BACKGROUND: There has been a growing amount of evidence indicating that excess visceral fat is associated with alterations in brain structure and function, including brain cortical thinning in adults. OBJECTIVES: This study aims to investigate the relationship between brain cortical thickness with obesity assessments, in adolescents. METHODS: In this study, we measured three different obesity assessments within an adolescent population (aged 15 - 18 years): body mass index (BMI), visceral fat ratio measured with an MRI and hepatorenal gradient measured with an ultrasound. Volunteers also underwent an MRI scan to measure brain structure. RESULTS: Results indicated that there was no relationship of BMI or hepatorenal gradient with brain cortical dimensions. However, there was a significant association between visceral fat ratio and an increase of cortical thickness throughout the brain. CONCLUSIONS: These results suggest that visceral fat, but not BMI, is correlated with cortical thickening in adolescence.

OBJECTIVES: To determine the prevalence of bifid mandibular condyle (BMC) detected on panoramic radiographs in a Brazilian population. METHODS: The panoramic radiographs of 50 080 radiographs taken from 1999 to 2006 were retrospectively reviewed in order to identify BMC cases. RESULTS: 9 (0.018%) of the 50 080 patients, 2 men and 7 women, with age ranging from 20 years to 74 years, had BMC. These conditions were unilateral in 7 (78%) subjects and bilateral in 2 (22%) subjects. All patients denied a history of any previous trauma or fracture of the mandible, pain, or trismus. CONCLUSIONS: BMC is an uncommon finding in a group of Brazilian subjects and, although it is rare, dentists should be aware of this condition on routine panoramic radiographs and its implications for function and appropriate treatment modalities.

The objective of the study was to assess knowledge and attitudes about human papillomavirus (HPV), cervical cancer, and Papanicolaou (Pap) smears among young women. A questionnaire was administered to 204 women aged 16-23 years, attending a public clinic. Data were gathered on sociodemographic characteristics, knowledge, and attitudes related to HPV. Overall, 92% of women reported current/previous sexual activity, 42% perceived themselves at high risk of acquiring a sexually transmitted disease, 67% did not know that HPV can cause cervical cancer/warts, and only 10% acknowledged that HPV might lead to cervical cancer. In general, women had a poor knowledge on HPV diagnosis/treatment, condyloma signs, and Pap smear test. The main reasons for not having a Pap smear test done before were embarrassment (63%) and fear of pain (61%). Knowledge of HPV infection and cervical cancer was low in this urban young population. Our findings recommend for greater HPV education of the public and health care practitioners.

Possible immunologic interaction between infection with human T lymphotropic virus type 1 (HTLV-1), a retrovirus, and the intestinal parasite Strongyloides stercoralis was investigated in persons infected with one or both agents. This was done by examining the cytokine responses of peripheral blood mononuclear cells (PBMC) to mitogens and Strongyloides antigen. PBMC of subjects infected with HTLV-1 spontaneously produced interferon (IFN)-gamma with levels that correlated inversely with serum IgE levels. HTLV-1-infected subjects also had poor interleukin (IL)-4 responses to mitogenic stimulation, unlike persons without HTLV-1 infection. It is postulated that the IFN-gamma produced by activated T cells in some HTLV-1-infected persons acts to down-regulate IL-4 with consequent reduction of serum IgE levels. The impaired IgE responses and other effects of IL-4 down-regulation may be contributing factors to more severe disease and impaired response to treatment of strongyloidiasis in some HTLV-1-infected persons.

RESUMO

The authors discuss an uncommon case in the literature of a carcinoid tumor of in the bifurcation of common hepatic duct. The tumor was surgically resected and the duct was reconstructed by Y of Roux's technic. The patient had a good evolution and no tumoral lesion was found in post operative exams during the last six months. The authors present a literature review and discuss diagnosis, treatment and prognosis.

RESUMO

Com o objetivo de aumentar a irrigaçäo sanguínea cerebral e do membro superior direito, foi rrealizada uma derivaçäo fêmoro-axilar direita em uma paciente portadora de oclusöes e estenoses dos ramos supra-aórticos outras doenças associadas que näo permitiam a toracotomia. Embora os resultados näo tenham sido plenamente satisfatórios, este procedimento mostrou-se hemodinamicamente viável e uma alternativa válida para o tratamento da insuficiência vascular cerebral e de membros superiores, em casos eecíficos.

RESUMO

Angiographic studies were performed on 60 of 394 patients diagnosed as having thoracic outlet syndrome. Ten of the patients studied angiographically presented with obstruction of contrast material in the axilla with refilling of the axillary artery through the external mammary and shoulder arteries. The obstruction was caused by lower trunk brachial plexus compression. This seldom known cause of hyperabduction syndrome occurred in 2.5 per cent of the patients examined for thoracic outlet syndrome. Surgical treatment consists of loosening the axillary artery by tying off the external mammary and circumflex arteries, followed by section and anastomosis of the axillary artery anteriorly to the brachial plexus. Angiography is considered a basic diagnostic procedure, mainly when concomitant compression occurs and gives good dynamic information on the location and importance of the compression.

From June 1980 to June 1985, 42 patients were subjected to patch graft angioplasties using glutaraldehyde tanned bovine pericardium. Life-table analysis showed a 100% patency in the grafted areas. Histological studies made on specimens recovered at 7 and 19 months showed no major deterioration of the original structure. Fibrous cell infiltration was not extensive enough to be harmful. Elastic fibers were relatively preserved, with some dissociation. No endothelialization, infection, thrombosis, or aneurysm dilatation was detected. Angiographic control up to 37 months showed patency and normal internal surface of the graft. It appears from this study that bovine pericardium is a good alternative as a vascular graft material.
