RESUMO

Radioactivity is a process in which the nuclei of unstable atoms spontaneously decay, producing other nuclei and releasing energy in the form of ionizing radiation in the form of alpha (α) and beta (ß) particles as well as the emission of gamma (γ) electromagnetic waves. People may be exposed to radiation in various forms, as casualties of nuclear accidents, workers in power plants, or while working and using different radiation sources in medicine and health care. Acute radiation syndrome (ARS) occurs in subjects exposed to a very high dose of radiation in a very short period of time. Each form of radiation has a unique pathophysiological effect. Unfortunately, higher organisms-human beings-in the course of evolution have not acquired receptors for the direct "capture" of radiation energy, which is transferred at the level of DNA, cells, tissues, and organs. Radiation in biological systems depends on the amount of absorbed energy and its spatial distribution, particularly depending on the linear energy transfer (LET). Photon radiation with low LET leads to homogeneous energy deposition in the entire tissue volume. On the other hand, radiation with a high LET produces a fast Bragg peak, which generates a low input dose, whereby the penetration depth into the tissue increases with the radiation energy. The consequences are mutations, apoptosis, the development of cancer, and cell death. The most sensitive cells are those that divide intensively-bone marrow cells, digestive tract cells, reproductive cells, and skin cells. The health care system and the public should raise awareness of the consequences of ionizing radiation. Therefore, our aim is to identify the consequences of ARS taking into account radiation damage to the respiratory system, nervous system, hematopoietic system, gastrointestinal tract, and skin.

BACKGROUND: Atelectasis is a condition characterized by the collapse and nonaeration of lung regions and is considered a manifestation of an underlying disease process. The goal of atelectasis treatment is the restoration of volume loss. In the range of different treatment options, chest physiotherapy is often used as a first-line approach, and some cases require bronchoscopic interventions. METHODS: In this case series, we describe a modified bronchoscopic treatment procedure using pressure-controlled bronchoscopic segmental insufflation with surfactant application. RESULTS: The proposed approach resulted in significant improvement of lung volume across a range of patients including massive lobar, atypical rounded atelectasis in previously healthy patients, and in a particularly challenging case involving an infant suffering from spinal muscular atrophy type I. CONCLUSION: The modified segmental insufflation-surfactant instillation technique offers a safe and promising easily implementable treatment of persistent atelectasis caused by different underlying disease processes with positive long-term outcomes.

Diabetes mellitus, a chronic disease of metabolism, is characterized by a disordered production or cellular utilization of insulin. Diabetic foot disease, which comprises the spectrum of infection, ulceration, and gangrene, is one of the most severe complications of diabetes and is the most common cause of hospitalization in diabetic patients. The aim of this study is to provide an evidence-based overview of diabetic foot complications. Due to neuropathy, diabetic foot infections can occur in the form of ulcers and minor skin lesions. In patients with diabetic foot ulcers, ischemia and infection are the main causes of non-healing ulcers and amputations. Hyperglycemia compromises the immune system of individuals with diabetes, leading to persistent inflammation and delayed wound healing. In addition, the treatment of diabetic foot infections is challenging due to difficulty in accurate identification of pathogenic microorganisms and the widespread issue of antimicrobial resistance. As a further complicating factor, the warning signs and symptoms of diabetic foot problems can easily be overlooked. Issues associated with diabetic foot complications include peripheral arterial disease and osteomyelitis; accordingly, the risk of these complications in people with diabetes should be assessed annually. Although antimicrobial agents represent the mainstay of treatment for diabetic foot infections, if peripheral arterial disease is present, revascularization should be considered to prevent limb amputation. A multidisciplinary approach to the prevention, diagnosis, and treatment of diabetic patients, including those with foot ulcers, is of the utmost importance to reduce the cost of treatment and avoid major adverse consequences such as amputation.

