RESUMO

Eighty female adolescents living in the area where goiter is endemic were examined. The rates of increases in height and weight under 10 years were found to lag behind and, on the contrary, to excel their standard values in adolescence. The accelerated increases in height and weight in girls aged 11 to 15 years were attended by rises in morbidity rates. The number of children with normal sexual development in the area of goiter endemics was reduced to 28.8% and the prevalence of menstrual dysfunction increased up to 54.8%. Impaired menstrual function was most frequently recorded in girls with changed levels of follicle-stimulating and interstitial cell-stimulating hormones.

RESUMO

The authors provide materials of a clinical follow-up of 23 children suffering from rheumatoid arthritis (RA) with an outcome of amyloidosis of the internal organs, supported by morphological examination in 13 patients. 10 patients were treated with colchicine in a dose of 1-2 mg/day lasting from 6 months to 2 years and 7 months. The beneficial effect was attained in 7 children. In 3 patients with RA standing over 8 years, the results were negative. Colchicine merits attention as an effective drug for the treatment of secondary amyloidosis in RA patients.

RESUMO

In order to estimate the liver status and hemodynamics in children with non-rheumatic cardiomyopathies, 130 schoolchildren were examined. Of these, 60 children were diagnosed to have hypertrophic and 70 dilated cardiomyopathy. In both cardiomyopathy varieties, alterations in the liver were established to be of secondary character and associated with greater circulation abnormalities. The patients manifested an appreciable increase of the diameter of the hepatic veins and reduction in them of the blood flow rate. Adequate cardiotropic therapy brought about a noticeable decrease in the echographic signs of liver congestion.

To study the morphofunctional state of the heart, 49 children aged 1 to 15 years with dilated cardiomyopathy (DCMP) were examined. The diagnosis was established on the basis of comprehensive clinical and instrumental examinations. It was discovered as a result of echocardiography that children with dilated cardiomyopathy show considerable dilatation of the cardiac chambers, accompanied by moderate hypertrophy of the myocardium (mainly of the left ventricle) and by appreciable impairment of the function of myocardial contraction and relaxation together with derangement of heart pump function. The overwhelming majority of patients with DCMP manifested relative failure of the mitral valve which plays an important role in the impairment of intracardiac and central hemodynamics. It should be noted that echographic alterations seen in DCMP are not specific. However, they provide an idea of the gravity of heart damage, that is of importance in outlining the policy of treatment and prophylactic measures.

RESUMO

Echography of the pancreas in children suffering from chronic pathology of the gastroduodenal and biliary systems may reveal a considerable incidence and the gravity of pancreatic injuries. The relationship may be found between alterations in the pancreas and the gravity of the pathology in question as well as duration, process spreading, gastric secretion reduction, and the disease entity. The results derived well agree with the reported data on pathomorphology and laboratory diagnosis of pancreatic injuries in the above-indicated pathology and provide evidence in favour of the high information content of echography. A considerable incidence and the intensity of the echographic signs of pancreatic lesions in children suffering from chronic pathology of the gastroduodenal and biliary systems support the appropriateness of echographic studies of the pancreas in the given patients' group.

Phentolamine has been shown to improve peripheral hemodynamics and have a favorable central hemodynamic effect, reflected in clinical improvement, in children with bradycardia (11 patients with second- or third-degree atrioventricular block) and signs of circulatory insufficiency.
