Background and study aims In the last decade, gastroenterologists have been attempting to use endoscopy to reproduce the great success of traditional surgical suture techniques. Despite recent advances, we still lack a reliable method that results in a permanent suture with minimal incidence of suture failure. This was an experimental study in pigs with an innovative technique that applied basic surgical concepts to endoscopy to evaluate the effectiveness of a novel suture technique. Methods The procedures were performed on six live pigs under general anesthesia. Endoscopic mucosal resection (EMR) first was performed in the stomach, exposing the submucosal or muscularis propria layers. A novel device, a transparent chamber cap (DASE), was developed to aspirate the gastric wall, allowing the sutures to reach deep layers. The aspiration was performed with a standard gastroscope to which the novel cap was distally attached. Three sutures aligned were defined as a plication. Each pig received two or three plications and was placed on a liquid diet for 14 days after the procedure. The pigs were sacrificed at 4 and 8 weeks and the sutures were reviewed. Results The technique was feasible in all animals. Of 16 plications, only one failed. One perforation occurred after EMR. There were no other complications or adverse events. Permanent fusion of the gastric wall was confirmed by histology in all cases. Conclusions This study showed that basic principles of surgery can be applied endoscopically to ensure a permanent suture with reduced chances of failure. These findings can help to pave the way for more effective bariatric endoscopic techniques.

BACKGROUND: Laparoscopic sleeve gastrectomy (LSG) is currently the most frequently performed bariatric procedure in Turkey. The goal of weight reduction surgery is not only to decrease excess weight, but also to improve obesity related comorbidities and quality of life (QoL). AIM: To evaluate the impact of LSG on patient quality of life, weight loss, and comorbidities associated with morbid obesity according to the updated BAROS criteria. METHODS: Eleven hundred thirty-eight adult patients were undergone to LSG by our bariatric surgery team between January 2013 and January 2016. A questionnaire (The Bariatric Analysis and Reporting Outcome System - BAROS) was published on social media. The data on postoperative complications were collected from hospital database. RESULTS: Number of respondants was 562 (49.4%). Six of 1138 patients(0.5%) had leakage. All patients who had leakage were respondants. The overall complication rate was 7.7%. After a mean period of 7.4±5.3 months(1-30), mean excess weight loss was 71.3±27.1% (10.2-155.4). The respondants reported 772 comorbidities. Of these, 162 (30%) were improved, and 420 (54.4%) were resolved. The mean scores for QoL were significantly increased after LSG (range, p<0.05 to <0.001). Of the 562 patients, 26 (4.6%) were classified as failures; 86 (15.3%) fair; 196 (34.9%) good; 144 (25.6%) very good, and 110 (19.6%) excellent results according to the updated BAROS scoring system. CONCLUSION: LSG is a highly effective bariatric procedure in the manner of weight control, improvement in comorbidities and increasing of QoL in short- and mid-term.

RESUMO Racional: A gastrectomia vertical laparoscópica (LSG) é atualmente o procedimento bariátrico mais frequentemente realizado na Turquia. O objetivo da operação de redução de peso não é apenas diminuir o excesso de peso, mas também melhorar as comorbidades e a qualidade de vida relacionadas à obesidade (QoL). Objetivo: Avaliar o impacto do LSG na qualidade de vida dos pacientes, perda de peso e comorbidades associadas à obesidade mórbida de acordo com os critérios BAROS atualizados. Métodos: Estudo não-randomizado de intervenção comportamental e de saúde pública. Um total de 1138 pacientes adultos foram submetidos a LSG entre janeiro de 2013 e janeiro de 2016. Um questionário (The Bariatric Analysis and Reporting Outcome System - BAROS foi utilizado. Os dados sobre complicações pós-operatórias foram coletados do banco de dados hospitalar. Resultados: Responderam ao questionário 562 (49,4%) pacientes. Seis de 1138 pacientes (0,5%) tiveram deiscência e todos estes responderam a pesquisa. A taxa geral de complicações foi de 7,7%. Após período médio de 7,4±5,3 meses (1-30), a perda média de excesso de peso foi de 71,3±27,1% (10,2-155,4). Os questionados relataram 772 comorbidades. Destes, 162 (30,0%) foram melhorados e 420 (54,4%) foram resolvidos.Os escores médios de QoL foram significativamente aumentados após LSG (p<0,05 a <0,001). Dentre os resultados dos 562 pacientes, 26 (4,6%) foram classificadas como falhas; 86 (15,3%) regular; 196 (34,9%) bom;144 (25,6%) muito bom; e 110 (19,6%) excelente de acordo com para o sistema de pontuação BAROS atualizado. Conclusão: O LSG é procedimento bariátrico altamente efetivo para controle de peso, melhora nas comorbidades e aumento da QoL em curto e meio prazos.