Leptin is a polypeptide which is mostly produced in white fat tissue and is an important proinflammatory, proangiogenic, proinvasive and mitotic factor. There is ever more evidence suggesting the key role of leptin in the occurrence of breast cancer. The aim of the study was to investigate serum leptin levels in patients with benign breast tumors, as well as in various breast cancer phenotypes, taking into account leptin levels connected to menopausal status and body mass index (BMI). The study included 97 patients having their breast tumor surgically removed. Serum leptin level was determined by ELISA method in all study patients. Study results showed that significantly more women, regardless of having malignant or benign tumors, were postmenopausal and had a significantly higher level of leptin compared to the premenopausal group. The highest level of leptin was recorded in the group of postmenopausal obese women compared to other postmenopausal women but also compared to premenopausal women. According to BMI alone, obese women had a significantly higher level of leptin regardless of the type of tumor. The most significant differences in leptin levels observed through BMI were found in the Luminal B1 group. In conclusion, serum leptin level was shown to be a good diagnostic parameter suggesting a higher possibility of breast cancer development.

Engineered beige adipose tissues could be used for screening therapeutic strategies or as a direct treatment for obesity and metabolic disease. Microvascular fragments are vessel structures that can be directly isolated from adipose tissue and may contain cells capable of differentiation into thermogenic, or beige, adipocytes. In this study, culture conditions were investigated to engineer three-dimensional, vascularized functional beige adipose tissue using microvascular fragments isolated from both healthy animals and a model of type II diabetes (T2D). Vascularized beige adipose tissues were engineered and exhibited increased expression of beige adipose markers, enhanced function, and improved cellular respiration. While microvascular fragments isolated from both lean and diabetic models were able to generate functional tissues, differences were observed in regard to vessel assembly and tissue function. This study introduces an approach that could be employed to engineer vascularized beige adipose tissues from a single, potentially autologous source of cells.

Objetivo: Determinar la Dosis Diaria Definida (DDD) de antibióticos utilizados en los Servicios de Medicina Interna de cuatro hospitales públicos en Panamá. Métodos: Se escogió una muestra a conveniencia de 4 hospitales públicos de segundo nivel de atención de la seguridad social y Ministerio de Salud, atendiendo características en cuanto a ubicación en provincias, población atendida, servicios médicos ofrecidos y con Dosis Unitaria en funcionamiento. Se obtuvieron datos de los pacientes hospitalizados entre los meses de enero a marzo del 2017 y la información era registrada en una base de datos anotándose generales del paciente, diagnósticos, antibióticos, dosis, frecuencia, tiempo de tratamiento. Igualmente se anotaron los consumos y costos unitarios de todos los antibióticos usados en los hospitales durante el año 2016, para calcular la DDD y Dosis Diaria Prescrita. Resultados: La oxacilina fue el antibiótico con mayor DDD días cama con 60.5, luego le siguen ceftriaxona (45.2), cefalotina (35.5), clindamicina (35.5). Por grupo farmacológico, las cefalosporinas tienen consumos más altos con 87.6 DDD días cama, la penicilina MRSA 60.5, la lincosamidas con 35.5 DDD días cama, seguido de las quinolonas con 29.3. Conclusiones: Los resultados pueden ser utilizados para implementar programas para mejorar el uso racional y gasto de antibióticos a nivel hospitalario y que la DDD se use para medir el consumo y a la vez propiciar el control de antibióticos a través de comunicaciones escritas con los médicos tratantes. Se deben realizar estudios de DDD días cama relacionando con los aspectos microbiológicos para relacionar mejor el uso de los antibióticos. (provisto por Infomedic International)


