Anesthesia with propofol is frequently associated with hypotension. The TRPA1 gene contributes to the vasodilator effect of propofol. Hypotension is crucial for anesthesiologists because it is deleterious in the perioperative period. We tested whether the TRPA1 gene polymorphisms or haplotypes interfere with the hypotensive responses to propofol. PCR-determined genotypes and haplotype frequencies were estimated. Nitrite, nitrates, and NOx levels were measured. Propofol induced a more expressive lowering of the blood pressure (BP) without changing nitrite or nitrate levels in patients carrying CG+GG genotypes for the rs16937976 TRPA1 polymorphism and AG+AA genotypes for the rs13218757 TRPA1 polymorphism. The CGA haplotype presented the most remarkable drop in BP. Heart rate values were not impacted. The present exploratory analysis suggests that TRPA1 genotypes and haplotypes influence the hypotensive responses to propofol. The mechanisms involved are probably other than those related to NO bioavailability. With better genetic knowledge, planning anesthesia with fewer side effects may be possible.

Introduction  Opioids are the mainstay of pain management in critically ill patients. However, recent attention to their adverse effects in the intensive care unit (ICU) has led to the use of strategies that aim to reduce these side effects. Among these strategies, there are multimodal analgesia protocols, which prioritize pain management and employ a combination of different analgesics to spare excessive doses of opioids and sedatives in continuous infusion. Objective The objective of this study is to evaluate the impact of a multimodal analgesia protocol on clinical outcomes and consumption of sedatives and analgesics in two intensive care units. Methods  We conducted a single-center, quasi-experimental, retrospective, and prospective cohort study comparing clinical outcomes and consumption of sedatives and analgesics before and after the implementation of a multimodal pain management protocol in critically ill adult patients. We included 465 patients in 2017 (pre-intervention group) and 1508 between 2018 and 2020 (post-intervention group). Results In the analysis of the primary outcome, there was a significant reduction in mortality between 2017 and 2020 (27.7% - 21.7%, p=0.0134). There was no statistical difference in mechanical ventilation time or concerning the infection rate. Patients who received the multimodal analgesia protocol had a decrease of 24% regarding mean fentanyl intake and a progressive reduction in morphine milligram equivalents (MME) (8.4% - 19%). There was an increasing trend in the use of adjuvant analgesics and morphine in preemptive and therapeutic analgesia. Conclusion The implementation of a multimodal pain control protocol significantly reduced morbidity and mortality and the use of opioids in the ICU.

Melanin is the basis of coloration in many animals, and although it is often used in communication, thermoregulation, or camouflage, melanin has many other physiological functions. For example, in polluted habitats, melanin can have a detoxifying function. Melanic coloration would help to sequester in the skin the heavy metal contaminants from inside the body, which will be expelled to the exterior when the skin is sloughed. Moreover, animals should have evolved more melanic colorations in more polluted habitats ("industrial melanism" hypothesis). We examined whether the fossorial amphisbaenian reptile, Trogonophis wiegmanni, is able to eliminate heavy metals, derived from soil pollution by seagull depositions, through sloughing its skin. Our results suggest a covariation between levels of soil pollution by heavy metals and the concentration of heavy metals in the sloughed skins of amphisbaenians. This suggests that amphisbaenians may expel heavy metals from their bodies when they slough the skins. We also tested whether amphisbaenians inhabiting soils with higher levels of heavy metal pollution had darker (melanin-dependent) body colorations. However, contrary to predictions from the "industrial melanization" hypothesis, we found a negative relationship between soil pollution and proportions of melanic coloration. This contradictory result could, however, be explained because heavy metals have endocrine disruption effects that increase physiological stress, and higher stress levels could result in decreased melanogenesis. We suggest that although amphisbaenians might have some detoxifying mechanism linked to melanin in the skin, this process might be negatively affected by stress and result ineffective under conditions of high soil pollution.

