The present study aimed to analyse the integrated histopathological lesions (IHLs) of the gill, liver and intestine of Catla catla exposed to the different doses of UV-B radiation. Gill exhibited the lesions like hypertrophy, hyperplasia, vacuolation, fusion of the gill filaments, rupture in the gill lamellae, epithelial cell lifting and necrosis. The UV-B-exposed liver of Catla showed the lesions like the degeneration of nucleus, the disarrangement of hepatocytes, sinusoidal vacuolation, epitheliod histiocyst, hepatocellular adenoma, exocrine adenoma, cyst formation and diffused epithelial necrosis (DEN). UV-B-exposed intestine showed the lesions like the distortion of columnar epithelial cells (CECs), distortion in lamina propria (LP), disruption in brush border (BB), vacuolation in LP, the presence of submucosal mass (SM), the degeneration of nucleus, the presence of tactoid bodies, the presence of aschoff nodules and metatypical cell carcinoma. These histopathological alterations can be considered as the main blocking alterations of the growth and absorption as well as the final production of fish which can cause a serious loss in total yield to fish farmers which can interrupt the profitable economical production of fish.

Coastal ecosystems are vital for maintaining the biodiversity and human livelihoods, but they are increasingly subjected to anthropogenic pressures, including pollution from various sources. Present work intends to assess the possible threats in coastal ecosystem as well as coastal fish species, in particular, through haematological parameters caused due to exposure of environmental contaminants like polycyclic aromatic hydrocarbons (PAHs), potentially toxic metals (PTMs), etc. This study analysed the haematological parameters and probable toxicity levels in two important coastal fish species, viz., Mystus sp. and Mugil sp. widely available in Digha coastal belt. Different haematological parameters, such as WBCs (White Blood Cells), Lym (Lymphocytes), Gran (Granulocytes), Mid (Monocytes), RBCs (Red Blood Cells), HCT (Haematocrit) value, MCV (Mean Corpuscular Volume), MCH (Mean Corpuscular Haemoglobin), MCHC (Mean Corpuscular Haemoglobin Concentration), RDW- CV (Red Cells Distribution Width-Co-efficient of Variation), RDW- SD (Red Cells Distribution Width-Standard Deviation), PLT (Total Platelet Count), MPV (Mean Platelet Volume), PDW- SD (Platelet Distribution Width-Standard Deviation), PDW- CV (Platelet Distribution Width-Co-efficient of Variation), PCT (Plateletcrit), PLCR (Platelet Large Cell Ratio), PLCC (Platelet Large Cell Count) and many others were measured directly through Erba H360 Haematology Analyser, simultaneously air dried blood smear was stained by Haematoxylin-Eosin(H-E) and Giemsa stain for assessing morphometric alterations of RBCs, WBCs, platelets as well as to determine the differential counts of WBCs by observing through Leica DM2000 microscope. Evidence of several abnormalities in the erythrocyte's nucleus (ENAs) and the abundance of abnormal celled erythrocytes (ECAs), carcinoma (lymphoproliferative disorder, polycythaemia vera, Hodgkin lymphoma and non-Hodgkin lymphoma), elevation of WBCs content, Lym %(Lymphocyte percentage), Eo(Eosinophils), monocytes, HCT and gross depletion of Ne(Neutrophils), basophils, and PLCR levels indicated a sign of major impact of contamination to two intoxicated fishes which may also affect the human being through food chain and may result into leukaemia in mammalian species, finally. However, comprehensive evaluation of the long-term impacts of the contaminants like PAHs and/or PTMs, etc., on fish populations, human health risk and coastal ecosystem is required to be addressed.

