This retrospective study aimed to assess the value of 24-hour ambulatory blood pressure monitoring (ABPM) in distinguishing primary from secondary hypertension in pediatric patients. Our study was conducted on 293 patients referred to a pediatric nephrology clinic over 11 years. Various ABPM parameters were analyzed, including daytime and nighttime systolic and diastolic blood pressures, heart rate, and blood pressure load. Among the participants, 74% were normotensive (white-coat hypertension), 21.5% had primary hypertension, and 4.4% had secondary hypertension. There were no significant differences in the analyzed variables between primary and secondary hypertension groups. Our findings suggest that ABPM might not reliably differentiate between the two in this cohort. As white-coat hypertension becomes more prevalent, ABPM remains a valuable tool in preventing unnecessary workups in children without sustained hypertension. However, our study did not identify specific endpoints for distinguishing primary from secondary hypertension.

Abstract The aim of the present study is to assess the effects of selenium nanoparticles on the growth, hematology and nutrients digestibility of Labeorohita fingerlings. Fingerlings were fed with seven isocaloric sunflower meal-based diet supplemented with different concentrations of nanoparticles naming T1 to T7 (0, 0.5, 1, 1.5, 2, 2.5, and 3 mg/kg), with 5% wet body weight while chromic oxide was used as an indigestible marker. After experimentation for 90 days T3 treated group (1mg/kg -1Se-nano level) showed the best result in hematological parameters (WBCs 7.97 ×103mm-3, RBCs 2.98 ×106 mm-3 and Platelet count 67), nutrient digestibility (crude protein: 74%, ether extract: 76%, gross energy: 70%) and growth performance (weight gain 13.24 g, weight gain% 198, feed conversion ratio 1.5, survival rate 100%) as compared to the other treatment groups. Specific growth rates were found significantly higher in T5 than in other groups. The present study indicated positive effect of 1 mg/kg Se-nanoparticles on growth advancement, hematological parameters, and nutrients digestibility of L. rohita fingerlings.


Resumo O objetivo do presente estudo é avaliar os efeitos das nanopartículas de selênio no crescimento, hematologia e digestibilidade dos nutrientes de alevinos de Labeo rohita. Os alevinos foram alimentados com sete dietas isocalóricas à base de farinha de girassol suplementada com diferentes concentrações de nanopartículas, nomeando T1 a T7 (0, 0,5, 1, 1,5, 2, 2,5 e 3 mg / kg), com 5% do peso corporal úmido enquanto o óxido crômico foi usado como um marcador indigesto. Após a experimentação por 90 dias, o grupo tratado com T3 (nível 1mg / kg -1Se-nano) mostrou o melhor resultado em parâmetros hematológicos (WBCs 7,97 × 103mm-3, RBCs 2,98 × 106mm-3 e contagem de plaquetas 67), digestibilidade dos nutrientes (proteína bruta: 74%, extrato de éter: 76%, energia bruta: 70%) e desempenho de crescimento (ganho de peso 13,24 g, ganho de peso % 198, taxa de conversão alimentar 1,5, taxa de sobrevivência 100%) em comparação com os outros grupos de tratamento. As taxas de crescimento específicas foram encontradas significativamente mais altas em T5 do que em outros grupos. O presente estudo indicou efeito positivo de 1 mg / kg de nanopartículas de Se no avanço do crescimento, parâmetros hematológicos e digestibilidade de nutrientes de alevinos de L. rohita.

