Background: Psychiatry is a specialized field of medicine that focuses on the diagnosis, treatment, and prevention of mental health disorders. With advancements in technology and the rise of artificial intelligence (AI), there has been a growing interest in exploring the potential of AI language models systems, such as Chat Generative Pre-training Transformer (ChatGPT), to assist in the field of psychiatry. Objective: Our study aimed to evaluates the effectiveness, reliability and safeness of ChatGPT in assisting patients with mental health problems, and to assess its potential as a collaborative tool for mental health professionals through a simulated interaction with three distinct imaginary patients. Methods: Three imaginary patient scenarios (cases A, B, and C) were created, representing different mental health problems. All three patients present with, and seek to eliminate, the same chief complaint (i.e., difficulty falling asleep and waking up frequently during the night in the last 2°weeks). ChatGPT was engaged as a virtual psychiatric assistant to provide responses and treatment recommendations. Results: In case A, the recommendations were relatively appropriate (albeit non-specific), and could potentially be beneficial for both users and clinicians. However, as complexity of clinical cases increased (cases B and C), the information and recommendations generated by ChatGPT became inappropriate, even dangerous; and the limitations of the program became more glaring. The main strengths of ChatGPT lie in its ability to provide quick responses to user queries and to simulate empathy. One notable limitation is ChatGPT inability to interact with users to collect further information relevant to the diagnosis and management of a patient's clinical condition. Another serious limitation is ChatGPT inability to use critical thinking and clinical judgment to drive patient's management. Conclusion: As for July 2023, ChatGPT failed to give the simple medical advice given certain clinical scenarios. This supports that the quality of ChatGPT-generated content is still far from being a guide for users and professionals to provide accurate mental health information. It remains, therefore, premature to conclude on the usefulness and safety of ChatGPT in mental health practice.

The COVID-19 outbreak resulted in the shutdown of athletic training facilities. Although the effects of these restrictions on daily activity and sleep patterns have been widely analyzed, the employed tools often lacked accuracy, and were based on subjective measures. This study assessed the effects of home confinement on objective physical activity (PA), physiological and sleep parameters in active individuals. Sixteen male elite fitness coaches (age: 29±3 years; height: 183±6 cm; body mass: 82±5 kg, body mass index: 24.7±1.8 kg/m2) participated in this retrospective study. One-way analysis of variance was conducted to analyze selected PA, physiological and sleep parameters collected by smartwatch (Garmin Fenix 6 pro, USA) data during four consecutive months [i.e., pre-confinement, 1st and 2nd months of confinement, and post-confinement, year 2020]. Ramadan intermittent fasting (RIF) month occurred during the 2nd month of confinement. Compared to pre-confinement, significant changes were registered for almost all parameters during the 1st and/or the 2nd month of confinements (p<0.001), with (i) higher values for resting heart rate, sleep latency, and total, light and rapid eye movements sleep times (% change=7-523 %), and (ii) lower values for PA parameters, calories/day spent, average and highest respiratory rates, and deep sleep time during the home confinement period (% change=5-36 %). During the post-confinement month, all parameters regained pre-confinement values. In conclusion, home confinement-induced detraining negatively influenced the objective measurements of cardiorespiratory and sleep parameters among fitness coaches with a deeper effect during the 2nd month of home confinement, possibly due to the effect of RIF.

BACKGROUND: The circadian clock regulates the function of the immune system, the replication of viruses, and the magnitude of infections. The aim of this study was to analyse whether hospital attendance in Coronavirus disease 2019 (COVID-19) patients presents a diurnal variation. METHODS: Data from the electronic medical records of 1094 COVID-19 patients who presented to a Health Centre in Qatar during the month of July 2020 was retrospectively analysed. The following demographic (i.e. time of day (TOD), sex, age), clinical (i.e. cycle threshold (CT), temperature, oxy-haemoglobin saturation and resting heart-rate), biochemical (i.e. uraemia, glycaemia and albuminia) and haematological (i.e. leukocytes, erythrocytes ad platelets) parameters were collected. RESULTS: Univariate analysis showed a significant effect of TOD on hospital admission (p < 0.001), with patients attending the health care centre more during the active behavioural phase (08h00-00h00) compared to the resting phase (00h00-08h00). COVID-19 infection blunted the circadian rhythms of core body temperature, neutrophils and leukocytes family and shifted the circadian rhythms of resting heart-rate and uraemia. Correlation analysis showed a near perfect negative correlation between the age of patients and the TOD (r=-0.97), with older patients attending the care centre earlier during the day. CONCLUSION: COVID-19 infection affected the circadian rhythms of the host through disrupting the circadian rhythms of core temperature and innate immunity mediators. Old patients attend the health care centre earlier compared to younger ones. However, CT during polymerase chain reaction-test was unaffected by the TOD, which limits the conclusion that COVID-19 viral infection exhibits diurnal variation.