Measles, mumps, and rubella (MMR) are highly infectious viral diseases affecting young children and have high secondary attack rates. Present MMR vaccines show consistent seroconversion rates for anti-measles and anti-rubella antibodies with variable responses for anti-mumps antibodies. Most common strains for MMR vaccines, currently available in India, are the Edmonston-Zagreb measles strain, Leningrad Zagreb (L-Z) mumps strain, and the RA 27/3 rubella strain. L-Z strain of mumps virus has been found to be associated with aseptic meningitis by different studies from different parts of the world including India. Recently, a novel freeze-dried MMR vaccine developed by Zydus Lifesciences (Zyvac MMR) contains Edmonston Zagreb measles strain, Hoshino mumps strain, and RA 27/3 rubella strain. The Hoshino strain is WHO approved and was found to induce interferon gamma production. This review article aims to provide a comprehensive appraisal of the data available on the safety and immunogenicity of the novel MMR vaccine.

Background: A previous study compared insulin sensitivity indices for the detection of double diabetes (DD) in Indian adolescents with type-1 diabetes (T1D) and derived a cut-off to predict future risk for the development of metabolic syndrome (MS) in adolescents with T1D. We conducted the current study with the aim to validate these cut-offs for detecting DD among Indian subjects with T1D from various geographical locations. Methods: This multicentric cross-sectional study included 161 Indian adolescents with T1D. Demographic, anthropometric, clinical, and biochemical data were collected using standard protocols. Insulin sensitivity (IS) was calculated using various equations developed to determine insulin sensitivity in subjects with T1D. Metabolic syndrome was diagnosed using International Diabetes Federation (IDF) Consensus Definition 2017. Results: We report 4.3% prevalence of MS in Indian adolescents with T1D with an additional 29.8% of study participants at risk of development of MS. Low High density lipoprotein (HDL) (23.6%) was the commonest abnormal component of the MS definition. Insulin sensitivity calculated by an equation derived by the SEARCH group was the most appropriate index to identify MS and metabolic risk in Indian adolescents with T1D. The proposed cut-off of 5.48 had high specificity, positive predictive value, and negative predictive value in identifying the risk of the development of DD. Conclusions: Insulin sensitivity calculated by the equation proposed by the SEARCH group together with cut-offs derived in earlier study may be used effectively to identify risk of development of MS/DD in Indian adolescents with T1D from various geographical locations.

Face masks, a prime component of personal protective equipment (PPE) items, have become an integral part of human beings to survive under the ongoing COVID-19 pandemic situation. The global population requires an estimated 130 billion face masks and 64 billion gloves/month, while the COVID-19 pandemic has led to the daily disposal of approximately 3.5 billion single-use face masks, resulting in a staggering 14,245,230.63 kg of face mask waste. The improper disposal of face mask wastes followed by its mismanagement is a challenge to the scientists as the wastes create pollution leading to environmental degradation, especially plastic pollution (macro/meso/micro/nano). Each year, an estimated 0.15-0.39 million tons of COVID-19 face mask waste, along with 173,000 microfibers released daily from discarded surgical masks, could enter the marine environment, while used masks have a significantly higher microplastic release capacity (1246.62 ± 403.50 particles/piece) compared to new masks (183.00 ± 78.42 particles/piece). Surgical face masks emit around 59 g CO2-eq greenhouse gas emissions per single use, cloth face masks emit approximately 60 g CO2-eq/single mask, and inhaling or ingesting microplastics (MPs) caused adverse health problems including chronic inflammation, granulomas or fibrosis, DNA damage, cellular damage, oxidative stress, and cytokine secretion. The present review critically addresses the role of face masks in reducing COVID-19 infections, their distribution pattern in diverse environments, the volume of waste produced, degradation in the natural environment, and adverse impacts on different environmental segments, and proposes sustainable remediation options to tackle environmental challenges posed by disposable COVID-19 face masks.

