Aim    Myocardial infarction (MI) affects the working-age group and cause many absences and lost days of work. Some occupational factors effect in the prognosis of MI patients. The objective of this study was to determine predictors of early, late and no return to work (RTW) after MI.Material and methods    In this cohort study, 240 pre-employed, male patients with MI from April 2020 through February 2022 provided data about their demographic, occupational, psychosocial, and medical information. Data was also collected about the treatment they received as patients, their feelings about socioeconomic support, and RTW time. RTW within two weeks after MI was defined as early RTW. The relationships of these variables and with early RTW and with late or no RTW were analyzed.Results    Ninety-four patients (39.6 %) returned to work within two weeks after MI, whereas 207 patients (87.3 %) returned to work by the end of six months. Many variables, including coworker support, were associated with early RTW in a univariate analysis. Regression analysis revealed that age, coworker support, marital status, the patient's own estimated RTW time, the number of the vessels with occlusion, and comorbidity were predictors of early RTW. Of these factors, only coworker support would be subject to modification.Conclusions    This study indicates that improving support from coworkers can increase early RTW after MI.

RESUMO

Pachydermoperiostosis (PDP) is a rare disease that mimics the clinical and radiographical manifestations of acromegaly. Therefore, it should be considered as one of the differential diagnoses in the evaluation of acromegalic patients. In this study, we discussed a case of PDP in a 24-year-old simple worker working in a food industry factory and reviewed the work restrictions caused by the complications of the disease.

INTRODUCTION: Return to work after a kidney transplantation in a previously functioning person is determined by a number of medical and non-medical factors. In this regard, this study was to investigate the factors that influence return to work in kidney transplant recipients. METHODS: This retrospective cohort study was conducted in patients with kidney transplant in the main nephrology center in Tehran (December to April 2022). The primary outcome was the cumulative rate of return to work at 3, 6, and 12 months after transplantation. The secondary outcome was to compare the occupational, individual, and disease-related factors between patients who had returned to work and those who had not. RESULTS: Among 214 kidney transplant recipients, the overall cumulative rate of return to work after kidney transplantation at 3, 6 months, and 12 months were 44.4%, 63.1% and 69.6%; respectively. According to the univariate analysis, male sex, age less than 40 years, nonphysical jobs, job satisfaction, employer support, partner support, and absence of diabetes mellitus significantly affected the time of return to work (P < .05). According to the multivariate analysis, absence of diabetes mellitus, nonphysical jobs and job satisfaction had greater impact on the time of return to work (P < .05). CONCLUSION: The results showed that older age, female gender, having a physical job, the presence of diabetes mellitus, lack of job satisfaction, and employer and partner support are associated with not returning to work in these patients and adjusting factors linked to the work environment and support of colleagues and supervisors might play an important role in improving the general condition of these patients.  DOI: 10.52547/ijkd.7210.

In this study, we investigated a 42-year-old man working in a refractory brick (RB) production line who had allergic contact dermatitis (ACD) due to skin exposure to chromium (Cr). He had visited a dermatologist several times over a five-month period and although he had been medically treated, the symptoms reappeared after he returned to work and resumed exposure. Finally, with the announcement of the definite diagnosis of ACD through a patch test, it was decided to exclude him from exposure, and after 20 days, the symptoms went through the recovery process. No new recurring episodes were reported during the six-month follow-up period.

Background Manganese, as an essential element, has neurotoxic effects on basal ganglia and causes parkinsonism, dystonia, and cognitive symptoms in exposed individuals. Transcranial sonography (TCS) is a noninvasive and easily accessible imaging modality for detecting the accumulation of trace elements in the basal ganglia. Methodology In a cross-sectional study of foundry workers of one of the automobile manufacturing companies in 2019, the prevalence of parkinsonism was assessed through neurological examination and brain parenchymal sonography or TCS. The prevalence of parkinsonism according to age, smoking, work experience, marital status, and exposure to manganese was determined. Results Among 83 male workers, the prevalence of parkinsonism according to neurological examination, substantia nigra hyperechogenicity on TCS, lentiform nucleus hyperechogenicity, and totally was 33.7%, 9.6%, 10.8%, and 42.2%, respectively. The association between the prevalence of parkinsonism and age, smoking, work experience, marital status, and manganese exposure was evaluated. Parkinsonism according to lentiform nucleus hyperechogenicity was associated with smoking (odds ratio [OR] (95% confidence interval [CI]) = 26.63 (2.38-178.71)) and work experience (OR (95% CI) = 7.18 (0.84-61.32)). Conclusions According to this study, the prevalence of parkinsonism based on neurological examination or brain sonography findings was 42.2%. The implementation of this combined screening method might facilitate earlier detection of affected individuals among manganese-exposed workers.