The physiological systems and biological applications that have arisen during the past 15 years depend heavily on the microscale and nanoscale fluxes. Microchannels have been utilized to develop new diagnostic assays, examine cell adhesion and molecular transport, and replicate the fluid flow microenvironment of the circulatory system. The various uses of MHD boundary flow in engineering and technology are extensive, ranging from MHD power generators and the polymer industry to MHD flow meters and pumps and the spinning of filaments. In this investigation, the (Magnetohydrodynamic) MHD flow of Prandtl nanofluid is investigated along with mixed convection, energy activation, microorganism, and chemical reaction. The flow model is considered through partial differential equations in dimensionless form which is then integrated numerically via considering the Bvp4c technique. The outcome is numerous emerging physical parameters over velocity profile, temperature, mass concentration, and microorganism with the separate pertinent quantities such as the Prandtl fluid parameter, elastic fluid parameter, magnetic field, mixed convection parameter, activation energy, chemical reaction, Brownian motion, thermophoretic force, Prandtl number, and Schmidt number. The friction factor, rate of heat transfer and Sherwood number, and density of microbes are revealed numerically and graphically. The outcomes indicate that the Prandtl fluid parameter and elastic fluid parameter tend to enhance the velocity profile. It is also noted that the Prandtl fluid parameter depreciates the thermal rate with the addition of the concentration profile while the opposite trend is recorded for activation energy. Obtained numerical outcomes are correspondingly compared with the current statistics in limiting cases and a close match is obtained.

The wettability of nano-SiO2 surface was transformed from the inherent hydrophilicity to functional superhyderophobicity by coating Trimethoxy (vinyl)silane modifier, and the resultant surface showed contrasting wettability for water and oil (Superhydrophobic and Superoleophilic), which is a desired characteristic for the membranes used in oil-water separation. Initially Trimethoxy (vinyl)silane coated SiO2 nanoparticles (TMVS@SiO2) were synthesized by hydrolysis and poly-condensation reactions, and this nano dispersion was spray coated on the annealed stainless-steel mesh surface, whose resulting hierarchical surface texture brought about the desired wettability, with the water-surface-air (θWA) and oil-surface-air (θOA) interfacial contact angles of 150° and 0° respectively. In addition to the wettability studies (contact angles), FTIR, morphological, and elemental characterizations of the TMVS@SiO2 coated surfaces were carried out to understand the alterations that have taken place on the TMVS@SiO2 surface that in turn rendered superhydrophobicity and superoleophilicity to the surface. The FTIR absorption peaks indicate that after modifying SiO2 with TMVS, the -OH groups on SiO2 surface are clearly replaced by -CH3. The morphological studies indicated that modification of SiO2 leads to better cross-linking between coating composition and nanoparticles and EDS spectra and elemental mapping of the modified surface showed the presence of Si, O and C elements. Finally, this surface was tested for its efficiency and stability as a membrane in the process of separating oil and water from the oily water using gravity driven method. The oil-water separation efficiency was estimated to be 99% for this membrane and also it was found to be quite stable as the surface effectively retained this oil-water separation efficiency even after 10 cycles of separation process.

Appropriate surface wettability of membranes and materials are of an extreme importance for targeting separation of mixtures/emulsions such as oil from water or conversely water from oil. The development of super-wettable membranes and materials surfaces have shown remarkable potential for recovering water from oil-water emulsion while offering maximum resistance to fouling. The availability of clean and potable water has been regarded as an important global challenge for coming human generations. Oil and gas industry is continuously producing immense quantities of waste stream regarded as produced water which contains oil dispersed in water along with other several components. Treating such immense quantities of oily wastewater is of utmost need for recovering precious water for possible reuse or safe disposal. Various technologies have been developed for targeting the separation of oil-water emulsions or mixtures to harness useful potable water and oil as products. Membrane-based separations or use of porous materials such as mesh have been explored in literature for separation of oil-water mixtures/emulsions. Given the unique features of special hydrophilicity, ease of tunability, control of molecular weight, abundant availability, and potential for commercial scale up, chitosan has been extensively used for modifying membranes/meshes or preparing composites with other materials for oil-water separations. This review has described in detail the synthesis, methods of modification and application of chitosan-based super-wettable membranes/meshes and porous materials for oil-water separation. The special wettability features including super-hydrophobicity/superoleophilicity, super-oleophobicity/super-hydrophilicity and super-hydrophilicity/underwater super-oleophobicity of various chitosan-based membranes and materials have been discussed in detail in the review. The strategies for enhancing or developing special wettability for target specific applications have also been discussed. Finally, the challenges, their respective importance have been identified along with a discussion on possible solutions to these challenges.