Nutrient inadequacies among picky-eaters have adverse effects on growth and development. Oral nutritional supplements (ONS) along with dietary counseling (DC), rather than DC alone as reported in our earlier publication, promoted growth among picky-eating Indian children aged from >24 m to ≤48 m with weight-for-height percentiles lying between the 5th and 25th (based on WHO Growth Standards) over 90 days. This paper presents the contribution of ONS to nutrient adequacy, dietary diversity, and food consumption patterns in children (N = 321). Weight, height, and dietary intakes, using 24-h food recalls, were measured at baseline (Day 1) and at Days 7, 30, 60, and 90. Nutrient adequacy, dietary diversity score (DDS), and food intake adequacy were calculated in both the supplementation groups (ONS1 + DC and ONS2 + DC; n = 107 in each group) and the control group (DC-only; n = 107). Supplements increased nutrient adequacy in both of the ONS + DC groups relative to control (p < 0.05). The proportions of children with adequate nutrient intakes increased significantly at Day 90 in the supplemented groups as compared to in the control group (p < 0.05), especially for total fat, calcium, vitamin A, vitamin C, and thiamin. Although no significant differences were observed in DDS in any of the groups, the percentage of children consuming ≥4 food groups in a day had increased in all the groups. Consumption of fruit and vegetables and cereals had increased significantly from baseline to Day 90. ONS along with dietary counseling was found to have improved nutritional adequacy without interfering with the normal food consumption patterns of picky-eating children at nutritional risk.

BACKGROUND: This study assessed safety and immunogenicity of Serum Institute of India Pvt Ltd (SIIPL)'s tetanus toxoid (TT), diphtheria toxoid (DT), and acellular pertussis booster vaccine (Tdap). RESEARCH DESIGN AND METHODS: In this Phase II/III, multicenter, randomized, active-controlled, open-label study, 1500 healthy individuals, aged 4-65 years, were randomized to receive a single dose of SIIPL Tdap or comparator Tdap vaccine (Boostrix®; GlaxoSmithKlines, India). Adverse events (AEs) during initial 30 minutes, 7-day, 30-day post-vaccination were assessed. Blood samples were taken before and 30 days post-vaccination for immunogenicity assessment. RESULTS: No significant differences in incidence of local and systemic solicited AEs were observed between the two groups; no vaccine-related serious AEs were reported. SIIPL Tdap was non-inferior to comparator Tdap in achieving booster responses to TT and DT in 75.2% and 70.8% of the participants, respectively, and to pertussis toxoid (PT), pertactin (PRN), and filamentous hemagglutinin (FHA) in 94.3%, 92.6%, and 95.0% of the participants, respectively. Anti-PT, anti-PRN, and anti-FHA antibody geometric mean titers in both the groups, were significantly higher post-vaccination compared to pre-vaccination. CONCLUSIONS: Booster vaccination with SIIPL Tdap was non-inferior to comparator Tdap with respect to immunogenicity against tetanus, diphtheria, and pertussis and was well tolerated.

OBJECTIVES: To assess the risk factors of intravenous immunoglobulin (IVIG)-resistant Kawasaki disease (KD) and to evaluate the performance of the three Japanese risk-scoring systems, namely the Kobayashi, Egami, and Sano scores in predicting IVIG resistance among the Indian patients. METHODS: Prospective observational study on children admitted with KD at Institute of Child Health, Kolkata, over a period of 16 months, from January 2019 to April 2020. The study included 70 KD patients all of whom were treated with IVIG. Clinical parameters, laboratory variables, and risk scores were compared between the IVIG-responsive and the IVIG-resistant groups. RESULTS: A total of 31.4% were IVIG non-responders. Skin rash was found to be significantly associated with IVIG-resistant KD. The IVIG-resistant group had higher total bilirubin, lower albumin, higher CRP levels, and higher ALT and AST levels. High Kobayashi score, high Egami score, and high Sano score were significantly associated with IVIG resistance, individually. Sano score had the highest sensitivity (81.8%) and Kobayashi score had the highest specificity (77.1%) in our cohort. CONCLUSION: The presence of skin rash, high total bilirubin, high CRP, high AST, high ALT, and low albumin were important predictors of IVIG resistance in our population. Among the three scores, Sano score is the most reliable in identifying potential non-responders to IVIG. But Sano score lacked good specificity. Therefore, Indian KD patients may need an exclusive scoring system to predict non-responsiveness to IVIG so that a more aggressive therapy can be instituted at the earliest. Key points • Early prediction of IVIG-resistant KD is necessary to limit cardiac injuries. • Sano score has high sensitivity to predict IVIG resistance in Indian population.

