BACKGROUND: The aim of this study is to evaluate the stress distributions of a primary molar tooth restored with a stainless steel crown (SSC) using resin and glass ionomer luting cements by Finite Element Analysis (FEA). METHODS: Original DICOM data of a primary molar was used to create a 3D model. One model was prepared as a tooth model with SSC. A 30 µm cement layer was used in model. Two different luting cements were tested in the study: self-cure adhesive resin cement, and glass ionomer cement. Vertical and oblique loads of 330 N were applied to simulate maximum bite force and lateral forces in the occlusal contact areas of the models. Maximum von Mises stress values in the models were evaluated as MPa. RESULTS: The maximum von Mises stress value was observed in the force application and general occlusal contact areas for all models. The maximum von Mises stress values were higher in the tooth model with SSC using self-cure adhesive resin cement (478.09 MPa and 214.62 MPa) than in the tooth model with SSC using glass ionomer cement (220.06 MPa and 198.72 MPa) in both vertical and oblique loading, respectively. CONCLUSIONS: Depending on the magnitude of the bite force on the SSC, fracture of the luting cement materials could occur if the stress exceeds the endurance limit of the luting cement. Cementation with glass ionomer cement may help to reduce stress levels in SSC restorations of primary molars in children.

RESUMO

We report an elderly woman with vascular risk factors and recurrent cardioembolic strokes in whom the stroke aetiology was finally ascertained to be a calcified amorphous tumour of the heart after repeated negative investigations for embolic aetiology over 2 years. This report discusses the clinical and imaging characteristics of calcified amorphous tumours of the heart with emphasis of recent advances in cardiac imaging.

The aim of this study is to evaluate the stress distributions of low and high viscosity bulk-fill composite resins at class II MOD inlay cavity in primary molar tooth using Finite Element Analysis (FEA). Original DICOM data of a primary molar tooth from a research archive was used to create a 3D model. Two models were prepared as Model 1: the tooth model without restoration (control group) and Model 2: the tooth model with class II MOD inlay restoration. Two different bulk-fill composite resins were tested in study: Model 2 A (class II MOD inlay cavity model restored with low viscosity bulk-fill composite resin) and Model 2B (class II MOD inlay cavity model restored with high viscosity bulk-fill composite resin). Occlusal vertical loading of 232 N was applied to the teeth in occlusal contact areas. Maximum Von Mises stress values in the models for enamel, dentin, and restorative material were evaluated as MPa. More intense stress accumulation is observed in enamel than in dentin. In addition, more stress values were determined in Model 2B (206.15 MPa, 32.76 MPa, 128.95 MPa) than in Model 2 A (203.39 MPa, 29.77 MPa, 120.61 MPa) for enamel, dentin and restorative material, respectively.

In this work, we introduce LiNi0.8Mn0.15Al0.05O2 (NMA), which is cobalt-free and has a high nickel content, and a conductive composite material to NMA by supporting it with a three-dimensional (3D) graphene aerogel (GA). With an easy freeze-drying approach, NMA nanoparticles are properly dispersed on graphene sheets, and GA creates a strong and conductive framework, significantly improving the structure and conductivity. The structure of the pure NMA and NMA/graphene aerogel (NMA/GA) composite was investigated by X-ray diffraction (XRD) and field emission scanning electron microscopy (FE-SEM). XRD and FE-SEM analyses clearly indicated that ultrapure NMA structures are homogeneously dispersed among the GAs. In addition, the composite structure was examined using transmission electron microscopy (TEM) to determine the dispersion mechanisms. The electrochemical cycling performance of the pure NMA and NMA/GA composite was evaluated by rate capacitance, cyclic voltammetry (CV), and electrochemical impedance spectroscopy (EIS). The synthesized NMA/GA was able to provide 89.81% specific capacity retention after the 500th cycle at C/2. The average charge/discharge rates of the obtained cathode show good electrochemical results and exhibit capacities of 190.2,186.3, 185.2, 176.2, 161.2,142.6, and 188.5 mAh g-1 at C/20, C/10, C/5, C, 3C, 5C, and C/20, respectively. EIS data showed an improvement in the impedance of the composite containing GA. According to the results of the electrochemical tests, NMA nanoparticles formed a conductive network with its porous structure thanks to GA, formed a protective layer on the surface, prevented the side reactions between the cathode and the electrolyte, decreased the impedance of the cathode, and increased the redox kinetics. In addition, the changes in the structure were investigated in the NMA/GA composite cathode by XRD, FE-SEM, and Raman analyses at the end of the 50th, 250th, and 500th cycles. In summary, the NMA/GA cathode is expected to play an important role in lithium-ion batteries (LIBs) by taking advantage of its easy synthesis and excellent cycle stability.