Objective. Determine the Defined Daily Dose of antibiotics used in the Internal Medicine Services of 4 public hospitals in Panama. Methods. A convenience sample of 4 public hospitals of the second level of social security care and the Ministry of Health was chosen, considering characteristics in terms of location in the provinces, population served, medical services offered and with Unit Dose in operation. Data were obtained from hospitalized patients between the months of January to March 2017 and the information was recorded in a database, noting general patient reports, diagnoses, antibiotics, dose, frequency, treatment time. Likewise, the consumptions and unit costs of all antibiotics used in hospitals during 2016 were recorded to calculate the DDD and Prescribed Daily Dose. Results. Oxacillin was the antibiotic with the highest DDD bed days with 60.5, followed by ceftriaxone (45.2), cephalothin (35.5), and clindamycin (35.5). By pharmacological group, cephalosporins have higher intakes with 87.6 DDD bed days, penicillin MRSA 60.5, and lincosamides with 35.5 DDD bed days, followed by quinolones with 29.3. Conclusions. The results can be used to implement programs to improve the rational use and expense of antibiotics at the hospital level and that the DDD is used to measure the consumption and at the same time promote the control of antibiotics through written communications with the treating physicians. Bed-day DDD studies should be carried out relating to microbiological aspects to better relate the use of antibiotics. (provided by Infomedic International)

An accurate in vitro model of human adipose tissue could assist in the study of adipocyte function and allow for better tools for screening new therapeutic compounds. Cell culture models on two-dimensional surfaces fall short of mimicking the three-dimensional in vivo adipose environment, while three-dimensional culture models are often unable to support long-term cell culture due, in part, to insufficient mass transport. Microfluidic systems have been explored for adipose tissue models. However, current systems have primarily focused on 2D cultured adipocytes. In this work, a 3D human adipose microtissue was engineered within a microfluidic system. Human adipose-derived stem cells (ADSCs) were used as the cell source for generating differentiated adipocytes. The ADSCs differentiated within the microfluidic system formed a dense lipid-loaded mass with the expression of adipose tissue genetic markers. Engineered adipose tissue showed a decreased adiponectin secretion and increased free fatty acid secretion with increasing shear stress. Adipogenesis markers were downregulated with increasing shear stress. Overall, this microfluidic system enables the on-chip differentiation and development of a functional 3D human adipose microtissue supported by the interstitial flow. This system could potentially serve as a platform for in vitro drug testing for adipose tissue-related diseases.

This study aimed at investigating the synthesis, characterization, and search for a biotechnological application proposal for poly [(R)-3-hydroxybutyric acid] (PHB) grafted with the n-hydroxyethyl acrylamide (HEAA) monomer. The novel copolymer was prepared by 60Co gamma radiation-induced-graft polymerization. The effect of different solvents in the graft polymerization; the degree of grafting, crystallinity, and hydrophilicity; the morphology and the thermal properties were evaluated. The polyurethane fabricated from the grafted PHB was suggested as a scaffold. The enzymatic degradation behavior and the spectroscopic, morphological, mechanical, and biological properties of the composites were assessed. According to the results, the successful grafting of HEAA onto PHB was verified. The grafting was significantly affected by the type of solvent employed. A decreased crystallinity and increased hydrophilicity of the graft copolymer, concerning the PHB, was found. An increased roughness was observed in the morphology of the polymer after grafting. The thermodynamic parameters, except for the glass transition temperature, also decreased for the synthetic biopolymer. The intended use of these scaffolds for skin tissue engineering was supported by a proper degradability and degree of porosity, improved mechanical properties, the optimal culture of human fibroblasts, and its transfection with a plasmid vector containing an enhanced green fluorescent protein.

The aim of this study was to examine the quality of life and to report on the utility and QALY measures in patients before and after coronary artery bypass grafting (CABG); to investigate whether the SF-12 is comparable with the SF-36 for measuring health-related quality of life of patients with CABG; and to determine the impact of individual predictors on poor quality of life assessment after rehabilitation. This prospective study was conducted between January 2017 and December 2018 at the University Hospital Center Osijek, at three time points: pre-operation, 1 month after surgery, and after rehabilitation. The study was conducted with the SF-36 and SF-12 health questionnaires on 47 participants. After rehabilitation, there was a significant improvement in all domains of quality of life. The highest score was given to the change in pain (BP); mean scores were 63.8 (95% CI 56.9 to 70.6) (p = 0.001). The lowest grade (the lowest quality) after rehabilitation was in the domain of limitations due to physical difficulties (RP); arithmetic mean was 48.5 (95% CI 41 to 55.9) (p < 0.001). Quality-adjusted life-year was 0.41 (95% CI 0.38-0.44) after the CABG. The results of this study show that patients with coronary heart disease have poor quality of life before surgery. One month after the surgery, the quality of life improved, but was still inadequate. One year after surgery, satisfactory results were obtained in almost all subscales. The SF-36, SF-12, and its components, can be used effectively in patients with CABG. Age, gender, lifestyle, and risk factors in our sample of participants are not predictors of poor quality of life assessment after rehabilitation.

