RESUMO

Centrifugal blood pumps can be used for treating heart failure patients. However, pump thrombosis has remained one of the complications that trouble clinical treatment. This study analyzed the effect of impeller shroud on the thrombosis risk of the blood pump, and predicted areas prone to thrombosis. Multi-constituent transport equations were presented, considering mechanical activation and biochemical activation. It was found that activated platelets concentration can increase with shear stress and adenosine diphosphate(ADP) concentration increasing, and the highest risk of thrombosis inside the blood pump was under extracorporeal membrane oxygenation (ECMO) mode. Under the same condition, ADP concentration and thrombosis index of semi-shroud impeller can increase by 7.3% and 7.2% compared to the closed-shroud impeller. The main reason for the increase in thrombosis risk was owing to elevated scalar shear stress and more coagulation promoting factor-ADP released. The regions with higher thrombosis potential were in the center hole, top and bottom clearance. As a novelty, the findings revealed that impeller shroud can influence mechanical and biochemical activation factors. It is useful for identifying potential risk regions of thrombus formation based on relative comparisons.

Extracorporeal centrifugal blood pumps are used to treat cardiogenic shock. Owing to the imbalanced excitation or initial assembly configurations, the variation in the impeller axial position has the potential to affect the blood pump performance. This study compared the hydrodynamics and hemolysis outcomes at different impeller axial positions via numerical simulations. The result shows that pressure difference of the blood pump decreased with increasing impeller axial position, with decreasing by 4.5% at a flow rate of 2 L/min. Under axial impeller motion close to the top pump casing, average wall shear stress and scalar shear stress reached their maximum values (64.2 and 29.1 Pa, respectively). The residence time in the impeller center hole and bottom clearance were extended to 0.5 s by increasing impeller axial position. Compared to the baseline blood pump, hemolysis index increased by 12.3% and 24.3% when impeller axial position is 2.5 and 4.0 mm, respectively. As a novelty, the findings reveal that the impeller axial position adversely affects hemolysis performance when the impeller is close to the pump casing. Therefore, in the development process of centrifugal blood pumps, the optimal axial position of the impeller must be defined to ensure hemodynamic performance.

The hollow fiber membrane modules act as dehumidifiers and regenerators to avoid gas-liquid entrainment problems in direct-contact dehumidification systems. A solar-driven hollow fiber membrane dehumidification experimental rig was designed to investigate its performance from July to September in Guilin, China. The dehumidification, regeneration, and cooling performance of the system between 8:30 and 17:30 are analyzed. The energy utilization of the solar collector and system is investigated. The results show that solar radiation has a significant influence on the system. The hourly regeneration of the system has the same trend as the temperature of solar hot water, which ranges from 0.13 g/s to 0.36 g/s. The regeneration capacity of the dehumidification system is always larger than the dehumidification capacity after 10:30, which increases the solution concentration and the dehumidification performance. Further, it ensures stable system operation when the solar radiation is lower (15:30-17:50). In addition, the hourly dehumidification capacity and efficiency of the system ranges from 0.15 g/s to 0.23 g/s and 52.4 to 71.3%, respectively, with good dehumidification performance. The COP of the system and solar collector have the same trend, in which their maximum values are 0.874 and 0.634, respectively, with high energy utilization efficiency. The solar-driven hollow fiber membrane liquid dehumidification system performs better in regions with larger solar radiation.

This study aimed to assess whether genetic variants of dendritic cell-associated C-type lectine-1 (Dectin-1), Toll-like receptor 2 (TLR2), Toll-like receptor 4 (TLR4), and myeloid differentiation primary response 88 (MyD88) influence the susceptibility to pulmonary invasive fungal disease (IFD) in patients with acute myeloid leukemia (AML) from a Chinese Han population. Eight single nucleotide polymorphisms (SNPs) of Dectin-1 (rs16910526, rs3901533, and rs7309123), TLR2 (rs5743708), TLR4 (rs4986790 and rs4986791) and MyD88 (rs4988453 and rs4988457) in the genomic DNA of 172 adult AML patients were genotyped. Pulmonary IFD was diagnosed as proven or probable according to the 2008 European Organization for Research and Treatment of Cancer/Invasive Fungal Infections Cooperative Group and the National Institute of Allergy and Infectious Diseases Mycoses Study Group (EORTC/MSG) consensus guidelines. SNPs that were significant in the univariate analysis were further analyzed using the multiple logistic regression analysis to determine their association with the occurrence of pulmonary IFD. The mRNA expression of Dectin-1 was detected according to the genotype by quantitative realtime PCR (qRT-PCR), and the correlation of this expression with the occurrence of pulmonary IFD in AML patients was analyzed. Two Dectin-1 intron SNPs (rs3901533 and rs7309123) were found to be significantly associated with the susceptibility to pulmonary IFD in AML patients in a Chinese Han population. Significant associations were noted between pulmonary IFD and Dectin-1 rs3901533 dominant model (G/T+G/G vs. T/T, OR: 2.158; 95% CI: 1.109-4.2, P=0.02), Dectin-1 rs3901533 G allele (OR: 2.201; 95% CI: 1.206-4.019, P=0.01), or Dectin-1 rs7309123 C allele (OR: 1.919; 95% CI: 1.047-3.518, P=0.03). There were no significant associations between pulmonary IFD and the remaining Dectin-1 SNPs (rs16910526), TLR2 (rs5743708), TLR4 (rs4986790 and rs4986791) or MyD88 (rs4988453 and rs4988457). In conclusion, two Dectin-1 SNPs (rs3901533 and rs7309123) are associated with increased susceptibility to pulmonary IFD in AML patients in a Chinese Han population.