INTRODUCTION: Nocturnal pain is a common complaint in rotator cuff related shoulder pain (RCRSP), and there is no study in literature that has evaluated the relationship between nocturnal pain severity and other evaluation parameters in RCRSP. The objective of the study was to investigate the relationship between the severity of nighttime pain and demographic and clinical data including physical examination findings, magnetic resonance imaging (MRI) findings, disability, kinesiophobia, and quality of life scores in patients diagnosed with RCRSP. MATERIALS AND METHODS: We assessed 61 patients (52.4% female) using the Visual Analog Scale (VAS) for nighttime and daytime, Shoulder Pain and Disability Questionnaire (SPADI), Tampa Scale of Kinesiophobia (TSK), and Short Form-36 (SF-36). Demographic and clinical data were recorded and rotator cuff specific examinations were performed and documented. RCRSP lesions were evaluated in terms of tendonitis and rupture by MRI. RESULTS: The nighttime VAS score was positively correlated with SPADI disability and total scores, and daytime VAS score, and negatively correlated with SF-36 physical function subscale. The nighttime VAS score was associated with presence of calcific tendonitis in supraspinatus, infraspinatus tendinosis, and subscapularis tendinosis. On multivariate analysis with a linear regression model, presence of subscapularis tendinosis was an independent predictor of nighttime VAS score. In shoulder clinical tests, the nighttime VAS score correlated with only Neer test positivity. CONCLUSIONS: Our results suggest that there is a relationship between nocturnal pain severity and disability. Presence of subscapularis tendinosis appears to be a predictor of nocturnal pain severity. Additionally, patients with positive Neer test may experience more nocturnal pain.

PURPOSE: The neurogenic bladder symptom score (NBSS) represents the effect of disease-specific symptoms on the quality of life in patients with neurogenic bladder (NB). The purpose of this study was to assess the psychometric properties of the Turkish version of the NBSS in spinal cord injury (SCI) and multiple sclerosis (MS) patients. METHODS: The cross cultural adaptation was achieved through forward and back translation of the items of the original version by an expert committee. Face and content validity were evaluated in a prepatient group. Internal consistency and test-retest reliability were used for reliability assessment in 102 patients. Validity was examined using Short Form-12 and King's Health Questionnaire. RESULTS: Eighty-four patients with SCI and 18 with MS were included in the study. Cronbach's alpha values for the total score and the incontinence, storage/voiding, and consequences domains were 0.90, 0.91, 0.81, and 0.63, respectively. The intraclass correlation coefficients were >0.80 for all subdomains and the overall score. The correlation analyses indicated that the Turkish version has good construct validity. CONCLUSIONS: Our results showed that the Turkish version of the NBSS has good reliability and validity in patients with SCI and MS and can be used to evaluate NB symptoms. PURPOSE: To compare muller muscle conjunctival resection (MMCR) and external levator advancement (ELA) in terms of objective and subjective symmetry in unilateral ptosis. METHODS: The patients who underwent unilateral MMCR (group1, n = 25) and ELA (group2, n = 25) were reviewed retrospectively. With the written algorithm, margin reflex distance-1 (MRD1), localization of the contour peak, and the percentage of overlapping curvatures (POC) of both upper eyelid curves were calculated. Semi-automated measurements were compared with the manual measurements. The correlation between objective and subjective symmetry was evaluated. RESULTS: Preoperative levator functions were similar in both groups (13.93 ± 3.25 mm and 13.3 ± 2.86 mm, respectively). We found strong correlations between manual and semi-automated measurements (ICC: 0.942, 95% CI = 0.924-0.956, p < 0.001). Preoperative MRD1 (2.15 ± 0.90 vs 1.51 ± 1.01, p = 0.022) and POC was lower in group 2 (66% vs 47.2%, p = 0.01). In the postoperative period, the increase in MRD1 and POC were similar in both groups (p = 0.2 and p = 0.7 respectively), and the contour peak displaced temporally (p < 0.01) providing a symmetric peak in both groups. Subjective symmetry increased as the difference in MRD1 between two eyes decreased (r = -0.456, p = 0.001), and POC increased (r = 0.396, p = 0.004). CONCLUSIONS: Besides subjective symmetry, MMCR and ELA are equally effective in ensuring symmetry objectively, as measured with MRD1 and contour peak symmetry and POC in unilateral ptosis. POC may be used as a criterion of symmetry in the evaluation of ptosis.