The specifics of short-term physical exercise are similar to the immediate reaction demands placed on police officers. Identifying the physiological predisposition to short-term high-intensity exercise in male law enforcement officers will assist in understanding their metabolism and make a significant contribution to a much more personal and individualized workout program. This will improve physical fitness of individual officers, improving their preparedness for such times of emergency. This cross-sectional study was conducted to investigate the responses of hematological (erythrocytes, hemoglobin, hematocrit, leucocytes, monocytes, neutrophils, lymphocytes), hormonal (testosterone, cortisol, melatonin), biochemical (glucose, uric-acid, lactate, creatine-phosphokinase) data to short-term maximal exercise in male police officers (n = 20). Blood samples were collected before- and after- the running-based anaerobic sprint test (RAST), and biological values were corrected for fluid shifts. Data were mean ± standard deviation of differences (= after minus before RAST). After the RAST, values of cortisol, lactate, neutrophils, lymphocytes, and monocytes increased significantly by 7.01 ± 37.36 mmol/l, 7.55 ± 1.67 mmol/l, 0.17 ± 0.26 103/µl, 0.61 ± 0.28 103/µl, and 0.10 ± 0.13 103/µl, respectively. After the RAST, values of melatonin, uric-acid, creatine-phosphokinase, hemoglobin, and hematocrit decreased significantly by -13.24 ± 4.60 pg/ml, -13.28 ± 14.35 µmol/l, -10.23 ± 10.13 IU/l, -2.01 ± 0.81 g/dl, and -4.46 ± 0.59%, respectively. Biological data of male police officers were affected by sprint test. Understanding changes in biological data following short-term maximal exercise can further assist in a better understanding of anaerobic metabolism, which will be helpful to find available methods for coaches to quantify training loads.

This study aimed to assess the effects of the lunar cycle on diurnal variation of biological, i.e, hormonal, biochemical, and hematological, profiles of diurnally active healthy men. Blood samples of 20 males were collected on four occasions [full moon (FM) and new moon (NM), in the morning (06:00-07:00 h) and evening (19:00-20:00 h)]. The results showed that melatonin and testosterone levels and neutrophils count were lower during the FM as compared to the NM in the morning (p < .001; d = 4.13, p < .001; d = 3.84, p < .01; d = 0.77, respectively) and evening (p < .001; d = 6.36, p < .001; d = 4.03, p < .05; d = 1.07, respectively) samples. However, cortisol level was higher during the FM compared to the NM, in the morning (p < .001; d = 0.74) and evening (p < .001; d = 3.54). Hemoglobinemia was higher only in the evening during the FM compared to the evening of the NM (p < .01; d = 1.22). In summary, this study confirmed that lunar cycle can affect human biological parameters independently of time of day.

PURPOSE: To investigate the optimal pretaper duration on match running performance in a professional soccer team. METHODS: The training load was monitored during daily training sessions and matches during 2 seasons according to different periodization strategies. Matches' running distances were collected using match analysis system. The data were analyzed in 3 types of mesocycle blocks of 5 (M5), 4 (M4), and 3 weeks (M3), concludes all of them by 1 taper week. RESULTS: Significant decreases in the training load during the taper weeks compared to standard weeks were observed in 3 types of mesocycle blocks (d ≥ 5; P < .01). An increase in overall match running performance was observed in matches played after the taper weeks compared to matches played after the standard weeks during M4 for all speed ranges (d ≥ 1.3; P < .05). The increase was only observed in low-intensity running (d = 1.3; P < .04) and total distance, low-intensity running, and intense running (d ≥ 1.3; P < .05) in M5 and M3, respectively. Match running performance following the taper weeks between the 3 different mesocycle durations was significantly higher in M4 for the number of high-speed running, sprinting, and high-intensity running (P < .05). The greatest enhancement of match running performance was observed at M4 when the training load was decreased by approximately 18% during the tapering period. CONCLUSION: This study suggests that a period of 3 standard weeks of training followed by 1 taper week is the optimal taper strategy when compared to different pretaper durations.