This article reviews the application of highly heat and pressure resistant polyimide material for the development of membranes/materials that exhibit unique super-wettability, the characteristics pivotal for the efficient separation of oil-water mixture and emulsion. The polymerization of imide monomer in polyimide brings about the required porosity in the material, which in turn renders the crucial surface roughness, which is instrumental for establishing the desired super-wettability on the polyimide based membrane materials, in addition to the mechanical and thermal robustness. The membrane as the oil-water filtering medium can be either oil passing or water passing depends on the individual wettability of the membrane surface for oil and water, which in turn depend on the respective solid-liquid interfacial energy and the hierarchical surface roughness. Superhydrophobic/superoleophobic wetting characteristic of the surface repels water and allows oil to pass through the membrane medium, and the major disadvantage of this kind of oil/water separation is the rapid oil fouling of the membrane pores and the consequent less efficiency for oil water separation. On the other hand, the membrane surface engineered to have the Superhydrophilic/underwater superoleophobic wetting characteristics can be water passing, and the easy fouling of the membrane surface can be minimized. In the case of polyimide materials, there are lot of scopes to engineer the physical properties like surface energy and surface roughness of the membrane surface in order to obtain the required wettability. There have been many works focused on the application of different variants of polyimide materials for developing membrane for oil water separation. In this review, we present an itemized review of various works on polyimide materials based oil/water separation in terms of chemical, physical, structural and surface characteristics of the material.

Semiconducting membranes integrated with nanomaterials have placed themselves in new emerging researches tremendously for seawater desalination, oil-water separation, disinfection, removal of inorganic as well as organic pollutants. Howbeit, only nanoparticles unified membranes show quite a lot lags in their performance, although some of these particles associated with the demerits of high cost. In contrast, graphitic carbon nitride incorporated membranes offered improved aforementioned properties corresponding to absolute essential qualities such as cost-effective, environmentally friendly, easy-to-operate, green manufacturing, anti-fouling, and low energy consumption. Moreover, their high mechanical strength, high stability against harsh environment and long-term utilization without flux reduction are strong plus. Even though there are some undeniable downsides of these membranes in real world applications as bulk synthesis, consistent dispersion of graphitic carbon nitride, low photocatalytic efficiency etc. Accordingly, in the present article, these frailties of the membranes having graphitic carbon nitride as a filler and their respective synthesis procedures and properties are discussed. A comprehensive analysis over the application of semiconducting graphitic carbon nitride incorporated membranes with and without special surface modification; and exploration of the future challenges and difficulties associated to these membranes are also reviewed. Consequently, the current article provides brief overview about graphitic carbon nitride integrated composite membranes as well as their applications, and it finished up with new thoughts of further improvements/modifications to overcome their shortcomings in actual environmental conditions.

CONTEXT: There is an alarming rise in the incidence of chronic and recurrent dermatophytosis (CRD) in India. Many factors including tinea unguium may be responsible for it. AIMS: To evaluate various epidemiological and clinical factors including the presence of tinea unguium as a risk factor for CRD. SETTINGS AND DESIGN: This was a case-control study in which patients attending the dermatology outpatient department of a tertiary care hospital in February-March 2019 were recruited. A total of 80 consecutive clinically diagnosed patients with CRD as per the case definition (cases) were selected. Another 80 consecutive patients with dermatophytosis other than CRD (controls) were also selected. Patients were clinically evaluated with special attention for the presence of tinea unguium. RESULTS: Among the total of 80 cases, 44 (55%) and 36 (45%) were diagnosed to have chronic dermatophytosis and recurrent dermatophytosis respectively. CRD was relatively uncommon in patients younger than 20 years. Sharing of linen, family history, and topical corticosteroid abuse were also frequent among patients with CRD. Tinea unguium was present in six cases (7.5%) and two controls (2.5%) which was not statistically significant (P = 0.27). CONCLUSIONS: The current epidemic of CRD may be primarily due to a pathogen with certain specific epidemiological and clinical determinants. It may be primarily a skin pathogen with less or no affinity toward the hair and nail.