BACKGROUND/AIMS: Today, one of the ways to access medical information is the internet. Our objective was to develop a measurement tool to assess the quality of online medical videos. METHODS: Online videos covering a variety of subjects (COVID-19, low back pain, weight loss, hypertension, cancer, chest pain, vaccination, asthma, allergy, and cataracts) were evaluated using our Medical Quality Video Evaluation Tool (MQ-VET) by 25 medical and 25 non-medical professionals. Exploratory factor analysis, Cronbach's alpha, and correlation coefficients were used to assess the validity and reliability of the MQ-VET. RESULTS: The final MQ-VET consisted of 15 items and four sections. The Cronbach's alpha reliability coefficient for the full MQ-VET was 0.72, and the internal consistency for all factors was good (between 0.73 and 0.81). The correlation between the DISCERN questionnaire scores and MQ-VET scores was significant. CONCLUSION: Collectively, our findings indicated that the MQ-VET is a valid and reliable tool that will help to standardize future evaluations of online medical videos.

OBJECTIVE: To determine ergonomic problems when working from home during the COVID-19 pandemic with regard to musculoskeletal pain, sleep conditions, physical activity, resting, equipment, and productivity. METHODS: In this cross-sectional study, office workers who switched to working from home during the COVID-19 pandemic participated in an online survey (n = 194). The data were interpreted using descriptive and multivariate regression analysis. RESULTS: A significant increase in back pain was revealed (P < 0.001). Significant weight gain was observed (P < 0.001) connected to a decrease in physical activity and an increase in the consumption of junk food. Despite the negative health impact, participants self-reported an increase in productivity. CONCLUSION: The self-reported productivity was higher during the working from home, but a declining trend in employee health can be observed including low back pain and weight gain.

OBJECTIVES: This study aimed to test the hypothesis that fibromyalgia patients feel older than their actual age and to investigate the associations between their subjective age and clinical parameters such as cognition, depression, anxiety, widespread pain, sleep, and fatigue. METHODS: This observational cross-sectional study enrolled 176 patients with newly diagnosed fibromyalgia and 89 controls. Subjective age was determined by asking the question "how old do you feel?", and the difference between the physiological and subjective ages was calculated. Depression, anxiety, fatigue, sleep cognition, and widespread pain levels in the subjects were evaluated, and multivariate stepwise regression analysis was used to determine the factors explaining the variation in the difference between actual and subjective age. RESULTS: Of the fibromyalgia patients, 75% felt older than their actual age, whereas 45% of the controls felt younger. Regression analysis revealed that depression, widespread pain, and fatigue explained nearly half of the variation in the subjective age and the difference between actual and subjective age. CONCLUSIONS: Fibromyalgia patients feel older than their actual age, and this subjective age is associated with depression, widespread pain, and fatigue. Further studies should investigate usage of subjective age perception in differential diagnosis of fibromyalgia.

