RESUMO

A case report of Jarisch-Herxheimer (JHR) reaction on a 10th day of Leptospirosis caused by Leptospira Pomona. JHR occurs as a complication of an antibiotic treatment of various spirochetes and may lead to respiratory distress syndrome, renal failure, hepatic insufficiency, and multiple organ failure. This case represents a skin and cardio-vascular form of JHR with no lung involvement. The patient was treated with benzylpenicillin and low dexamethasone doses for 5th day of the disease with a shift to ceftriaxone and high doses of methylprednisolone. The fastest diagnosis of a sporadic zoonotic disease, early start of antibiotic therapy, and adequate doses of corticosteroids are key to the successful treatment of leptospirosis.

OBJECTIVE: The aim: To identify clinical and epidemiological features of meningococcal infection on the initial day of a patient's medical consultation, as well as the efficacy of laboratory examinations. PATIENTS AND METHODS: Materials and methods: A retrospective analysis of 76 patients' histories diagnosed with meningococcal disease was carried out. CONCLUSION: Conclusions: Patients in the Transcarpathian region mainly develop an atypical form of meningococcal disease. Only half of all patients diagnosed with meningococcemia had a classical hemorrhagic rash. Generalized forms of meningococcal disease may proceed with normal or subfebrile temperature and without severe leukocytosis. We doubt the use of bacteriological methods of laboratory diagnosis due to their low effectiveness. The most sensitive method of laboratory diagnosis is a microscopic examination of blood smear, and cerebrospinal fluid.

OBJECTIVE: The aim: To study the effect of concomitant obesity in patients with bronchial asthma on quality of life and psychoemotional state depending on the phase of the disease. PATIENTS AND METHODS: Materials and methods: 176 patients with bronchial asthma in different phases of the disease with normal weight, pre-obesity and obesity were examined. The quality of life and psycho-emotional state of the patients was determined by the «Sent George Respiratory Question¼ and the Spielberger-Hanin and Beck tests. RESULTS: Results: All studied patients, showed significant violations of the psychoemotional sphere, a decrease in quality of life compared to healthy individuals (p<0.05). The remission phase revealed a deterioration in the quality of life of patients with overweight and obesity compared to patients with normal weight (p<0.05), in the exacerbation phase - with obesity and normal body weight (p < 0.05). Exacerbation negatively affected the psycho-emotional sphere in patients with normal body weight and obesity (p<0.05). CONCLUSION: Conclusions: The quality of life was reduced in all studied patients, regardless of body mass index(p<0.05), and the exacerbation of the disease worsened it (p < 0.05), increased depressive tendencies in patients with normal body weight and obesity (p<0.05). In patients with obesity, the worst quality of life indicators were found in the remission phase (p<0.05) and more pronounced depressive tendencies in the exacerbation phase than in the pre-obesity group (p<0.05). The most important indicators determining the course of bronchial asthma are quality of life and body mass index.