Anaesthesia with propofol is frequently associated with hypotension, which is at least partially attributable to increased nitric oxide (NO) formation derived from the activation of protein kinase C (PKC)/endothelial NO synthase (NOS3) axis. In this cross-sectional study, we tested whether PRKCA (which encodes PKCα) polymorphisms, or haplotypes, and interactions among PRKCA and NOS3 polymorphisms affect the hypotensive responses to propofol. We collected venous blood samples from 164 patients before and 10 min after propofol administration. Genotypes were determined by PCR and haplotype frequencies were estimated. Nitrite and NOx (nitrites+nitrates) levels were measured by using an ozone-based chemiluminescence assay and the Griess reaction, respectively. We used multifactor dimensionality reduction to test interactions among PRKCA and NOS3 polymorphisms. Propofol promoted enhanced blood pressure-lowering effects and increased nitrite levels in subjects carrying GA + AA genotypes for the rs16960228 and TC + CC genotypes for the rs1010544 PRKCA polymorphisms, and the CCG haplotype. Moreover, genotypes for the rs1010544 PRKCA polymorphism were associated with higher or lower blood pressure decreases in response to propofol depending on the genotypes for the rs2070744 NOS3 polymorphism. Our findings suggest that PRKCA genotypes and haplotypes impact the hypotensive responses to propofol, possibly by modifying NO bioavailability, and that PRKCA-NOS3 interactions modify the blood pressure-lowering effects of propofol.

Soil degradation may have strong negative consequences for soil biodiversity, but these potential effects are understudied and poorly understood. Concentration of nesting seabirds may be a source of soil pollution by heavy metals, which are incorporated into the food chain and may have toxicological effects in vertebrates, especially in fossorial animals with low dispersal ability. We examined whether contamination by heavy metals, derived from seagull depositions, and other soil characteristics, may affect the levels of faecal glucocorticoid metabolites (as a potential indicator of physiological stress) of the fossorial amphisbaenian reptile Trogonophis wiegmanni. We found a relationship between soil pollution by heavy metals and increased levels of faecal corticosterone metabolite of the amphisbaenians that live buried in those soils. This can be due to the strong endocrine disruption effect of heavy metals. In addition, there was an independent effect of the soil texture, with amphisbaenians showing higher levels of faecal corticosterone metabolite in soils with less sand and more silt and clay, which are more energetically costly to dig. Long-term exposure to high glucocorticoid levels might have serious effects on health state and fitness of fossorial animals that may be unnoticed. Our study emphasizes that, to prevent future conservation problems, we need to perform periodic surveys on the physiological health state of the little-known subterranean biodiversity.

We analyse the paper "The spread of SARS-CoV-2 in Spain: Hygiene habits, sociodemographic profile, mobility patterns and comorbidities" authored by Rodríguez-Barranco et al. (2021), published in Environmental Research, vol.192, January 2021. The study was carried out under challenging conditions and provides original data of great value for exploratory purposes. Nevertheless, we found that the authors have not considered the potential effect of the multiple hypothesis testing carried out until obtaining the final model on the increased occurrence of false discoveries by mere chance. After adjusting the results provided in the paper for the effects of multiple testing, we conclude that only one of the five factors cited as statistically significant and relevant in the article, living with someone who has suffered from COVID-19, remained significantly related to the relative prevalence of COVID-19. Therefore, the preeminent role given in the analysed work to walking the dog as one of the main transmission routes of COVID-19 probably does not correspond to an actual effect. Instead, until replicated by other studies, it should be considered a spurious discovery.

Interactions among plant species and their soil biota drive plant-soil feedbacks (PSFs) that play a major role in the dynamics and diversity of plant communities. Among the different components of the soil community, pathogens are considered to be the main drivers of negative PSFs. Despite this, the number of studies that have experimentally quantified the contribution of soil pathogens to PSFs remains considerably low. Here we conducted a greenhouse experiment with oomycete-specific fungicide to quantify the contribution of soil pathogens, and particularly oomycete pathogens, to individual and pairwise PSFs in forest communities. We used as a case study Mediterranean mixed forests dominated by Quercus suber and invaded by the oomycete pathogen Phytophthora cinnamomi. The fungicide treatment was crossed with a competition treatment to explore how conspecific neighbors might modify pathogen effects. To place the results of the experiment in a wider context, we also conducted a systematic review of published papers that explicitly used fungicide to explore the role of pathogens in PSF experiments. Our experimental results showed that oomycete pathogens were the main drivers of individual PSFs in the study forests. Oomycete effects varied among tree species according to their susceptibility to P. cinnamomi, driving negative PSFs in the highly susceptible Q. suber but not in the coexistent Olea europaea. Oomycete-driven PSFs were not modified by intraspecific competition. Oomycete pathogens were also major contributors to negative pairwise PSFs assumed to promote species coexistence. Results from the systematic review supported the novelty of our experimental results, since only three studies had previously used oomycete-specific fungicide in a PSF context and none in systems invaded by exotic oomycetes. Overall, our results provide novel evidence of oomycete pathogens (including the exotic P. cinnamomi) as fundamental drivers of negative individual and pairwise PSFs with implications for species coexistence in invaded communities. Although in the short-term invasive pathogens might contribute to species coexistence by causing self-limitation in dominant species, strong inter-specific variation in self-limitation might undermine coexistence in the long-term. Because of the increasing number of exotic oomycetes worldwide, further attention should be given to oomycetes as drivers of PSFs in plant communities.