The effects of caregiver burden during the inpatient rehabilitation period have not yet been investigated. The purpose of this study was to evaluate the burden on stroke survivors' caregivers during the inpatient rehabilitation period, and to compare the associations of robot-assisted gait training and conventional therapy with caregiver burden. Our randomized, crossover, prospective study included 63 stroke survivors and their caregivers, who were randomly assigned to one of two groups. The patients in group I received robot-assisted gait training for 2 weeks, followed by conventional therapy for a further 2 weeks. The patients in group II received conventional therapy for 2 weeks followed by robot-assisted gait training for a further 2 weeks. The caregiver burden inventory, beck depression index, and beck hopelessness scale were administered to the caregivers at day 0, on the "switch day" (day 15), and day 30. Before inpatient rehabilitation, 18 (35%) of the caregivers had somewhat elevated scores on the caregiver burden inventory; however, at the end of rehabilitation, 42 (66.6%) of the caregivers were in a high-burden state. The caregiver burden inventory scores differed significantly between baseline and the end of rehabilitation in both groups. Caregiver depression scores also increased significantly in both groups (p < 0.0001), while hopelessness scores increased only in group II (p = 0.027). Caregiver burden increased during the inpatient stroke rehabilitation period. The rapidly spreading COVID-19 pandemic has affected many people worldwide. Due to the high infectivity, countries make calls to stay at home or take measures such as lockdowns to ensure that people are least affected by the virus. Meanwhile, infected people are getting treatments: people who are slightly affected are quarantined at home, and those who are heavily affected are treated in hospitals. Hence there is an excessive increase in the hospital workload. This causes physical fatigue in healthcare professionals. Along with the increasing workload, the fear of being infected and infecting the environment causes psychological problems in healthcare professionals. It is important to protect healthcare professionals and provide them with suitable working conditions. For this reason, besides the provision of protective equipment such as gloves, overalls, mask, and glasses that are necessary for the protection of healthcare workers from the virus, healthcare services should also be planned very carefully. One of the critical issues is planning the shift schedules of the physicians. In this study, we handle the preparation of a physician shift schedule of a hospital in Turkey during the COVID-19 pandemic. The hospital has established three new COVID-19 related departments and the aim is to provide continuous service in the new departments while maintaining the workload in the existing departments. We propose a mixed integer programming (MIP) model to address the shift scheduling problem and transform it into a decision support system (DSS). The resulting schedules minimize the exposure of the physicians to the virus with a balanced workload while maintaining the healthcare service in all departments.

BACKGROUND: Zirconia has become a popular biomaterial in dental implant systems because of its biocompatible and aesthetic properties. However, this material is more fragile than titanium so its use is limited. OBJECTIVES: The aim of this study was to compare the stresses on morse taper implant systems under parafunctional loading in different abutment materials using three-dimensional finite element analysis (3D FEA). METHODS: Four different variations were modelled. The models were created according to abutment materials (zirconia or titanium) and loading (1000 MPa vertical or oblique on abutments). The placement of the implants (diameter, 5.0 × 15 mm) were mandibular right first molar. RESULTS: In zirconia abutment models, von Mises stress (VMS) values of implants and abutments were decreased. Maximum and minimum principal stresses and VMS values increased in oblique loading. VMS values were highest in the connection level of the conical abutments in all models. CONCLUSIONS: Using conical zirconia abutments decreases von Mises stress values in abutments and implants. However, these values may exceed the pathological limits in bruxism patients. Therefore, microfractures may be related to the level of the abutment.

The objective of this study was to evaluate the effect of embryo quality and developmental stages on pregnancy rate in beef heifer recipients. The present study used 168 Simmental breed cows as donors, and 618 beef cattle breed heifers as recipients. The quality and developmental stages of the collected embryos were evaluated according to the criteria specified by the International Embryo Technology Society. Accordingly, the embryos in the compact morula, early blastocyst, blastocyst, and expanded blastocyst stages that were of Code I (excellent) and Code II (good) quality levels were transferred as fresh embryos to the recipient heifers. Prior to the transfer, the recipients were synchronized using the Ovsynch protocol, and the embryos obtained were transferred to 618 beef heifers. Pregnancy examinations were performed on days 30 and 60. On day 30, the pregnancy rates with Code I and Code II embryos were determined as 44.15% and 32.58%, respectively. According to the developmental stages, the pregnancy rates with Code I quality compact morula, early blastocyst, blastocyst, and expanded blastocyst were determined as 44.64%, 45.67%, 45.83%, and 33.33%, respectively. The rates of pregnancy with Code II quality compact morula, early blastocyst, and blastocyst were determined as 32.03%, 32.14%, and 50.0%, respectively. In conclusion, the pregnancy rates with Code I quality embryos were found to be higher compared with Code II embryos (P < 0.05). It was also determined that the embryonic developmental stages had no effect on the pregnancy rate (P > 0.05).

