterns. In recent years, activity wristbands have become popular because they record continuous data in real time. Hence, comprehensive validation studies are needed to analyze the performance and reliability of these devices in the recording of sleep parameters. OBJECTIVE: This study compared the performance of one of the best-selling activity wristbands, the Xiaomi Mi Band 5, against polysomnography in measuring sleep stages. METHODS: This study was carried out at a hospital in A Coruña, Spain. People who were participating in a polysomnography study at a sleep unit were recruited to wear a Xiaomi Mi Band 5 simultaneously for 1 night. The total sample consisted of 45 adults, 25 (56%) with sleep disorders (SDis) and 20 (44%) without SDis. RESULTS: Overall, the Xiaomi Mi Band 5 displayed 78% accuracy, 89% sensitivity, 35% specificity, and a Cohen κ value of 0.22. It significantly overestimated polysomnography total sleep time (P=.09), light sleep (N1+N2 stages of non-rapid eye movement [REM] sleep; P=.005), and deep sleep (N3 stage of non-REM sleep; P=.01). In addition, it underestimated polysomnography wake after sleep onset and REM sleep. Moreover, the Xiaomi Mi Band 5 performed better in people without sleep problems than in those with sleep problems, specifically in detecting total sleep time and deep sleep. CONCLUSIONS: The Xiaomi Mi Band 5 can be potentially used to monitor sleep and to detect changes in sleep patterns, especially for people without sleep problems. However, additional studies are necessary with this activity wristband in people with different types of SDis. Background: Lower quantity and poorer sleep quality are common in most older adults, especially for those who live in a nursing home. The use of wearable devices, which measure some parameters such as the sleep stages, could help to determine the influence of sleep quality in daily activity among nursing home residents. Therefore, this study aims to analyse the influence of sleep and its changes concerning the health status and daily activity of older people who lived in a nursing home, by monitoring the participants for a year with Xiaomi Mi Band 2. Methods: This is a longitudinal study set in a nursing home in [Details omitted for double-anonymized peer reviewed]. The Xiaomi Mi Band 2 will be used to measure biomedical parameters and different assessment tools will be administered to participants for evaluating their quality of life, sleep quality, cognitive state, and daily functioning. Results: A total of 21 nursing home residents participated in the study, with a mean age of 86.38 ± 9.26. The main outcomes were that sleep may influence daily activity, cognitive state, quality of life, and level of dependence in activities of daily life. Moreover, environmental factors and the passage of time could also impact sleep. Conclusions: Xiaomi Mi Band 2 could be an objective tool to assess the sleep of older adults and know its impact on some factors related to health status and quality of life of older nursing homes residents. OBJECTIVES: To better understand the impact of visitor restrictions on nursing home residents and their families as well as strategies and actions that were conducted in nursing homes during the COVID-19 pandemic. METHODS: A scoping review was carried out in October 2020. Several electronic databases were used: Cochrane Plus, Scopus, Web of Sciences and PubMed. 725 results were identified. We included 10 articles. RESULTS: Thematic analysis obtained the following categories: the impact of COVID-19 on nursing home residents' lives and their families, procedures and frameworks of nursing homes during and after lockdown, and solutions and resources implemented by health care professionals to improve the connection between older people and their families. CONCLUSIONS: Visitor restrictions have a high impact on the health and well-being of older adults' and their families. The main strategies and solutions employed to reduce social isolation and facilitate the communication between older adults' and their families were the use of information and communication technologies, family support groups, and the assignment of reference staff to each family. CLINICAL IMPLICATIONS: The strategies and solutions mentioned should be internationally considered by health care providers in nursing homes to improve connections between family and older adults.

BACKGROUND: Presently the use of technological devices such as wearable devices has emerged. Physical activity monitoring with wearable sensors is an easy and non-intrusive approach to encourage preventive care for older adults. It may be useful to follow a continuous assessment of the risk of falling. The objective is to explore the relationship between the daily activity measured by Xiaomi Mi Band 2 and the risk of falling of older adults residing in or attending care facilities. METHODS: A cross-sectional study was conducted on three different institutions located in Galicia (autonomous community) (Spain). RESULTS: A total of 31 older adults were included in the study, with a mean age of 84 ± 8.71 years old. The main findings obtained were that a greater number of steps and distance could be related to a lower probability of falling, of dependency in basic activities of daily living, or of mobility problems. CONCLUSIONS: The importance of focusing on daily steps, intrinsically related to the objective assessment of daily physical activity, is that it is a modifiable factor that impacts different aspects of health and quality of life.