This paper addresses the effects of atmospheric conditions on the spread of the SARS-CoV-2 coronavirus and its associated disease, COVID-19. For this purpose, we assess the limitations of bioclimatic correlative models to explain the geographic distribution of SARS-CoV-2 in the context of medical geography. Overall, there is a broad consensus that the global distribution of COVID-19 is not random but conditioned by environmental drivers. However, as the COVID-19 distribution becomes global, including tropical climates, the evidence reveals that atmospheric conditions explain, at most, only a limited amount of the space-time dynamics of SARS-CoV-2. Therefore, the usefulness of approaches based on bioclimatic envelopes is in question since the dominant route for the spread of COVID-19 seems to be the anthroposphere's non-stationary environment. In this sense, there is a need to clarify further the role of different transmission routes at multiple scales and outdoor and indoor environments beyond bioclimatic envelopes. At this time, the possible influence of the weather in COVID-19 spread is not sufficient to be taken into account in public health policies. Hence, until reliable bioclimatic envelopes of SARS-CoV-2, if any, are found, caution should be exercised when reporting, as this could have unforeseen consequences.

PURPOSE: Propofol anesthesia is usually accompanied by hypotensive responses, which are at least in part mediated by nitric oxide (NO). Arginase I (ARG1) and arginase II (ARG2) compete with NO synthases for their common substrate L-arginine, therefore influencing the NO formation. We examined here whether ARG1 and ARG2 genotypes and haplotypes affect the changes in blood pressure and NO bioavailability in response to propofol. METHODS: Venous blood samples were collected from 167 patients at baseline and after 10 min of anesthesia with propofol. Genotypes were determined by polymerase chain reaction. Nitrite concentrations were measured by using an ozone-based chemiluminescence assay, while NOx (nitrites + nitrates) levels were determined by using the Griess reaction. RESULTS: We found that patients carrying the AG + GG genotypes for the rs3742879 polymorphism in ARG2 gene and the ARG2 GC haplotype show lower increases in nitrite levels and lower decreases in blood pressure after propofol anesthesia. On the other hand, subjects carrying the variant genotypes for the rs10483801 polymorphism in ARG2 gene show more intense decreases in blood pressure (CA genotype) and/or higher increases in nitrite levels (CA and AA genotypes) in response to propofol. CONCLUSION: Our results suggest that ARG2 variants affect the hypotensive responses to propofol, possibly by modifying NO bioavailability. TRIAL REGISTRATION: NCT02442232.

Emergent diseases are an increasing problem in forests worldwide. Exotic pathogens are now threatening forests where pathogens have not traditionally been considered to be major ecological drivers of tree demography, such as water-limited Mediterranean forests. However, how pathogens might limit regeneration in invaded forests is largely unknown. Here we used fungicide to analyse the impact of soil-borne oomycete pathogens on seedling establishment at community level in Mediterranean forests invaded by the exotic oomycete Phytophthora cinnamomi. Fungicide effects were modelled as a function of the tree neighbourhood composition, the seed mass of the target species, and the abiotic environment. Fungicide application had positive effects on seedling performance that varied in magnitude and spatial structure among coexisting species. Seed mass predicted fungicide effects on seedling emergence, but not on survival or growth. Positive fungicide effects were modulated by levels of abiotic resources, mainly water, increasing with soil moisture. Our results support a novel role for soil-borne oomycete pathogens as one more axis of the regeneration niche of woody species in water-limited forests. Given the increasing numbers of exotic oomycete pathogens worldwide, more research is needed to understand the role of this relevant microbial group as a factor shaping seedling establishment.