Efficacy and safety data on quadrivalent influenza vaccines (QIVs) for immunization of Indian children are scarce. This phase 3, registration study evaluated the immunogenicity, safety, and tolerability of a QIV in Indian children aged 6-35 months (Group 1) and 3-17 y (Group 2). Subjects received one or two doses (0.5 mL each) of the study vaccine based on their priming status. Immunogenicity (post-vaccination geometric mean fold increase in hemagglutination inhibition [HI] titers and proportion of patients with seroprotection and seroconversion against the four influenza strains), unsolicited adverse events (AEs), and tolerability were analyzed. Among 118 subjects enrolled in each group, the geometric mean(standard deviation) fold increase in HI titers against A(H3N2), A(H1N1), B(Victoria), and B(Yamagata) strains were 31.7(5.33), 10.5(6.06), 4.1(5.70), and 8.6(5.34) in Group 1 and 14.0(4.37), 9.2(4.26), 14.3(6.73), and 14.4(5.41) in Group 2, respectively. Seroprotection was achieved by 91.2%, 83.3%, 41.2%, and 68.4% subjects in Group 1 and 100%, 95.8%, 73.7%, and 89.8% subjects in Group 2, respectively. Seroconversion was achieved by 87.7%, 66.7%, 41.2%, and 64.9% subjects in Group 1 and 89.0%, 78.8%, 69.5%, and 75.4% subjects in Group 2, respectively. Vaccination site pain and fever were the most common local and systemic reactions, respectively. Systemic reactions were more frequent in Group 1 (16.9% vs 7.6%). Most subjects (>90%) did not experience inconvenience within 7 d of vaccination; <10% in both groups reported unsolicited AEs. The present study was intended to determine the possible influences of direct field application of choline chloride into pond water in addition to farm-made-aqua-feed under a semi-intensive culture system on the growth and biochemical parameters of two Indian major carps (IMCs), Catla catla (Catla) and Labeo rohita (Rahu), and two air-breathing species, Clarias batrachus (Magur) and Anabas testudineus (Koi), cultured in a ratio of 2:5:1:1 (Catla/Rahu/Magur/Koi) in three experimental ponds for a period of 90 days during the breeding season (June to August). Results were compared with control (C: fed only with farm-made-aqua-feed) and treatment (T: P1 and P2: farm-made-aqua-feed plus choline chloride into pond water directly at the rate of 350 g bigha-1 fortnightly or 350 g per 1600 square meter fortnightly). A significant increasing trend was observed in the growth parameters including total length-final (TLF), standard length-final (SLF), mean weight-final (MWF), % gain of mean total length (MTL), % gain of mean standard length (MSL), % weight gain (WG), specific growth rate (SGR) % per day, and survivability %. However, a reverse pattern was noticed in the food conversion ratio (FCR) both in IMCs and air-breathing fish species under choline supplementation. Serum biochemical responses, e.g., total protein (PRO), lactate dehydrogenase (LDH), glucose (GLU), and calcium (Ca) showed significant enhancement, and alkaline phosphatase (ALP), alanine amino transaminase (ALT), aspartate amino transaminase (AST), cholesterol (CHOLES), and triglycerides (Trig) showed gradual significant reduction during the breeding season under choline exposure. Treated fishes showed prevention from liver dysfunction and fatty liver formation, and increased body crude protein content. Results indicated favorable growth and yield, which may benefit fish farmers during their culture practices, and the output fish species under choline supplementation resulted in quality food-fish for human consumption.