The development of digital supports for people with autism has increased considerably in recent years. Technology designers and developers have interpreted the needs and learning styles of people with autism in different ways. As a result, there are generic, non-specific or heterogeneous guidelines for the design and development of technology for people with autism. This study aims to identify and describe the recommended elements to support graphical user interface design for children with Autism Spectrum Disorder (ASD), considering the stakeholders' perspective, engaged in a computer application development. A qualitative, longitudinal, multicentre study was carried out. A sample of 39 participants belonging to four groups of stakeholders participated: children with autism, family members, professionals with experience in the intervention with children with autism, and professionals with expertise in the design and development of assistive technology. The techniques used to formalise the collection of information from participants were semi-structured interviews and observation. MAXQDA 2020 software (Verbi Software, Berlin, Germany) was used to analyse the data. The result is a guide with suggestions to support an interface design that emerges from the stakeholder perspectives. This study provides useful information to offer alternatives for children with ASD and facilitate the understanding of daily life.

RESUMO

(1) Background: Work stress is one of the most relevant issues in public health. It has a significant impact on health, especially the development of mental disorders, causing occupational imbalance. There is a growing interest in the development of tools with a positive effect on workers. To this end, wearable technology is becoming increasingly popular, as it measures biometric variables like heartbeat, activity, and sleep. This information may be used to assess the stress a person is suffering, which could allow the development of stress coping strategies, both at a professional and personal level. (2) Methods: This paper describes an observational, analytical, and longitudinal study which will be set at a research center in A Coruña, Spain. Various scales and questionnaires will be filled in by the participants throughout the study. For the statistical analysis, specific methods will be used to evaluate the association between numerical and categorical variables. (3) Discussion: This study will lay the foundation for a bigger, more complete study to assess occupational stress in different work environments. This will allow us to begin to understand how occupational stress influences daily life activity and occupational balance, which could directly enhance the quality of life of workers if the necessary measures are taken.

(1) Background: Sleep disorders are a common problem for public health since they are considered potential triggers and predictors of some mental and physical diseases. Evaluating the sleep quality of a person may be a first step to prevent further health issues that diminish their independence and quality of life. Polysomnography (PSG) is the "gold standard" for sleep studies, but this technique presents some drawbacks. Thus, this study intends to assess the capability of the new Xiaomi Mi Smart Band 5 to be used as a tool for sleep self-assessment. (2) Methods: This study will be an observational and prospective study set at the sleep unit of a hospital in A Coruña, Spain. Forty-three participants who meet the inclusion criteria will be asked to participate. Specific statistical methods will be used to analyze the data collected using the Xiaomi Mi Smart Band 5 and PSG. (3) Discussion: This study offers a promising approach to assess whether the Xiaomi Mi Smart Band 5 correctly records our sleep. Even though these devices are not expected to replace PSG, they may be used as an initial evaluation tool for users to manage their own sleep quality and, if necessary, consult a health professional. Further, the device may help users make simple changes to their habits to improve other health issues as well. Trial registration: NCT04568408 (Registered 23 September 2020).

Introduction: Falls are the second leading cause of accidental or non-intentional deaths worldwide and are the most common problem as people age. The primary purpose of addressing falls is to detect, prevent, treat, and reduce their incidence and consequences. Previous studies identified that multifactorial programs, an interprofessional team, and assistive technology are required to address falls in older adults effectively. Accordingly, the research question is as follows: what are the scope, type of studies, and approaches and strategies to fall risk using technology in the existing occupational therapy literature regarding interventions to address the effects of falls in older adults on daily living? Methods: This scoping review was carried out in January 2020 through Biblioteca Virtual de Salud España, C.I.N.A.H.L., Cochrane Plus, OTSeeker, PubMed, Scopus, and Web of Science. Results: Twelve papers were included. We analyzed the year and journal of publication, authors' affiliation, and design of the study, and thematic categories. There were three themes: participants' characteristics, type of intervention, and fall approach and type of technology used. Discussion and Conclusions: The literature obtained is scarce. It is considered to still be an emerging theme, especially when considering the use of technology for occupational therapy.

