Resumen Objetivo: presentar los aprendizajes de una sistematización como método, para la construcción de conocimiento basado en la experiencia vivida por los propios investigadores de varios proyectos sobre la educación en crianza, en el campo de la salud pública, llevado a cabo en la vereda Granizal, Antioquia, durante el período 2013-2019. Materiales y método: se empleó la sistematización de experiencias y los participantes fueron los mismos investigadores. Se inició con la organización de los materiales, los diarios de campo de los círculos de investigación temática y relatorías de las reuniones del equipo de investigación y de los actores comunitarios. Se siguió con la elaboración de una matriz con la recuperación histórica. Y, finalmente, de las reflexiones grupales, surgieron categorías y relaciones para la construcción teórica de visión de conjunto. Resultados: se presenta la propuesta teórica de la sistematización y los procedimientos para implementarla, orientada por cinco momentos: vivir la experiencia, formular un plan de sistematización, recuperar el proceso vivido, las reflexiones de fondo y los puntos de llegada. Se destacan cuatro rasgos de la sistematización: una investigación creativa; basada en un proceso de construcción colectiva; para aprender de la práctica y construir conocimiento; y como escenario transformador de esta y de quienes la realizaron. Conclusión: la "sistematización de la sistematización" permitió hacer conciencia sobre la propia praxis investigativa. Muestra la comprensión del grupo acerca de la conceptualización y metodología de esta perspectiva investigativa. Trasciende la recopilación y organización de la información del proceso vivido en la medida en que la reconstrucción de la experiencia es una comprensión más amplia que transforma la práctica de los sujetos involucrados.


Abstract Objective: to present the learning of a systematization as a method, for the construction of knowledge based on the experience lived by the researchers of several projects on parenting education in the field of public health carried out in the rural settlement of Granizal, Antioquia, during the period 2013-2019. Materials and method: the systematization of experiences was used and the participants were the researchers. It began with the organization of the materials, the field diaries of the thematic research circles and reports of the meetings of the research team and community actors. It was continued with the elaboration of a matrix with the historical recovery. And, finally, reflections, categories and relationships emerged from the group for the theoretical construction of the overall vision. Results: the theoretical proposal of the systematization and the procedures to implement it are presented, guided by five moments: living the experience, formulating a systematization plan, recovering the lived process, the background reflections and the arrival points. Four features of systematization stand out: creative research; research based on a process of collective construction; learning from practice and building knowledge; and as a transforming scenario of this research and of those who carried it out. Conclusion: the "systematization of the systematization" allowed raising awareness of the research praxis itself. It shows the understanding of the group on conceptualization and methodology of this research perspective. It transcends the collection and organization of the information of the process lived to the extent that the reconstruction of the experience is a broader understanding that transforms the practice of the subjects involved.


Resumo Objetivo: apresentar as aprendizagens de uma sistematização como método, para a construção de conhecimento baseado na experiência vivida pelos próprios pesquisadores de vários projetos sobre a educação em criação, no campo da saúde pública, levado a cabo no povoado Granizal, Antioquia, durante o período 2013-2019. Materiais e método: utilizou-se a sistematização de experiências os participantes foram os mesmos pesquisadores. Iniciou-se com a organização dos materiais, os diários de campo dos círculos de pesquisa temática e relatorias das reuniões da equipe de pesquisa e dos atores comunitários. Conseguiu-se com a elaboração de uma matriz com a recuperação histórica. E, finalmente, das reflexões grupais, surgiram categorias e relações para a construção teórica de visão de conjunto. Resultados: apresenta-se a proposta teórica da sistematização e os procedimentos para implementá-la, orientada por cinco momentos: viver a experiência, formular um plano de sistematização, recuperar os processos vividos, as reflexões de fundo e os pontos de chegada. Destacam-se quatro rasgos da sistematização: uma pesquisa criativa; baseada em um processo de construção coletiva; para aprender da prática e construir conhecimento; e como cenário transformador desta e de quem a realizaram. Conclusão: a "sistematização da sistematização" permitiu fazer consciência sobre a própria práxis pesquisaria. Amostra a compreensão do grupo acerca da conceptualização e metodologia desta perspectiva pesquisaria. Trascende a recopilação e organização da informação do processo vivido na medida em que a reconstrução da experiência é uma compreensão mais ampla que transforma a prática dos sujeitos envolvidos.

