RESUMO

In direct plasma injection scheme, the relation between the ion current from a laser ion source and the accelerated beam current by a radio-frequency quadrupole (RFQ) linac was studied by a series of the following analyses. First, ion current at beam extraction was calculated from measured plasma parameters using a scaling law. Second, the beam emittance at the entrance of acceleration electrode region was evaluated by the simulation of the ion extraction out of the ion source plasma using the result of the first calculation. Last, the accelerated beam current could be reproduced by the simulation of beam acceleration using the results of the second. The second calculation revealed the time variation in the injected beam parameter to the RFQ linac. Then it was proved that the time structure of the accelerated beam was different from that of the injection beam because the degree of the matching between the injected beam emittance and the RFQ acceptance varied with time.

RESUMO

The idea of direct plasma injection scheme (DPIS) was proposed in 2000. This new technique has been studied and proven to accelerate intense ion beams. To provide medium mass ions with highly charged states, small tabletop solid lasers were used for plasma production. Based on the measured plasma properties, aluminum and carbon ions were accelerated with more than 60 mA of current. The next experiments will use an radio frequency quadrupole designed for q/m=1/6 and explore beam productions using targets up to silver, and future work will explore production up to uranium. The DPIS has been established and is ready to be used with various accelerators which require pulsed high current, high charge state ion beams.

In a sample of 21 children and adolescents on hospital haemodialysis, nurses identified a substantial percentage of the children (about half) as having marked problems in psychological adjustment. Nearly half of the children were also regarded as showing poor compliance with aspects of treatment other than dialysis. More children were rated as disturbed by nursing than by research psychiatric assessments; there was limited congruence between nurses' and parent/child ratings of problems in physical well-being or in treatment compliance. The reasons for these discrepancies are explored. Our results highlight the importance that psychological aspects in the nursing of children with chronic renal failure on hospital haemodialysis are recognised.

We studied the effects on the family of childhood chronic renal failure by contrasting children with varying degrees of severity of illness with healthy controls. Disruption of family life was most common in families with the more severely ill children (on hospital haemodialysis) and there was a tendency for more mental health problems in the parents in this group. Although more of these parents reported that the child's illness had had an impact on the marriage, marital break up was not related to illness status. There were indications that illness had resulted in an increase of subjective stress but also of support in a number of psychosocial areas for parents, especially those in the more severely affected group. Nevertheless at the time of assessment, fewer mothers in this group were deriving support from their social life and fewer fathers of ill children from their jobs or finances.

RESUMO

Problems in psychiatric adjustment were found to be more common in children and adolescents with chronic renal failure (22 subjects on hospital haemodialysis and 22 with less severe renal failure) than in healthy matched controls. There was a trend towards more definite and marked psychological difficulties in patients with more severe renal condition. However, the less severely physically ill children tended to have increased difficulties in school adjustment and they reported more feelings of loneliness. The findings have implications on the management of children with chronic renal failure.

RESUMO

The patient presented with progressive joint deformity, a hoarse voice, subsequent cachexia, and myoclonic seizures. She was first seen aged 22 months and died aged 6 years. A diagnosis of Farber's disease was made by demonstrating a deficiency of acid ceramidase both in leucocytes and fibroblasts.

RESUMO

We describe our impressions gained in the process of systematic interviewing of children with chronic renal failure. In spite of improved techniques of medical treatment in recent years, many of these children and their families remain under considerable strain. This is caused by anxieties about the child's general prospects for the future, problems in accepting the illness and treatment, particularly with a late onset of the condition, and multiple hospital admissions with occasional life-threatening incidents. In spite of their less serious nature, chronic features such as poor growth and urinary incontinence seem to be particularly worrying for many children and their parents. We feel that it is worth considering how to improve the supports for these families, helping parents to be more aware of community facilities that are available, and striving to improve communication between the different agencies (hospital, general practitioners, schools, etc.) involved.
