BACKGROUND: To evaluate the reliability of a new instrument to clinically measure the anteroposterior relationship of the maxillary central incisors to the forehead. METHODS: An analytical cross-sectional study using a sample of 84 (67% female and 33% male) young adult participants was conducted. Measurements were performed according to Element II of the Andrews' Six Elements of Orofacial Harmony by assessing the horizontal distance between the facial axis point of the maxillary central incisor and the forehead's anterior limit line. The instrument tested was a transparent plastic template having a leveling meter, reference lines, and an embedded millimeter ruler. Two orthodontists evaluated the sample twice with a minimum of a two-week interval between evaluations. The reliability was evaluated using the intraclass correlation coefficient. Repeated-measures ANOVA was also performed. RESULTS: Good to excellent intra- and interexaminer agreement, with intraclass correlation coefficient values of 0.874 (95% confidence interval 0.800-0.921; P < 0.001) and 0.876 (95% confidence interval 0.739-0.948; P < 0.001), respectively, were calculated. There were no statistically significant differences between all the repeated measurements assessed (P = 0.820). CONCLUSIONS: The instrument showed good to excellent reliability for determining the anteroposterior relationship of the maxillary central incisors to the forehead and could be used to help orthodontists and maxillofacial surgeons indirectly evaluate the anteroposterior position of the maxilla.

La maloclusión Clase III es un problema esquelético anteroposterior en la que se puede observar relaciones alteradas de los componentes óseos, dentarios y musculares; siendo el componente esquelético alterado con mayor frecuencia la deficiencia maxilar en sentido sagital y transversal. La intervención temprana de esta anomalía es reportada con un mayor porcentaje de éxito. Con la utilización de la máscara de protracción asociada a una disyunción maxilar se producen cambios beneficiosos en la arquitectura facial. La edad del paciente, la colaboración y el manejo adecuado del sistema son los factores que conducen a resultados exitosos y estables. El objetivo de este reporte es describir el tratamiento de un paciente en etapa de la dentición mixta con el uso de este dispositivo. Consideraciones finales: El uso de la máscara facial de Petit, asociado con expansión maxilar, fue útil en la corrección de la deficiencia maxilar y la maloclusión Clase III.


Class III malocclusion is an anteroposterior skeletal problem in which altered relationships of the bones, teeth and muscular components can be observed; the skeletal component being most frequently altered in the sagittal and transverse maxillary deficiency. Early treatment of this anomaly is reported with a higher success rate. The protraction mask associated with a maxillary disjunction produces beneficial changes in the facial architecture. The patient's age, collaboration and proper management of the system are the factors that lead to successful and stable results. The purpose of this report is to describe the treatment of a patient in the stage of the mixed dentition with the use of this device. Final considerations: The use of the Petit facial mask, associated with maxillary expansion, was useful in the correction of maxillary deficiency and class III malocclusion

El periodonto es la estructura a través de la cual el ortodoncista mueve los dientes, por lo tanto el éxito de un tratamiento ortodóncico depende en parte de la integridad de la salud de los tejidos periodontales. Como ciertos movimientos ortodóncicos pueden afectar el periodonto y los tejidos gingivales, es básica una valoración periodontal previa al tratamiento. Problemas periodontales no diagnosticados y/o no tratados a tiempo, como defectos mucogingivales y recesiones gingivales, pueden empeorar con la terapia ortodóncica. El presente caso describe a una paciente de sexo femenino de 13 años de edad con recesión gingival a nivel de piezas 31 y 41 y apiñamiento dentario. El tratamiento periodontal consistió en un injerto de encía libre para aumentar la encía queratinizada y aumento de fondo de surco vestibular antes de iniciar el tratamiento de ortodoncia. El objetivo de este reporte de caso es presentar el manejo adecuado en la prevención y control de las recesiones gingivales para asegurar un mejor pronóstico a través de un trabajo interdisciplinario entre periodoncia y ortodoncia.


The periodontium is the structure through which the orthodontist moves your teeth, so the success of orthodontic treatment depends in part on the integrity of the health of periodontal tissues. Because some orthodontic tooth movement may affect the periodontal and gingival tissues, periodontal assessment is essential prior to orthodontic treatment. Undiagnosed periodontal problems and / or untreated in time, as Mucogingival defects and gingival recession may worsen with orthodontic treatment. This case describes a female patient 13 years of age with gingival recession at parts 31 and 41 and dental crowding. Periodontal treatment consisted of a free gingival graft to increase the gingiva and increased background surcus vestibular before starting orthodontic treatment. The aim of this case report is to present the best conduct to be taken in the prevention and control of gingival recessions to ensure a better outcome through a management interdisciplinary between periodontics and orthodontics.
