RESUMO

To investigate the associations between corneal curvature (CC) and other anterior segment biometrics in young myopic adults. In this retrospective multi-center study, 7893 young myopic adults were included. CC and other anterior segment biometrics were measured by Scheimpflug imaging (Pentacam). CC was defined as SimK at central 3 mm area, and other anterior segment biometrics included white-to-white corneal diameter (WTW), central corneal thickness (CCT), corneal volume (CV) at 3 mm, 5 mm, and 7 mm area, anterior corneal astigmatism (ACA), posterior corneal astigmatism (PCA), anterior corneal eccentricity (ACE) and asphericity (ACAP), posterior corneal eccentricity (PCE) and asphericity (PCAP), anterior chamber depth (ACD), and anterior chamber volume (ACV). Univariate regression analyses were used to assess the associations between CC and other anterior segment biometrics, and multivariate regression analyses were further performed to adjusted for age, gender and spherical equivalent. CC was higher in patients of female gender and higher myopia (all P < 0.05). Eyes in higher CC quartiles had lower WTW, thinner CCT, lower CV at 3 mm and 5 mm, lower ACD, and lower ACV (all P < 0.001), but had larger ACA, larger PCA, less PCE and less PCAP (all P < 0.001), compared to eyes in lower CC quartiles. The trends of CV at 7 mm, ACE and ACAP were inconsistent in different CC quartiles. After adjusting for age, gender and spherical equivalent with multivariate linear regression, CC was positively correlated to CV at 7 mm (ßs = 0.069), ACA (ßs = 0.194), PCA (ßs = 0.187), ACE (ßs = 0.072), PCAP (ßs = 0.087), and ACD (ßs = 0.027) (all P < 0.05), but was negatively correlated to WTW (ßs = - 0.432), CCT (ßs = - 0.087), CV-3 mm (ßs = - 0.066), ACAP (ßs = - 0.043), PCE (ßs = - 0.062), and ACV (ßs = - 0.188) (all P < 0.05). CC was associated with most of the other anterior segment biometrics in young myopic adults. These associations are important for better understanding of the interactions between different anterior segment structures in young myopic patients, and are also useful for the exploration of the pathogenesis of myopia.

Purpose: To assess the efficacy and safety of intrastromal lenticule implantation for the treatment of hyperopia. Methods: A systematic search of PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Internet, and Wan Fang Database identified studies on small-incision intrastromal lenticule implantation for hyperopia correction until January 2023. The Joanna Briggs Institute (JBI) critical appraisal tool was used to assess the quality of the retrospective research, and the Methodological Index for Non-randomized Studies (MINORS) was used to assess the quality of the prospective research. This study included postoperative visual outcomes, corneal morphology, and biomechanical outcomes. Results: A total of 456 articles were identified, of which 10 were included in the meta-analysis. Ten single-arm studies involving 190 eyes were included. A meta-analysis demonstrated that corneal intrastromal lenticule implantation treatment significantly improved hyperopia. Uncorrected distance visual acuity (UDVA) significantly improved compared to the preoperative value (p = 0.027), corrected distance visual acuity showed no difference compared to the preoperative value (p = 0.27), and 87% eyes have no loss of one or more lines in the Snellen lines of CDVA (p < 0.00001). There was a significant difference between the spherical equivalent refractive (SE) and preoperative examination (p < 0.00001), 52% of eyes had ±0.5 diopters (D) postoperative SE (p < 0.00001), and 74% eyes had ±1.0 D postoperative SE (p < 0.00001). The central corneal thickness (CCT) increased by 72.68 µm compared to that preoperatively (p < 0.00001), and corneal curvature increased by 4.18D (p < 0.00001). The Q-value decreased by 0.82 (p < 0.00001), and higher-order aberration (HOA) decreased by 0.66 (p < 0.00001). Conclusion: Small-incision intrastromal lenticule implantation may be an effective solution for correcting hyperopia. The effect of improved vision is significant, but further exploration is needed for changes in corneal biomechanics and long-term safety.Systematic review registration: https://www.crd.york.ac.uk/PROSPERO/, identifier: CRD42023432343.

