BACKGROUND: Airway stenting is an established procedure for treating oncological emergencies in patients with airway disorders. In patients with airway hemorrhage, respiratory conditions may worsen during stenting. Bronchial artery embolization (BAE) is useful to prevent bleeding from the bronchus. We aimed to evaluate the efficacy and safety of airway stenting after BAE in patients with malignant airway disorders. METHODS: The medical records of all patients who underwent airway stenting following BAE at the National Hospital Organization Okayama Medical Center between 2016 and 2023 were retrospectively reviewed. RESULTS: Thirteen procedures (11 silicone Y stents, one hybrid stent, and one self-expandable metallic stent) were performed. The median duration from BAE to airway stenting was one day (range: 1-5 days). Nine patients experienced tumor shrinkage, and none experienced severe bleeding after BAE during the stent procedure. No other major complications were associated with the stent placement. The median survival time after stenting was 169 days (range; 24-1086). No serious complications caused by BAE, such as spinal cord infarction, were observed. CONCLUSIONS: Airway stent placement was safely performed after BAE without severe bleeding or acute respiratory failure. BAE, followed by airway stenting, is useful.

A woman in her mid-50s was admitted to our hospital with airway stenosis secondary to mediastinal lymph node enlargement. An AERO stent was placed under rigid bronchoscopy. Immediately after stent placement, tissue sampling was performed on the lymph nodes. Metastatic lesions were found to have an EGFR mutation (exon 19 deletion). Consequently, osimertinib treatment was initiated 15 days after stent placement. The tumour partially responded to osimertinib, and the airway stenosis improved. The patient underwent stent removal 66 days after stent placement. Our findings indicate that temporary oncological emergencies due to airway stenosis may be bridged by airway stenting.

A 75-year-old woman with stage IVB (cT2bN3M1b) lung adenocarcinoma was administered nivolumab, ipilimumab, carboplatin, and paclitaxel. Fourteen days after receiving chemotherapy, she experienced an impaired consciousness and a cerebrospinal fluid analysis revealed high protein levels and pleocytosis. She was diagnosed with nivolumab- and ipilimumab-induced encephalitis and was treated with corticosteroids which were tapered to 10 mg/day, with no symptom recurrence. She died 18 weeks after the initial presentation, as the cancer worsened. An autopsy showed encephalitis and CD8+ lymphocyte infiltration around the blood vessels. Thus, immune-related adverse events should be suspected and treatment should be initiated for patients presenting with an impaired consciousness when concurrently being treated with nivolumab and ipilimumab.

A 70-year-old woman with anti-aminoacyl-tRNA synthetase (ARS) antibody-positive interstitial lung disease (ARS-ILD) received daily medications and regular cyclophosphamide cycles for recurring exacerbations. Approximately four years after immunosuppression initiation, the patient was admitted for progressive dyspnea on exertion. Chest computed tomography (CT) findings were suggestive of acute exacerbation. Despite intensified immunosuppressive treatment, the radiographic findings worsened, and serum Krebs von den Lungen-6 (KL-6) levels increased. A bronchoalveolar lavage fluid (BALF) examination revealed amorphous globules and alveolar macrophages with eosinophilic granules. Owing to negative anti-GM-CSF antibody tests, a diagnosis of secondary pulmonary alveolar proteinosis (PAP) was established.

A 75-year-old woman with stage IVB (cT3N3M1c) extensive disease small-cell lung cancer was treated with carboplatin, etoposide, and atezolizumab. Ten days after pegfilgrastim initiation, during the second chemotherapy cycle, she experienced back pain. Contrast-enhanced computed tomography revealed soft tissue thickening around the descending aorta and brachiocephalic artery. She was diagnosed with atezolizumab and pegfilgrastim-induced large-vessel vasculitis (LVV) and was treated with prednisolone, which was tapered and discontinued after 14 weeks, with no symptom recurrence. LVV should be included in the differential diagnosis of patients with nonspecific body pain when pegfilgrastim and immune checkpoint inhibitors are used in combination.