CONTEXT: The endoscopic gastric perforation is a consequence of some endoscopic procedures and now a way to manage abdominal organs. This is the reason why endoscopists are studying a safe endoscopic repair. OBJECTIVE: To evaluate an endoscopic closure method for the gastric opening in natural orifice transenteric surgery DESIGN: Short-term survival animal study. METHODS: Ten White Landrace pigs underwent a gastric perforation of 1.8 cm in diameter under general anesthesia. The opening was repaired with stitch assembled in a T-tag anchor placed through the gastric wall with a needle. A plastic transparent chamber, adapted to the endoscope tip protected the abdominal organs from the needle puncture outside the stomach. Six T-tags were placed in most cases and the stitches were tied with a metallic tie-knot, forming three sutures. The animals received liquids in the same operative day. One shoot antibiotic was used. The leakage test was performed with a forceps and by air distention. RESULTS: No complication was detected in the postoperative course. One month later the endoscopy revealed a scar and some suture material was observed in all animals. The antral anterior gastric wall was clear with few adhesions in the laparotomy performed in the same time. The adhesions were intense in an animal in which a cholecystectomy was performed before the repair. CONCLUSION: The endoscopic repair using T-tag and a protector chamber is feasible, easy to perform and safe. Further studies are needed to show the real value of this kind of procedure.

CONTEXTO: A perfuração gástrica pode ser consequência de alguns procedimentos endoscópicos, atualmente, produzida intencionalmente para acesso a alguns órgãos com o advento da cirurgia transluminal endoscópica por orifícios naturais. Esta é a razão para que os endoscopistas estudem uma maneira segura de reparar estas lesões por via endoscópica. OBJETIVO: Avaliar um novo método de fechamento das perfurações gástricas utilizadas para acesso ao NOTES. MODELO DE ESTUDO: Pesquisa em modelos animais com curto tempo de sobrevida. MÉTODO: Dez porcos da raça White Landrace, foram submetidos a perfuração gástrica de 1,8 cm, monitorizados e sob anestesia geral. A abertura gástrica foi reparada com dispositivo especial constituído por agulha em forma de T montada com fio (T-tag), inserida, por via endoscópica, nas bordas do ferimento perfurando a parede gástrica e fixando-se a ela como uma âncora. Uma câmara plástica protetora, especialmente desenvolvida, foi adaptada à ponta do endoscópio para proteção dos órgãos subjacentes. Seis T-tags foram inseridos na maioria dos casos e os pontos foram amarrados e fixados com auxílio de outro dispositivo metálico de contenção do nó denominado “tie-knot”, formando três suturas. Um teste de vazamento do tipo manobra do borracheiro era realizado ao término do procedimento. Os animais recebiam líquido no pós-operatório imediato. Foi usada profilaxia antibiótica. RESULTADOS: Não houve complicações. Um mês depois, a endoscopia de controle revelou cicatriz e alguns restos de sutura. A laparotomia, feita no mesmo tempo, revelou poucas aderências na face anterior do antro. CONCLUSÃO: O reparo endoscópico com T-tags e câmara protetora parece ser efetiva, fácil e segura. Estudos maiores e com maior tempo são necessários para confirmar estes resultados e a utilidade deste procedimento.

BACKGROUND: The risk of intraabdominal contamination is a critical consideration during most natural orifice translumenal endoscopic surgery (NOTES) procedures. OBJECTIVE: The objective of this study was to determine a safe and efficient pathway for the endoscope in a transgastric NOTES procedure. DESIGN AND SETTINGS: A pilot experimental study in live pigs was performed. METHODS: Five White Landrace pigs, weighing approximately 30-35 kg, underwent the placement of a device consisting of an expandable sheath, the distal portion of which was composed of a fully covered self-expanding metal stent, and an introducer made with an outer catheter, a pushing catheter, and an inner, guiding catheter. The sheath was attached to the stent by suturing it in place. The initial gastric opening was made by means of a needle knife papilotome with electrocoagulation, aimed to the anterior gastric wall. Then, it was dilated with an endoscopic 1.8-cm balloon. The set was introduced over a Savary guidewire. After the set placement, the outer tube was slowly retrieved. Finally, the delivery system was removed from the pig, leaving the entire endoscopic port in place. RESULTS: The expandable gastric port was placed without difficulty in all animals. Endoscope insertion into the expandable gastric port was very easily performed. The endoscope had a wide range of movement inside the peritoneal cavity. The gastric port sealed the gastric wall, avoiding gross contamination of the peritoneal cavity and maintaining the pneumoperitoneum without excessive inflation of the intestine. CONCLUSIONS: Use of a gastric port can minimize contamination of the peritoneal cavity due to the spillage of gastric contents during a transgastric NOTES procedure and can also facilitate performance of the procedure.

