RESUMO
Esophageal achalasia is an uncommon disease in pediatrics. With an insidious clinical presentation, diagnosis is delayed. Here we describe a case of esophageal achalasia in a 16-year-old girl, with the typical delay in consultation and diagnosis. Although pneumatic balloon dilatation has been described as the best therapeutic option for type II achalasia, it was ineffective in our patient and she required Heller extramucosal myotomy with gastroesophageal fundoplication for reflux.
La acalasia esofágica es una patología infrecuente en la edad pediátrica. Su presentación clínica es insidiosa, lo que causa un retraso en el diagnóstico. Se presenta un caso de acalasia esofágica en una niña de 16 años, que tuvo la demora característica en la consulta y el diagnóstico. A pesar de que se describe la dilatación neumática con balón como la mejor opción terapéutica para la acalasia tipo II, en nuestra paciente fue inefectiva y requirió miotomía extramucosa de Heller con funduplicatura gastroesofágica antirreflujo.

Introducción. La usabilidad en un sistema de teleconsulta afecta directamente la eficiencia y efectividad de la atención médica remota. Objetivo. Evaluar la usabilidad de la teleconsulta durante la pandemia por COVID-19. Población y método. Estudio de corte transversal. Incluimos a los cuidadores de niños/as de 1 mes a 12 años. Evaluamos la usabilidad mediante el Telehealth Usability Questionnaire adaptado en español. Además, evaluamos datos socioeconómicos. Resultados. Tasa de respuesta del 70,2 % (n = 221). La mayoría eran mujeres, edad promedio 33 años, con educación secundaria y cobertura de salud pública. El 87,8 % eligió atención telefónica y el 88,2 % tenía su primera teleconsulta. Alta satisfacción general con puntuaciones menores en facilidad de uso y aprendizaje en videollamadas. Conclusión. La teleconsulta mostró alta usabilidad, independientemente de la modalidad, para cuidadores de niños/as de 1 mes a 12 años.

Introducción. Las estrategias sanitarias basadas en tecnologías de la información y la comunicación (TIC) podrían perpetuar la inequidad en salud, especialmente en poblaciones vulnerables. Existen escasas herramientas validadas para evaluar el acceso a las TIC en pediatría en nuestro medio. Objetivos. Construir y validar un cuestionario para evaluar el acceso a las TIC para cuidadores de pacientes pediátricos. Describir las características de acceso a las TIC y evaluar si existe correlación entre los tres niveles de la brecha digital. Población y métodos. Construimos y validamos un cuestionario que luego administramos a cuidadores de niños entre 0 y 12 años. Las variables de resultado fueron las preguntas del cuestionario para los tres niveles de brecha digital. Además, evaluamos variables sociodemográficas. Resultados. Administramos el cuestionario a 344 cuidadores. El 93 % poseía celular propio y el 98,3 % utilizaba internet por red de datos. El 99,1 % se comunicaba a través de mensajes de WhatsApp. El 28 % había realizado una teleconsulta. La correlación entre las preguntas fue nula o baja. Conclusión. Por medio del cuestionario validado, evaluamos que los cuidadores de pacientes pediátricos de 0 a 12 años poseen en su mayoría celular, se conectan por red de datos, se comunican principalmente a través de WhatsApp y obtienen pocos beneficios a través de TIC. La correlación entre los diferentes componentes del acceso a las TIC fue baja.