The interactions between white-to-white corneal diameter (WTW) and other ocular biometrics are important for planning of refractive surgery and understanding of ocular structural changes in myopia, but such interactions are rarely investigated in young myopic adults. This is a retrospective study involving 7893 young myopic adults from five centers. WTW and other ocular biometrics were measured by Pentacam. The ocular biometrics included anterior corneal curvature (AK) and posterior corneal curvature (PK), central corneal thickness (CCT) and corneal volume (CV), anterior and corneal eccentricity and asphericity, anterior corneal astigmatism (ACA) and posterior corneal astigmatism, anterior chamber depth (ACD), and anterior chamber volume (ACV). The ocular biometrics were compared among eyes of different WTW quartiles. Multivariate linear regression was used to assess the linear associations between WTW and other ocular biometrics adjusting for age, gender and spherical equivalent. In eyes of different WTW quartiles, other ocular biometrics were also significantly different (all P < 0.05). After adjusting for age, gender and spherical equivalent, WTW was positively correlated to AK (ß = 0.26 to 0.29), ACA (ß = 0.13), anterior corneal asphericity (ß = 0.05), PK (ß = 0.33 to 0.34), posterior corneal asphericity (ß = 0.13), ACD (ß = 0.29), and ACV (ß = 40.69), and was negatively correlated to CCT (ß = - 6.83), CV (ß = - 0.06 to - 0.78), anterior corneal eccentricity (ß = - 0.035), and posterior corneal eccentricity (ß = - 0.14) (all P < 0.001). In conclusion, we found that in young myopic adults, larger WTW was associated with thinner corneal thickness, flatter corneal curvature, more anterior corneal toricity, less corneal eccentricity and asphericity, and broader anterior chamber. Our findings may fill in the gap of literature, and help us better understand how the anterior segment structures interact with the WTW in myopia.

PURPOSE: The aim of this study was to introduce an optimized version of the Corvis Biomechanical Index for Chinese populations (cCBI). DESIGN: Retrospective, multicenter clinical validity enhancement study. METHODS: Patients were included from 7 clinics in Beijing, Shenyang, Guangzhou, Shanghai, Wenzhou, Chongqing, and Tianjin, China. Logistic regression was used to optimize the values of the constants of the CBI, based on database 1 as the development dataset (6 of 7 clinics), to create a new version of the index named cCBI. The factors of the CBI (A1Velocity, ARTh, Stiffness Parameter-A, DARatio2mm, and Inverse Integrated Radius) and the cutoff value were kept the same (0.5). With the formation of cCBI determined, it was validated on database 2 (1 of the 7 clinics). RESULTS: Two thousand four hundred seventy-three patients (healthy and keratoconus) were included. In database 2, the area under the curve of the cCBI was 0.985 with 93.4% specificity and 95.5% sensitivity. In the same dataset, the original CBI produced an area under the curve of 0.978 with 68.1% specificity and 97.7% sensitivity. There was a statistically significant difference between the receiver operating characteristic curve of cCBI and CBI (De Long P = .0009) CONCLUSION: The new cCBI for Chinese patients was shown to be statistically significantly better when compared with CBI to separate healthy from keratoconic eyes. The presence of an external validation dataset confirms this finding and suggests the use of cCBI in everyday clinical practice to aid in the diagnosis of keratoconus in patients who are of Chinese ethnicity.

