RESUMO

The up-to-date possibilities of transfusion therapy are presented in the article. The authors analyzed their own experience and made a review of literature. The study considers the mechanisms of treatment, immunological and infectious risks, indications and contraindications for application of transfusion therapy in surgical practice.

RESUMO

The genetic polymorphism of metabolizers of tobacco smoke carcinogens can influence individual susceptibility to lung cancer. The study was concerned with the Mspl-polymorphism of the CYP1A1 gene responsible for encoding aryl hydrocarbon hydroxylase. It also plays a role in the activation of polycyclic aromatic hydrocarbons (PAH). The CYP1A1 alleles and genotype distribution in 146 lung cancer patients was compared with that in 230 healthy donors. Another control group consisted of 259 "cancer-resistant" subjects, i.e. tumor-free smokers and non-smokers aged 75 and more. The CYP1A1 allele incidence (19%) in patients with squamous lung cancer was significantly higher than in the control cohorts (11%) which is consistent with the leading role of PAH in the etiology of this pathology.

RESUMO

Sixty-five patients with the fourth stage colon cancer were subjected to the combined surgical and immunotherapy. The following conclusions are made: (1) surgical elimination of the bulk of tumor mass is a necessary prerequisite for effective immunotherapy; (2) vaccination with autological tumor cells accompanied with bacille bilié de Calmette-Guérin (BCG) as the adjuvant and with interleukin-2 as the immunostimulator effectively prevents metastasizing after successful surgery; (3) the vaccine must necessary contain living tumor cells adequately presenting tumor antigens; and (4) in some cases, immunotherapy causes undesirable autoimmune complications. They can be registered by corresponding inflammation control methods.

Bone marrow films from 20 patients with acute lymphoblastic leukemia (ALL) and 40 patients with acute nonlymphoblastic leukemia (ANLL) were investigated using Ag-NOR staining. The control group comprised 12 donors. The activity of nucleolar organizer regions (NOR) in the whole population of megakaryocytes (MK) and in groups of MK with 1-3, 4-6, 7 and more lobules per nucleus was assessed. The division into groups made it possible to judge about NOR activity in MK with different ploidy. The MK with both high ploidy and high NOR activity in residual megakaryocytopoiesis in patients with ALL are associated with good response to chemotherapy and complete remission, whereas these with low ploidy and low NOR activity suggest poor prognosis. In patients with ANLL no stable correlation between NOR activity and prognosis was found.

Aim-To evaluate the haemopoietic function of bone marrow blood forming cells in human chronic myeloid leukaemia (CML) by means of silver staining of nucleolar organiser region (AgNOR).Methods-Nucleoli were investigated in bone marrow blast cells and in erythroid, granulocytic, and megakaryocytic cells from 10 haematologically healthy subjects and from 26 patients with chronic myeloid leukemia (17 in benign phase, nine with blast crisis). The investigation was done before treatment, by means of a one step silver staining method. In every case 50 to 100 blasts, promyelocytes, myelocytes, immature (pronormoblastic and basophilic normoblastic) and mature (polychromatic normoblastic) erythroid elements, and megakaryocytes were evaluated for the mean numbers of nucleoli and for the average number of AgNORs per nucleus. Student's t test was used to compare the patient and control groups. Other statistical analyses were carried out by means of the computer assisted "HEMA" system.Results-Compared with controls, activation of NORs was noticed only in CML blasts, while there was a decrease in NORs in the erythroid elements, promyelocytes, and megakaryocytes. The AgNOR score of polychromatic normoblasts and megakaryocytes started to decrease in the benign stage of CML, whereas a similar decrease in pronormoblasts, basophilic normoblasts, and promyelocytes was detected only in patients with CML blast crisis.Conclusions-The loss of AgNOR sites in cell series in CML may be related to intrinsic defects in their proliferation.

The activity of nucleolar organizer regions (NOR) in megakaryocytes (MK) from 21 patients with acute lymphoblastic leukemia (ALL), 35 patients with acute nonlymphoblastic leukemia (ANLL) and 12 patients with blast crisis of chronic myeloid leukemia (BC CML) were studied by silver staining. The control group consisted of 12 donors. The activity of NOR in the whole population of MK and in groups of MK with 1-3, 4-6, 7 and over lobules per nucleus (MK 1, 2 and 3 class, respectively) were taken into account. The activity of NOR in the group of first class MK from patients with acute leukemia (ALL and ANLL) was detected low. On the contrary, it was high in the group of third-class MK of those patients, as compared to the control. The activity of NOR in MK of BC CML patients was significantly lower as compared both to the control and to patients with acute leukemia. The cause of changing the functional state of MK in varying hematology disease has been discussed.

The activity of nucleolar organizer (NO) in megakaryocytes (MG) from 8 donors, 10 patients with immune thrombocytopenias (IT), 17 patients with chronic myelocytic leukemia (CML) and 14 patients with multiple myeloma (MM) was studied by silver staining. The average number of nucleoli in MG of normal donors comprised 21.8 per nucleus with a range from 16.6 to 33.7. It was significantly lower in MG of CML patients, and, on the contrary, it was higher in MG of IT patients. The average number of Ag grains per nucleus reflecting their activity in relation to ribosomal RNA synthesis was found to be the highest (127 +/- 32.1) in MG of IT patients but rather low (43.2 +/- 7.2) in CML patients as compared to those of the control (76.5 +/- 11.1) and MM patients (86.0 +/- 5.6). The differences in the functional state of MG in varying diseases as well as possibilities of using this new approach in hematology have been discussed.