The aim of this study was to evaluate the effects of the transfer side, transfer location, cervix transfer score, type and diameter of corpus luteum (CL) during embryo transfer on pregnancy rates in beef heifers. Progesterone-based synchronization and superovulation protocol were applied to Simmental cows used as donors (n = 168). Uterine flushings were performed on day 7 following artificial insemination. Obtained Code I (excellent or good) and II (fair) quality embryos were transferred to recipient beef heifers (n = 561). During embryo transfer, side of transfer (right or left), transfer location (the cranial or middle third of uterine horn), cervix transfer score (easy, moderate or difficult) and type (CLa, CLb and CLc) and diameter of CL were determined. Pregnancy rates following the transfer of Code I and II embryos were 44.66% and 33.07%, respectively (p < .05). The rates of pregnancy after transfers to the right and left uterine horn were 37% and 42.2%, respectively (p > .05). The pregnancy rates were 41.2%, 34.9% and 30.3% for cervix transfer scores as easy, moderate and difficult, respectively (p > .05). Pregnancy rates after transfer to the cranial third and middle third were 41.06% and 29.67%, respectively (p < .05). According to types of CL, pregnancy rates were 31.7%, 40.4% and 45.3% for CLa, CLb and CLc, respectively (p < .05). Moreover, it was found that as the CL diameter increased, the pregnancy rates increased. As a result, it was concluded that there was no effect of side of transfer and cervix transfer score, but embryo quality, transfer location, type and diameter of CL had significant effects on the pregnancy rate during embryo transfer in beef heifers.

OBJECTIVES: This study aims to investigate whether fibromyalgia (FM) and work-related musculoskeletal disorders can be differentiated in working females by comparing their pain on nonworking and working days. PATIENTS AND METHODS: The study included 142 female workers (mean age 30.0±6.5 years; range, 18 to 50 years) from five different work areas: 27 factory workers, 27 janitors, 25 data automation employees, 31 nurses, and 32 physiotherapists. Demographic characteristics were recorded. FM was diagnosed according to 2016 criteria of the American College of Rheumatology. The extended version of the Nordic Musculoskeletal Questionnaire was used to evaluate the participants' musculoskeletal complaints and the severity of their pain. Pain was assessed with visual analog scale (VAS) scores on working and nonworking days. Differences in the participants' VAS-Pain on working and nonworking days were compared. Sensitivity, specificity, receiver operating characteristic (ROC) curve and area under the curve (AUC) were used. RESULTS: Of the 142 working females, 32 (22.5%) were diagnosed with FM. There was a significant difference in nonworking day VAS-Pain scores between the FM patients and the work-related musculoskeletal disorder patients (p<0.001). Analysis of ROC curve for VAS-Pain difference scores yielded AUC of 0.860 (95% confidence interval=0.774-0.945) (p<0.001). ROC analysis identified 1.5 centimeters of VAS-Pain difference score as the cut-point for differentiating work-related musculoskeletal disorders and FM resulting in sensitivity of 97% and specificity of 96%. FM patients had significantly higher rates of positive answers to "visit health professionals," "take medication," and "sick leave" questions compared to patients with work-related musculoskeletal disorders (p<0.001). CONCLUSION: The amount of the decrease in pain on nonworking days may help differentiate work-related musculoskeletal disorders from FM in working females. Therefore, evaluating pain on nonworking days may help clinicians diagnose and treat FM correctly.

OBJECTIVE: This study explored the role of central sensitization (CS) pain in patients with various rheumatic diseases using the CS inventory (CSI). METHODS: A total of 193 patients of mean age 50.72 ± 9.65 years were included; they were divided into four different groups in terms of their rheumatic diseases. Patients with rheumatoid arthritis (RA), spondyloarthropathy (SpA), osteoarthritis (OA), and fibromyalgia syndrome (FMS) were evaluated in tertiary care rheumatology/pain medicine settings. Disease duration and activity, the Bath Ankylosing Spondylitis Disease Activity Index, the Disease Activity Score-28, and pain severity (evaluated using a visual analog scale) were assessed, and the Turkish version of the CSI administered. RESULTS: CS syndromes were present in almost half the patients (45% of SpA, 41% of RA, 62% of OA, and 94% of FMS patients). We found no significant relationship between disease activity and the CSI-A scores in SpA or RA patients (p = 0.731 and p = 0.390, respectively). As expected, the CSI-A scores were highest in the FMS group (p = 0.000), but were similar in the other groups (p < 0.05). CS-related syndromes (CSI-B conditions) were present at similar frequencies in the RA, SpA, and OA groups, but were less common in the FMS group (p = 0.000). CONCLUSIONS: The CSI usefully detects CS pain in patients with rheumatic diseases. Treatment of such pain can enhance the quality of daily life in patients with rheumatic diseases.Key Point• Central sensitization pain is common in patients with rheumatic diseases including rheumatoid arthritis, spondyloarthropathies, and osteoarthritis.

