RESUMO

Background: HLA-G, a major histocompatibility complex of non-classical class Ib, plays a key role in the development of the primary tumors to metastatic stages. The aim of this study was to investigate HLA-G expression in oral squamous cell carcinomas and its relationship with clinicopathological factors. Methods: After immunohistochemical staining for HLA-G with 63 formalin fixed and paraffin embedded blocks (33 oral squamous cell carcinoma and 30 normal oral mucosa samples), staining intensity, percentage of stained cells and final immunoreactivity score were evaluated, along with other variables. Results: Staining intensity, percentage of stained cells and final immunoreactivity scores in oral squamous cell carcinomas were higher than those in normal oral mucosa (all P=0.001). The staining intensity in the parenchyma of squamous cell carcinoma cells was significantly associated with the clinical tumor stage (P=0.022) and the group with lymphatic metastasis exhibited a higher staining percentage (P=0.026). Staining intensity and immunoreactivity score (IRS) exhibited a significant but inverse correlation with survival rate (P=0.004 and P=0.018, respectively) and a significant direct relationship with clinical stage (P=0.001 and P=0.001). Conclusion: The results supported a role of HLA-G in development of oral squamous cell carcinomas and metastasis to lymph nodes. It might be useful in molecular-targeted therapy.

BACKGROUND: Cyclo-oxygenase-2 (COX-2) is an early response gene that is induced by growth factors, oncogenes and carcinogens and its expression is increased in various tumors. Increased expression of COX-2 plays a significant role in the development and growth of tumors by interfering in biological processes such as cell division, cellular immunity, cell adhesion, apoptosis, and angiogenesis. This study aimed to investigate the immunohistochemical expression of COX-2 in keratocystic odontogenic tumor (KOT) in comparison with ameloblastoma and dentigerous cyst with regards to different clinical behavior and histopathological features of these lesions. MATERIALS AND METHODS: Paraffined blocks of 45 cases including 15 cases of dentigerous cyst, 15 cases of KOT and 15 cases of ameloblastoma were stained with immunohistochemical method for COX-2. Five high-power fields of each sample were evaluated to determine the percentage of stained cells and the intensity of staining. Degree of immunoreactivity was obtained from the sum of two. Statistical evaluation was performed by the Kruskal-Wallis and ANOVA Mann-Whitney test (P < 0.05). RESULTS: Overexpression of COX-2 in ameloblastoma and KOT was observed compared with dentigerous cyst (P < 0.001). However, no significant difference was observed between the expression of COX-2 in ameloblastoma and KOT (P = 0.148). CONCLUSION: The COX-2 expression in odontogenic tumors such as ameloblastoma and cystic neoplasm with aggressive behavior such as KOT increases. However, it does not seem that COX-2 affects the development and growth of cysts with noninvasive behavior like dentigerous cyst.

STATEMENT OF THE PROBLEM: Determining the crown-root ratio is crucial in many dental clinical decisions. There are no reliable data presented for Iranian population. PURPOSE: The aim of this study was to assess the root-crown ratio of permanent teeth with regards to the relationship of gender and jaw type using panoramic radiographs. The reliability of method was also assessed. MATERIALS AND METHOD: The crown and root lengths of teeth were measured by a digital caliper on panoramic radiographs of 185 individuals except for the third molars. A total of 1994 teeth was studied and 50.8% of the teeth belonged to males and 49.2% belonged to females. The modified Lind method was applied. RESULTS: The highest mean root-crown ratios in both arches of both genders were obtained in the second premolars, followed by the first premolars and canines. In both genders, the lowest root-crown ratios were found in the maxillary central incisors. In male patients, the mean root-crown ratio was higher (p= 0.003) than that of females. Using Bland-Altman analysis, a non-significant difference of 0.006 (95% CI: -0.012-0.024) and 0.0002 (95% CI: -0.011-0.011) were found for intra-observer and inter-observer agreement, respectively. CONCLUSION: Assessment of the root-crown ratio in permanent dentition could be performed on panoramic radiographs with acceptable reproducibility in an Iranian Population. Considering observed differences, our findings suggest that for the accurate assessment, this ratio must be calculated for male and female patients and also for the upper and lower jaws, separately.

