RESUMO

STUDY OBJECTIVE: Determine the impact of varying doses of fibrinogen concentrate and 4-factor prothrombin complex concentrate on clotting time as measured by thromboelastometry in an in-vitro model of dilutional coagulopathy. DESIGN: In-Vitro Study. SETTING: Tertiary academic center. PATIENTS: 31 healthy term singleton gestation patients. INTERVENTIONS: Blood was analyzed and diluted 95% with crystalloid. Washed red blood cells were added to simulate red blood cell transfusion. Two levels of fibrinogen repletion were then added to samples to simulate fibrinogen repletion in massive transfusion. Finally, 4-factor prothrombin complex concentrate (10 U/kg, 15 U /kg, or 25 U/kg) adjusted for body weight and estimated blood volume was added. MEASUREMENTS: Samples were analyzed by thromboelastometry, and the main outcome was a FIBTEM clotting time > 80s. MAIN RESULTS: FIBTEM clotting times were prolonged after dilution. After repletion with fibrinogen and prothrombin complex concentrates 7/31 (22.5%) of samples had a prolonged FIBTEM clotting time (> 80s) in the 50% fibrinogen repletion arm and 0 (0%) had a prolonged clotting time in the 100% fibrinogen repletion arm. FIBTEM clotting times approached their baseline levels at each dose of prothrombin complex concentrate. Median clotting time in the 100% fibrinogen repletion arm was under 80s prior to the administration of prothrombin complex concentrate. CONCLUSIONS: Commonly cited doses for prothrombin complex concentrates in hemorrhage might be too high for the obstetric patient. After fibrinogen correction alone, several samples required no further correction, highlighting the importance of frequent testing at the point of care. Limitations of this study include the in vitro study design and ability to directly apply findings to patient care. Further studies are needed to elucidate the ideal dose of prothrombin complex concentrate for obstetric hemorrhage. RESUMO

Severe liver dysfunction requiring transplantation is a major event at any stage of life, but for those requiring liver transplantation while pregnant, two lives hang in the balance. Available evidence on this relatively rare event consists solely of case reports and have yet to be reviewed or synthesized. We performed a systematic literature review and analyzed reports of 22 patients who underwent liver transplantation during pregnancy. This review describes the reported etiologies of hepatic failure in pregnant patients requiring transplantation, perioperative anesthetic management techniques, and the maternal and fetal clinical outcomes.

Factor X (FX) deficiency is a rare coagulopathy that may cause bleeding complications in parturients. The literature on rotational thromboelastometry (ROTEM; Instrumentation Laboratory, Bedford, MA) to guide factor repletion and neuraxial placement during partuition is limited. We present a 17-year-old parturient with FX deficiency after vaginal delivery with neuraxial anesthesia without bleeding complications. After FX concentrate administration, thromboelastometry was utilized to risk-stratify and manage her coagulopathy peripartum. Thromboelastometry may be a valuable adjunct to conventional monitoring in patient management. A limitation of this report is that coagulation tests and thromboelastometry were not assessed at identical timepoints.

RESUMO

The utilization of large-scale databases for research in medical fields, including anesthesiology, has increased in popularity over the last decade, likely due to their structured content and relative ease of access. These databases have been used in a variety of perioperative studies, including analyses of risk stratification, preoperative testing, complications, and cost. While these databases contain a wealth of information that allows for an abundance of research opportunities, there are unique limitations to their use. A comprehensive understanding will afford the anesthesiology researcher the knowledge and tools to not only better interpret studies that utilized these databases, but also to conduct analyses of their own. This review details the content and composition of these databases, highlights the advantages of and limitations to their use, and offers information about their access and cost.

BACKGROUND: Despite a growing interest in simulated learning, little is known about its use within regional anesthesia training programs. In this study, we aimed to characterise the simulation modalities and limitations of simulation use for US-based resident and fellow training in regional anesthesiology. METHODS: An 18-question survey was distributed to regional anesthesiology fellowship program directors in the USA. The survey aimed to describe residency and fellowship program demographics, modalities of simulation used, use of simulation for assessment, and limitations to simulation use. RESULTS: Forty-two of 77 (54.5%) fellowship directors responded to the survey. Eighty per cent of respondents with residency training programs utilized simulation for regional anesthesiology education, while simulation was used for 66.7% of fellowship programs. The most common modalities of simulation were gel phantom models (residency: 80.0%, fellowship: 52.4%) and live model scanning (residency: 50.0%, fellowship: 42.9%). Only 12.5% of residency programs and 7.1% of fellowship programs utilized simulation for assessment of skills. The most common greatest limitation to simulation use was simulator availability (28.6%) and funding (21.4%). CONCLUSIONS: Simulation use for education is common within regional anesthesiology training programs, but rarely used for assessment. Funding and simulator availability are the most common limitations to simulation use.

BACKGROUND AND OBJECTIVES: Cannabinoids have an expanding presence in medicine. Perioperative patients' perceptions of the effectiveness of these compounds, and acceptance if prescribed for pain, have not been previously described. Our primary objective was to describe patients' beliefs regarding the potential effectiveness of cannabinoids for the treatment of acute and chronic pain, as well as gauge patient acceptance of these compounds if prescribed by a physician. In addition, demographic and pain history data were collected to elucidate the predictors of the aforementioned patient attitudes. Secondarily, we sought to characterize the subgroup of patients who reported marijuana use. Predictors of marijuana use, effectiveness, and adverse effects were also reported for this subgroup. METHODS: An anonymous questionnaire was administered to 501 patients in the preoperative registration area at Mount Sinai Hospital, New York, New York. The questionnaire was designed to collect data on patient demographics, presence of pain, pain severity, use of pain medication, history of illicit-drug use, tobacco use, cannabis use, patient beliefs about the potential effectiveness of marijuana for acute and chronic pain and their willingness to use cannabis for pain, if prescribed by a physician. Normality of distributions for continuous variables was assessed with skewness and kurtosis measures. A logistic regression model was used to assess the demographic and medical characteristics of marijuana users compared with nonusers. The effectiveness of marijuana in dealing with pain and adverse effects associated with its use were examined using exploratory principal component analysis. RESULTS: More than 80% of this cohort of preoperative patients believed that marijuana could be at least somewhat effective for the treatment of pain after surgery and would be willing to use cannabinoid compounds if prescribed by their physician. Predictors of positive attitudes toward marijuana included history of marijuana use, pain history, and being a marijuana nonuser of white race. Approximately 27% of the respondents reported a history of marijuana use. Younger patients, those with higher levels of pain in the last 24 hours, and those who found standard therapies to be less effective for their pain were more likely to use marijuana. CONCLUSIONS: Patients generally believe that marijuana could be at least somewhat effective for the management of pain and are willing to use cannabinoid compounds for this indication, if prescribed by a physician.