PURPOSE: To evaluate the clinical results of primary topography-guided femtosecond laser-assisted in situ keratomileusis (TG-FS-LASIK) using the Pentacam-measured anterior corneal astigmatism axis (ACA) or manifest refractive astigmatism axis (MRA). METHODS: In this prospective cohort study, all eyes were treated with primary TG-FS-LASIK using the manifest cylinder. Thirty-two right eyes were treated using ACA with axis disparity > 5° in the experimental group, and 32 right eyes were treated using MRA with axis disparity ≤ 5° in the control group. Visual, refractive outcomes, and corneal higher-order aberrations were evaluated. Vector analysis of astigmatism was performed using Alpins method with the ASSORT software. RESULTS: The mean logMAR UDVA in the experimental group was - 0.12 ± 0.06, - 0.05 ± 0.08, and - 0.08 ± 0.08 at 1-week, 1-month, and 3-month follow-up, whereas - 0.05 ± 0.06, - 0.12 ± 0.06, and - 0.14 ± 0.06 in the control group (p = 0.017, p < 0.001, and p = 0.003). At 3-month follow-up, 79% eyes achieved a UDVA of 20/16 or better, 9% gained one line of corrected distance visual acuity, the mean manifest cylinder was - 0.375 ± 0.254 D, 84% showed a manifest cylinder within ± 0.50 D, the mean magnitude of difference vector was 0.41 ± 0.35 D, the mean absolute angle of error (AE) was 7.36 ± 6.21°, and 41% exhibited an arithmetic AE within ± 5° in the experimental group, whereas 94%, 19%, - 0.203 ± 0.148 D, 100%, 0.21 ± 0.15 D, 3.39 ± 3.68°, and 69% in the control group (all p < 0.05). The postoperative and preoperative corneal spherical aberrations were comparable in both groups (p > 0.05). CONCLUSION: Primary topography-guided FS-LASIK using Pentacam-measured anterior corneal astigmatism axis exhibited slightly inferior clinical results to that using the manifest refractive astigmatism axis. Both methods achieved comparable postoperative and preoperative corneal spherical aberrations.

Purpose: The aim of this study was to evaluate the difference between Caucasian and Chinese healthy subjects with regards to Corvis ST dynamic corneal response parameters (DCRs). Methods: Two thousand eight hundred and eighty-nine healthy Caucasian and Chinese subjects were included in this multicenter retrospective study. Subsequently, Chinese eyes were matched to Caucasians by age, intraocular pressure (IOP), and Corneal Thickness (CCT) using a case-control matching algorithm. The DCRs assessed were Deformation Amplitude (DA) Applanation 1 velocity (A1v), integrated radius (1/R), deformation amplitude ratio (DAratio), stiffness parameter at applanation 1 (SPA1), ARTh (Ambrósio's Relational Thickness to the horizontal profile), and the novel Stress Strain Index (SSI). Results: After age-, CCT-, and IOP- matching, 503 Chinese were assigned to 452 Caucasians participants. Statistical analysis showed a statistical significant difference between Chinese and Caucasian Healthy subjects in the values of SPA1 (p = 0.008), Arth (p = 0.008), and SSI (p < 0.001). Conversely, DA, A1v, DAratio, and 1/R were not significantly different between the two ethnical groups (p > 0.05). Conclusion: We found significant differences in the values of the DCRs provided by the Corvis ST between Chinese and Caucasian healthy subjects.

PURPOSE: To compare myopia and astigmatic correction after small-incision lenticule extraction (SMILE) with or without prophylactic crosslinking (SMILE Xtra). SETTING: Shenyang Aier Eye Hospital, Central South University, China. DESIGN: Retrospective study. METHODS: Patients with comparable manifest sphere and cylinder undergoing SMILE Xtra or SMILE were enrolled. The crosslinking (CXL) energy was 2.7 J/cm2. Only right eyes were selected. Visual and refractive changes were evaluated for 1 year. Astigmatic correction was analyzed using Alpins method. RESULTS: Thirty-six eyes undergoing SMILE Xtra and 40 eyes undergoing SMILE were enrolled. The uncorrected distance visual acuity at 1-day visit was lower after SMILE Xtra than that after SMILE (P = .01). At 12 months, the mean manifest refraction spherical equivalent (MRSE) and manifest cylinder were 0.08 ± 0.32 diopters (D) and -0.29 ± 0.23 D in SMILE Xtra group, whereas -0.25 ± 0.29 D and -0.22 ± 0.19 D in SMILE group (P < .01 and P = .135), respectively. Thirty-four eyes (94%) and 32 eyes (89%) in SMILE Xtra group and 36 eyes (91%) and 39 eyes (98%) in SMILE group exhibited target MRSE and manifest cylinder within ±0.50 D (P = .771 and P = .294), respectively. Compared with SMILE group, spherical correction index (SCI), correction index (CI), and difference vector were higher in SMILE Xtra group since 1-week follow-up (all P < .05). SCI and CI were slightly more than 1.0 after SMILE Xtra even at postoperative 12-month follow-up. CONCLUSIONS: With CXL protocol of 30 mW/cm2 for 90 seconds, SMILE Xtra exhibited comparable astigmatic correction with SMILE up to 1-year follow-up, although slight spherical equivalent and astigmatic overcorrection were evident after SMILE Xtra.