Hypophysiotropic thyrotropin-releasing hormone (TRH) neurons function as metabolic sensors that regulate the thyroid axis and energy homeostasis. Less is known about the role of other hypothalamic TRH neurons. As central administration of TRH decreases food intake and increases histamine in the tuberomammillary nuclei (TMN), and TMN histamine neurons are densely innervated by TRH fibers from an unknown origin, we mapped the location of TRH neurons that project to the TMN. The retrograde tracer, cholera toxin B subunit (CTB), was injected into the TMN E1-E2, E4-E5 subdivisions of adult Sprague-Dawley male rats. TMN projecting neurons were observed in the septum, preoptic area, bed nucleus of the stria terminalis (BNST), perifornical area, anterior paraventricular nucleus, peduncular and tuberal lateral hypothalamus (TuLH), suprachiasmatic nucleus and medial amygdala. However, CTB/pro-TRH178-199 double-labeled cells were only found in the TuLH. The specificity of the retrograde tract-tracing result was confirmed by administering the anterograde tracer, Phaseolus vulgaris leuco-agglutinin (PHAL) into the TuLH. Double-labeled PHAL-pro-TRH boutons were identified in all subdivisions of the TMN. TMN neurons double-labeled for histidine decarboxylase (Hdc)/PHAL, Hdc/Trh receptor (Trhr), and Hdc/Trh. Further confirmation of a TuLH-TRH neuronal projection to the TMN was established in a transgenic mouse that expresses Cre recombinase in TRH-producing cells following microinjection of a Cre recombinase-dependent AAV that expresses mCherry into the TuLH. We conclude that, in rodents, the TRH innervation of TMN originates in part from TRH neurons in the TuLH, and that this TRH population may contribute to regulate energy homeostasis through histamine Trhr-positive neurons of the TMN.

INTRODUCTION: Introduction: cervical cancer is the second most common cancer in women in the world. It is associated with a high body mass index. However, the phase angle has not been determined in women with cervical cancer. Electrical vector bioimpedance has been validated to assess body composition, nutritional status, and cell membrane integrity in cancer patients using phase angle. Objetive: to evaluate phase angle, body composition by electrical vector bioimpedance, and dietary intake in women with cervical diagnosis who are users of a second-level care hospital in San Luis Potosí, Mexico. Methods: an observational, cross-sectional, and analytical study. Seventy women with a diagnosis of cervical cancer were studied in the oncology service. Phase angle was measured, and the frequency of food from the Nutritional Habits Assessment System and a questionnaire to measure socioeconomic level were applied. Results: the data of the phase angle in a mean of 4.66 ° ± 0.87 ° with a range of 2.9 ° to 6.2 ° In a multiple linear regression model formed for phase angle as a dependent variable, and evolution of the cervical cancer, body mass index, skeletal muscle mass, extracellular water, fructose, saturated fatty acids, and trans fatty acids as independent variables, had an R2 value of 0.748 with a p-value of 0.001. Conclusion: women with cervical cancer have integrity damage of the cell membrane with a lower phase angle than the reference population.


INTRODUCCIÓN: Introducción: el cáncer cervicouterino es el segundo cáncer más común en las mujeres en el mundo. Se asocia a un índice de masa corporal elevado. Sin embargo, no se ha determinado el ángulo de fase en las mujeres con cáncer cervicouterino. La bioimpedancia vectorial eléctrica está validada para evaluar la composición corporal, el estado nutricional y la integridad de la membrana celular en pacientes con cáncer mediante el ángulo de fase. Objetivo: evaluar el ángulo de fase, la composición corporal por bioimpedancia vectorial eléctrica y el consumo dietético en mujeres con diagnóstico de cáncer cervicouterino usuarias de un hospital de segundo nivel de atención en San Luis Potosí, México. Métodos: estudio observacional, transversal y analítico. Se estudiaron 70 mujeres con diagnóstico de cáncer cervicouterino en el servicio de oncología. Se realizó la medición del ángulo de fase y se aplicó la frecuencia de alimentos del Sistema de Evaluación de Hábitos Nutricionales, así como un cuestionario para medir el nivel socioeconómico. Resultados: el ángulo de fase presentó una media de 4,66 ° ± 0,87 ° con un rango de 2,9 ° a 6,2 °. El modelo de regresión lineal múltiple formado para el ángulo de fase como variable dependiente y la evolución del cáncer cervicouterino, el índice de masa corporal, la masa de músculo esquelético, el agua extracelular, la fructosa, los ácidos grasos saturados y los ácidos grasos trans como variables independientes, tuvo un valor de R2 de 0,748 con un valor de p de 0,001. Conclusión: las mujeres con cáncer cervicouterino mostraron daños en la integridad de la membrana celular, con un ángulo de fase menor que el de la población de referencia.

The sudden outbreak of coronavirus disease 2019 (COVID-19) revealed the need for fast and reliable automatic tools to help health teams. This paper aims to present understandable solutions based on Machine Learning (ML) techniques to deal with COVID-19 screening in routine blood tests. We tested different ML classifiers in a public dataset from the Hospital Albert Einstein, São Paulo, Brazil. After cleaning and pre-processing the data has 608 patients, of which 84 are positive for COVID-19 confirmed by RT-PCR. To understand the model decisions, we introduce (i) a local Decision Tree Explainer (DTX) for local explanation and (ii) a Criteria Graph to aggregate these explanations and portrait a global picture of the results. Random Forest (RF) classifier achieved the best results (accuracy 0.88, F1-score 0.76, sensitivity 0.66, specificity 0.91, and AUROC 0.86). By using DTX and Criteria Graph for cases confirmed by the RF, it was possible to find some patterns among the individuals able to aid the clinicians to understand the interconnection among the blood parameters either globally or on a case-by-case basis. The results are in accordance with the literature and the proposed methodology may be embedded in an electronic health record system.