INTRODUCCIÓN: La piomiositis es una infección bacteriana agudasubaguda del músculo esquelético. OBJETIVO: Estimar la incidencia de piomiositis en pacientes internados, describir e identificar factores de riesgo para bacteriemia y hospitalización, y evaluar diferencias entre Staphylococccus aureus sensible y resistente a meticilina (SASM y SARM). PACIENTES Y MÉTODOS: Estudio descriptivo, retrospectivo, observacional, con pacientes de 1 mes a 18 años de edad, internados entre el 1 de enero de 2008 y 31 de diciembre de 2018. Variables: sexo, edad, hacinamiento en el hogar, existencia de lesión previa, estacionalidad, localización anatómica e imágenes, antibioterapia previa, estadio clínico, parámetros de laboratorio, cultivos y antibiograma, días de tratamiento intravenoso (IV), de internación, de fiebre y bacteriemia. RESULTADOS: Se incluyeron 188 pacientes. Incidencia: 38,9 casos / 10.000 admisiones (IC95 % 33,7 - 44,9). Días de internación y tratamiento IV: 11 (RQ 8-15 y RQ 8-14, respectivamente). El desarrollo de bacteriemia se asoció a PCR elevada (p = 0,03) y fiebre prolongada (p < 0,001). No hubo diferencias en la evolución y parámetros de laboratorio entre SASM y SARM. La leucocitosis (p = 0,004), neutrofilia (p = 0,005) y bacteriemia (p = 0,001) se asociaron a mayor estadía hospitalaria. CONCLUSIONES: Este estudio recaba la experiencia de más de 10 años de niños internados con diagnóstico de piomiositis y proporciona información sobre sus características. Se describen parámetros asociados a bacteriemia y estadía hospitalaria.
BACKGROUND: Pyomyositis is an acute-subacute bacterial infection of skeletal muscle. AIM: To estimate the incidence of pyomyositis in hospitalized patients, describe and identify risk factors for bacteremia and hospitalization, and evaluate differences between MSSA and MRSA. METHODS: Descriptive, retrospective, observational study with patients aged 1 month to 18 years hospitalized between January, 1, 2008 and December 1, 2018. Variables: sex, age, home overcrowding, previous injury, seasonality, anatomical location and images, previous antibiotherapy, clinical stage, laboratory, cultures and antibiogram, days of intravenous (IV) treatment, hospitalization, fever and bacteremia. RESULTS: 188 patients were included. Incidence: 38.9 cases/10,000 admissions (95% CI 33.7 - 44.9). Days of hospitalization and IV treatment: 11 (RQ 8-15 and RQ 8-14, respectively). The development of bacteremia was associated with elevated CRP (p = 0.03) and prolonged fever (p < 0.001). There were no differences in the evolution and laboratory parameters between MSSA and MRSA. Leukocytosis (p = 0.004), neutrophilia (p = 0.005), and bacteremia (p = 0.001) were associated with a longer hospital stay. CONCLUSIONS: This study collects the experience of more than 10 years of hospitalized children diagnosed with pyomyositis and provides information on its characteristics. Parameters associated with bacteremia and hospital stay are described.

Introducción: la cuarentena adoptada durante la pandemia por COVID-19 significó grandes cambios en el cuidado de los pacientes con diabetes, pudiendo afectar el control metabólico. Objetivos: evaluar si existieron modificaciones en el control metabólico en menores de 18 años con diabetes tipo 1, seguidos en el Servicio de Nutrición y Diabetes del Hospital General de Niños Pedro de Elizalde (HGNPE), luego de las medidas de aislamiento social implementadas durante la pandemia por COVID-19. Estimar la frecuencia de complicaciones agudas y la variación del puntaje Z de Índice de masa corporal (IMC). Materiales y métodos: estudio observacional, longitudinal, retrospectivo. Se incluyeron pacientes de 1 a 18 años con diabetes tipo 1 de más de un año de evolución, con seguimiento regular en el HGNPE, que hayan sido evaluados, con determinación de hemoglobina glicosilada, entre diciembre de 2019 y marzo de 2020 (precuarentena). De estos pacientes, aquellos que asistieron a un control posterior, entre diciembre de 2020 y marzo de 2021, fueron considerados para un análisis comparativo. Resultados: se incluyeron 88 pacientes, de los cuales 59 se realizaron un control en el segundo período. La HbA1c inicial fue de 8,8% (IIC 25-75= 7,42-9,4) y la posterior de 8,3% (IIC 25-75= 7,2-9,95), el puntaje z de IMC inicial fue de 0,35 ± 1,02 y el posterior de 0,37 ± 0,97, siendo en ambos casos diferencias no significativas. Presentó complicaciones agudas el 15% (n= 9) de los pacientes. Conclusión: no se encontraron cambios significativos de la HbA1c y del puntaje z de IMC entre los períodos comparados
Introduction: the lock down during the COVID-19 pandemic meant great changes in the care of patients with diabetes, which could affect metabolic control. Objectives: to assesse whether there were changes in metabolic control in children under 18 years of age with type 1 diabetes followed up in the Nutrition and Diabetes Service of the Pedro de Elizalde Children's General Hospital (HGNPE), after the social isolation measures implemented during the COVID-19 pandemic and estimate the frequency of acute complications and the variation of the Z-score of Body Mass Index (BMI). Materials and methods: longitudinal, retrospective, observational study. Patients aged 1 to 18 years old with type 1 diabetes of more than one year of evolution, with regular follow-up in the HGNPE, who had been evaluated, with determination of glycosylated hemoglobin, between December 2019 and March 2020 (pre-quarantine) and between December 2020 and March2021. Of these patients, those who attended a subsequent control between December 2020 and March 2021 were considered for a comparative analysis. Results: 88 patients were included, of which 59 underwent a control in the second period. The initial HbA1c was 8,8% (IIC 25-75 = 7,42-9,4) and the subsequent one was 8,3% (IIC 25-75 = 7,2-9,95), the z-score of initial BMI was 0,35 ± 1,02 and the subsequent one was 0,37 ± 0,97, with non-significant differences in both cases. 15% (n = 9) of the patients had acute complications. Conclusion: no significant changes were found in HbA1c and BMI z-score between the periods compared