The current study is aimed to assess the ecotoxicological effects of toxic metals and seasonal shift of the physicochemical characteristics of soil in an endogeic earthworm Metaphire posthuma of industrially contaminated soil of Calcutta leather complex. The accumulation of cadmium, chromium, lead and mercury was quantitated in whole earthworms and coelomocytes. The accumulation of metals was derived to be high in the coelomocytes than whole earthworms. Morphofunctional shift in coelomocytes indicated a high level of metal toxicity in soil inhabitants. The shift in differential coelomocyte count and cellular damage including intense cytoplasmic vacuolation and membrane blebbing of coelomocytes of M. posthuma of contaminated soil were suggestive to a state of immunocompromisation in the same species. Shift in the generation of nitric oxide and activity of inducible nitric oxide synthase indicated a possible immunosuppression in earthworm. Depletion in the acetylcholinesterase activity of coelomocytes indicated neurotoxicity of metals leached from the dumped wastes in Calcutta leather complex. Integrated biomarker response based analysis was carried out to assess the biomarker potential of experimental endpoints of M. posthuma to monitor metal toxicity in soil.

The problem of poor nutrition with impaired growth persists in young children worldwide, including in India, where wasting occurs in 20% of urban children (<5 years). Exacerbating this problem, some children are described by their parent as a picky eater with behaviors such as eating limited food and unwillingness to try new foods. Timely intervention can help prevent nutritional decline and promote growth recovery; oral nutritional supplements (ONS) and dietary counseling (DC) are commonly used. The present study aimed to determine the effects of ONS along with DC on growth in comparison with the effects of DC only. Enrolled children (N = 321) were >24 to ≤48 months old, at malnutrition risk (weight-for-height percentile 3rd to 15th), and described as a picky eater by their parent. Enrollees were randomized to one of the three groups (N = 107 per group): ONS1 + DC; ONS2 + DC; and DC only. From day 1 to day 90, study findings showed significant increases in weight-for-height percentile for ONS1 + DC and for ONS2 + DC interventions, as compared to DC only (p = 0.0086 for both). There was no significant difference between the two ONS groups. Anthropometric measurements (weight and body mass index) also increased significantly over time for the two ONS groups (versus DC only, p < 0.05), while ONS1 + DC significantly improved mid-upper-arm circumference (p < 0.05 versus DC only), as well. ONS groups showed a trend toward greater height gain when compared to DC only group, but the differences were not significant within the study interval. For young Indian children with nutritional risk and picky eating behaviors, our findings showed that a 90-day nutritional intervention with either ONS1 or ONS2, along with DC, promoted catch-up growth more effectively than did DC alone.

The coronavirus disease 2019 (COVID-19) pandemic, caused by the novel virus severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), began in December 2019 in China and has led to a global public health emergency. Previously, it was known as 2019-nCoV and caused disease mainly through respiratory pathways. The COVID-19 outbreak is ranked third globally as the most highly pathogenic disease of the twenty-first century, after the outbreak of SARS-CoV and Middle East respiratory syndrome in 2002 and 2012, respectively. Clinical, laboratory, and diagnostic methodology have been demonstrated in some observational studies. No systematic reviews on COVID-19 have been published regarding the integration of COVID-19 outbreaks (monitoring, fate and treatment) with environmental and human health perspectives. Accordingly, this review systematically addresses environmental aspects of COVID-19 outbreak such as the origin of SARS-CoV-2, epidemiological characteristics, diagnostic methodology, treatment options and technological advancement for the prevention of COVID-19 outbreaks. Finally, we integrate COVID-19 outbreaks (monitoring, fate and treatment) with environmental and human health perspectives. We believe that this review will help to understand the SARS-CoV-2 outbreak as a multipurpose document, not only for the scientific community but also for global citizens. Countries should adopt emergency preparedness such as prepare human resources, infrastructure and facilities to treat severe COVID-19 as the virus spreads rapidly globally.

