RESUMO

BACKGROUND: The analysis of mutual relations between HPV infection and the expression of cancer gene products and proliferative activity in cervical carcinomas and dysplasias. MATERIALS AND METHODS: The expression of p53, c-erbB-2 oncoproteins and proliferative activity (Ki-67) was evaluated immunohistochemically in 41 cervical carcinomas and 29 dysplasias. HPV infection (type 16, 18) was assessed by in situ hybridization technique. RESULTS: HPV-positive carcinomas were found in 68.3% of cases. HPV type 16 infection were detected in 54% and HPV 18 in 39% of carcinomas. Simultaneous appearance of both virus types was shown in 25% of carcinomas. In dysplastic lesions, HPV infection was observed in 62.1% of cases. HPV type 16 was found in 34.5% and HPV 18 in 44.8% of patients. Both virus types were found in 17.2% of dysplasias. HPV infection was more extensive in cervical carcinomas than in dysplasias. Similarly the expression of oncoproteins was more intensive and referred to a higher percentage of cells in carcinomas. No relations between p53, c-erbB-2 overexpression and HPV infection were found. Ki-67 activity was found in a higher percentage of HPV-positive than in HPV-negative, both carcinomas and dysplasias. CONCLUSIONS: HPV infection, especially accompanied by increase of proliferative activity in dysplasias may define the cell subpopulation predisposed to malignant process development. The employment of in situ hybridization technique appears to be useful in detecting the viral infection in cytological smears even with no morphological changes in the cells.

RESUMO

The authors presented their consultations with young females at the out-patient clinic from 1983-1995 and compared them with their consultations during an earlier period. According to the authors, the frequency and character of the issues consulted changed as a result of a greater need for this type of help in general. The authors stressed the particular role of establishing thorough medical case histories on the gynaecology of the pubescent period. They concluded that effective child and pubescent female gynaecology could, in nearly every case, protect the patient against disorders and complications during in maturity and over the reproductive period.

By the determination of arylsulphatase A activity (EC 3.1.6.1) in the blood serum and urine obtained from 66 women using the modified method by Lee-Vaupel and Conzelmann it was noticed the increase in the enzyme activity during the pregnancy comparing to the non-pregnant group. The highest enzyme activity was observed in the III trimester of pregnancy. In the following stages of delivery (I, II, III) it was assumed the increase in enzyme activity in urine. The highest enzyme activity in urine was observed in the stage III, and in the serum--in the stage II. It was compared the enzyme activity in primiparae and multiparae proving, that in the serum nd urine this activity is higher in the stages I and II in multiparae, and in the stage III in primiparae.

RESUMO

A rare case is described of primary benign tumour of small intestine mesentery misdiagnosed as ovarian tumour. The diagnostic difficulties are presented resulting from small number or total absence of symptoms characteristic of this type of disease and from the demonstrated in gynaecological examination connection of the lesion with the genitals. The proper diagnosis is established usually during surgical operation, the extent of which depends on the vascularization of the tumour and on its connection with intestinal wall.

The authors discuss 47 cases in which the HSG examination showed secondary tubular unpatency, then verified by laparoscopy. The patients were divided in three groups. The patients who belonged to the first group had two-sides tubular unpatency recognized by HSG. In 26 of these patients the HSG result was confirmed by laparoscopy, in 3 cases--it was not. The second group were 14 patients with partial tubular unpatency showed by HSG. In 10 of these cases the laparoscopic examination gave results consistent with HSG, in 4 cases--contradictory. The last group was formed by 6 patients with no tubular unpatency recognized through HSG. 4 of these recognitions were confirmed by laparoscopy, while 2 were not. In the opinion of the authors supported by the results the HSG examination may be considered only as a preliminary element of tubular patency examination. In case of planned microsurgery the only qualifying or disqualifying examination is the laparoscopy with introduction of colour contrast.

The authors discuss 84 cases of laparoscopic examination of women with primary or secondary infertility. The patients qualified for this examination had undergone at least 26 weeks of conventional treatment with no effect. In 7 cases the reproductive organ was found to be in order, with fallopian tubes fully patent. In 43 cases tubular inpatency was found. The remaining patients suffered from other reproductive organ disorders. Therefore, the laparoscopic examination made detailed recognition of the causes of infertility possible and thus helped to establish the proper treatment. Additionally, in some cases it enabled the immediate removal of the source of infertility.

Authors demonstrate 21 cases with primary dysmenorrhoea at girls aged 14-17 years, which were observed between 1989-1992 and present way of treatment. In their opinion administration of nonsteroid, antiphlogistic medicaments and the spasmolytics is the most effective method.

RESUMO

Authors discuss current hypotheses related to the causes of osteoporosis and its possible therapy. One of the frequent menopausal disorders is osteoporosis, i.e. decrease in bone density, which may eventually lead to the pathological fractures and marked deformities of the skeleton. Etiology of the postmenopausal osteoporosis is not clearly explained. However, one may assume that hormonal disturbances, especially estrogen deficit, plays an important causative role. The treatment is difficult. It is usually aimed at preventing of bone loss.