The use and deployment of mobile devices across society is phenomenal with an increasing number of individuals using mobile devices to track their everyday health. However, there is a paucity of academic material examining this recent trend. Specifically, little is known about the use and deployment of mobile heart monitoring devices for measuring palpitations and arrhythmia. In this scoping literature review, we identify the contemporary evidence that reports the use of mobile heart monitoring to assess palpitations and arrhythmia across populations. The review was conducted between February and March 2018. Five electronic databases were searched: Association for Computing Machinery (ACM), CINHAL, Google Scholar, PubMed, and Scopus. A total of 981 records were identified and, following the inclusion and exclusion criteria, nine papers formed the final stage of the review. The results identified a total of six primary themes: purpose, environment, population, wearable devices, assessment, and study design. A further 24 secondary themes were identified across the primary themes. These included detection, cost effectiveness, recruitment, type of setting, type of assessment, and commercial or purpose-built mobile device. This scoping review highlights that further work is required to understand the impact of mobile heart monitoring devices on how arrhythmias and palpitations are assessed and measured across all populations and ages of society. A positive trend revealed by this review demonstrates how mobile heart monitoring devices can support primary care providers to deliver high levels of care at a low cost to the service provider. This has several benefits: alleviation of patient anxiety, lowering the risk of morbidity and mortality, while progressively influencing national and international care pathway guidelines. Limitations of this work include the paucity of knowledge and insight from primary care providers and lack of qualitative material. We argue that future studies consider qualitative and mixed methods approaches to complement quantitative methodologies and to ensure all actors' experiences are recorded.

Objetivos: el Envejecimiento Activo se centra en la participación de las personas mayores en los asuntos sociales, económicos, culturales, espirituales y cívicos, y no únicamente en la capacidad de estar activos física y mentalmente. La Terapia Ocupacional, con sus conocimientos y herramientas, puede tener un papel fundamental en la promoción del Envejecimiento Activo. El objetivo es estudiar la relación entre laTerapia Ocupacional y la promoción del Envejecimiento Activo, a partir de las percepciones de terapeutas ocupacionales que trabajan en recursos de atención a personas mayores de la ciudad de A Coruña. Metodología: El estudio se enmarca en la investigación cualitativa, dentro del paradigma fenomenológico, utilizando como técnicas de recogida de datos la entrevista semi estructurada y el cuaderno de campo. La muestra del estudio está conformada por 10 profesionales de terapia ocupacional que ejercen su trabajo en centros de día y residencias de A Coruña. Resultados: A pesar de la importancia de la Terapia Ocupacional en los programas de Envejecimiento Activo, se observa en los participantes un desconocimiento importante sobre el fenómeno, que queda patente en la dificultad para justificar su práctica profesional, así como los beneficiosque aporta la disciplina a las personas mayores. Conclusiones: Se considera muy importante incluir información y educación en los planes de estudio y programas de formación de trabajadores sanitarios, dando a conocer los principios básicos del Envejecimiento Activo. También es muy importante continuar desarrollando trabajos que profundicen en la relación entre el Envejecimiento Activo y laTerapia Ocupacional.


Objectives: the Active Aging focuses on the participation of older people in social, economic, cultural, spiritual and civic affairs, not only in the ability to be physically and mentally active. Occupational Therapy with knowledge and tools can play a key role in promoting Active Aging. The aim was to study the relationship between Occupational Therapy and promoting Active Aging, from the perceptions of occupational therapists working in care resources at people over the city of A Coruna. Methodology: The study is part of qualitative research, within the phenomenological paradigm, using semistructured interview and field notebook as data collection techniques. The sample of the study is formed by 10 professionals of occupational therapy who work in care day centers and residences of A Coruña. Results: Despite the importance of Occupational Therapy in Active Aging programs, we can see in participants a significant lack of knowledge about the phenomenon, which is evident in the difficulty to justify their practice, as well as the benefits of the discipline for elderly. Conclusions: It is considered very important to include information and education in the curricula and training programs for health workers, revealing the basic principles of Active Aging. It is also very important to continue to work unwrapping delve in to the relationship between Active Aging and Occupational Therapy.

Objetivos: en la actualidad, se ha podido observar un envejecimiento progresivo de la población, lo que ha aumentado el interés acerca de la jubilación, ya que la duración de esta será cada vez más prolongada y afectará a los sistemas laborales, sanitarios y educativos, así como a la propia organización de la sociedad. El objetivo de este estudio es explorar y conocer las vivencias ocupacionales de los participantes en su etapa de jubilación. Método: Se ha utilizado la investigación cuantitativa y cualitativa, empleándose como métodos de recogida de datos un cuestionario, una entrevista semiestructurada y un diario de campo. La muestra estuvo conformada por 13 personas jubiladas. Resultados: Existen inconsistencias entre los resultados obtenidos en el cuestionario y lo que los participantes refieren en la entrevista, ya que en el cuestionario otorgan a la jubilación un significado positivo mientras en sus discursos se refleja lo contrario. Además, presentan limitaciones para comprender el concepto oficial de jubilación e identificarse con él. Por último, ha destacado la influencia del clima en el desempeño ocupacional y la controversia existente en cómo se entiende el ocio según el entorno en el que viva la persona (urbano o rural). Conclusiones: Se concluye que la distribución del tiempo es uno de los cambios más significativos de la jubilación, existiendo contradicciones en cuanto al significado que se le otorga a este concepto. Los resultados obtenidos también reflejan la necesidad de la preparación previa a la jubilación, aunque inicialmente muchos participantes no identifiquen esta necesidad como propia.


