INTRODUCTION: Owing to the different results from the series that evaluate the behavior of the bladder cancer according to the age at the moment of the diagnosis, our objective is based on valuing the characteristics and behaviour according to age of appearance. METHODS: A retrospective study of bladder cancer diagnosed in our area during decade 1993-2003, distributed in 3 intervals of age and some characteristics and behaviour are valued. RESULTS: Elderly patients present greater tumors, non differentiated and with greater rate of progression to infiltrated. Moreover the age, the pathological stage and the tumorlike degree appear as independent significant factors in the multivariant study. CONCLUSIONS: In our experience, the patients greater than 70 years present neoplasms of similar clinical characteristics, although pathologically more aggressive, with greater percentage of progression and worse survival.

Adenolipoma of the skin is an unusual variant of lipoma recently described by Hitchcock et al. and characterized by the presence of normal eccrine sweat glands within a lipoma. We report a case and review the literature. A 45-year-old woman presented with a slow-growing, painless nodule on the thigh, clinically considered to be lipoma. Microscopically it comprised an adipose-tissue proliferation with a single eccrine secretory coil and associated duct in the periphery and in the center of the nodule. This benign lesion has been termed adenolipoma because of the presence of adipose tissue and eccrine glands. It probably represents only a histological curiosity in which the eccrine glands are entrapped by the adipose proliferation. Adenolipoma of the skin is a distinct lesion that can occur in the dermis or subcutaneous tissue.

An extraordinary case of encapsulated fat necrosis characterized by its large size, diffuse formation of pseudomembranes, and tendency to recur after excision is reported. A 67-year-old Caucasian woman suffering from morbid obesity was admitted for diagnosis and surgical treatment of a soft tissue mass showing a longest diameter of 14 cm and lying adjacently to the scar from previous appendicectomy. Histopathologic features were consistent with a nodular-cystic encapsulated fat necrosis with diffuse pseudomembranous transformation. Eight months after surgery, a new larger mass (longest diameter of 18 cm) sharing identical histopathologic features appeared in the same location. Encapsulated fat necrosis is a well-defined entity even though several names have been proposed for this condition, including mobile encapsulated lipoma, encapsulated necrosis, or nodular-cystic fat necrosis. Its pathogenesis seems to be related to ischemic changes secondary to previous trauma. It may occasionally show degenerative changes, including dystrophic calcifications and presence of pseudomembranes. To our knowledge, these are the first reported cases of encapsulated fat necrosis presenting as lesions of such size and showing diffuse formation of pseudomembranes; these particular features made diagnosis difficult and led to consideration of a wide range of potential diagnostic possibilities. This case expands the clinico-pathologic spectrum of membranocystic fat necrosis, including the potential ability of this subcutaneous fatty tissue abnormality to recur after surgical excision. RESUMO

We report on the case of a young man with a longstanding tumor in the left proximal tibia, in whom we had the opportunity to correlate both radiologic (plain films plus MRI) and bone scintigraphic findings with those of biopsy and clinical follow-up. The final diagnosis was bone fibrous dysplasia. The correlation between noninvasive diagnostic findings is discussed, and the convenience of using such test combination is also stressed in order to endorse a noninvasive approach to such patients, given the low malignant potential, the exception being those patients in whom there is a definite fractures risk.

RESUMO

Consulting in child psychology in collaboration with the pediatric hospital service, gave way to a growing interest to learn and take in consideration psychological problems of children. Six drawings by children are presented about subjects of hospitalization and in which the physician takes on a familiar psychological value that goes beyond the diagnosis and somatic treatment. The affective states which children show in their drawings when they are hospitalized and the work of the physician are discussed. Some psychoprophylactic means are suggested for hospitalization, surgical operations and serious somatic illnesses that can lead to a premature death. The child's biography and what the physician can observe in his family visits, are basic elements in the psychosomatic approach. The drawing gives elements about the internal state of the child in the hospital atmosphere and preliminary indications on individual psychotherapy treatment.