Resumen (analítico) Se presenta la experiencia de un proceso educativo orientado a promover transformaciones en las personas y sus condiciones de vida, a partir de una investigación-acción sobre crianza. Durante dos años se realizaron 40 círculos de investigación temática con mujeres desplazadas por la violencia en un asentamiento periférico colombiano, orientados por la educación popular. Con la información registrada en diarios de campo se hizo un análisis hermenéutico. Los temas generadores fueron machismo, violencia, identidad, sexualidad, afecto, autoridad, norma, castigo, oportunidades, sueños, amor, organización social, entre otros. Partiendo de situaciones límite se construyeron códigos narrativos, los cuales permitieron configurar una problematización. La investigación temática permitió realizar una educación en crianza alternativa y constituyó un proceso pedagógico que motivó cambios en la vida de las participantes.


Abstract (analytical) This paper presents the experience of a child rearing educational process. This work aimed to promote transformations in people and their living conditions and is based on Action Research for child rearing. For two years, 40 Thematic Research circles were held in a peripheral Colombian settlement. The circles were implemented with women displaced by violence and guided using the principles of popular education. A hermeneutical analysis was conducted with information recorded in field diaries. The main topics identified were: sexism, violence, identity, sexuality, affection, authority, norms, punishment, opportunities, dreams, love, social organization and others. Narrative codes were constructed to problematize this data. This Thematic Research facilitated an alternative child rearing education and was an educational process that motivated changes in the lives of participants.


Resumo (analítico) Este artigo apresenta a experiência de um processo educacional que procura promover impacto e mudanças nas pessoas e nas suas condições de vida, com base numa Pesquisa-Ação sobre o cuidado. Durante dois anos, foram realizados 40 círculos de Investigação Temática com mulheres deslocadas pela violência num assentamento periférico colombiano, orientado pela educação popular. Foi efetuada uma análise hermenêutica com as informações registadas em diários de campo. Emergiram temas como o machismo, a violência, a identidade, a sexualidade, o afeto, a autoridade, a norma, a punição, as oportunidades, os sonhos, o amor, a organização social, entre outros. Com as situações-limites, foram construídos códigos narrativos para definir a problemática. A Investigação Temática permitiu uma educação sobre um cuidado alternativo e foi um processo educativo que motivou mudanças na vida dos participantes.

Breast cancer (BRCA) is the most frequent cancer type that afflicts women. Unfortunately, despite all the current therapeutic strategies, many patients develop chemoresistance hampering the efficacy of treatment. Hence, an early indicator of therapy efficacy might aid in the search for better treatment and patient survival. Although emerging evidence indicates a key role of the purinergic receptors P2X7 and A2A in cancer, less is known about their involvement in BRCA chemoresistance. In this sense, as the chemotherapeutic treatment stimulates immune system response, we evaluated the expression and function of P2X7 and A2A receptors in CD8+ T cells before and four months after BRCA patients received neoadjuvant chemotherapy. The results showed an increase in the levels of expression of P2X7 and a decrease in the expression of A2A in CD8+ T cells in non-chemoresistant (N-CHR) patients, compared to chemoresistant (CHR) patients. Interestingly, in CHR patients, reduced expression of P2X7 occurs along with a decrease in the CD62L shedding and the production of IFN-γ. In the case of the A2A function, the inhibition of IFN-γ production was not observed after chemotherapy in CHR patients. A possible relationship between the modulation of the expression and function of the P2X7 and A2A receptors was found, according to the molecular subtypes, where the patients that were triple-negative and human epidermal growth factor receptor 2 (HER2)-enriched presented more alterations. Comorbidities such as overweight/obesity and type 2 diabetes mellitus (T2DM) participate in the abnormalities detected. Our results demonstrate the importance of purinergic signaling in CD8+ T cells during chemoresistance, and it could be considered to implement personalized therapeutic strategies.

Electrical burn injuries are one of the most severe forms of trauma. This study aims to investigate the infection complications in electrical burn patients in a referral hospital in Mexico City. A longitudinal retrospective study was conducted, involving electrical burn patients admitted from April 2011 to December 2016. Demographic and clinical data including type of electric burns, infection complications, and mortality was sought. Data were collected at admission and daily until discharge. Number and type of infections and microorganism isolations were sought. Risk factors for death were analyzed. A total of 111 patients were included, of which 96.4% were males, mean age of 31.6±16.22, most injuries were high voltage associated. The total body surface area average was 27.8% ± 19.63. The overall infection rate was 72.9 cases per 100 patients. Mortality was observed in 4 (3.6%) patients. About 59.1% (443/749) had growth for Gram-negative bacteria. Multidrug-resistant Pseudomonas aeruginosa was the most frequent microorganism isolated. Fungi were present in 4.9% of cases. Electrical burn injuries occurred in young males in our study. Infection was frequent, most of them caused by Gram-negative rods with an important rate of antimicrobial resistance; however, an important microbial diversity was present.