Aims: nowadays has been observed a progressive aging of the population which has increased the interest about retirement, as the duration of this will be longer and will affect labour, health and education systems, as well as the proper organization of society. The objective of this study is to explore and learn about the occupational experiences of older recently retired in the urban environment of the province of A Coruña. Methods: It has been used for the study the quantitative and qualitative research, being used as methods of data collection a questionnaire, a semistructured interview and a field journal homemade. The sample consisted of 13 retired people. Results: Thereare inconsistencies between the results of the questionnaire and refer participants in the interview, because the retirement questionnaire give a positive meaning while in his speech is the opposite. Furthermore, they have limitations to understand the official concept of retirement and identify with him. Finally, it highlighted the influence of climate on occupational performance and the controversy about how leisure is understood as the environment in which the person lives (urban or rural). Conclusions: We conclude that the distribution of time is one of the most significant changes in retirement, existing contradictions about the meaning given to it. The results also reflect the need for pre - retirement preparation, although initially many participants do not identify this need as their own.

INTRODUCTION: Whooping cough is a re-emerging disease. We describe the investigation of an outbreak of whooping cough and the measures of control adopted. METHODS: The event was reconstructed through a longitudinal study of incidence. In addition to the notified cases, an active search from the list of those who attended summer camps was made through telephone calls. An epidemiological survey was applied to all cases; vaccination history was confirmed with computerised clinical history and the obtaining of samples for analytical confirmation was proposed. The description of the outbreak was made through the epidemic curve, the attack rates, the relative risk and the linear trend by ages and the vaccination coverage. RESULTS: Of the 30 cases that appeared, 22 (73.3%) were among the members of the summer camps. In these, the attack rate was 21.8%, 26.7% among the children and adolescents increasing linearly with the age. The large majority (86.4%) of this last group were correctly vaccinated, including the fifth dose at the age of 4-6 years. Through the meticulous study of the first cases and the telephone calls to those who attended the summer camp, 90% of the cases of the outbreak were detected. CONCLUSIONS: The active search of cases allowed an outbreak of whooping cough with a high attack rate to be studied in children and adolescents with a high vaccination coverage, and the application of control measures that contributed to stop the outbreak.

OBJECTIVE: Combination therapies of fluorouracil (FU) with irinotecan (CPT-11) and docetaxel plus cisplatin have been proven to be active in metastatic gastric cancer. In this paper, we present the results of a phase III trial in which these two combinations given sequentially were compared to mitomycin C (MMC) monochemotherapy in an adjuvant setting. METHODS: 169 patients with radically resected gastric cancer were randomized to receive CPT-11 (180 mg/m2 day 1), leucovorin (100 mg/m2 days 1-2), FU (400-600 mg/m2 days 1-2, q 14; for four cycles; FOLFIRI regimen), followed by docetaxel (85 mg/m2 day 1), cisplatin (75 mg/m2 day 1, q 21; for three cycles; arm A), or MMC (8 mg/m2 days 1-2 as 2-hour infusion, q 42; for four cycles; arm B). All patients had histologically confirmed gastric carcinoma with nodal positivity or pT3/4. A total of 166 patients (85 in arm A and 81 in arm B) were treated. Adjuvant treatment was completed in 76% of the patients in arm A and in 70% of the patients in arm B. The main grade 3/4 side effects recorded were neutropenia in 35%, with only 1 febrile patient, and diarrhea in 11% in arm A, and thrombocytopenia in 10% and neutropenia in 7% in arm B. The FOLFIRI regimen and docetaxel/cisplatin given in sequence was well tolerated and feasible in adjuvant setting. This sequence treatment currently represents the experimental arm of an ongoing multicenter trial.

RESUMO

Pressäo negativa no ouvido médio é um achado frequente. O seu significado clínico, entretanto, näo está ainda perfeitamente conhecido quanto à associaçäo com perda auditiva e prevalência em crianças assintomáticas. Uma mostra de setenta e três escolares assintomáticos foi submetida a timpanometria, screening audiométrico e otoscopia. Encontramos pressäo negativa no ouvido médio (maior que -110mmH2O) em 16,4 por cento das crianças. Deste grupo com pressäo negativa, apenas 25 por cento apresentaram perda auditiva. A pressäo do ar do ouvido médio pode estar negativa e passar despercebida em grande número de crianças em idade escolar. Caso näo leve a uma otite média secretora, esta alteraçäo só raramente poderá ser suficiente para prejudicar a aprendizagem, visto que a maioria das crianças com pressäo negativa apresentou audiçäo normal no presente estudo