It is essential to know the normal range of the interocular symmetry of the cornea (ISC) for keratoconus diagnosis and corneal substitutes design. In the present study we investigated the interocular symmetry of corneal biometrics in 6,644 Chinese myopic patients from multiple ophthalmic centers. Corneal biometrics of both eyes were exported from the Pentacam instrument. Interocular symmetry of the corneal biometrics was analyzed by Spearman's correlation test, intraclass correlation coefficient (ICC) analysis and Bland-Altman plot. Significantly strong interocular correlations were found in anterior and posterior corneal curvatures, corneal diameter, corneal thickness, corneal volume, corneal eccentricity, and corneal asphericity (r = 0.87-0.98, all P < 0.001). Moderate interocular correlations were observed in whole corneal astigmatism (r = 0.78) and posterior corneal astigmatism (r = 0.73). ICC between the right and left eyes was 0.94-0.98 for anterior and posterior corneal curvatures, corneal diameter, corneal thickness and corneal volume, 0.80-0.88 for corneal eccentricity and asphericity, and 0.73-0.79 for corneal astigmatism (all P < 0.001). Bland-Altman plot showed that the 95% limit of agreement between both eyes was narrow and symmetric in most of the corneal biometrics, suggesting strong interocular agreements in these corneal biometrics. In conclusion, significant interocular symmetry of corneal biometrics is observed in Chinese myopia patients. Extra attention should be paid to patients with interocular corneal asymmetry.

OBJECTIVE: The aim of this study was to determine the prevalence of preoperative dry eye disease and evaluate tear film function in refractive surgery candidates in China. METHODS: In this prospective noninterventional cross-sectional study, refractive surgical candidates from 13 preselected eye hospitals in China were recruited from July 2015 to February 2016. Patient histories, subjective symptoms, tear film breakup time (TBUT), ocular surface fluorescein staining, and Schirmer I tests (SIT), were assessed to conduct subgroup analysis. RESULTS: A total of 1,849 patients were recruited, 41.4% were diagnosed with dry eye disease (766/1,849) and 44.9% (830/1,849) of subjects had a positive history of contact lens (CL) wear. The overall mean TBUT and SIT values were 7.3 ± 3.7 s and 15.2 ± 8.8 mm, respectively. The total prevalence of ocular surface fluorescein staining was 23.46% (422/1,849); 44.62% of patients had TBUT <5 s and 23.20% of patients had SIT <5 mm. CL wearers were observed to have a higher prevalence of dry eye than non-CL wearers (54.1 vs. 35.2%, OR = 2.17, 95% CI: 1.77-2.65). CONCLUSIONS: In this study, the most common abnormal finding in dry eye disease was tear film instability. A high proportion of refractive surgery candidates have preexisting dry eye disease and a history of CL wear prior to surgery. Careful attention should be given to the evaluation of preoperative dry eye in refractive surgery candidates.

PURPOSE: To compare small incision lenticule extraction with prophylactic cross-linking (SMILE Xtra) and femtosecond laser-assisted in situ keratomileusis with prophylactic cross-linking (FS-LASIK Xtra) in their safety, efficacy, predictability, and stability. METHODS: This is a prospective cohort study. Forty-eight eyes subjected to SMILE Xtra and 90 eyes subjected to FS-LASIK Xtra were enrolled. The enrolled eyes exhibited either borderline corneal topography or high myopia with thin cornea. The following parameters were evaluated before surgery and at 1 day; 1 week; and 1, 3, 6, and 12 months after surgery: corrected distance visual acuity, uncorrected distance visual acuity, manifest refractive spherical equivalent (MRSE), keratometry values, and thinnest corneal thickness. RESULTS: After 1 month, no eye showed loss of 1 or more lines of corrected distance visual acuity. At 12 months, uncorrected distance visual acuity was 20/20 or better for 98% and 99% eyes in the SMILE Xtra and FS-LASIK Xtra groups, respectively (P = 0.657). For 93.8% eyes in the SMILE Xtra group and 90% eyes in the FS-LASIK Xtra group, the achieved MRSE was within ±0.50 diopter of the intended MRSE (P = 0.669). The correlation coefficients for predictability of MRSE correction by SMILE Xtra and FS-LASIK Xtra were 0.987 and 0.983, respectively. The mean MRSE values, keratometry values, and thinnest corneal thickness measured at the different time points showed no significant intra- or intergroup differences. CONCLUSIONS: Our evaluations of 1-year outcomes suggest that the safety, efficacy, predictability, and stability of SMILE Xtra are comparable with those of FS-LASIK Xtra.

