AIM: To investigate the association of clinical variables and polymorphisms (tag SNPs) in the interleukin 4 (IL4) gene, with the prognosis of avulsed and replanted teeth. METHODOLOGY: Ninety-four patients who suffered avulsion and had their teeth replanted and endodontically treated were included. Periapical radiographs were obtained soon after tooth replantation and after 1 year. For genotypic IL4 gene analysis, the DNA from oral mucosa cells was collected, and polymorphisms were investigated by real-time PCR. Univariate and multivariate analyses were performed to verify the association of clinical and genetic variables and the outcome of the replanted teeth (P < 0.05). RESULTS: After multivariate analysis, extra-alveolar time longer than 1 h was significantly associated with external root resorption. No significant association was observed between IL4 gene polymorphisms and root resorption. CONCLUSION: No association between root resorption and IL4 gene polymorphisms was observed. An extra-alveolar time of more than 1 h was associated with a susceptibility for external root resorption. Replanting the tooth in its socket immediately is the most important factor to maintain a healthy root surface.

OBJECTIVE: The study aimed to verify the potential correlation between the detected amount of gram-negative bacteria and the radiographic sizes of the lesions in patients with symptomatic and asymptomatic apical periodontitis. Furthermore, to evaluate whether the expression of receptor activator of NF-κB ligand (RANKL) and osteoprotegerin (OPG) and the RANKL/OPG ratio are differentially regulated in both groups. DESIGN: Twenty patients with periapical lesions were divided into two groups: symptomatic (SYM) n=10 and asymptomatic (ASYM) n=10. After periapical surgery, the lesions were collected and processed for histological examination, and immunohistochemistry. The percentage of RANKL- and OPG-immunopositive areas relative to the total area of the microscopic field was calculated. For gram staining, the number of gram-negative cells per microscopic field was assessed. The radiographs of each patient were processed and measured. The Student's t-test and the Pearson correlation coefficient were performed. RESULTS: The SYM group showed a significantly higher number of gram-negative cells (p=0.007) when compared to the ASYM group. A higher number of gram-negative bacteria occurred more frequently in larger periapical lesions and the SYM group (p=0.03). The expression for RANKL and OPG and the RANKL/OPG ratio were not significantly different between the groups. There was a significant positive correlation between the number of bacteria and OPG levels in the SYM group (p=0.01). CONCLUSION: The number of bacteria seems to influence the symptoms and the radiographic size of a periapical lesion. Gram-negative bacteria may play an important role in OPG activity in the SYM group.

AIM: To assess the effect of sodium chloride concentration in fluoridated solutions during the electrochemical dissolution of fractured rotary endodontic instruments. METHODOLOGY: Two solutions were assessed (solution 1: NaF 12 g L-1  + NaCl 1 g L-1 , pH = 5.0; and solution 2: NaF 12 g L-1  + NaCl 180 g L-1 , pH = 5.0) using two tests: the ProTaper Universal F1 (PTU F1) instrument polarization test and the polarization test for intracanal PTU F1 fragments fractured in mandibular incisors. In the first test, two sets of five instruments were separately and partially immersed in each solution, and the electrical current was evaluated over 30 min. In the second test, 45 PTU F1 instruments were fractured within the root canals of mandibular incisors and subjected to potentiodynamic polarization for 30 min. The electrical current and the variations in the length of PTU F1 fragments were measured. The data were analysed statistically (anova and Wilcoxon and Mann-Whitney tests, respectively). RESULTS: Solution 2 was associated with more corrosive effects in both tests. In the first test, the PTU F1 instruments immersed in solution 2 had a higher electrical current (P < 0.001) and had a total dissolution time of approximately 540 s. In the second test, a larger difference between the baseline and final lengths of the fragments was noted in solution 2 (P = 0.011). CONCLUSION: Saturation of fluoridated solution with sodium chloride led to an increase in electrical current and microscopic reductions in the length of fractured instrument fragments subjected to electrochemical dissolution.

OBJECTIVES: To assess the influence of metallic posts in the detection of simulated vertical root fractures (VRFs) using the following imaging examinations: 2 cone beam CT (CBCT) systems [CBCT1: NewTom(®) 3G (QR Srl, Verona, Italy) and CBCT2: i-CAT Next Generation(®) (Imaging Sciences International, Hatfield, PA)] and film and digital radiographs. Additionally, the influence of the orientation of the fracture line in the detection of VRFs was evaluated. METHODS: 100, human, single-rooted endodontically treated premolars were divided into 5 groups (Group 1: with posts and buccolingual VRFs, Group 2: with posts and mesiodistal VRFs, Group 3: without posts and with buccolingual VRFs, Group 4: without posts and with mesiodistal VRFs, and Group 5: with posts and without VRFs). The premolars were placed in human mandibles and imaged using the four examination modalities. The sensitivity and the specificity of each examination in the experimental groups were calculated. The data were analysed using Student's t-test. RESULTS: The presence of metallic posts reduced the sensitivity of the CBCT1 system (p = 0.0244). Digital radiographs and the CBCT1 and CBCT2 systems had a higher sensitivity in detecting buccolingual fractures in teeth with posts, whereas film and digital radiographs had a higher sensitivity in detecting buccolingual fractures in teeth without posts (p < 0.05). The CBCT1 examination demonstrated the lowest specificity (p < 0.05). CONCLUSIONS: The presence of metallic posts did not influence the sensitivity of most of the examinations, excluding the CBCT1 system. The fracture line orientation may influence VRF detection.