The aim of this study was to compare the physiological responses between a vibration induced cycling step protocol (Vib) and normal cycling (without vibration, no-Vib). Eighteen moderate trained males (age 24.1 ± 4.3 years; weight 76.5 ± 10.5 kg; height 178.0 ± 6.4 cm) have participated in this study. They randomly performed two gradual maximal exercise tests on two separate days using a new bike that automatically induces vibration cycling and the Corival cycle ergometer. The choice of two different bikes was made because of the impossibility to recreate the same power output without altering the cycling cadence on the vibration Bike. Both protocols were matched for power output and cycling cadence incrementations. Oxygen uptake (VO2), carbon dioxide production (VCO2), ventilation (VE), heart rate (HR), blood lactate and rating of perceived exertion (RPE) during each stage were continuously recorded. No statistical differences were founded for all variables when comparing the Vib to no-Vib trials, except a higher ventilation during the vibration trial at submaximal levels. The results of this study do not confirm those of previous studies stated that Vib increased metabolic demands during cycling exercise. Added vibration stimulus to an incremental cycling protocol does not affect physiological parameters.

The aim of this study was to explore the effect of the lunar cycle and time of day on short-term maximal performance, wellness and mood state. Twenty healthy active men performed a running anaerobic sprint test (RAST) at four sessions (full moon (FM) and new moon (NM) in the morning and in the evening). Before each testing session, participants completed a rating scale of mood states (POMS) and a psychometric rating scale of perceived wellness (Hooper's questionnaire). Rating of perceived exertion (RPE) was collected just after the RAST. A higher rating of sleep was observed in FM compared to NM upon the morning and the evening (FM morning vs. NM morning, p < .01; d = 1.38) and (FM evening vs. NM evening, p < .001; d = 2.62). POMS subscales data revealed no significant lunar cycle effect. Peak power (PP), average power (AP) and minimal power (MP) were greater in the evening following NM compared to FM (NM evening vs. FM evening, p< .01, <0.01, <0.05; d = 0.77, = 0.78, = 0.68, respectively). Likewise, RAST performance was greater in the evening compared to the morning for both lunar cycle phases (FM evening vs. FM morning, p< .001, <0.01, <0.01; d = 1.08, = 1, = 0.85) and (NM evening vs. NM morning, p< .001, <0.01, <0.01; d= 1.08, = 1, = 0.85), respectively, for PP, AP, and MP. RPE was higher in the FM evening compared to the NM evening and in the FM morning compared to the NM morning (p < .001, <0.001; d = 1.38, = 2.18, respectively). These results suggest that the lunar cycle and time of day might affect short-term maximal performance with better performance on NM evening associated with a greater perception of sleep quality compared to FM. Objectives: The present review provides a summary of basketball referees' game activities, fitness assessment, cognitive function, and energy expenditure (EE) during official games.Methods: Analyzing the literature on basketball referees. Literature review conducted using Pub Med, WOK, SCOPUS, and Google Scholar, with search criteria including the terms 'referees', 'game officials', 'basketball' and performance terms such as 'physiological', 'physical demands', 'decision-making', and 'mental fatigue'.Results: the relevant studies (n = 19) showed that referees cover ~5000 m during a game with mean heart rates (HR) of ~140 beats.min-1 (>75% HRmax) and oxygen uptake (VO2) of ~52 ml.min-1.kg-1 (~86.19% VO2max). The YO-YO intermittent recovery test level 1 (YO-YO-IRT1) performance has revealed a direct validity to assess the intermittent aerobic activity as it was correlated to the on-field activity of the referees. The estimated referees' EE during a game exceeds 500 kcal (~5 kcal.kg-1.h-1). The threshold of low intensity and high intensity could be considered as lower than 3.8 METs and higher than 9.8 METs, respectively. The physical abilities of referees decrease gradually with the chronological age. However, older referees (>32 years old) have a significantly better quality of officiating than young referees. The perceptual-cognitive demands of basketball refereeing are also discussed in the present review.Conclusion: Basketball referees' game-analysis showed an intermittent activity (~1:12 ratio of high-intensity effort/recovery). The YO-YO-IRT1 could be used as a specific fitness test to estimate their aerobic capacity. The referees' EE showed a "moderate energy intensity" when expressed by the metabolic equivalent (~5 METs). A specific training program should be prescribed for the referees, taking into consideration their age. The cognitive processes influencing basketball referees' decisions should be studied in further researches.