Including posterior corneal astigmatism (PCA) into consideration may increase the accuracy of astigmatism correction after corneal refractive surgery. In the present study we aim to investigate the distribution pattern of PCA in a large number of myopic patients from multiple ophthalmic centers. There were 7829 eyes retrospectively included in the study. Pentacam data of the eyes were retrieved from the machine and only results with image quality labelled with 'OK' were included. Distribution of PCA was slightly positively skewed (Skewness = 0.419, Kurtosis = 0.435, KS P < 0.0001). Mean PCA was 0.34 ± 0.14 D (range: 0.00 D-0.99 D). PCA was ≥ 0.25 D in 74.91% of the eyes and was ≥ 0.50 D in 11.61% of the eyes. In 97.55% of the eyes the steep meridian of PCA was vertical (SMV). PCA magnitude was significantly higher in eyes with SMV PCA (P < 0.0001) or high manifest astigmatism (MA, P < 0.0001). There was a significant correlation between anterior corneal astigmatism (ACA) magnitude and PCA magnitude in all of the eyes (r = 0.704, P < 0.0001). There was also a trend of decreasing frequency and magnitude of SMV PCA with aging (both P < 0.0001). In conclusion, PCA is present in myopic patients having corneal refractive surgery and PCA magnitude is increased with higher MA or ACA. Consideration of the impact of PCA on laser astigmatism correction may be necessary.

PURPOSE: To investigate the impact of cap thickness on the formation of an opaque bubble layer (OBL) during small incision lenticule extraction procedures. METHODS: In total, 100 eyes from 50 patients were prospectively examined. One of two corneal cap thicknesses was randomly assigned to each eye and differed in the contralateral eye: 110 µm in one eye and 140 µm in the other. OBL area and density were quantitatively assessed. RESULTS: The proportion of OBL areas in the anterior lenticule plane was 11.70% ± 7.35% in the 110-µm group, which was significantly higher than the 140-µm group (6.64% ± 4.68%, P < .001). For OBL areas located in the posterior lenticule plane, mean areas for the 110-µm group were also higher than those for the 140-µm group (1.32% ± 1.20% and 0.94% ± 0.59%, respectively; P = .002). Mean gray values of the OBL in the posterior lenticule plane were slightly different between the two groups (P < .001), but no significant difference in OBL of the anterior lenticule plane was observed (P = .055). Eyes with a 110-µm cap thickness had more focal OBLs, revealed by cap scanning (chi-square = 10.256, P = .001). RESUMO