Objective Airway stenting is an established procedure for treating various airway disorders. The AERO stent (Merit Medical Systems, South Jordan, UT, USA) is a fully covered self-expandable metallic stent approved for use in Japan in 2014. However, its effectiveness in treating malignant airway disorders in patients with a poor performance status remains unclear. Therefore, we investigated the safety and efficacy of the AERO stent in patients with malignant airway disorders and a poor performance status. Patients and Methods We retrospectively reviewed the medical records of all patients who underwent AERO stent placement at our institute between April 2016 and March 2022, and 21 patients underwent 25 procedures for malignant airway disorders. All AERO stenting procedures were performed using an over-the-wire delivery system with flexible and/or rigid bronchoscopy. Results Eighteen of the 21 patients (85.7%) had a poor general condition (Eastern Cooperative Oncology Group performance status 3 or 4). AERO stents were successfully placed in 23 of the 25 procedures and migrated in the remaining 2 cases. Complications occurred in 10 cases, with infection being the most common (3 cases). Fourteen patients (66.6%) showed an improvement in their performance status. In addition, 5 of the 6 intubated patients were extubated following AERO stenting, and 11 patients subsequently received anticancer treatment. Conclusion The placement of the AERO stent is useful in patients with a poor performance status, including those who are intubated and afflicted with malignant airway disorders.

Light-in-flight recording by holography (LIF holography) is an ultrafast imaging technique for recording light pulse propagation as a motion picture. In this study, we propose and demonstrate multiple motion picture recordings of light pulse propagation by use of LIF holography with angular multiplexing. We set incident angles of reference light pulses to remove the difficulty in adjusting the optical path length difference between an object light pulse and reference light pulses and the complexity of the optical system. In the experiment, by using LIF holography with angular multiplexing, we succeeded in recording a propagating light pulse as two motion pictures with durations of 129.6 ps without an inseparable superimposition of the reconstructed images. In addition, cross talk between the recorded images, noise caused by cross-terms in an image plane, and the number of motion pictures that can be recorded are discussed.

A 73-year-old woman in complete remission from localized small-cell lung cancer associated with Lambert-Eaton myasthenic syndrome (LEMS) 22 years earlier was referred to our hospital and diagnosed with non-small-cell lung cancer. After three courses of pembrolizumab, an immune checkpoint inhibitor, the patient complained of muscle weakness, fatigue, ptosis, and dysarthria. The anti-voltage-gated calcium channel antibody level was elevated, and waxing was observed on a high-frequency repetitive stimulation test using an electromyogram. We diagnosed her with recurrence of LEMS as an immune-related adverse event (irAE) induced by pembrolizumab. After intravenous immunoglobulin therapy, the patient's symptoms improved, and she was discharged.

A multi-view phase measurement system based on single-shot phase-shifting digital holography is proposed to dynamically obtain three-dimensional (3-D) information of an unstable temperature field. The proposed system consists of a laser, three polarization imaging cameras, and the corresponding optical components. The laser beam emitted from the laser is separated by the fibers into three pairs that contain three object beams and three reference beams. The object beams pass through the object in three different directions and interfere with the reference beams at the image sensor plane respectively. The recording of the three cameras is triggered simultaneously, which enables the phase measurement of dynamic objects from different viewpoints. We successfully measured the 3-D distributions of an unstable temperature field in the experiments with the proposed system.

Ultrafast imaging techniques involving light propagation, which can record light-pulse propagation as a motion picture, are commonly applied in various fields. However, conventional ultrafast imaging techniques cannot obtain multiple motion- pictures with an ultrashort time difference. In this Letter, we propose an imaging technique to obtain double motion-pictures of propagating light pulses with an ultrashort time difference. To record double motion-pictures of propagating light pulses without superposition of the motion pictures, we employ a space-division multiplexing technique for recording holograms. Also, we construct and introduce an optical delay setup for generating light pulses with an ultrashort time difference. In the experimental demonstration of the proposed technique, we observe the first and second light propagations for a duration of 6.9 ps with a 1.78-ps time difference.

