RESUMO

Cancer, microbial infections, and water pollution are significant challenges the modern human population faces. Traditional treatments for cancer and infections often have adverse effects and ecological consequences, while chemical methods for water decontamination can produce harmful byproducts. Metal nanoparticles, particularly zinc oxide (ZnO) and silver (Ag) nanoparticles, show promise in addressing these issues. However, doping Ag on ZnO NPs may synergistically enhance biomedical and therapeutic effects with fewer adverse consequences and improved photocatalytic properties for wastewater treatment. This study aimed to create ZnO and ZnO-Ag nanoparticles through green synthesis and compare their anticancer, antimicrobial, and photocatalytic activity mechanisms. XRD studies determined the crystal diameters of ZnO NPs and ZnO-Ag NPs to be 12.8 nm and 15.7 nm, respectively, with a hexagonal wurtzite structure. The XPS and EDS analyses confirmed the presence of Ag on the ZnO NPs. ZnO NPs and ZnO-Ag NPs exhibited low aggregation in aqueous suspensions, with zeta potentials of -20.5 mV and -22.7 mV, respectively. Evaluating antimicrobial and antibiofilm activity demonstrates that ZnO-Ag NPs have superior potential to ZnO NPs and standard antibiotic drugs against E. coli, S. typhi, B. subtilis, S. aureus, C. albicans, and A. niger. The results of the in vitro cytotoxicity test indicated that on the NCI-H460 lung cancer cell line, ZnO NPs and ZnO-Ag NPs demonstrated IC50 values of 40 µg mL-1 and 30 µg mL-1, respectively. The photocatalytic degradation of methylene blue under direct sunlight revealed that ZnO and ZnO-Ag NPs degraded MB by 98% and 70% in 105 min, respectively. These results show that these nanomaterials may have great potential for treating the aforementioned issues.

Cancer is one of the most challenging, life-threatening illnesses to cure, with over 10 million new cases diagnosed each year globally. Improved diagnostic cum treatment with common side-effects are warranting for successful therapy. Nanomaterials are recognized to improve early diagnosis, imaging, and treatment. Recently, multifunctional nanocomposites attracted considerable interest due to their low-cost production, and ideal thermal and chemical stability, and will be beneficial in future diagnostics and customized treatment capacity. Stimuli-Responsive Hybrid Metal Nanocomposites (SRHMNs) based nanocomposite materials pose the on/off delivery of bioactive compounds such as medications, genes, RNA, and DNA to specific tissue or organs and reduce toxicity. They simultaneously serve as sophisticated imaging and diagnostic tools when certain stimuli (e.g., temperature, pH, redox, ultrasound, or enzymes) activate the nanocomposite, resulting in the imaging-guided transport of the payload at defined sites. This review in detail addresses the recent advancements in the design and mechanism of internal breakdown processes of the functional moiety from stimuli-responsive systems in response to a range of stimuli coupled with metal nanoparticles. Also, it provides a thorough understanding of SRHMNs, enabling non-invasive interventional therapy by resolving several difficulties in cancer theranostics.

RESUMO

Here, we report a rare case of migration of a foreign body has occurred from loin to scrotum. A 35-year-old male patient presented with right sided scrotal mass, on exploration it was found to be a metallic bullet. The patient had a history of gunshot injury in his loin 8 years back and was treated conservatively that time. The probable explanation to this mysterious incident may be that, the bullet might have entered the peritoneal cavity through the loin, but did not cause any extensive damage, so the patient well responded to the initial conservative management. With time, due to changes in abdominal pressure or may be due to gravitational force, it had migrated to the dependent scrotum through the patent processus vaginalis. So, this case signifies that a foreign body can migrate through the body planes and get settled far from the primary place.