PURPOSE: To evaluate the impact of cap thickness for small incision lenticule extraction (SMILE) on changes in corneal curvature and biomechanics. METHODS: One hundred eyes (50 patients) were enrolled in this prospective contralateral eye study. The difference in manifest refraction spherical equivalent (MRSE) in the same patient was less than 0.50 diopters. SMILE was performed with a randomized cap thickness of 110 µm in one eye and 140 µm in the other eye. MRSE, uncorrected distance visual acuity (UDVA), and corneal curvature and biomechanics were evaluated. The anterior and posterior surfaces of the corneal curvature (mean keratometry [Km] values 2, 4, and 6 mm from the pupil center) were measured by Pentacam HR (Oculus Optikgeräte, Wetzlar, Germany) and changes in corneal biomechanics at 6 months postoperatively by Scheimpflug technology. RESULTS: There was no significant between-group difference in UDVA or MRSE postoperatively. Postoperative changes in Km at the anterior surface (ΔKm-ant) in the 4-mm zone were significantly higher in the 110-µm group than in the 140-µm group at 1 day and 1, 3, and 6 months postoperatively (P = .043, .045, .003, and .049, respectively); at 3 months, the ΔKm-ant in the 6-mm zone was higher in the 110-µm group (P = .035). The changes in second appla-nation time, deformation amplitude, and integrated radius were significantly less in the 110-µm group (P = .031, .049, and < .001, respectively). CONCLUSIONS: A thicker corneal cap caused less change in anterior surface curvature after SMILE for moderate or low myopia, with no significant difference in UDVA and MRSE. OBJECTIVE: A retrospective observational cohort study was conducted to evaluate the efficacy and safety of tacrolimus in Osserman grade III and Osserman grade IV myasthenia gravis (MG) patients. PATIENTS AND METHODS: MG patients admitted to the First Affiliated Hospital of Guangzhou University of Chinese Medicine between June 2011 and January 2017 with grade III and grade IV according to the modified Osserman scale were recruited and received a telephone follow-up in September 2017. Patients treated with tacrolimus plus prednisone were compared with those treated without tacrolimus. The efficacy of tacrolimus was assessed using MG-activities of daily living (MG-ADL) score, Osserman classification, Myasthenia Gravis Foundation of America (MGFA) post intervention status (PIS), the number of hospitalizations, the number of myasthenic crises and deaths. The adverse drug effects of tacrolimus were monitored. RESULTS: A total of 124 patients were included. The tacrolimus group had a significantly lower MG-ADL score than the control group at follow-up (1.90 ± 2.27vs 2.97 ± 2.78, p = 0.029). The difference of MG-ADL score between baseline and after follow-up was significantly greater in the tacrolimus group than the control group (-7.20 ± 2.95 vs -5.52 ± 2.91, p = 0.003). Fewer patients were hospitalized in the tacrolimus group (p = 0.011). The Osserman classification, MGFA PIS, the number of myasthenic crises and deaths did not differ significantly between the two groups. Nineteen patients in the tacrolimus group had adverse drug reactions, but no severe adverse effects appeared. CONCLUSION: Our study suggested that tacrolimus could be an effective and safe treatment for Osserman grade III and Osserman grade IV MG patients.

PURPOSE: To investigate changes in intraocular scattering and Bowman's layer microdistortions after femtosecond laser small incision lenticule extraction using the continuous curvilinear lenticulerhexis technique (SMILE-CCL). METHODS: In this prospective study, 93 eyes of 93 patients (44 women, 49 men) were treated with SMILE-CCL. Mean age was 26.09 ± 4.39 years and mean spherical equivalent was -6.31 ± 2.16 diopters. The Objective Scatter Index (OSI) was examined with an objective optical quality analysis system. Bowman's layer microdistortions in the central 4-mm optical zone were measured using Fourier-domain optical coherence tomography. RESULTS: The mean OSI value increased from 0.67 preoperatively to 1.09 at 20 days, and then decreased to 0.84 at 3 months (P < .05). Microdistortions were observed in 62.37% of the eyes at 20 days and 48.39% at 3 months postoperatively (P < .05). The width of the microdistortions per meridian was 283.18 ± 197.19 µm at 20 days; this decreased to 156.00 ± 159.86 µm at 3 months (P < 0.05). Regression analysis models revealed that older age (b = 0.02, P = .03) and higher preoperative spherical equivalent (b = -0.09, P < .05) resulted in higher postoperative OSI values; no significant association was found between postoperative OSI values and microdistortion-related parameters (P > .05). Higher preoperative spherical equivalent (b = -0.28, P = .01) and thinner preoperative corneal thickness (b = -0.02, P = .01) were associated with more microdistortions. CONCLUSIONS: SMILE induced a temporary increase in OSI values and Bowman's layer microdistortions. The microdistortions did not influence intraocular scattering. Alzheimer's disease (AD) is characterized by excessive amounts of senile plaques and neurofibrillary tangles in the brain, and cerebrovascular pathologies in AD are attracting increasingly more attention. Krüppel-like factor (KLF) 2, a transcription regulator expressed in the mouse embryonic vasculature and involved in the regulation of vascular gene expression, serves as a protective factor in endothelial cells. However, whether KLF2 is involved in neurodegenerative disease, and especially in AD, remains unknown. In the present study, the effects of KLF2 in Aß-induced neurotoxicity were investigated. Firstly, we found that KLF2 expression decreased at both the RNA and protein levels in AD cases. The following results show that KLF2 was found to be decreased in both a time- and dose-dependent manner in response to Aß1-42 treatment in primary mouse brain microvascular endothelial cells. Overexpression of KLF2 attenuated Aß-induced oxidative stress, improved mitochondrial function, and reduced the rate of apoptosis. Furthermore, inhibition of KLF2 promoted Aß1-42-induced oxidative stress, exacerbated mitochondrial dysfunction, and increased the rate of apoptosis. Our data imply that the promotion of KLF2 expression in cerebral endothelial cells has the potential to be developed as a novel therapeutic strategy for the treatment of cerebral vascular dysfunction in AD.