El agua de consumo humano y su calidad son determinantes para la salud pública. Esta revisión pretende recopilar y analizar información acerca de la relación entre la enfermedad diarreica en niños menores de cinco (5) años y la contaminación de las fuentes de agua subterránea. Se consultaron las bases: PubMed, ScienceDirect, SpringerLink, SciELO y Google Scholar, sin limitación en fechas de publicación; utilizando los descriptores: agua subterránea, diarrea, enfermedad gastrointestinal infantil, contaminación microbiana, calidad del agua, diarrea infantil, agua potable, técnicas moleculares y técnicas bioquímicas, analizándose un total de ciento sesenta y nueve (169) publicaciones. Se encontró relación entre la contaminación microbiana del agua subterránea y la diarrea infantil. El agua subterránea se contamina debido a fugas de fosas sépticas, métodos inadecuados de manejo de desechos y escorrentías de agua de lluvia, determinando la prevalencia de diarrea infantil. De allí, la importancia de monitorear la calidad del agua como factor de riesgo, con la detección y cuantificación de bioindicadores, mediante métodos rutinarios y novedosos, e incorporar intervenciones dirigidas a mejorar la accesibilidad a fuentes de agua controladas y la educación sanitaria en la búsqueda de asegurar la protección del agua y la disminución en la prevalencia de la diarrea infantil. Water for human consumption and its quality are determinants for public health. This review aims to collect and analyze information about the relationship between diarrheal disease in children under five (5) years of age and contamination of groundwater sources. The bases: PubMed, ScienceDirect, SpringerLink, SciELO and Google Scholar, without limitation on publication dates, using the descriptors: groundwater, diarrhea, childhood gastrointestinal disease, microbial contamination, water quality, childhood diarrhea, drinking water, molecular techniques and biochemical techniques, were consulted, analyzing a total of one hundred sixty-nine (169) publications. A relationship was found between microbial contamination of groundwater and childhood diarrhea. Groundwater is contaminated due to septic tank leaks, inadequate methods of waste management and rainwater runoff, determining the prevalence of childhood diarrhea. From there, the importance of monitoring water quality as a risk factor, with the detection and quantification of bioindicators, through routine and novel methods, and incorporating interventions aimed at improving accessibility to controlled water sources and health education in the search to ensure water protection and the decrease in the prevalence of childhood diarrhea. Abstract Health professionals often teach about child rearing based on traditional and transmissionist educational models that prescribe knowledge and meanings without considering whether families have the means to carry out the type of child rearing they value. This article discusses the meanings that child rearing has for a group of caregivers in a settlement inhabited mainly by people forcibly displaced by the armed conflict in Colombia as a way to go forward in understanding child rearing education. This work is a partial product of an action research and education project based on the strategy of thematic investigation circles. The analysis of the findings from the perspective of justice identified three areas of injustice that caregivers face as they undertake child rearing: structural-material, symbolic and cognitive. Addressing health education initiatives, including child rearing education, from a social justice perspective, entails helping people to overcome the unjust conditions faced by the subaltern population. It is also essential that health professionals learn from the students with whom they interact in order to develop more relevant education that aims to promote individual and social transformation.


Resumen Es común que los profesionales de la salud realicen educación sobre la crianza desde modelos educativos tradicionales y transmisionistas, los cuales imponen conocimientos y significados sin tener en cuenta las oportunidades que tienen las familias para realizar la crianza que valoran. Este artículo presenta los significados que sobre la crianza tiene un grupo de cuidadoras de un asentamiento habitado principalmente por población en situación de desplazamiento forzado debido al conflicto armado que vive Colombia. Es un producto parcial de un proyecto de investigación acción/educación cuya estrategia central fueron los círculos de investigación temática. El análisis de los hallazgos, realizado desde una perspectiva de justicia, identificó tres ámbitos de injusticia que las cuidadoras afrontan a la hora de realizar sus crianzas: estructural-material, simbólica y cognitiva. Abordar las acciones en salud pública y la educación para la salud - en particular la educación sobre la crianza - desde una perspectiva de justicia social, se hace necesario para contribuir a superar las condiciones de injusticia de las poblaciones subalternas; además es fundamental para que los profesionales de salud aprendan de los educandos con los cuales interactúan, como requisito para el desarrollo de procesos educativos más pertinentes tendientes a promover la transformación individual y social.


Resumo É comum que os profissionais da saúde realizam educação sobre a criação desde modelos educativos tradicionais e os professores dedicados, os quais impõem conhecimentos e significados sem ter em conta as oportunidades que têm as famílias para realizar a criação que valoram. Este artigo apresenta os significados que sobre a criação tem um grupo de cuidadoras dum assentamento habitado principalmente por população em situação de êxodo forçado devido ao conflito armado que mora na Colômbia. É um produto parcial dum projeto de pesquisa ação/educação cuja estratégia central foram os círculos de pesquisa temática. O analise dos descobrimentos, realizado desde uma perspectiva de justiça, identificou três âmbitos de injustiça que as cuidadoras afrontam à hora de realizar sua criação: estrutural-material, simbólica e cognitiva. Abordar as ações em saúde pública e a educação para a saúde - em particular a educação sobre a criação - desde uma perspectiva de justiça social, se faz necessário para contribuir a superar as condições de injustiça da população subalterna; além é fundamental para que os professionais de saúde aprendam dos estudantes com os quais interatuam, como requisito para o desenvolvimento de processos educativos mais pertinentes tendentes a promover a transformação individual e social.