BACKGROUND: Most commonly practiced surgical "lay open" technique to treat fistula-in-ano (a common anorectal pathology) has high rate of recurrence and anal incontinence. Alternatively, a nonsurgical cost efficient treatment with Ksharasutra (cotton Seton coated with Ayurvedic medicines) has minimal complications. In our study, we have tried to compare these two techniques. MATERIALS AND METHODS: A prospective randomized control study was designed involving patients referred to the Department of General Surgery in RG Kar Medical College, Kolkata, India, from January 2010 to September 2011. RESULTS: Among 50 patients, 26 were in Ksharasutra and 24 were in fistulotomy group. 86% patients were male and 54% of the patients were in the fourth decade. About 74% fistulas are inter-sphincteric and 26% were of trans-sphincteric variety. Severe postoperative pain was more (7.7% vs. 25%) in fistulotomy group, while wound discharge was more associated with Ksharasutra group (15.3% vs. 8.3%). Wound scarring, bleeding, and infection rate were similar in both groups. Ksharasutra group took more time to heal (mean: 53 vs. 35.7 days, P = 0.002) despite reduced disruption to their routine work (2.7 vs. 15.5 days work off, P <0.001). Interestingly, pain experienced was less in Ksharasutra group, there was no open wound in contrast to fistulotomy and it was significantly cost effective (Rupees 166 vs. 464). CONCLUSION: Treatment of fistula-in-ano with Ksharasutra is a simple with low complications and minimal cost.

BACKGROUND: Silver sulfadiazine (SSD) has been the standard topical antimicrobial for burn wounds for decades. Recently, nanometer-sized silver particles are available which have high surface to volume ratio and remain effective even at a very low concentration and minimizes the chance for tissue toxicity due to silver. Hence, we conducted a randomized controlled trial to compare the effectiveness of topical SSD and nano-crystalline silver (AgNP) hydrogel in burn wounds management. MATERIALS AND METHODS: Study was conducted in the Burn Unit of IPGME&R & SSKM Hospital Calcutta, from January 2011 to August 2012. Patients with 2° burn injury were randomly allocated to SSD and AgNP treatment group. Clinical assessments of burn wound were done on every week till 4(th) week and on completion of treatment. RESULTS: Data for evaluation were obtained for 54 patients on SSD (2° deep-dermal cases 27) and 52 (2° deep-dermal cases 31) on AgNP treatment. Healing status of 2° deep-dermal burns was more satisfactory for AgNP group than SSD treatment at 4 weeks. Among patients receiving AgNP, 80.6% showed at least 50% healing of 2° deep-dermal wounds compared to 48.1% on SSD at 4 weeks (P = 0.001). The figures for complete healing at 4 weeks were, respectively, 4% and 0% (P = 0.116). CONCLUSIONS: AgNP can be an effective and superior alternative to SSD for burn wounds, particularly 2° deep-dermal burns. Healing can be expected, in general, in 6 to 8 weeks time, depending upon the extent of body surface involvement.

INTRODUCTION: High-grade gynaecomastia (Simon IIb and III) has tissue excess (skin excess, enlarged areola, and displaced nipple), which is best managed surgically; however, results of conventional breast reduction surgeries and liposuction is not very good. Aim of our study was to describe a combined technique to manage these problems to produce a good result. MATERIAL AND METHOD: This was a 2-year study among 12 patients of high grade gynaecomastia. Clinical and laboratory findings were normal. Pre-operatively in standing position, diameter of breast and areola, position of nipple, and amount of skin excess were marked. Under general anaesthesia, tumescent infiltration, circumareolar de-epithelisation of skin excess, and liposuction was completed. Redundant portion of the breast was sharply dissected and pulled out. Areola was fixed over pectoralis fascia at mid humerus level, just medial to the mid-clavicular line. Outer borders of the de-epithelised area were apposed by the purse-string effect of a subdermal suture, and further apposed by few half buried horizontal mattress sutures. Drains for 24 hour and compressive dressings for 6 weeks were used. RESULT: Mean age of presentation was 25.8 year; emotional discomfort was the chief complaint. Among 12 patients, 10 patients had bilateral gynaecomastia and 8 patients had enlarged and displaced nipple-areola complex. Average hospital stay was 2.41 days and recoveries were usually uneventful. CONCLUSION: The problem of tissue excess and tissue displacement in high grade gynaecomastia can be well managed by this combined circumareolar skin reduction and liposuction technique to achieve a scar-less flat male chest.