BACKGROUND: Corneal biomechanical properties are always compromised after corneal refractive surgeries thus leading to underestimated intraocular pressure (IOP) that complicates the management of IOP. We investigated the changes in postoperative baseline of IOP values measured with noncontact tonometer (NCT), ocular response analyzer (ORA) and corvis scheimpflug technology (CST) in the early phase after small incision lenticule extraction (SMILE). METHODS: Twenty-two eyes (-6.76 ± 1.39D) of 22 moderate and high myopes, (28.36 ± 7.14 years, 12 male and 10 female) were involved in this prospective study. IOP values were measured using a non-contact tomometer (NCT-IOP), an ocular response analyzer (corneal-compensated IOP, IOPcc and Goldmann-correlated IOP, IOPg) and a Corvis scheimpflug technology tonometer (CST-IOP) preoperatively, at 20 min and 24 h, postoperatively. Repeated measures analysis of variance (RM-ANOVA), Pearson's correlation analysis and multiple linear regression models (stepwise) were performed. Cut-off P values were 0.05. RESULTS: Except for IOPcc, NCT-IOP, IOPg, and CST-IOP values significantly decreased after SMILE procedure (All P values <0.05). ΔCCT, as well as ΔMRSE and ΔKm, did not significantly correlated with ΔNCT-IOP, ΔIOPcc, ΔIOPg or ΔCST-IOP, (all P values >0.05). Multiple linear regression models (stepwise) showed that the practical post-operative IOP value was the main predictor of the theoretical post-operative NCT-IOP, IOPcc and IOPg values (all P values <0.001). The postoperative applanation time 1 (AT1) value (B = 8.079, t = 4.866, P < 0.001), preoperative central corneal thickness (CCT) value (B = 0.035, t = 2.732, P = 0.014) and postoperative peak distance (PD) value (B = 0.515, t = 2.176, P = 0.043) were the main predictors of the theoretical post-operative CST-IOP value. CONCLUSIONS: IOP values are underestimated when assessed after SMILE by using NCT-IOP, IOPg and CST-IOP. The practical postoperative IOPcc value and theoretical post-operative CST-IOP value may be more preferable for IOP assessment in the early phase after SMILE. OBJECTIVE: To investigate the feasibility and safety of adult-to-adult living-related donor liver transplantation using a right lobe graft. METHODS: The clinical data of 2 cases of living-related donor liver transplantation performed between July, 2010 and November, 2010 were analyzed. RESULTS: Liver transplantation was performed using a right lobe graft including the middle hepatic vein in one case and a right lobe graft without the middle hepatic vein in the other. The ratio of graft volume to standard liver volume was 46.2% and 47.3% in the two cases, with GR/WR of 0.83 and 0.80, and donor residue liver of 42.1% and 39.5%, respectively. The donor operation lasted for 6.5 h and 5 h in the two cases with blood loss of about 200-250 ml without blood transfusion. The donors recovered uneventfully without any surgical complications, whose liver function was normal 7 days after the operation, and were discharged 14 days and 16 days after the surgery, respectively. The recipient operation lasted for 8 h and 7 h with blood loss of about 800-1000 ml. The right hepatic vein, hepatic artery, portal vein and bile duct reconstruction were performed by end-to-end anastomoses in the 2 recipients. Bile duct anastomosis stricture occurred in the first recipient 2 months after transplantation and was treated with percutaneous transhepatic cholangiography and drainage. The second recipient recovered smoothly without any complications. The recipients have so far survived 9 months and 5 months, respectively. CONCLUSION: Adult-to-adult living-related donor liver transplantation is a safe and effective option for treatment of end-stage liver diseases in the context of cadaveric liver graft shortage.