El objetivo de este estudio es presentar un caso en el que tras el fracaso del tratamiento endodóntico se observó una lesión quística en el periápice. El presente caso trata de una paciente femenina de 39 años de edad, sin antecedentes médicos relevantes quien asistió a la clínica odontológica SERODU, determinando a la palpación la presencia de una tumefacción de consistencia blanda que estaría afectando la integridad del tejido óseo, sin sintomatología. Las pruebas de vitalidad pulpar resultaron negativas. Al examen radio-gráfico se observó una sombra radiolúcida compatible con lesión periapical crónica que afecta a las piezas dentales 11 y 12. Por lo que se decidió llevar a cabo la cirugía periapical con apicectomía, sellado apical retrógrado y enucleación de la lesión quística, seguido de regeneración ósea guiada. A los tres meses de control, radiográficamente se encontró una zona radiopaca compatible con regeneración ósea con un halo radiolúcido de separación con el hueso basal original, y a los seis meses, se observó la regeneración ósea completa. La técnica de obturación retrógrada e injerto óseo constituye una técnica de tratamiento efectivo para el sellado del conducto a nivel apical y la regeneración de defectos periapi-cales de gran tamaño. The aim of this study is to present a case in which after the endodontic treatment failed, a cystic lesion was observed in periapical area. The present case concerns a 39 years old female patient without relevant medical history, attended SERODU dental clinic, on clinical examination during the palpation the presence of a soft consistency tumefaction that would be affecting the integrity bone tissue, without symptomatology. The pulp vitality tests turned out to be negative. A radiolucidal shadow compatible with chro-nic periapical lesion affecting 11 and 12 teeth was observed during the radiographic examination. So it was decided to perform periapical surgery with apicectomy, apical retrograde filling and cystic injury enucleation, followed by guided bone regeneration. At three months of control, radiographically a radiopaque area compatible with bone regeneration was found with a radiolucid halo separating with the original basal bone, and at six months, complete bone regeneration was observed. The retrograde filling and bone grafting technique is an effective treatment technique for apical canal sealing and regeneration of large periapical defects. PURPOSE: To estimate the prevalence of blindness, visual impairment, diabetes mellitus, and diabetic retinopathy in patients aged 50 years and older in the State of Nuevo León, Mexico. METHODS: Ninety-one clusters of 60 people aged 50+ were selected randomly and a rapid assessment of avoidable blindness (RAAB) was conducted. Participants had their visual acuity and cause of visual impairment assessed, underwent a random glucose test and fundoscopy under mydriasis if they had diabetes. The diabetic retinopathy (DR) degree was classified according to the Scottish diabetic retinopathy grading scheme. RESULTS: From the sample 5,055 (92.6%) people were examined. The blindness prevalence was 1.7% (95% Confidence Interval: 1.3-2.1%). Cataract (32.6%), DR (29.1%) and glaucoma (16.3%) were the leading causes of blindness. The prevalence of severe, moderate, and early visual impairment was 1.0%, 5.1%, and 7.7%, respectively. Among respondents, 31% had diabetes and 8.1% of them was not diagnosed prior to the study. Of all participants with diabetes, 50% had glucose levels of 200 mg/dl or higher and 15.7% had sight-threatening diabetic retinopathy. CONCLUSIONS: Besides strengthening of cataract intervention activities, more ophthalmic services for diabetic retinopathy and glaucoma control are needed in Nuevo León to provide timely intervention to prevent blindness.

Embryo transfer using cryopreserved B. indicus embryos results in low pregnancy rates. The low viability of B. indicus embryos is likely a result of their low freezability relative to embryos from other species, notably B. taurus. Freezability is probably related to the quantity of lipid droplets in the embryonic cells. However, the mechanism underlying this association is unknown. Using multiphoton and transmission electron microscopy, the proportion, volume, morphology and composition of lipid droplets in fresh and cryopreserved B. indicus and B. taurus embryos were studied. B. indicus embryos have more droplets than B. taurus and a greater percentage of lipid droplets in clusters (34% vs. 24%; p < 0.05). Also, B. indicus individual droplets are of greater volume than those of B. taurus (47 µm3 vs. 34 µm3 ; p < 0.05). After freezing, the percentage of clusters decreased in B. indicus (11% vs. 5%; p < 0.05), while in B. taurus they increased (2% vs. 7%; p < 0.05). Freezing influenced the lipid droplet morphology, increasing the number of cracked droplets in B. indicus embryos (33% fresh vs. 62% frozen; p < 0.05), but not in B. taurus embryos (35% fresh vs. 34% frozen; ns). Regarding composition, saturated lipid droplets prevail in B. indicus embryos, contrary to B. taurus embryos. We observed differences in lipid droplet organization, proportion, volume, morphology and composition between B. indicus and B. taurus embryos which are altered in opposite directions following freezing.