Under physiological conditions, the perivascular adipose tissue (PVAT) negatively modulates vascular contractility. This property is lost in experimental and human obesity and in the metabolic syndrome, indicating that changes in PVAT function may contribute to vascular dysfunction associated with increased body weight and hyperglycemia. The O-linked ß-N-acetylglucosamine (O-GlcNAc) modification of proteins (O-GlcNAcylation) is a unique posttranslational process that integrates glucose metabolism with intracellular protein activity. Increased flux of glucose through the hexosamine biosynthetic pathway and the consequent increase in tissue-specific O-GlcNAc modification of proteins have been linked to multiple facets of vascular dysfunction in diabetes and other pathological conditions. We hypothesized that chronic consumption of glucose, a condition that progresses to metabolic syndrome, leads to increased O-GlcNAc modification of proteins in the PVAT, decreasing its anti-contractile effects. Therefore, the current study was devised to determine whether a high-sugar diet increases O-GlcNAcylation in the PVAT and how increased O-GlcNAc interferes with PVAT vasorelaxant function. To assess molecular mechanisms by which O-GlcNAc contributes to PVAT dysfunction, thoracic aortas surrounded by PVAT were isolated from Wistar rats fed either a control or high sugar diet, for 10 and 12 weeks. Rats chronically fed a high sugar diet exhibited metabolic syndrome features, increased O-GlcNAcylated-proteins in the PVAT and loss of PVAT anti-contractile effect. PVAT from high sugar diet-fed rats for 12 weeks exhibited decreased NO formation, reduced expression of endothelial nitric oxide synthase (eNOS) and increased O-GlcNAcylation of eNOS. High sugar diet also decreased OGA activity and increased superoxide anion generation in the PVAT. Visceral adipose tissue samples from hyperglycemic patients showed increased levels of O-GlcNAc-modified proteins, increased ROS generation and decreased OGA activity. These data indicate that O-GlcNAcylation contributes to metabolic syndrome-induced PVAT dysfunction and that O-GlcNAcylation of eNOS may be targeted in the development of novel therapies for vascular dysfunction in conditions associated with hyperglycemia.

Propofol anesthesia is usually accompanied by hypotension, which is at least in part related to enhanced endothelial nitric oxide synthase (NOS3)-derived NO bioavailability. We examined here whether NOS3 polymorphisms (rs2070744, 4b/4a VNTR, rs3918226 and rs1799983) and haplotypes affect the changes in blood pressure and NO bioavailability induced by propofol. Venous blood samples were collected from 168 patients at baseline and after 10 min of anesthesia with propofol 2 mg/kg administered intravenously by bolus injection. Genotypes were determined by polymerase chain reaction and haplotype frequencies were estimated. Nitrite concentrations were measured by using an ozone-based chemiluminescence assay, while NOx (nitrites + nitrates) levels were determined by using the Griess reaction. We found that CT + TT genotypes for the rs3918226 polymorphism, the ba + aa genotypes for the 4b/4a VNTR and the CTbT haplotype were associated with lower decreases in blood pressure and lower increases in nitrite levels after propofol anesthesia. On the other hand, the TCbT and CCbT haplotypes were associated with more intense decreases in blood pressure and higher increases in nitrite levels in response to propofol. Our results suggest that NOS3 polymorphisms and haplotypes influence the hypotensive responses to propofol, possibly by affecting NO bioavailability.