PURPOSE: Interleukin-1 beta (IL-1ß), a pro-inflammatory cytokine, has been shown to influence breast cancer susceptibility. The relationship between its risk of breast cancer and IL-1ß-C31T polymorphism has been demonstrated, but the results remain controversial. Therefore, our study aimed to investigate the correlation between the IL-1ß-C31T gene polymorphism and susceptibility to breast cancer. METHODS: The genotype frequencies of IL-1ß-C31T polymorphism were compared between 204 breast cancer cases and 210 controls using polymerase chain reaction and restriction fragment length polymorphism techinques. Further multivariate binary logistic regression analyses were used to assess the association between IL-1ß-C31T polymorphism and breast cancer risk. RESULTS: The frequency of the T allele of IL-1ß-C31T polymorphism in breast cancer cases was significantly higher than that in the controls (56.1% vs. 47.9%). The frequencies of genotypes CC, CT, and TT in the cases were 22.1%, 43.6%, and 34.3%, respectively, while in the control group they were 24.3%, 55.7%, and 20.0%, respectively. There was a significant difference between the prevalence of TT genotype in the 2 groups (adjusted odds ratio [OR], 2.06; 95% confidence interval [CI], 1.16-3.66; p = 0.014). Breast cancer risk increased in women with TT genotype, body mass index (BMI) ≥ 25 kg/m2 (OR, 2.19; 95% CI, 1.09-4.36), late age at first birth (OR, 2.43; 95% CI, 1.29-4.56), postmenopausal status (OR, 3.15; 95% CI, 1.39-7.16), and negative smoking history (OR, 2.52; 95% CI, 1.32-4.82). Furthermore, increase in breast cancer risk among women diagnosed with invasive ductal carcinoma was associated with CT/TT genotypes (OR, 2.82; 95% CI, 1.38-5.76). CONCLUSION: The IL-1ß-C31T polymorphism affects breast cancer susceptibility, especially in women with late age at first birth, high BMI, postmenopausal status, negative smoking history, and invasive ductal carcinoma. Our study adds to the evidence about the importance of IL-1ß-C31T polymorphism in breast cancer susceptibility.

The most significant focal points of the embryo transfer technology are as follows: the selection of donors, the response of the selected donor to the superovulation protocol and the obtained number of the transferable embryos. For this purpose, it is suggested that donor selection can be done by anti-Müllerian hormone (AMH) levels, and embryo production is evaluated. AMH is secreted by the granulosa cells of primordial, pre-antral and antral follicles below 4 mm in the ovary, independent of FSH. Therefore, the aim of this study was to investigate the relationship between serum AMH levels and the number of corpus luteum (CL), total embryos and transferable embryos that were shaped after a uniform superovulation protocol. For this reason, 48 Simmental cows, which were located at General Directory of Agricultural Enterprises (region, province, etc. instead of the general directorate), were used as donors for the embryo transfer. Blood samples were taken at random, regardless of the stage of animal's sexual cycle. AMH levels were measured by enzyme-linked fluorescent assay (ELFA) method of the miniVIDAS® (bioMérieux SA) using AMH Bovine Test Kit. According to the statistical analyses of the obtained data, AMH levels were positively correlated with CL and total embryos (p < .05). No significant correlations between AMH and transferable embryos were approved (p > .05). It was also determined that each 200 pg/ml increase in serum AMH level resulted in one increase in CL number. Overall, considering the positive correlation between AMH level and the obtained number of CL and total embryos after a superovulation treatment, it was concluded that measuring blood AMH level prior to any further costly implementation may be an effective method in donor selection.
