RESUMO

AIM: To study the possible hematogenic route of dissemination of Chlamydia trachomatis and to analyze efficacy of methods of pathogen detection in clinical specimens (sera and scraping material). MATERIALS AND METHODS: Cultural method, electron microscopy, real-time PCR, immunofluorescent assay. RESULTS: C. trachomatis was detected in blood by using 2 tests (culture and PCR) in 95.2% of patients with confirmed Chlamydia infection. Chlamydia isolated from blood had infectious properties that could point to the presence of weakly studied hematogenic route of dissemination of C. trachomatis in host's organism. Study of diagnostic value of pathogen detection in serum showed that in case of chronic diseases of urogenital tract as well as extragenital diseases, rate of C. trachomatis detection in serum was significantly higher (61.1% of cases compared to 16.7% in scraping material). CONCLUSION: It is the first time when data about possible circulation of C. trachomatis in blood of patients was obtained. Detection of C. trachomatis in serum of patients with chronic and complicated forms of chlamydiosis provides essentially new approach for direct identification of the pathogen irrespectively from localization of infection's locus.

During the last STDs'epidemic in Russia (1994 - 2004) over 2.7 min people have been infected with syphilis. At present the structure of syphilis morbidity is characterized by 37% of latent forms, including 1.5% late latency. The increased level of late latency may result from: an ever-growing number of those with asymptomatic syphilis; disorderly and self-dependent usage of antibacterial preparations; spread of the virus chronic infections (herpes, hepatitis B, C) altering the macroorganism immune response; alcohol and drug abuse which decreases the efficacy of specific therapy. In general, chronization of the syphilitic infection may be caused by antigenically inert treponemal cell surface; paucity of outer membrane protein; residence of treponemas within an immunoprotective niche; uncompleted phagocytosis of treponemes with macrophages. Syphilis remains an "infectio magna" and demands thorough attention to all diagnostic and therapeutic procedures on every stage of the disease.

RESUMO

The results of the immunophoretic analysis of H. pylori antigenic structure are presented. Along with H. pylori O-antigen, specific surface antigen of acidic nature, relatively thermolabile with characteristics similar to those of K-antigens, was detected. A diagnosticum based on the coagglutination test for the rapid detection of H. pylori specific antigens in the patients' biological fluids (coprofiltrate, saliva) by the noninvasive method was developed. The circulation mode of H. pylori in human body and its correlation with the clinical symptoms of the disease was shown. H. pylori antigens were very frequently detected (in 65 - 83% of cases) in patients with acute bacterial enteric infections: shigellosis, salmonellosis, yersiniosis, campylobacteriosis. The newly developed method seems to be promising for the rapid diagnosis of H. pylori associated infections, for the prolonged monitoring and evaluation of the effectiveness of antibacterial therapy.

RESUMO

The influence of amyxin, the Russian oral analog of tilorone, on the human interferon and immune status has been evaluated. As revealed by this investigation the administration of amyxin has produced a rise in the content of serum interferon, an increase in the capacity of leukocytes and lymphocytes for synthesizing alpha- and gamma-interferon, the activation of NK and phagocytizing cells of peripheral blood. No essential influence of amyxin on the amount of B and T lymphocytes, their subpopulations and the levels of the main classes of immunoglobulins has been established.

Status of tears was studied in patients with penetrating wounds to the eyes using polarization optic microscopy to assess the type of morphological types of crystals, measurements of secretory IgA to assess local immunity status, and screening methods to estimate protein level from the relative coefficient. A total of 122 patients with penetrating corneal and corneoscleral wounds were examined, 48 of these without clinical signs of infection and 74 with clinical manifestations of an infection. For control 88 subjects without somatic diseases or ocular diseases were examined. Crystallography revealed predominance of two morphologic types, Nos. 3 and 5, whose crystal structures were taken as the normal types. Their detection rate was reduced in penetrating wounds to the eye. Secondary morphologic types Nos. 14 and 16 reliably predominated in "pure" injuries and a secondary morphologic type 14 M in patients with developing infection (in 19 patients this type predominated during the preclinical form of infection of the eye). Increased IgA levels were observed in patients with noninfected injuries and reduced ones during infection. An infection was found associated with reduced protein level.