The interest in endoscopic treatment of obese patients is growing as a result of the technical development of endoscopic devices and because several studies have shown that most patients do not need the substantial weight loss afforded by bariatric surgery procedures. Therefore, this article presents the whole range of endoscopic procedures used to treat obese patients. The balloon devices are discussed in more detail because they are currently available in the world market and clinical data are available. This article also describes new techniques and devices in the investigational stage.

BACKGROUD: The elastic band ligation is the method of choice for treatment of esophageal varices. The action mechanism is a mechanical varices compression with thromboses. Based on this concept we developed a ligature method using pretied loop made with polyamide thread for the treatment of esophageal varices. OBJECTIVE: The present study describes and evaluates the feasibility of the treatment of esophageal varices by the ligature method using pretied loop made with polyamide thread and analyzes the local changes of the ligations and the results, concerning safety, efficiency and complications of this procedure. PATIENTS AND METHODS: Between March, 1998 and May, 2000, 58 patients with esophageal varices were treated with pretied loop, made with polyamide thread (26 patients with schistosomiasis, 11 with alcoholic cirrhosis, 9 with hepatitis C, 5 with hepatitis B, 4 of unknown etiology, 2 with hepatitis B and C, and one with Budd-Chiari syndrome/ 42 men and 16 women/ average age of 47,67 ± 13,12 years, range 16-74). A plastic tube was attached to the endoscope tip featured as an accessory working channel, allowing the pretied loop made with polyamide thread to be conducted to the esophagus facilitated by a flexible metallic tube, to perform the esophageal varices ligature. A total of 506 ligatures were done, distributed in 223 sections (average of 2,26 ± 1,08 ligature, varying from 1 to 6 per section). The sessions were perform with the interval of 15 days, until the complete eradication of the esophageal varices. The ecoendoscopy was used as a complementary method to evaluate the varices eradication RESULTS: The esophageal varices were treated successfully in all patients. The complete eradication of varices was achieved in 47 (81,03%) patients. In 37 (63,79%) patients the ligatures resulted in pseudopolyps. It was not identified systemic complications or obits. The ecoendoscopy showed thrombosis in the pseudopolyps...


RACIONAL: A ligadura com bandas elásticas tem se tornado o tratamento de escolha das varizes esofágicas, devido aos bons resultados e as menores taxas de complicações quando comparada à esclerose. Seu mecanismo de ação baseia-se na compressão mecânica do cordão varicoso com interrupção do fluxo sangüíneo e posterior trombose. Tendo como base este mecanismo de ação, desenvolveu-se um método alternativo no qual o cordão varicoso é ligado com uma alça confeccionada com fio de poliamida OBJETIVO: Avaliar a viabilidade do tratamento das varizes esofágicas por método de ligaduras com alças pré-atadas confeccionadas com fio de poliamida, analisando as alterações locais, a segurança, a eficácia e as complicações deste método de tratamento. CASUíSTICA E MÉTODOS: Entre março de 1998 e maio de 2000, 58 pacientes com varizes de esôfago foram tratados com ligaduras com alças pré-atadas, confeccionadas com fio de poliamida (26 esquistossomóticos, 11 com cirrose alcoólica, 9 com hepatite C, 5 com hepatite B, 4 de causa não esclarecida, 2 com hepatite B e C e 1 com síndrome de Budd-Chiari; 42 pacientes eram do sexo masculino e 16 do sexo feminino, com média de idade de 47,67 ± 13,12 anos). Confeccionou-se artesanalmente um canal acessório que acoplado à extremidade do endoscópio, permitia que alças pré-atadas feitas com fio de poliamida fossem conduzidas ao esôfago com auxílio de tubo metálico flexível para se proceder às ligaduras das varizes esofágicas. No total, 506 ligaduras foram realizadas, distribuídas por 223 sessões (média 2,26 ± 1,08 ligaduras por sessão). As sessões foram realizadas com intervalos de 15 dias até a completa erradicação das varizes esofágicas. A ecoendoscopia foi utilizada como método complementar para avaliar a erradicação das varizes em 10 doentes RESULTADOS: O procedimento foi realizado com sucesso em todos os pacientes. CONCLUSION: The study indicates that endoscopic treatment using polyamide thread ligature is a safe, efficient, accessible and low cost method for treatment of esophageal varices treatment, demonstrating a new aspect, that is the obliteration of varices without necrosis, by formation of pseudopolyps.

RESUMO

To avoid bleeding after colonoscopic polypectomy, several procedures are being used. We describe a new method that consists of a homemade nylon loop that is applied and tied to the stalk of the pedunculated polyp and after which a conventional polypectomy is done. We have used this method in 15 patients and no complication was observed. The procedure is simple; the loop may be opened to large diameters to make it easier to use for large polyps. It has a low cost and seems to be safe and effective to prevent bleeding after endoscopic polypectomy.