BACKGROUND:  The purpose of this article was to determine the rate of return to work (RTW) and contributing factors after a one- and two-level anterior cervical discectomy and fusion (ACDF), a common spine surgery. Recognizing the contributing factors to RTW of occupationally active patients is important. METHODOLOGY: In this study, 68 patients were examined at three, six, and nine months after ACDF by the same team and same spinal surgeon at a single medical center, and the rate of RTW and contributing factors were determined. In this study, relationships were analyzed by the logistic regression method. RESULTS: The results of this study demonstrated that 77.9%, 82.4%, and 82.4% of workers had returned to work after three, six, and nine months, respectively. At nine months, 82.4% of the patients had returned to work, 19.6% returned to part-time work, and 80.4% had returned to their previous work. Conversely, 17.6% of the patients had not returned to work after nine months. In the logistic regression analysis, older age, longer absence from work before surgery, and less employer support were the related factors for no RTW. CONCLUSIONS:  Per the results, it may be concluded that nearly 82% of patients with ACDF had returned to work after nine months of follow-up. Lack of RTW is affected by older age, longer absence from work before surgery, and employer support. Planning according to these variables can reduce the burden of the problem.

BACKGROUND: Job satisfaction is one of the most important issues related to human resources in the organization which can reduce organizational productivity. In this regard, occupational health is an important science related to the health of the workforce in any organization, paying attention to it will help health promotion. The aim of the present study was to determine the relationship between the status of occupational health management and job satisfaction among farmers with a health promotion approach. MATERIALS AND METHODS: This cross-sectional study was conducted in 2020. The number of 209 farmers from all villages of Zabol was included by census method. Data were collected using a researcher-made questionnaire that consisting of three sections of demographic information, occupational safety and health status, and standard job satisfaction questionnaire. At first, the validity and reliability of the questionnaire were evaluated. In addition, 5-10 staff working in Agricultural Jihad Organization were asked about the content, clarity, and simplicity of the questionnaire terms. Content validity index and content validity ratio indexes were calculated as 0.68 and 0.83 for occupational health management and 0.69 and 0.83 for job satisfaction questionnaires, respectively. Instrument reliability was also assessed using the Cronbach's alpha coefficient, which was 0.872. Finally, after verifying the validity and reliability of the instrument, a questionnaire was used to collect the research data. Shapiro-Wilk, Wilcoxon, and Spearman correlation tests were used to analyze the findings. The collected data were analyzed using SPSS software version 19. Armonk; NY, USA: IBM Corp. RESULTS: The results of the study indicate that occupational health management was at a moderate to a high level (3.701) and job satisfaction (3.381) was at a moderate to a high level. The standard regression coefficient between occupational health management and job satisfaction was obtained as 0.571. With a confidence of 95%, it can be stated that occupational health management in Zabol city under the supervision of jihad-e-agriculture organization has a significant impact on farmers' job satisfaction. In addition, the research findings confirmed a positive and significant relationship between occupational health management and job satisfaction of employed farmers and showed that the implementation of occupational safety and health programs in the jihad-e-agriculture organization can affect the job satisfaction of farmers. CONCLUSION: According to the evaluation of research findings, job satisfaction and occupational health management in the Agricultural Jihad Organization are relatively favorable among farmers in Zabol city. It was argued that there is a relationship between occupational health management and job satisfaction, that the implementation of programs related to occupational safety and health management system such as Health and Safety Executive will promote health.

Manganese (Mn) is an essential element used in many industries, such as welding, foundries, the production of metal alloys, especially stainless steel, and the production of dry batteries, pesticides, paints, and explosives. Individuals are exposed to Mn through inhalation of fumes, dermal absorption, and ingestion. This metal is an essential trace element required for normal growth, development, and cellular homeostasis. It has also toxic effects on the central nervous system and can cause Parkinsonism symptoms in exposed patients. Studies on human and animal models reveal that neurons of the globus pallidus, the cerebellum, pons, red nucleus, the thalamus, cortex, and the anterior horn of the spinal cord could be affected by Mn toxicity. Although the diagnosis of manganese-induced Parkinsonism is primarily clinical, there are some supporting features on brain MRI images that may be helpful to objectively distinguish it. This study was designed to review the ways of exposure to Mn, clinical symptoms in case of exposure, and discover the relationship between exposure to Mn and Parkinsonism in the working population.

BACKGROUND: Quality of working life (QWL) refers to aspects of life quality, which are related to work settings. This study was designed to evaluate the QWL and its effective factors among nurses of Tehran University of Medical Sciences Hospitals. MATERIALS AND METHODS: In this cross-sectional descriptive-analytical study, the demographic questionnaire and work-related quality of life questionnaire were used to collect data among 435 nurses working in hospitals, affiliated to Tehran University of Medical Sciences. This article has been adapted from the approved project of Medical Ethics Committee of Tehran University of Medical Sciences (with the identification code 8911215132). RESULTS: The average QWL of nurses in the study was 50.56. There was a significant statistical relationship between gender, frequency of overtime, and working ward with QWL (P < 0.05). In a multivariate analysis, working ward had the only meaningful relationship. CONCLUSION: Due to low QWL of nurses in the emergency department, the need of interventions to improve conditions and ultimately to increase QWL of nurses working in the emergency room is being reminded.