AIM: To evaluate in vivo the accuracy of the Root ZX II (J. Morita) apex locator in controlling the apical extent of rotary instrumentation when using the Auto Apical Reverse (AAR) set at the levels 0.5, 1.0 and 1.5. METHODOLOGY: Thirty single-rooted premolar teeth scheduled for extraction were divided into three groups (n = 10), according to the AAR setting 0.5, 1.0 and 1.5. The root canals were prepared using ProTaper (Dentsply Maillefer). After rotary instrumentation, the last file used (F3) was manually introduced into the extent of the root canal preparation and fixed before tooth extraction. The apical third of the root was dissected until exposure of the file. The distance from the file tip to the major apical foramen was obtained. RESULTS: Measurements within the range -1.0 to 0.0 mm were obtained in 30% of the teeth with AAR 0.5, 50% with AAR 1.0 and in 0% with AAR 1.5. The proportions test revealed a significant difference between the AAR settings 1.0 and 1.5 (P = 0.0188). Overinstrumentation occurred in 70% of the teeth with AAR 0.5 and in 40% with AAR 1.0. The measurements short of the acceptable range occurred in 10% of the teeth with setting AAR 1.0 and in 100% of the cases with AAR 1.5. A significant difference was found when comparing the percentage of teeth in which the file tip was short and beyond the established range between groups, except when comparing AAR 0.5 and AAR 1.0. CONCLUSION: The AAR function of the Root ZX II was not an accurate method for controlling the apical extent of rotary instrumentation in vivo. The setting 0.5 presented overinstrumentation in most of the canals, the setting 1.5 was short in all cases, and the setting 1.0 provided an adequate working length in only 50% of the teeth.

AIM: To report an anatomical variation in maxillary first molars. SUMMARY: A case report is presented of a maxillary first molar requiring root-canal treatment that had two palatal roots with distinct canals and foramena at the apical level. Root-canal treatment was performed using mechanical instrumentation with NiTi files. An anatomical study on two extracted maxillary second molars with the same variation revealed their internal structure. This case demonstrates an uncommon anatomical condition and supplements previous reports of such cases affecting maxillary molars. KEY LEARNING POINTS: Clinicians should be aware of uncommon anatomical variations in maxillary molars. The majority of maxillary first molars have three roots and four canals. Maxillary molars may have two canals in the palatal root.

AIM: The objective of this study was to determine the effectiveness of 0.04 ProFile instruments during the removal of root fillings. METHODOLOGY: A total of 30 single-rooted mandibular canines were divided into three groups. The canals were accessed and then prepared with a reciprocating automated device and nickel-titanium files. Three different techniques of obturation were used: Thermafil system (group I), thermomechanical compaction (group II) and lateral condensation (group III). Sealer 26 was used in all three groups. Two weeks after obturation, the fillings were removed using 0.04 ProFile instruments following standardized sequence of size 90, 60, 45, 40, 35 and 30 at a speed of 300 rpm in a crown-down manner. The teeth were subsequently split with a chisel, photographs were taken with a colour video camera and the removal of filling material was assessed visually (by colour) by two examiners. The time required to achieve the working length was also recorded. RESULTS: ProFile instruments reached the working length in all cases. However, complete removal of gutta-percha occurred in only three specimens (two of group I, and one of group II). Thermafil plastic carriers were removed successfully in all specimens of group I. CONCLUSIONS: ProFile instruments with 0.04 taper were inadequate in completely removing the filling materials from the root canal system. However, the ideal working length was achieved rapidly.

With the development of rotary nickel-titanium files and their applications in Endodontics, the concepts of root canal preparation have changed. The objective of this research is to evaluate the transportation of simulated root canals caused by stainless steel ENDOflash rotary files compared to nickel-titanium ProFile files (.04, .06 and Orifice Shapers). Twenty-four simulated root canals with a 30 degrees curvature were randomly divided into 2 groups and prepared with ENDOflash or ProFile files according to manufacturers' instructions. Comparison between the simulated root canals before and after instrumentation showed significant differences (p < 0.001) between the groups, with greater transportation observed with ENDOflash files. One case of deformation was observed with a ProFile instrument (#20/.04) and there were no fractures.