PURPOSE: To assess the effect of combination probiotic Saccharomyces boulardii CNCM-I 3799 and Bacillus subtilis CU-1 in outpatient management of acute watery diarrhea in children. METHODS: A randomized double-blind placebo-controlled study was conducted in 180 participants aged six months to five years with acute mild to moderate diarrhea. All were enrolled from six centers across India and centrally randomized to receive S. boulardii CNCM-I 3799 and B. subtilis CU-1 or a placebo along with oral rehydration salts and zinc supplementation. Each participant was followed up for three months to assess recurrence of diarrhea. RESULTS: The mean duration of diarrhea in the probiotic and placebo groups were 54.16 hours and 59.48 hours, respectively. The difference in the duration of diarrhea in those administered with probiotic or placebo within 24 hours of diarrhea onset was 25.21 hours. Furthermore, the difference in duration of diarrhea was 13.84 hours (p<0.05) for participants who were administered with probiotics within 48 hours. There were no significant differences in the stool frequencies between the two arms. After three months, 15% in the probiotic group and 18.5% in the placebo group reported episodes of diarrhea. The mean duration of diarrhea was considerably lower in the probiotic group, 31.02 hours versus 48 hours in placebo (p=0.017). CONCLUSION: S. boulardii CNCM-I 3799 and B. subtilis CU-1 combination was effective in reducing the duration of diarrhea when administered within 48 hours of diarrhea onset. Similarly, it reduced recurrence of diarrhea and its intensity in the subsequent three months.

Although a nationwide lockdown was imposed in India amid COVID-19 outbreak since March 24, 2020, the COVID-19 infection is increasing day-by-day. Till June 10, 2021 India has recorded 29,182,072 COVID cases and 359,695 deaths. A number of factors help to influence COVID-19 transmission rate and prevalence. Accordingly, the present study intended to integrate the climatic parameters, namely ambient air temperature (AT) and relative humidity (H) with population mass (PM) to determine their influence for rapid transmission of COVID-19 in India. The sensibility of AT, H and PM parameters on COVID-19 transmission was investigated based on receiver operating characteristics (ROC) classification model. The results depicted that AT and H models have very low sensibility (i.e., lower area under curve value 0.26 and 0.37, respectively compared with AUC value 0.5) to induce virus transmission and discrimination between infected people and healthy ones. Contrarily, PM model is highly sensitive (AUC value is 0.912, greater than AUC value 0.5) towards COVID-19 transmission and discrimination between infected people and healthy ones and approximate population of 2.25 million must impose like social distancing, personal hygiene, etc. as strategic management policy. Therefore, it is predicted, India could be the next epicenter of COVID-19 outbreak because of its over population.

Polyaromatic compounds are the major, widespread contaminants in the aquatic environment. However, the adverse impacts of these compounds on blood pathophysiology (hematological profiling and serum biochemical responses) are poorly understood. As a consequence, this study was intended to evaluate the toxic effects of naphthalene, one of the polycyclic aromatic hydrocarbons, on the blood pathophysiology of Anabas testudineus using multiple end-point biomarker approach. A. testudineus was exposed to short-term (1 and 5 d) and long-term (10, 15, and 21 d) naphthalene concentrations, that is, T1 (0.71 mg/L indicates 25% of LC50) and T2 (1.42 mg/L indicates 50% of LC50 value). The results disclosed significant decrease in red blood cells, hemoglobin (Hb), packed cell volume, and platelet levels, while other blood parameters, namely, white blood cells, percent lymphocyte, mean cell volume, mean corpuscular Hb, and mean corpuscular Hb concentration showed enhanced levels under naphthalene intoxication. Results were more detrimental under T2 concentration. Cholesterol, glucose, calcium, high-density lipoprotein, and low-density lipoprotein levels gradually increased throughout the different exposure periods under T1 and T2 concentrations, while the triglyceride level gradually decreased during exposure periods. Finally, integrated biomarker responses (IBR) analysis indicated that serum biochemical parameters are more powerful than hematological parameters for determining the naphthalene-induced fish health status. Additionally, the IBR study clearly identified that long-term (>5 d) exposure was more harmful than short-term (<5 d) naphthalene exposure. So, these responses may be derived as biomarkers for monitoring naphthalene pollution in an aquatic ecosystem.