Chikungunya virus (CHIKV) is a re-emerging pathogen of global importance. We attempted to gain an insight into the organisation, distribution and mutational load of the virus strains reported from different parts of the world. We describe transmission dynamics and genetic characterisation of CHIKV across the globe during the last 65 years from 1952 to 2017. The evolutionary pattern of CHIKV was analysed using the E1 protein gene through phylogenetic, Bayesian and Network methods with a dataset of 265 sequences from various countries. The time to most recent common ancestor of the virus was estimated to be 491 years ago with an evolutionary rate of 2.78 × 10-4 substitutions/site/year. Genetic characterisation of CHIKV strains was carried out in terms of variable sites, selection pressure and epitope mapping. The neutral selection pressure on the E1 gene of the virus suggested a stochastic process of evolution. We identified six potential epitope peptides in the E1 protein showing substantial interaction with human MHC-I and MHC-II alleles. The present study augments global epidemiological and population dynamics of CHIKV warranting undertaking of appropriate control measures. The identification of epitopic peptides can be useful in the development of epitope-based vaccine strategies against this re-emerging viral pathogen.

INTRODUCTION: Methadone maintenance therapy (MMT) program helped to improve access to antiretroviral therapy (ART) among people who inject drugs (PWID) with human immunodeficiency virus (HIV). However, the time to treatment initiation (TTI) and outcomes of ART intervention in this population have scarcely been analyzed. OBJECTIVES: The aim of this study was to analyze the TTI and outcomes of ART among MMT clients in primary health-care centers in Kuantan, Pahang. MATERIALS AND METHODS: This was a retrospective evaluation of MMT clients from 2006 to 2019. The TTI was calculated from the day of MMT enrolment to ART initiation. The trends of CD4 counts and viral loads were descriptively evaluated. Cox proportional hazard model was used to analyze the survival and treatment retention rate. RESULTS: A total of 67 MMT clients from six primary health-care centers were HIV-positive, of which 37 clients were started on ART. The mean TTI of ART was 27 months. The clients who were given ART had a mean CD4 count of 119 cells/mm3 at baseline and increased to 219 cells/mm3 after 6 months of ART. Only two patients (5.4%) in the ART subgroup had an unsuppressed viral load. The initiation of ART had reduced the risk of death by 72.8% (hazard ratio = 0.27, P = 0.024), and they are 13.1 times more likely to remain in treatment (P < 0.01). CONCLUSION: The TTI of ART was delayed in this population. MMT clients who were given ART have better CD4 and viral load outcomes, helped reduced death risk and showed higher retention rates in MMT program.

Co-circulation of Chikungunya and Dengue viral infections (CHIKV and DENV) have been reported mainly due to transmission by common Aedes vector. The purpose of the study was to identify and characterise the circulating strains of CHIKV and DENV in DENV endemic region of New Delhi during 2016. CHIKV and DENV were identified in the blood samples (n = 130) collected from suspected patients by RT-PCR. CHIKV was identified in 26 of 65 samples (40%). Similarly, DENV was detected in 48 of 120 samples (40%). Co-infection with both the viruses was identified in five (9%) of the samples. Interestingly, concurrent infection with DENV, CHIKV and Plasmodium vivax was detected in two samples. CHIKV strains (n = 11) belonged to the ECSA genotype whereas DENV-3 sequences (n = eight) clustered in Genotype III by phylogenetic analysis. Selection pressure of E1 protein of CHIKV and CprM protein of DENV-3 revealed purifying selection with four and two positive sites, respectively. Four amino acids of the CHIKV were positively selected and had high entropy suggesting probable variations. Co-circulation of both viruses in DENV endemic regions warrants effective monitoring of these emerging pathogens via comprehensive surveillance for implementation of effective control measures.

OBJECTIVES: This study aims at analyzing the dimensions of the burden of out-of-pocket (OOP) health expenditure in rural and urban India between 1999-2000 and 2011-2012. STUDY DESIGN: Consumer Expenditure Survey data of the National Sample Survey for the period of 1999-2000 (55th round), 2004-2005 (61st round), and 2011-2012 (68th round) have been used in this study. METHODS: To analyze the dimension of OOP healthcare expenditure, this study has used headcount measure, which includes change in the percentage of population paying OOP health expenditure, concentration index of headcount, rank-weighted headcount, and gap measures, which includes average per capita gap (health expenditure), mean positive gap, concentration index, and rank-weighted gap for rural, urban, and all India level between 1999-2000 and 2011-2012. RESULTS: The authors have found that a large percentage of the population in rural and urban India is still not reporting health expenditure. There is a huge difference between per capita health expenditure in rural and urban areas during the study period. The percentage of the population that reported OOP health expenditure has increased at the bottom level, but the expenditure got more concentrated toward better-off people in both rural and urban areas of the country in this time period. It is also clear from the results that the concentration of average per capita gap (Co) in rural areas leans towards better-off people more than in urban areas. CONCLUSION: This study advocates for health sector reforms to protect households from the significant burden of expenditure on critical health care.