Propofol anesthesia is usually accompanied by hypotension. Studies have shown that the hypotensive effects of propofol increase in patients treated with angiotensin-converting enzyme inhibitors (ACEi). Given that both propofol and ACEi affect nitric oxide (NO) signaling, the present study tested the hypothesis that ACEi treatment induces pronounced hypotensive responses to propofol by increasing NO bioavailability. In this study we evaluated 65 patients, divided into three groups: hypertensive patients chronically treated with ACEi (HT-ACEi; n = 21), hypertensive patients treated with other antihypertensive drugs instead of ACEi, such as angiotensin II receptor blockers, ß-blockers or diuretics (HT; n = 21) and healthy normotensive subjects (NT; n = 23). Venous blood samples were collected at baseline and after 10min of anesthesia with propofol 2mg/kg administrated intravenously by bolus injection. Hemodynamic parameters were recorded at each blood sample collection. Nitrite levels were determined by using an ozone-based chemiluminescence assay, while NOx (nitrites+nitrates) levels were measured by using the Griess reaction. Additionally, experimental approaches were used to validate our clinical findings. Higher decreases in blood pressure after propofol anesthesia were observed in HT-ACEi group as compared with those found in NT and HT groups. Consistently, rats treated with the ACEi enalapril showed more intense hypotensive responses to propofol. The hypotensive effects of propofol were associated with increased NO production in both clinical and experimental approaches. Enhanced increases in nitrite levels after propofol anesthesia were observed in HT-ACEi patients compared with NT and HT groups. Accordingly, rats treated with enalapril showed increased vascular NO formation after propofol anesthesia compared with rats receiving vehicle. Our data show that ACEi enhance the hypotensive responses to propofol anesthesia and increase nitrite concentrations. These findings suggest that increased NO bioavailability may account for the enhanced hypotensive effects of propofol in ACEi-treated patients.

Plant-soil feedbacks (PSFs) play a relevant role as drivers of species abundance, coexistence, and succession in plant communities. However, the potential contribution of PSFs to community dynamics in changing forest ecosystems affected by global change drivers is still largely unexplored. We measured the direction, strength and nature (biological vs. chemical) of PSFs experienced by coexisting tree species in two types of declining Quercus suber forests of southwestern Spain (open woodland vs. closed forest) invaded by the exotic soil pathogen Phytophthora cinnamomi. To test PSFs in a realistic community context, we focused not only on individual PSFs (i.e., comparing the growth of a tree species on conspecific vs. heterospecific soil) but also calculated net-pairwise PSFs by comparing performance of coexisting tree species on their own and each other's soils. We hypothesized that the decline and death of Q. suber would alter the direction and strength of individual and net-pairwise PSFs due to the associated changes in soil nutrients and microbial communities, with implications for recruitment dynamics and species coexistence. In support of our hypothesis, we found that the decline of Q. suber translated into substantial alterations of individual and net-pairwise PSFs, which shifted from mostly neutral to significantly positive or negative, depending on the forest type. In both cases however the identified PSFs benefited other species more than Q. suber (i.e., heterospecific positive PSF in the open woodland, conspecific negative PSF in the closed forest). Our results supported PSFs driven by changes in chemical soil properties (mainly phosphorus) and arbuscular mycorrhizal fungi, but not in pathogen abundance. Overall, our study suggests that PSFs might reinforce the loss of dominance of Q. suber in declining forests invaded by P. cinnamomi by promoting the relative performance of non-declining coexisting species. More generally, our results indicate an increase in the strength of net PSFs as natural forests become disturbed by global change drivers (e.g., invasive species), suggesting an increasingly important role of PSFs in forest community dynamics in the near future.

Mast-seeding species exhibit not only a large inter-annual variability in seed production but also considerable variability among individuals within the same year. However, very little is known about the causes and consequences for population dynamics of this potentially large between-individual variability. Here, we quantified seed production over ten consecutive years in two Mediterranean oak species - the deciduous Quercus canariensis and the evergreen Q. suber - that coexist in forests of southern Spain. First, we calibrated likelihood models to identify which abiotic and biotic variables best explain the magnitude (hereafter seed productivity) and temporal variation of seed production at the individual level (hereafter CVi), and infer whether reproductive effort results from the available soil resources for the plant or is primarily determined by selectively favoured strategies. Second, we explored the contribution of between-individual variability in seed production as a potential mechanism of satiation for predispersal seed predators. We found that Q. canariensis trees inhabiting moister and more fertile soils were more productive than those growing in more resource-limited sites. Regarding temporal variation, individuals of the two studied oak species inhabiting these resource-rich environments also exhibited larger values of CVi. Interestingly, we detected a satiating effect on granivorous insects at the tree level in Q. suber, which was evident in those years where between-individual variability in acorn production was higher. These findings suggest that individual seed production (both in terms of seed productivity and inter-annual variability) is strongly dependent on soil resource heterogeneity (at least for one of the two studied oak species) with potential repercussions for recruitment and population dynamics. However, other external factors (such as soil heterogeneity in pathogen abundance) or certain inherent characteristics of the tree might be also involved in this process.