OBJECTIVE: To explore the relationship between driver mutations and clinical characteristics in patients with Philadelphia chromosome (Ph) negative myeloproliferative neoplasms (MPN), so as to provide evidence for diagno-sis and treatment of the disease. METHODS: The clinical data of 410 patients with classic Ph negative MPN including 150 cases of polycythemia vera (PV), 188 cases of essential thrombocythemia (ET) and 72 cases of primary myelofibrosis (PMF) from January 2013 to December 2016 in Fujian Medical University Union Hospital were retrospectively analyzed. The PCR or DNA sequencing were used for JAK2 V617F, JAK2 exon12, CALR and MPL W515L/K mutation analyses, and follow-up information on patients was updated by direct phone call or follow-up in outpatient. RESULTS: Among the 410 patients with Ph negative MPN, 136 (33.2%) cases were asymptomatic at diagnosis. 389 cases were sequenced and JAK2 V617F was detected in 87.1% (122/140) of PV, 64.1% (118/184) of ET, 64.6% (42/65) of PMF; JAK2 exon 12 mutation in 1 case of PV; MPL W515L/K mutation in 1 case of ET and PMF, respectively; CALR mutation in 18(9.8%) cases of ET and 5 (7.7%) cases of PMF. JAK2 V617F mutated PV patients ocourred in older age: the white blood cell count, platelet count and incidence of splenomegaly were higher than JAK2-negative PV cases(P<0.05). Compared with JAK2 V617F mutated ET patients, CALR mutated ET cases displayed younger age, lower leukocyte count, higher platelet count and lower incidence of thrombosis; JAK2-negative ET cases had younger age, lower leukocyte count, lower hemoglobin level, higher platelet count and lower incidence of thrombosis(P<0.05). The incidence of splenomegaly in JAK2 V617F or CALR mutated PMF patients was both higher than that in JAK2-negative PMF cases, but the incidence of leukemia transformation in JAK2-negative PMF patients was higher than that in JAK2 V617F mutated cases (P<0.05). CONCLUSION: The types of driver mutations are closely related with the clinical features and prognosis in Ph- negative MPN patients.

RESUMO

Myeloproliferative neoplasm(MPN) is clonal hematopoietic stem cell disorder characterized by abnormal proliferation and expansion of one or more myeloid lineages. BCR-ABL-negative MPN includes polycythemia vera (PV), essential thrombocythemia (ET) and primary myelofibrosis (PMF). The mutations of JAK2, CALR and MPL genes are involved in the pathogenesis of MPN that provided a more complete molecular diagnostic standard for MPN. More and more new mutated genes related to prognosis of MPN were discovered in the past few years, at same time it was found that cytokines were also involved in the genesis and development of MPN. These provide a theoretical basis for the diagnosis, risk stratification and treatment management of MPN. In this article, the mechanisms of MPN-related cytokines and mutated genes in the genesis and development of disease and prognosis characteristics are summarized.

The influence of excessive inorganic nitrogen (NO3-, NH4+, NO2-) on the pH, inorganic carbon system and air-sea CO2 flux was studied in laboratory seawater simulation experiments. The result indicates that the excessive inorganic nitrogen significantly has an important influence on the inorganic carbon system in all conditions. At the NO3- addition groups, HCO3- and p (CO2) increase while pH and CO3(2-) decrease when the concentration of NO3- < or = 37.60 micromol x L(-1). But high concentrations > or = 188 micromol x L(-1) is just the reverse. The seawater changes from carbon source to carbon sink of atmospheric CO2 with NO3- addition increased. At the NH4+ addition groups, HCO3- and p (CO2) decrease while pH and CO3(2-) increase when the concentration of NH4+ < or = 25.20 micromol x L(-1). However, when the concentration of NH4+ > or = 126 micromol x L(-1), the concentrations of HCO3- and p (CO2) increase with NH4+ addition and the seawater changes from carbon sink to carbon source of atmospheric CO2 and the intensity of carbon source has a positive relationship with the concentration of NH4+. At the NO2- addition groups, the concentrations of HCO3- and p (CO2) decrease while pH and CO3(2-) increase with time scale and the seawater acts as a strong carbon sink with NO2- addition of 7.90 micromol x L(-1). While at the other NO2- concentration levels, the carbon sink is weak in seawater. The Chl-a concentration of nutrient addition groups compared with control (delta Chl-a) shows a significant negative correlation with delta p (CO2) (r = -0.87, p < 0.000 1, n = 16). The main controlling factor to the variation of inorganic carbon system is the adaptation of phytoplankton to different form and concentration of nitrogen.

Based on the 8 cruises data of surface seawater heavy metals and other related environmental factors from 1997 to 2004 of the South Yellow Sea (SYS), distribution patterns, mechanisms controlling the distributions and pollution levels of heavy metals (As, Cd, Cu, Hg, Pb, Zn) were studied with the data of 2003-10, and 8-year-fluctuation trends of heavy metals were also discussed. The average concentrations of heavy metals in surface seawater were 2.33, 0.078, 1.41, 0.003 6, 0.37, 6.21 microg/L respectively. The average concentrations showed a relatively stable trend in 8 years, except Zn's distinct upward tendency. The distribution patterns corresponded to the distance away from the coastline,that was, the content of heavy metals (except Pb) was low in central area while high inshore. Those were responses of human activity in the marginal sea, however, in some local areas, Pb was controlled by atmospheric deposition, Cd was relative to pH and salinity, Hg was related to organic carbon, Cu and Zn were influenced by runoff and drainage, and the resuspending of sediments played important roles on the content and distribution of As. Compared to the Marine Water Quality Standard of China, heavy metals indicated that SYS was a first class sea, and Ecological Risk Index analysis showed that SYS was a low ecological risk sea. In conclusion, seawater quality of heavy metals in SYS surface seawater is relatively good in general.