RESUMEN En cualquier esfera de actuación, toda actividad profesional ha de realizar aportes significativos a los problemas del medio ambiente. Para ello, es fundamental una educación universitaria que tribute a una formación socialmente responsable. Generalmente, la educación ambiental en la universidad muestra un carácter asistemático, debido a la falta de proyección. El objetivo del artículo fue determinar la importancia social de promover la formación ambiental del estudiante universitario como un proceso formativo con toda su rigurosidad. Se exponen fundamentos esenciales de dicho proceso en sentido general y en particular, en la carrera de Arquitectura, basados en el método de análisis-síntesis y argumentación lógica, a partir del estudio de las principales premisas teóricas y empíricas que abordan la temática. Se concluye que es de gran relevancia incluir en los programas de arquitectura la formación ambiental como un proceso organizado, sistemático e integrador, que le permita al egresado cumplir con la responsabilidad social que de el se espera.


ABSTRACT In every scope of action, professional activity should make significant contributions to solve environmental problems. In order to achieve this, it is essential for higher education institutions to include a socially responsible education as part of their curriculum. Environmental education in higher education has traditionally shown an unsystematic character due to a lack of support. The objective of the present paper is to determine the social importance of promoting a rigorously designed environmental education program in a higher education context. The essential underpinnings of this process are presented. The design of the program was based on the analysis-synthesis and logical argumentation methods and was specifically focused to meet the needs of undergraduate students of architecture by taking into consideration the essential theoretical and empirical assumptions that address the subject matter. The paper concludes that it is highly relevant to include a well organized, systematic and integrative environmental education program as part of the undergraduate program in architecture. Such program will allow architecture graduates to comply with the social responsibility that is expected from them.

BACKGROUND: Hip and knee prosthetic replacements have proven to be the most appropriate treatment in the joints that do not benefit from medical or arthroscopic interventions; however, infections are the most feared complication. It is recommended that the incidence of infection should not exceed 2%. MATERIAL AND METHODS: This was an observational, prospective, longitudinal and observational study conducted in patients fitted with a prosthetic joint from August 2011 to July 2012. Patients were followed up pre and post-surgery for one year to identify a prosthetic infection, diagnosed using international parameters. We calculated the incidence of prosthetic infection, as well as the incidence density. RESULTS: A total of 339 patients (179 hip and 160 knee) were included. Variations in the time of pre-operative antibiotics' administration were observed. Six prosthetic infections were identified with an incidence rate of 1.79/339 arthroplasties, 2.2/179 hip procedures, and 1.25/160 knee arthroplasties. An incidence density of 0.02/year for hip arthroplasties and 0.11/year for knee procedures was identified. There were 4 infections of hip and 2 of knee. Five infections were acute and one chronic. The isolated microorganisms were E. faecalis, S. epidermidis (2), S. mitis, S. aureus and P. stomatis. CONCLUSIONS: The incidence of prosthetic joint infection in the first year of follow-up at our centre is within the recommended parameters. Surgical techniques and organizational practices influence the results.

La clasificación de la enfermedad renal crónica (ERC) realizada en 2002 por el grupo Kidney Disease Outcome Quality Initiative en 5 estadios con base en la tasa de filtración glomerular ha permitido estandarizar los parámetros de diagnóstico y ha sido de gran utilidad para el ejercicio de la práctica clínica, la realización de múltiples estudios de investigación, así como el desarrollo de políticas de salud pública a nivel mundial. Publicaciones posteriores han propuesto diferenciar la presencia de enfermedad renal en rin˜ ón nativo o trasplantado y subdividir el estadio 3 en A y B por ser un rango muy amplio en donde ocurren eventos muy importantes, sobre todo a nivel cardiovascular. Por su parte, las guías de práctica clínica Kidney Disease Improving Global Outcomes sustentan la proteinuria-albuminuria como factor pronóstico indiscutible de la ERC. A su vez, con respecto a la hipertensión arterial crónica, aunque no alcanza valor estadístico significativo como factor de riesgo independiente para la progresión de la ERC, la mayoría de los análisis evidencian que el control adecuado de la presión arterial es benéfico y retarda la progresión de la ERC en pacientes con proteinuria; tanto es así que es considerada como la segunda causa de ERC después de la diabetes mellitus, por delante de las glomerulonefritis. Con base en lo anterior y teniendo en cuenta que una enfermedad multifactorial como esta no puede ser evaluada con tan solo uno de los 3 principales factores de riesgo que más determinan su evolución, proponemos una nueva, práctica y fácil manera de clasificar la ERC tomando simultáneamente las 3 variables antes mencionadas, logrando un concepto más integral de la enfermedad. Este método diagnóstico, además de identificar la enfermedad, puede ser útil para determinar la severidad y predecir su riesgo de progresión. The classification of chronic kidney disease (CKD), made in 2002 by the Kidney Disease Outcome Quality Initiative group, includes 5 stages based upon the glomerular filtration rate. This has led to the standardisation of diagnostic parameters, and has been of great use in clinical practice, carrying out multiple research studies, as well as developing public health policies at aworld level. Later publications have proposed differentiating the presence of kidney disease in a native kidney or in a transplanted one, and subdividing stage 3 of CKD into A and B, since there is a wide range of disease in which very important events occur, especially at a cardiovascular level. Furthermore, the Kidney Disease Improving Global Outcomes clinical practice guidelines support proteinuria-albuminuria as an undebatable prognostic risk factor of CKD. At the same time, and regarding chronic high blood pressure, while it does not have significant statistical value as an independent risk factor for the progression of CKD, the majority of analyses suggest that adequate control of blood pressure is beneficial and delays the progression of CKD in patients with proteinuria, so much so that it is considered the second cause of CKD after diabetes mellitus and above glomerulonephritis. Based upon what has been mentioned, and taking into account that a multifactor disease like CKD cannot be evaluated with only one of 3 principal risk factors that most determine its outcome, a new, practical, and easy method for the classification of CKD is proposed, taking into account all the 3 aforementioned variables, thereby achieving a more integral concept of the disease. This diagnostic method to identify the disease may also be useful for determining the severity as well as predict the risk of progression. At the same way, having quantifiable parameters can be very useful as a prospective follow-up tool for therapeutic interventions...(AU)