Diagnosis of subclinical and early stage clinical periodontal dysfunction could prevent from further socioeconomic burden. The aim of this study was to assess the diagnostic applicability of nitric oxide and its end-metabolites in periodontal tissue health and disease. Forty-two patients were enrolled and divided into three groups according to gingivitis (GI) and clinical attachment level (CAL) indices: a healthy group (GI<1, CAL<1), b: gingivitis (GI>1, CAL>1) and c: periodontitis (CAL>1) with 14 patients in each group. Unstimulated saliva and gingival crevicular fluid (GCF) were collected. Samples were evaluated for nitrite, nitrate and total nitric oxide contents with the ELISA method. In addition, CAL, GI, plaque index (PI), decay, missing, filling (DMFT) and bleeding index (BI) scores were also recorded. Except for GCF nitrite content (P= 0.89), there was an increasing trend for measured biomarkers in both saliva and GCF (Periodontitis> gingivitis> healthy periodontium, P< 0.05). Data remained stable after simultaneous adjustment for DMFT and BI scores as confounding factors. Sensitivity, specificity, positive predictive value, negative predictive value, cut point and p- value were as the followings: GCF nitrate (0.71, 0.11, 0.29,0.43, 4.97, P= 0.04), nitric oxide GCF ( 0.64, 0.18, 0.28, 0.5, 10.12, P= 0.04), nitrite saliva (0.93, 0.96,0.93,0.96,123.48, P< 0.001), salivary nitrate (0.93, 0.96, 0.93, 0.96, 123.6, P< 0.001), salivary nitric oxide (0.93, 0.96, 0.93, 0.96, 246.65, P <0.001). Our results revealed that NO plays an important role in the process of destruction of periodontal tissues. Within the limitation of our study, detecting NO biomarker and its end metabolites in saliva is of more value to assess the periodontal health comparing to GCF.

BACKGROUND: Testosterone may be an important factor causing depression in the elderly men. The purpose of this study was to determine the relationship between testosterone levels and depressive symptoms in older men in Amirkola, Iran. METHODS: This cross- sectional study is a part of the Amirkola Health and Aging Project (AHAP) that involves people aged 60 and above living in Amirkola, a small town in northern Iran. The testosterone levels were measured using ELISA on morning blood samples (ngr / ml) and depressive symptoms were identified using Geriatric Depression Scale (GDS). The data were collected and analyzed. RESULTS: Eight hundred thirty elderly men with the mean age of 70.02±7.7 years were included. On the basis of GDS criteria, 593 individuals had no depressive symptoms and 237 had at least one of these symptoms. The mean serum testosterone level in men without symptoms of depression (4.94±4.22) ngr/ml and was higher than in those with such symptoms (4.19±3.65) ngr/ml (P=0.011). Also, there was a significant inverse correlation between the testosterone levels and number of depressive symptoms (P=0.015, r=-0.084). After adjusting with age and educational levels, and living alone (OR=2.6, 95% CI: 1.17-5.82, P=0.02), testosterone levels (OR=1.67, 95% CI: 1.03-2.72, P=0.038) had the greatest impact on the development of depression. CONCLUSION: The results of this study showed a significant inverse relationship between serum testosterone levels and depressive symptoms in elderly men.

BACKGROUND: Many evidences showed that cyclooxygenase-2 (COX-2), an enzyme that catalyzes the synthesis of prostaglandins, has an important role in carcinogenesis. It has been observed in experimental models that selective COX-2 inhibitors suppress the formation of tumors including tongue carcinoma. The aim of this study was to investigate the immunohistochemical expression of COX-2 in oral squamous cell carcinoma (OSCC) compared to normal oral mucosa and oral dysplasia. MATERIALS AND METHODS: A total of 60 paraffined blocks (20 cases of OSCC, 20 cases of oral epithelial dysplasia and 20 cases of normal oral mucosa were included in this study and immunohistochemical staining was done for COX-2 expression. From each sample, 5 high power fields were assessed to determine the percentage of stained cells and staining intensity. Immunoreactivity was obtained by multiplying the above two cases. Data were analyzed with using the Kruskal-Wallis test ANOVA, Mann-Whitney test and least significant difference and P<0.05 was declared as significant. RESULTS: High level of COX-2 expression was found in OSCC and dysplasia compared to normal mucosa. Furthermore, a positive correlation was found between COX-2 expression and severity of dysplasia. However, no significant difference between low grade and high grade tumors was found. CONCLUSION: The result of the present study supports from the role of COX-2 in carcinogenesis and progression of premalignant lesion to malignancy.
