BACKGROUND: Six previous reports have documented a vulvar metastasis from breast cancer; one has noted involvement of the Bartholin's gland. CASE: A 53-year-old woman presented with an enlarging vulvar mass. Evaluation also identified a breast lesion. Surgical excision confirmed an invasive lobular breast carcinoma with a distant metastatic focus on the vulva. CONCLUSION: This is the first report in the English literature to document a vulvar metastasis from an invasive lobular carcinoma and the second to identify involvement of the Bartholin's gland. Careful gynecologic surveillance is needed in women with breast cancer.

A 50-yr-old woman with chronic ulcerative colitis developed severe restrictive lung disease, which progressed after total colectomy and cessation of sulfasalazine therapy. Open lung biopsy showed severe interstitial fibrosis and bronchiectasis. The patient's condition did not improve with steroid treatment. It is concluded that the patient's lung disease may represent an extracolonic manifestation of ulcerative colitis. A review of the literature is presented.

The design of laboratory archives depends to a large extent on frequency and type of access required. This article reports a study in which all requests for laboratory archival data were tabulated carefully. Such requests are infrequent and usually involve specific test results. An inexpensive, efficient, and convenient method of storing such data is described, employing a laboratory computer and magnetic tape-to-microfiche software. The efficiency of this system permits indefinite storage which may be desirable in view of the medico-legal climate.

A 68-year-old woman with ochronosis was admitted with congestive heart failure. A typical ejection murmur of aortic stenosis was ausculated and was documented on a phonocardiogram. The patient subsequently died, and at autopsy extensive deposition of ochronotic pigment was found on the aortic valve. This valve had no other anatomic abnormalities. Thus ochronosis should be considered in the differential diagnosis of aortic stenosis.