PURPOSE: To assess postmatch perceived exertion, feeling, and wellness according to the match outcome (winning, drawing, or losing) in professional soccer players. METHODS: In total, 12 outfield players were followed during 52 official matches where the outcomes (win, draw, or lose) were noted. Following each match, players completed both a 10-point Borg scale modified by Foster and an 11-point Hardy and Rejeski scale rating of perceived feeling. Rating of perceived sleep quality, stress, fatigue, and muscle soreness was collected separately on a 7-point scale the day following each match. RESULTS: Player rating of perceived exertion was higher by a very large magnitude following a loss compared with a draw or a win and higher by a small magnitude after a draw compared with a win. Players felt more pleasure after a win compared with a draw or loss and more displeasure after a loss compared with draw. The players reported a largely and moderately better perceived sleep quality, less stress, and fatigue following a win compared with a draw or a loss and a moderately bad perceived sleep quality, higher stress, and fatigue following a draw compared with a loss. In contrast, only a trivial-small change was observed in perceived muscle soreness between all outcomes. CONCLUSION: Match outcomes moderately to largely affect rating of perceived exertion, feeling, sleep quality, stress, and fatigue, whereas perceived muscle soreness remains high regardless of the match outcome. However, winning a match decreases the strain and improves both pleasure and wellness in professional soccer players.

We studied the relationship between daily training load (TL) experienced by professional soccer players and the Hooper questionnaire reflecting their perceived quality of sleep, fatigue, stress and delayed onset muscle soreness. During a 16-week training period, the rating of perceived exertion and duration were collected after each training session, and daily TL was calculated from 14 professional soccer players. The Hooper questionnaire was completed every day before the first training session and the Hooper's score (HS) was then calculated. The daily TL and HS were 379.9 ± 198.3 AU and 16.2 ± 5.1, respectively. Pearson correlation showed significant relationships (p < 0.01) between TL and perceived fatigue, muscle soreness, sleep and stress. Our findings revealed that the perceived sleep, stress, fatigue and muscle soreness are moderately related to the daily TL in professional soccer players. The Hooper questionnaire is both a simple and useful tool for monitoring perceived wellness and psychometric players' status of professional soccer players.

Perceived changes due to training monotony, strain, sleep, stress, fatigue, muscle soreness and the influence of specific training sessions on the affective valence were explored in professional soccer players. Seventeen players completed the Hooper questionnaire, the ratings of perceived exertion and feeling scale (FS) every training/match day before and during the soccer season. Higher players' training loads were recorded during pre-season when compared with in-season period (2558.1 ± 262.4 vs. 1642.8 ± 169.3 a.u., p < 0.01; respectively). The ratings of sleep, stress, fatigue and muscle soreness in pre-season were higher than those observed during in-season (p < 0.01) whereas the feeling score was lower (p < 0.01). Furthermore, training sessions, including technical/tactical work, induced an improved feeling score but linked with a lower training load when compared with sessions focus on physical emphasis (p < 0.01). Pre-season period of training induces a significantly more strenuous and exhausting demands on professional soccer players compared with the in-season period at the elite level.

This study aimed to examine: (i) the effect of decreasing training load (TL) during taper weeks on physical match activities in professional soccer players, and (ii) to disclose the relationship between weekly TL and physical match activities. Rating of perceived exertion was collected after each training session and match to quantify the TL in 19 professional players over 17 standard and 7 taper weeks during the season. Physical match activities were quantified by a computerised match analysis system and compared between standard training and taper weeks. Compared to standard weeks, the duration and frequency of training sessions during the taper weeks decreased (-21.7% and -18.8%, respectively; P < 0.01) with no change in intensity (-4.8%; P = 0.09). Consequently, the weekly TL decreased during the taper weeks (-25.5%; P < 0.01). Increases in distance covered by intense running (+15.1%; P < 0.05), high-intensity running (HIR) (+15.7%; P < 0.01), number of sprints (+17.8%; P < 0.05) and number of high-speed runs (+15.7%; P < 0.05) were observed during the seven matches played after the taper weeks. High relationships were observed between TL and HIR distance covered, number of HIR and number of sprints (r = -0.53; r = -0.55; r = -0.65, respectively; P < 0.01). Decreasing TL during taper weeks by reducing training duration and frequency but maintaining intensity was associated with an increase in physical activities during matches. However, it needs to be determined whether tapering or other match factors led to the changes in match activity.