Esta reflexión pedagógica surge de una experiencia académica que se ha desarrollado desde 2004 en la Facultad de Medicina de la Universidad de Antioquia, está enmarcada en el trabajo comunitario con los habitantes de la vereda Granizal (Bello, Antioquia). Se propone avanzar en la estructuración de un currículo integrado teniendo en cuenta las limitaciones del currículo actual. Para lograrlo se plantea la formación profesional de los médicos en los escenarios extramurales diferentes a los tradicionales como el salón de clase y el hospital. Se busca desarrollar en el estudiante habilidades clínicas, reflexivas, pensamiento crítico, actitud investigativa y de comunicación, reforzando la competencia de aprender a aprender y el aprendizaje significativo. Adicionalmente se favorece la interdisciplinariedad, la integración entre básicas-clínicas y teoría-práctica para resolver las necesidades sociales y generar conciencia ético-política y valores de los estudiantes en formación...


A presente reflexão pedagógica começou a partir de uma experiência acadêmica concreta que teve lugar, em 2004, na Faculdade de Medicina da Universidade de Antioquia, e foi desenvolvida com base no trabalho com os moradores da comunidade Granizal, zona rural do município de Bello, Antioquia. Propõe-se mudar o currículo atual (que tem limitações) para um currículo integrado. A principal estratégia para atingir tal mudança é priorizar as comunidades como o foco central da formação, além das aulas e os hospitais. Na comunidade, os discentes desenvolvem habilidades clínicas de reflexão e investigativas, obtendo a competência de aprender a aprender e a aprendizagem significativa. This pedagogical reflection originated from academic experience at the School of Medicine, University of Antioquia, based on community work among the inhabitants of the Granizal community, in the rural zone of the municipality of Bello, Antioquia, which started in 2004. It is proposed to move towards an integrated curriculum, given the limitations of the current one. To achieve this change, the main strategy is to prioritize communities as the central focus of the training, thus going beyond the classroom and the hospital. It is sought to develop reflective clinical skills, critical thinking and investigative and communicative attitudes among the students, thus strengthening their capacity for learning to learn and meaningful learning. During the last 20 years multiple roles of the nitric oxide gas (•NO) have been uncovered in plant growth, development and many physiological processes. In seed plants the enzymatic synthesis of •NO is mediated by a nitric oxide synthase (NOS)-like activity performed by a still unknown enzyme(s) and nitrate reductase (NR). In green algae the •NO production has been linked only to NR activity, although a NOS gene was reported for Ostreococcus tauri and O. lucimarinus, no other Viridiplantae species has such gene. As there is no information about •NO synthesis neither for non-vascular plants nor for non-seed vascular plants, the interesting question regarding the evolution of the enzymatic •NO production systems during land plant natural history remains open. To address this issue the endogenous •NO production by protonema was demonstrated using Electron Paramagnetic Resonance (EPR). The •NO signal was almost eliminated in plants treated with sodium tungstate, which also reduced the NR activity, demonstrating that in P. patens NR activity is the main source for •NO production. The analysis with confocal laser scanning microscopy (CLSM) confirmed endogenous NO production and showed that •NO signal is accumulated in the cytoplasm of protonema cells. The results presented here show for the first time the •NO production in a non-vascular plant and demonstrate that the NR-dependent enzymatic synthesis of •NO is common for embryophytes and green algae.

For the commercially established process of paclitaxel production with Taxus chinensis plant cell culture, the size of plant cell aggregates and phenotypic changes in coloration during cultivation have long been acknowledged as intangible parameters. So far, the variability of aggregates and coloration of cells are challenging parameters for any viability assay. The aim of this study was to investigate simple and non-toxic methods for viability determination of Taxus cultures in order to provide a practicable, rapid, robust and reproducible way to sample large amounts of material. A further goal was to examine whether Taxus aggregate cell coloration is related to general cell viability and might be exploited by microscopy and image analysis to gain easy access to general cell viability. The Alamar Blue assay was found to be exceptionally eligible for viability estimation. Moreover, aggregate coloration, as a morphologic attribute, was quantified by image analysis and found to be a good and traceable indicator of T. chinensis viability.