Breast cancer is the most common malignancy in the population of women under 40 years of age. Young age is an independent factor for poor prognosis. In this research, we tried to establish other factors for poor prognosis in stage I-III breast cancer. The following parameters were observed: tumor size, lymph node status, histologic grade, hormonal receptor status, Ki-67 prognostic index, Her2 neu status, histologic type of the tumor, local recurrence and metastases. Logistic regression was used to evaluate the effect of specific factors on the probability of lethal outcome and development of distant metastases. Our patients showed a predominance of T1 tumor (49.4%), had positive lymph nodes (62%) and most of them were pN1 (61.2%). Up to one-third of patients had triple negative status. Ki-67 proliferation index was high (25%). Multicentric tumor was detected in 23% of patients. There was no difference in overall survival between the two types of surgical procedures. Patients with pN0 status had better overall survival. Breast cancer in the population of young women has a more aggressive nature. Study results indicated positive lymph node status as an independent factor for poor prognosis of stage I-III breast cancer.

- A young woman with breast cancer is considered to be a woman younger than 40. According to the literature, breast cancer in the population of young women usually is of a higher histologic grade, unfavorable hormonal status, and overall higher mortality rate when compared with breast cancer occurring in older population. We compared pathologic and immunohistochemical features of breast carcinoma in women under 40 years of age with the respective features in women over 60 years of age. The following parameters were observed in these two groups: tumor size, lymph node status, histologic grade, hormonal receptor status, Ki-67 prognostic index, Her2/neu status, and histologic type of the tumor. Early onset breast carcinoma was found to have a higher frequency of tumor grade 3 (29% vs. 17%) and estrogen receptor negativity (45% vs. 23%). In the group of young women, breast carcinoma was mostly multicentric (23% vs. 5%), triple-negative (32% vs. 10%), and was found to have higher proliferation index Ki-67 (25% vs. 10%). Our results confirmed differences between the young and older groups of patients. In the group of young women, we found predominantly unfavorable prognostic parameters of the disease.

OBJETIVO: Caracterizar pacientes con indicación de cesárea atendidas en el servicio de ginecología y obstetricia de la clínica médica previsional Asunción ­ MINSA, Juigalpa Chontales, Nicaragua ,2016. DISEÑO: Estudio descriptivo, cuantitativo de corte transversal. Está constituido por 161 mujeres que se les practicó cirugía vía cesárea, se recolectó la información de expedientes clínicos médico legales, del departamento de archivo, que cumplieròn los criterios de inclusión correspondientes. RESULTADOS: El 86.96%se trataba de mujeres en el rango de edad entre 20-35 años, en un 72.05% de educación universitaria, el 54.04% laboraban de acuerdo a su formación profesional, 58.39% eran casadas y el 75.78% procedían del área urbana. De acuerdo a los factores gineco - obstétricos, las pacientes en el estudio no tenían antecedentes personales no patológicos hasta en un 96.89%, con embarazos de término hasta en 83.23%, 42.24% tenían cesáreas anteriores y hasta 72.05% no tenían antecedentes patológicos personales, el 100% recibió atención prenatal, el 86.34% recibió más de 4 atenciones prenatales. 43.48% no presentarón factores de riesgo durante su embarazo y 91.93% no tuvierón factores de riesgo durante el parto. La principal indicación de cesárea hasta en 36.65% precisamente fue cesárea anterior. CONCLUSIONES: Con respecto a las características sociodemográficas, todos estos factores no favorecierón a indicación absoluta de cesárea, no presentarón factores ginecológicos y obstétricos de relevancia que hayan favorecido su indicación absoluta de cesárea, la cual fue la principal causa de indicación para realizar interrupción del embarazo vía alta actual