Soil-borne pathogens are a key component of the belowground community because of the significance of their ecological and socio-economic impacts. However, very little is known about the complexity of their distribution patterns in natural systems. Here, we explored the patterns, causes and ecological consequences of spatial variability in pathogen abundance in Mediterranean forests affected by oak decline. We used spatially explicit neighborhood models to predict the abundance of soil-borne pathogen species (Phytophthora cinnamomi, Pythium spiculum and Pythium spp.) as a function of local abiotic conditions (soil texture) and the characteristics of the tree and shrub neighborhoods (species composition, size and health status). The implications of pathogen abundance for tree seedling performance were explored by conducting a sowing experiment in the same locations in which pathogen abundance was quantified. Pathogen abundance in the forest soil was not randomly distributed, but exhibited spatially predictable patterns influenced by both abiotic and, particularly, biotic factors (tree and shrub species). Pathogen abundance reduced seedling emergence and survival, but not in all sites or tree species. Our findings suggest that heterogeneous spatial patterns of pathogen abundance at fine spatial scale can be important for the dynamics and restoration of declining Mediterranean forests.

The density of soil seed banks is normally highest at the soil surface and declines monotonically with depth. Sometimes, for a variety of reasons, peak density occurs below the surface but, except in severely disturbed soils, it is generally true that deeper seeds are older. In seasonally dry habitats that develop deep soil cracks during the dry season, it is possible that some seeds fall down cracks and rapidly become deeply buried. We investigated this possibility for three dominant clonal perennials (Scirpus maritimus, S. litoralis, and Juncus subulatus) in the Doñana salt marsh, a nontidal marsh with a Mediterranean climate located in southwest Spain. Two species, which shed most of their seed during the dry season and have seeds with low buoyancy, had bimodal viable seed depth distributions, with peak densities at the surface and at 16-20 cm. A third species, which shed most seeds after soil cracks had closed and had seeds with high buoyancy, had viable seeds only in surface soil. Bimodal seed bank depth distributions may be relatively common in seasonally dry habitats with fine-textured soils, but their ecological significance has not been investigated.

The effect of salinity level and extended exposure to different salinity and flooding conditions on germination patterns of three salt-marsh clonal growth plants (Juncus subulatus, Scirpus litoralis, and S. maritimus) was studied. Seed exposure to extended flooding and saline conditions significantly affected the outcome of the germination process in a different, though predictable, way for each species, after favorable conditions for germination were restored. Tolerance of the germination process was related to the average salinity level measured during the growth/germination season at sites where established individuals of each species dominated the species cover. No relationship was found between salinity tolerance of the germination process and seed response to extended exposure to flooding and salinity conditions. The salinity response was significantly related to the conditions prevailing in the habitats of the respective species during the unfavorable (nongrowth/nongermination) season. Our results indicate that changes in salinity and hydrology while seeds are dormant affect the outcome of the seed-bank response, even when conditions at germination are identical. Because these environmental-history-dependent responses differentially affect seed germination, seedling density, and probably sexual recruitment in the studied and related species, these influences should be considered for wetland restoration and management.

O presente trabalho é uma analise retrospectiva de 218 casos de nutriçäo enteral monomérica realizada em pacientes de diversas enfermarias do Hospital das Clínicas da Faculdade de Medicina de Ribeiräo Preto no período de 1979 a 1988. A duraçäo média desse suporte nutricional foi de 24 dias, a média de idade dos pacientes de 47,6 anos sendo 77,5% do sexo masculino. Os diagnosticos mais comuns foram desnutriçäo por neoplasias, doenças intestinais inflamatorios, sindrome do intestino curto e pore alcoolismo. Houve ganho de peso em 67, 1% dos casos. Apos o uso da nutriçäo enteral monomerica, os valores de proteínas totais e albumina foram maiores que os iniciais, sendo que essa diferença foi mais acentuada naqueles pacientes que receberam a nutriçäo enteral por um período de tempo maior. As complicaçöes metabolicas foram as mais frequentes, no entanto apresentaram intensidade suficiente para interromper o suporte nutricional. A nutriçäo enteral monomerica, alem das facilidades na sua manipulaçäo e comntrole adequadoda oferta dos diferentes nutrientes, representou uma terapeutica dos pacientes que necessitam de suporte nutricional