CONTEXT: Laryngeal sensitivity is important in the coordination of swallowing coordination and avoidance of aspiration. OBJECTIVE: To briefly review the physiology of swallowing and report on our experience with laryngeal sensitivity evaluation among patients presenting dysphagia. TYPE OF STUDY: Prospective. SETTING: Endoscopy Department, Hospital Sírio-Libanês. METHODS: Clinical data, endoscopic findings from the larynx and the laryngeal sensitivity, as assessed via the Flexible Endoscopic Evaluation of Swallowing with Sensory Testing (FEESST) protocol (using the Pentax AP4000 system), were prospectively studied. The chi-squared and Student t tests were used to compare differences, which were considered significant if p < or = 0.05. RESULTS: The study included 111 patients. A direct association was observed for hyperplasia and hyperemia of the posterior commissure region in relation to globus (p = 0.01) and regurgitation (p = 0.04). Hyperemia of the posterior commissure region had a direct association with sialorrhea (p = 0.03) and an inverse association with xerostomia (p = 0.03). There was a direct association between severe laryngeal sensitivity deficit and previous radiotherapy of the head and neck (p = 0.001). DISCUSSION: These data emphasize the association between proximal gastroesophageal reflux and chronic posterior laryngitis, and suggest that decreased laryngeal sensitivity could be a side effect of radiotherapy. CONCLUSIONS: Even considering that these results are preliminary, the endoscopic findings from laryngoscopy seem to be important in the diagnosis of proximal gastroesophageal reflux. Study of laryngeal sensitivity may have the potential for improving the knowledge and clinical management of dysphagia.

CONTEXTO: A sensibilidade da laringe é importante para coordenar a deglutição e prevenir a aspiração. OBJETIVO: Revisar os mecanismos fisiológicos de coordenação da deglutição e reportar a experiência do Hospital Sírio Libanês com a avaliação da sensibilidade da laringe nos pacientes com disfagia. TIPO DE ESTUDO: Prospectivo. LOCAL: Instituição privada (Departamento de Endoscopia do Hospital Sírio Libanês, São Paulo, Brasil), pacientes ambulatoriais. MATERIAL E MÉTODOS: Os dados clínicos, os achados endoscópicos na laringe e a sensibilidade da laringe avaliada pelo protocolo FEESST (Flexible Endoscopic Evaluation of Swallowing with Sensory Testing) utilizando o sistema Pentax AP4000 foram analizados prospectivamente. Diferenças foram comparadas pelo teste de t de Student ou pelo teste do chi-quadrado, sendo consideradas significantes diferenças com o p < ou = 0,05. RESULTADOS: 111 pacientes foram incluídos no estudo. Houve uma associação direta entre hiperplasia e hiperemia da região da comissura posterior com historia de globus histérico (p = 0,01) e regurgitação (p = 0,04); associação direta entre hiperemia da região da comissura posterior com sialorréia (p = 0,03) e associação inversa com xerostomia (p = 0,03); e associação direta entre déficit grave da sensibilidade da laringe e radioterapia prévia na cabeça e pescoço (p = 0,001). DISCUSSAO: Estes dados enfatizam a associação entre refluxo gastroesofágico proximal e a laringite posterior crônica; e sugerem que a radioterapia tem como um efeito colateral a redução da sensibilidade da laringe. CONCLUSÕES: Mesmo considerando que estes resultados são preliminares, o aspecto laringoscópico da laringe é importante para o diagnóstico do refluxo gastroesofágico proximal e o estudo da sensibilidade da laringe pode aumentar nosso conhecimento e melhorar o manejo clínico da disfagia.

Racional - A colangiopancreatografia retrógrada endoscópica desempenha, desde a sua introdução, papel cada vez mais importante no diagnóstico e tratamento dos distúrbios biliopancreáticos. Objetivo - Apresentar uma análise da utilização dos métodos endoscópicos alternativos de acesso à via biliar nas situações em que esta não foi alcançada pelo método convencional. Casuística e Método - Entre janeiro de 1997 e dezembro de 2000, 207 pacientes foram submetidos a colangiopancreatografia endoscópica retrógrada pelo método convencional, sendo que 20 destes também foram submetidos aos métodos alternativos. Resultados - O método convencional isolado obteve sucesso em 86,5% dos casos com 2,42% de complicações. A utilização dos métodos alternativos contribuiu significativamente para elevação desta taxa para 94,7%, sem aumento de complicação. Conclusões - Os métodos convencionais constituem procedimentos de eleição no acesso à via biliar pelo seu alto índice de sucesso e baixa morbidade. Os métodos alternativos constituem importante recurso diagnóstico e terapêutico nos casos de insucesso dos métodos convencionais.