BACKGROUND: Suicide and mood disorders (especially major depressive disorder (MDD) and bipolar affective disorder (BD)) represent a significant global health burden. Major depressive disorder and bipolar affective disorder have been associated with increased risk for suicide. Some specific suicide risk factors might be found in underlying individual personality traits. Specific personality features may predispose an individual to mood disorders (MDD or BD) hence increased suicide risk. The specificity of this research is in the assessment of personality features during the acute phase of illness immediately after suicide attempt which resulted in psychiatric inpatient treatment. SUBJECTS AND METHODS: The study included 119 unrelated Caucasian participants with MDD-severe depressive episode without psychotic symptoms (MDD) and BD-severe depressive episode without psychotic symptoms (BD-sDE). Both groups of patients with MDD and BD-sDE were divided into the suicide attempters and non-suicidal group. The diagnoses of the severe depressive episode without psychotic symptoms in major depressive disorder (MDD; F32.2) and bipolar disorder (BD-sDE; F31.4) were made according to ICD-10 (WHO 1992) diagnostic criteria. Methods of suicide attempts were also assessed according to ICD-10 and a self-report questionnaire, the Temperament and Character Inventory (TCI) was applied. RESULTS: The participants who exhibited suicide attempt had significantly higher scores on harm-avoidance (HA) (p<0.001), significantly lower score on persistence (PS) (p=0.037) and lower score, however not statistically significant, on novelty-seeking (NS) (p=0.319) regarding temperament dimensions. In character dimensions, the patients with suicidal attempt had significantly lower scores on self-directedness (SD) (p<0.001) and significantly lower scores on cooperativeness (CO) (p=0.001). CONCLUSION: Patients who had suicide attempt may have some significantly different personality traits than non-suicidal patients with mood disorders. The combination of high harm-avoidance (HA) and low self-directedness (SD) may be specific for depressive episode while the combination of high HA, novelty-seeking (NS), and self-transcendence (ST) with low SD may be related to suicide attempts during the depressive episode in bipolar disorder. The novelty-seeking (NS), self-transcendence (ST) and self-directedness (SD) may be specific for suicidal group of bipolar patients.

The incidence of Multilocular cystic renal cell carcinoma (MCRCC) in literature is very low and confounding MCRCC with cystic nephroma (CN) is even more unusual. The aim of this report is to present a case of MCRCC and emphasize the importance of the preoperative radiologic evaluation and immunohistochemical staining confirmation to obtain an accurate diagnosis. A 73-year-old woman presented with a history of 4-month right flank pain. CT showed a Bosniak type III renal mass. After laparoscopic partial nephrectomy the initial report was cystic nephroma. Immunohistochemical staining was performed being positive for Epithelial Membrane Antigen thus changing the diagnosis to MCRCC. Multilocular cystic renal cell carcinoma cannot reliably be distinguished from cystic nephroma neither by physical examination nor by radiologic evaluation; immunohistochemical staining assay is useful to differentiate between these conditions allowing an accurate diagnosis and proper follow-up.

The topological analysis of the electron density for electronic excited states under the formalism of the quantum theory of atoms in molecules using time-dependent density functional theory (TDDFT) is presented. Relaxed electron densities for electronic excited states are computed by solving a Z-vector equation which is obtained by means of the Sternheimer interchange method. This is in contrast to previous work in which the electron density for excited states is obtained using DFT instead of TDDFT, that is, through the imposition of molecular occupancies in accordance with the electron configuration of the excited state under consideration. Once the electron density of the excited state is computed, its topological characterization and the properties of the atoms in molecules are obtained in the same manner that for the ground state. The analysis of the low-lying π→π* singlet and triplet vertical excitations of CO and C6H6 are used as representative examples of the application of this methodology. Altogether, it is shown how this procedure provides insights on the changes of the electron density following photoexcitation and it is our hope that it will be useful in the study of different photophysical and photochemical processes.