BACKGROUND: To develop an understanding of current practices in the management of transient secondary hypothyroidism in pediatric postoperative cardiopulmonary bypass (CPB) patients. METHODS: Electronic survey comprising a 10-item questionnaire was sent to sixty-four high volume pediatric heart centers in the United States and United Kingdom. Survey participants included cardiologists, intensivists, cardiothoracic surgeons, and advanced practice providers. A retrospective chart review was also performed at a large regional referral center in the Midwest on subjects 0-18 years old who underwent CPB from 2005-2015. Information obtained included a unique identifier, date of birth, age, procedure performed, CPB time, date of surgery and date and type of Thyroid Function Test (TFT) ordered. RESULTS: 1,153 individuals from 64 congenital heart centers were contacted via email to participate in the electronic survey. In the 3-month response window, 129 completed surveys were received from cardiologists (55%), intensivists (17%), surgeons (15%), "other" (8%), and advanced practice providers (5%). This yielded a response rate of 11.2%. Of the 129 respondents, only 10 providers routinely order TFTs prior to (n=7) and after (n=1) CPB or when clinically indicated (n=2). All 10 providers order thyroid stimulating hormone test, 7 order thyroxine, and 3 order triiodothyronine. Only 1 provider routinely treats children with prophylactic thyroid hormone replacement therapy after CPB. Our retrospective review included 502 CPB events with 442 unique patients. Of the events, 20 patients received preoperative TFT testing while 11 received postoperative testing. CONCLUSIONS: There is a general lack of uniformity in the evaluation, diagnosis, and treatment of transient secondary hypothyroidism in pediatric postoperative CPB patients.

By March 2015, 30% of all Medicare beneficiaries were enrolled in Medicare Advantage (MA) plans. Research to date has not explored the impacts of MA market penetration on individual or population health outcomes. The primary objective of this study is to examine the relationships between MA market penetration and the beneficiary's portfolio of cardiometabolic diagnoses. This study uses 2004 to 2008 Medical Expenditure Panel Survey (MEPS) Household Component data to construct an aggregate index that captures multiple diagnoses in one outcome measure (Chronic Disease Severity Index [CDSI]). The MEPS data for 8089 Medicare beneficiaries are merged with MA market penetration data from Centers for Medicare and Medicaid Services (CMS). Ordinary least squares regressions are run with SAS 9.3 to model the effects of MA market penetration on CDSI. The results suggest that each percentage increase in MA market penetration is associated with a greater than 2-point decline in CDSI (lower burden of cardiometabolic chronic disease). Spill-over effects may be driving improvements in the cardiometabolic health of beneficiary populations in counties with elevated levels of MA market penetration.

Inflammatory bowel diseases (IBDs), consisting mainly of ulcerative colitis (UC) and Crohn's disease (CD), are important immune-mediated diseases of the gastrointestinal tract. The etiology of the disease includes environmental and genetic factors. Its management presents a constant challenge for gastroenterologists and conventional surgeon. 5-Amninosalicylates, antibiotics, steroids, and immune modulators have been used to reduce the symptoms and for maintenance of remission. Unfortunately, long-term usage of these agents has been found to lead to severe toxicities, which are deterrent to the users. Pre-clinical studies carried out in the recent past have shown that certain dietary agents, spices, oils, and dietary phytochemicals that are consumed regularly possess beneficial effects in preventing/ameliorating UC. For the first time, this review addresses the use of these dietary agents and spices in the treatment and prevention of IBD and also emphasizes on the mechanisms responsible for their effects.