An 18-year-old man was admitted to our hospital with pneumonia 4 days after he initiated vaping. The patient did not show improvement after ceftriaxone and azithromycin treatment. The cell count of the bronchoalveolar lavage fluid (BALF) revealed 64% eosinophils and 18% lymphocytes. Based on the BALF findings, the patient met the current diagnostic criteria and was diagnosed with vaping-induced acute eosinophilic pneumonia (AEP). AEP caused by nicotine-free vaping is rare in Japan. Thus, in cases of AEP, the patient's history of cigarette smoking as well as vaping should be considered.

We propose a numerical simulation method of the hologram-recording process for light-in-flight recording by holography (LIF holography) based on fast Fourier transform (FFT) to improve the efficiency of the simulation. Because it is crucial to consider the difference in the optical-path length between the object and reference light pulses, we modify a point-spread function by considering the optical-path lengths of the object and reference light pulses and whether both pulses interfere with each other in LIF holography. The computational time was shortened by 5.5×105 times for the 4,096×4,096 resolution of the hologram using the proposed method. We evaluate the proposed method by calculating the root mean square error (RMSE) of the reconstructed holographic images. The RMSEs were relatively small considering the effect of speckle noise; these results effectively demonstrate the validity of the proposed method. Moreover, we reconstruct the moving pictures of light pulse propagation from the hologram generated by the proposed method. We compare the simulation and experimental results, and succeed in qualitatively demonstrating the validity of the proposed method.

We experimentally demonstrate a motion picture imaging technique that can record a magnified image of light pulse propagation with extending the recordable time of digital light-in-flight recording by holography. We constructed an optical system that achieves a recordable time extension and an observation of a magnified image of light pulse propagation. As a result, we experimentally succeeded in recording light pulse propagation with a 7.45 magnification rate with extending the recordable time. The recordable time of the motion picture was 714 fs, which is twice that of the conventional one.

We propose a simultaneous imaging technique of both sound propagations and spatial distribution of acoustic frequencies. We experimentally demonstrated the proposed technique for the acoustic waves of frequencies 39,500 and 40,500 Hz, which have close sound pressure. The sounds were recorded at the framerate of 100,000 fps by parallel phase-shifting digital holography. To obtain the distribution of the acoustic frequencies, the short-time Fourier transform analysis was applied. The simultaneous imaging was carried out by assigning the frequencies and the pixel values of the phase-difference images to the components of HSL color space. The images obtained by the proposed technique represent the frequencies with the hue in addition to the sound propagations with the luminance. We succeeded in imaging the spatiotemporal evolution of the spatial frequencies of the sounds.

This study determined the influence of intravenous (i.v.) oncolytic vaccinia virus mpJX-594 (mpJX) on antitumor activity of anti-programmed death receptor-1 antibody (aPD1) in functional and metastatic pancreatic neuroendocrine tumors (PanNETs). One i.v. dose of mpJX, engineered for mice with the same plasmid design as clinical virus Pexa-Vec, was administered alone or with repeated dosing of aPD1 (mpJX+aPD1) to two contrasting genetic models of PanNET: one developing benign insulin-secreting tumors (RIP1-Tag2;C57BL/6J mice) and the other developing liver metastases (RIP1-Tag2;AB6F1 mice). Experiments revealed that aPD1 had synergistic actions with mpJX on CD8+ T cell and natural killer (NK) cell influx, apoptosis, and suppression of proliferation in PanNETs. After mpJX+aPD1, the 53-fold increase in apoptosis (5 days) and 85% reduction in proliferation (20 days) exceeded the sum of mpJX and aPD1 given separately. mpJX+aPD1 also stabilized blood insulin and glucose in mice with functional PanNETs, regressed liver metastases in mice with aggressive PanNETs, and prolonged survival of both. The findings revealed that mpJX+aPD1 converted "cold" PanNETs into immunogenic tumors with widespread cytotoxic T cell influx, tumor cell killing, and suppression of proliferation. Reduction of tumor insulin secretion from functional PanNETs prolonged survival, and anti-metastatic actions on aggressive PanNETs reduced the metastatic burden to less than before treatment. The findings support the efficacy of the vaccinia virus with aPD1 for functional and metastatic PanNETs.