La enfermedad de Paget de la mama es una manifestación cutánea infrecuente de cáncer de mama que tiene mejor pronóstico cuando se detecta de manera temprana. Se presenta el caso de una mujer de 56 años, que asistió a consulta en cuidado primario en la ciudad de Cali para la evaluación del resultado de una citología cervicouterina. Se le realizó examen mamario preventivo y se encontró una lesión en areola-pezón de características eritematodescamativas de dos años de evolución. Los estudios anatomopatológicos y de inmunotipificación confirmaron enfermedad de Paget. El reporte histológico de la pieza quirúrgica extraída durante la cuadrantectomía con resección del complejo areola-pezón, diagnosticó un carcinoma intraductal subyacente. La utilización del método clínico en cuidado primario, en una paciente sin lesión palpable y con mamografías negativas para malignidad, fue la clave del éxito en la detección temprana y recuperación de la paciente que ha evolucionado de manera favorable. Paget's disease of the breast is a rare cutaneous manifestation of breast cancer which has a better prognosis when detected early. We report the case of a 56 year-old woman who attended consultation in primary care in the city of Cali to evaluate the result of uterine cervical cytology. She underwent preventive breast exam and a descamative erythematous nipple-areola lesion of two years of evolution was found. Pathological and immunotyping studies confirmed Paget's disease. The histological report of the surgical specimen removed during lumpectomy with resection of the nipple-areola complex, diagnosed an underlying intraductal carcinoma. The use of the clinical method in primary care in a patient without palpable lesion and malignancy negative mammograms, was the key to success in early detection and recovery of the patient who has performed well. BACKGROUND: According to data from the World Health Organization and UNICEF from year 2009, iron deficiency is the most widespread nutritional deficiency worldwide. This deficiency causes an imbalance between needs and iron supply, which consequently results in anemia. Around the world, two million people suffer from anemia, half of which is due to iron deficiency. The most impacted groups are children and teenagers, due to their highest requirements derived from the growing process, and women in their reproductive age, due to their loss of iron derived from menstruating or to their highest iron needs during pregnancy. This increase in needs is not satisfied by the regular diet, since it includes an insufficient amount and/or low bioavailability of iron. PURPOSE: To share with the medical community treating pregnant women the experience of an expert group so that they always bear in mind the repercussions caused by anemia during pregnancy, know more about the diagnostic possibilities and have a reference point for prescribing iron supplements. METHOD: The consensus method was used through the expert panel group technique. Two rounds were taken for structuring the clinical questions. The first one was to facilitate working groups their focusing in the clinical topics and the population of interest; the second one was to aid in posing specific questions observing the Patient, Intervention, Compare and Outcome (PICO) structure. The primary and clinical secondary study variables were defined by the working groups from the previously developed questions and during the face-to-face working period, according to the natural history of the disease: risk factors, diagnostic classification, (either pharmacological or non pharmacological) treatment and prognosis. The level of evidence and clinical recommendation was classified based on the Evidence Classification Level and Clinical Recommendation of the Medicine Group based on Evidence from Oxford University. RESULTS: In Mexico, 20.6% of pregnant women suffer from anemia, especially those between 15 and 16 years old, who prevail in 42.4% and 34.3% percent, respectively. Almost half the cases are due to iron deficiency. This type of anemia is associated with a higher risk of pre-term delivery, of low birth weight and perinatal death. The first assessment of an anemic pregnant woman shall include the medical history, a physical examination and the quantification of the erythrocyte indices, serum concentrations of iron and ferritin. The measurement of this last one has the highest sensitivity and specificity for diagnosing iron deficiency. Daily oral iron supplementation, at a 60-to-120 mg dosage, may correct most of mild-to-moderate anemias. The most appropriate treatment is with iron salts (iron sulfate, polimaltose iron complex or iron fumarate). In case of intolerance to iron sulfate or fumarate, polimaltose iron is a better tolerated option. Treatment shall be administered until the hemoglobin values are > 10.5 g and ferritin is between 300 and 360 microg/dL, and such levels shall be observed for at least one year. Parenteral administration is an alternative for patients with a severe intolerance to oral administration; even when the possibility of anaphylaxis shall be considered it is lower when using ferrous sacarate. Transfusion is reserved for patients with hemoglobin lower than 7 g/dL or having an imminent cardio-respiratory decompensation. CONCLUSIONS: Iron deficiency is the highest prevailing nutritional deficiency worldwide and its consequences during pregnancy may be highly risky for both the mother and her child. Anemia diagnosis may easily be achieved through a blood analysis including the serum ferritin determination. Serum iron measurement shall not be used as the only marker to set the diagnosis. It is important to rule out other causes, in addition to the deficiencies, which produce anemia in a patient. It is essential to suggest the administration of iron supplements not only during the antenatal period but also after birth o even after a miscarriage to fulfill the need for depleted iron. In severe anemias (hemoglobin being lower than 9.0 g/L), iron doses higher than 120 mg a day may be required. Treatment shall always begin orally, and if this is not well tolerated, parenteral administration shall be used.