INTRODUCTION: Pediatricians are in a strategic position to prevent smoking. OBJECTIVES: To estimate the prevalence of smoking among pediatric residents, analyze associated factors, describe preventive actions, and assess the differences observed over the past decade. METHODS: Cross-sectional study with a self-administered, anonymous survey conducted among pediatric residents from eight Argentine hospitals. Smoking habit, associated factors, and attitude towards patients' or their parents' smoking were evaluated; results were compared to those obtained in 2002. RESULTS: Out of 448 surveyed physicians, 20.1% smoked. There were no significant differences between smokers and non-smokers in terms of gender, having children, number of on-call shifts, and having a supervisor who smokes. Having a parent who smoked was a risk factor for tobacco use only among women (OR: 1.98; 95% CI: 1.09-3.61; p = 0.01). Surveyed residents living with a couple had a lower smoking rate (OR: 0.57; 95% CI: 0.34-0.96; p = 0.03). Only 18.1% referred having an active behavior towards smoking patients, no differences were observed between smokers and non-smokers. In addition, there were no differences in smoking prevalence from 2002, but there was a higher rate of residents who advised their patients (32.4% versus 26.1%; p < 0.01) and warned them against smoking (37.7% versus 18.6%; p < 0.01), and of those who received information on this topic during their training (63.6% versus 39.8%; p < 0.01). CONCLUSIONS: Among resident pediatricians, 20.1% were smokers, with a higher prevalence among women with a father or mother who is a smoker. The rate of active behavior regarding patients or parents who smoke was very low. In spite of official policies, tobacco use in this group has not changed over the past decade, but there was an increase in the rate of those who received information during their training and of those who advised their patients of smoking risks.

Introducción. Los pediatras se encuentran en una posición estratégica para actuar en la prevención del tabaquismo. Objetivos. Estimar la prevalencia del tabaquismo en residentes de pediatría, analizar los factores asociados, describir su conducta preventiva y evaluar las diferencias en la última década. Métodos. Estudio transversal, por encuesta anónima, autoadministrada, que incluyó a residentes de pediatría de ocho hospitales de la Argentina. Se evaluaron el hábito de fumar, los factores asociados, la actitud hacia el consumo de tabaco de los pacientes o los padres, y se compararon los resultados con los de 2002. Resultados. De 448 encuestados, 20,1% fumaban. No hubo diferencias signifcativas entre fumadores y no fumadores respecto del sexo, los hijos, el número de guardias y el tener un jefe fumador. El padre fumador fue un factor de riesgo para tabaquismo solo en las mujeres (OR 1,98; IC 95% 1,09 a 3,61; p= 0,01). Los que vivían en pareja fumaban menos (OR 0,57; IC 95% 0,34 a 0,96; p= 0,03). Solo 18,1% refrieron tener una conducta activa frente al tabaquismo de los pacientes, sin diferencias entre fumadores y no fumadores. No hubo diferencia con la prevalencia del tabaquismo de 2002, pero sí mayor proporción de residentes que aconsejaban (32,4% contra 26,1%; p <0,01) y advertían a sus pacientes (37,7% contra 18,6%; p <0,01), y que recibieron información durante su formación (63,6% contra 39,8%; p <0,01). Conclusiones. El 20,1% de los médicos residentes de pediatría eran fumadores, siendo más frecuente en las mujeres con un padre fumador. La conducta activa frente al tabaquismo de los pacientes o sus padres fue muy baja. Pese a las políticas ofciales, el tabaquismo en este grupo no se ha modifcado en la última década, aunque aumentó la proporción de quienes recibieron información durante su formación, y aconsejan y advierten a los pacientes sobre sus riesgos.