A 41-year-old man with exertional dyspnea was referred to our hospital. Chest computed tomography (CT) showed a pulmonary arteriovenous malformation (PAVM) in the left lingular lobe, and magnetic resonance imaging showed a brain abscess. After antimicrobial therapy, the patient underwent thoracoscopic lingulectomy of the PAVM. Pathological examination revealed lung metastases of papillary thyroid cancer (PTC) that were undetectable by CT. The patient underwent total thyroidectomy and D2b lymphadenectomy for the PTC (the pathological stage was T1bN2M1, Stage II). After surgery, the patient received 100 mCi of 131Iodine; post-treatment scans revealed only neck (remnant) uptake and the patient continued with thyroid hormone replacement therapy. To the best of our knowledge, this is the first report of a case of combined PAVM and occult lung metastases of PTC. Clinicians should remember that they may detect micro lung metastases of any cancer when investigating resected lung specimens.

Spatiotemporal information about light pulse propagation obtained with femtosecond temporal resolution plays an important role in understanding transient phenomena and light-matter interactions. Although ultrafast optical imaging techniques have been developed, it is still difficult to capture light pulse propagation spatiotemporally. Furthermore, imaging through a three-dimensional (3-D) scattering medium is a longstanding challenge due to the optical scattering caused by the interaction between light pulse and a 3-D scattering medium. Here, we propose a technique for ultrafast optical imaging of light pulses propagating inside a 3D scattering medium. We record an image of the light pulse propagation using the ultrashort light pulse even when the interaction between light pulse and a 3-D scattering medium causes the optical scattering. We demonstrated our proposed technique by recording converging, refracted, and diffracted propagating light for 59 ps with femtosecond temporal resolution.

Oncolytic vaccinia viruses have promising efficacy and safety profiles in cancer therapy. Although antitumor activity can be increased by manipulating viral genes, the relative efficacy of individual modifications has been difficult to assess without side-by-side comparisons. This study sought to compare the initial antitumor activity after intravenous administration of five vaccinia virus variants of the same Western Reserve backbone and thymidine kinase gene deletion in RIP-Tag2 transgenic mice with spontaneous pancreatic neuroendocrine tumors. Tumors had focal regions of infection at 5 days after all viruses. Natural killer (NK) cells were restricted to these sites of infection, but CD8+ T cells and tumor cell apoptosis were widespread and varied among the viruses. Antitumor activity of virus VV-A34, bearing amino acid substitution A34K151E to increase viral spreading, and virus VV-IL2v, expressing a mouse IL2 variant (mIL2v) with attenuated IL2 receptor alpha subunit binding, was similar to control virus VV-GFP. However, antitumor activity was significantly greater after virus VV-A34/IL2v, which expressed mIL2v together with A34K151E mutation and viral B18R gene deletion, and virus VV-GMCSF that expressed mouse GM-CSF. Both viruses greatly increased expression of CD8 antigens Cd8a/Cd8b1 and cytotoxicity genes granzyme A, granzyme B, Fas ligand, and perforin-1 in tumors. VV-A34/IL2v led to higher serum IL2 and greater tumor expression of death receptor ligand TRAIL, but VV-GMCSF led to higher serum GM-CSF, greater expression of leukocyte chemokines and adhesion molecules, and more neutrophil recruitment. Together, the results show that antitumor activity is similarly increased by viral expression of GM-CSF or IL2v combined with additional genetic modifications.
