Playgrounds are designed to be a safe, enjoyable, and effective means to promote physical activity in children and adolescents. The purpose of this study was to conduct a meta-analysis to determine the effectiveness of playground interventions for improving accelerometer-assessed ambulatory moderate-to-vigorous physical activity (MVPA) and to identify common aspects of playground interventions that may be beneficial to promote behavior change. An internet database search was performed. The final analyzed sample of studies was obtained from several criteria, including being a playground-based intervention targeting children or adolescents, having a control or comparison group, having an accelerometer-assessed MVPA outcome target variable, and reporting of the mean difference scores' variability. A random-effects model meta-analysis was employed to obtain pooled effect sizes. Ten studies (n = 10) were analyzed from the internet search. The weighted pooled effect (Hedges' g) across all studies was Hedges' g = 0.13, 95% CI: 0.02-0.24, p = 0.023. There was moderate study heterogeneity (I2 = 55.3%) but no evidence for publication bias (p = 0.230). These results suggest that school-based playground interventions have a small effect on increasing accelerometer-assessed MVPA within the pediatric population. The playground should still be an environmental target during school or community-based interventions aimed at providing opportunities to promote MVPA.

BACKGROUND: Gross motor skills are postulated to have a bidirectional relationship with physical activity (PA); however, no study has tested this relationship before and after a summer break. The purpose of this study was to examine the bidirectional relationships between school PA and gross motor skills in children before and after a summer break. METHODS: Participants were a sample of 440 children recruited from 3 low-income schools (age = 8.9 ± 1.2 years, mean ± SD). PA was assessed as average school-day step counts using Yamax DigiWalker pedometers (Yamasa Tokei Keiki, Tokyo, Japan) worn for 5 consecutive school days. Gross motor skills were assessed using the Test for Gross Motor Development, 3rd edition. Data were collected at 2 timepoints: at the end of spring semester (T1) and at the beginning of the subsequent fall semester (T2). An age- and body mass index-adjusted cross-lagged model was employed to relate T1 school step counts with T2 gross motor skills and T1 gross motor skills with T2 school step counts. RESULTS: T1 gross motor skills significantly predicted T2 school step counts (ß = 0.24, 95 % confidence interval (95%CI): 0.08-0.40, p = 0.003); however, T1 school step counts did not predict T2 gross motor skills (ß = 0.04, 95%CI: -0.06 to 0.14, p = 0.445). The model explained 35.4% and 15.9% of the variances of T2 gross motor skills and T2 school step counts, respectively. Additional analyses indicated that these relationships were driven primarily by ball skills. CONCLUSION: The relationship between gross motor skills and school PA was not bidirectional; however, higher gross motor skills, specifically ball skills, predicted higher school PA after a 3-month summer break.

The Apple Watch is one of the most popular wearable devices designed to monitor physical activity (PA). However, it is currently unknown whether the Apple Watch accurately estimates children's free-living PA. Therefore, this study assessed the concurrent validity of the Apple Watch 3 in estimating moderate-to-vigorous physical activity (MVPA) time and active energy expenditure (AEE) for school-aged children under a simulated and a free-living condition. Twenty elementary school students (Girls: 45%, age: 9.7 ± 2.0 years) wore an Apple Watch 3 device on their wrist and performed prescribed free-living activities in a lab setting. A subgroup of participants (N = 5) wore the Apple Watch for seven consecutive days in order to assess the validity in free-living condition. The K5 indirect calorimetry (K5) and GT3X+ were used as the criterion measure under simulated free-living and free-living conditions, respectively. Mean absolute percent errors (MAPE) and Bland-Altman (BA) plots were conducted to assess the validity of the Apple Watch 3 compared to those from the criterion measures. Equivalence testing determined the statistical equivalence between the Apple Watch and K5 for MVPA time and AEE. The Apple Watch provided comparable estimates for MVPA time (mean bias: 0.3 min, p = 0.91, MAPE: 1%) and for AEE (mean bias: 3.8 kcal min, p = 0.75, MAPE: 4%) during the simulated free-living condition. The BA plots indicated no systematic bias for the agreement in MVPA and AEE estimates between the K5 and Apple Watch 3. However, the Apple Watch had a relatively large variability in estimating AEE in children. The Apple Watch was statistically equivalent to the K5 within ±17.7% and ±20.8% for MVPA time and AEE estimates, respectively. Our findings suggest that the Apple Watch 3 has the potential to be used as a PA assessment tool to estimate MVPA in school-aged children.

This study examined the inter-model agreement between the Fitbit Flex (FF) and FF2 in estimating sedentary behavior (SED) and physical activity (PA) during a free-living condition. 33 healthy adults wore the FF and FF2 on non-dominant wrist for 14 consecutive days. After excluding sleep and non-wear time, data from the FF and FF2 was converted to the time spent (min/day) in SED and PA using a proprietary algorithm. Pearson's correlation was used to evaluate the association between the estimates from FF and FF2. Mean absolute percent errors (MAPE) were used to examine differences and measurement agreement in SED and PA estimates between FF and FF2. Bland-Altman (BA) plots were used to examine systematic bias between two devices. Equivalence testing was conducted to examine the equivalence between the FF and FF2. The FF2 had strong correlations with the FF in estimating SED and PA times. Compared to the FF, the FF2 yielded similar SED and PA estimates along with relatively low measurement discords and did not have significant systematic biases for SED and Moderate-to-vigorous PA estimates. Our findings suggest that researchers may choose FF2 as a measurement of SED and PA when FF is not available in the market during the longitudinal PA research.

MotionSense HRV is a wrist-worn accelerometery-based sensor that is paired with a smartphone and is thus capable of measuring the intensity, duration, and frequency of physical activity (PA). However, little information is available on the validity of the MotionSense HRV. Therefore, the purpose of this study was to assess the concurrent validity of the MotionSense HRV in estimating sedentary behavior (SED) and PA. A total of 20 healthy adults (age: 32.5 ± 15.1 years) wore the MotionSense HRV and ActiGraph GT9X accelerometer (GT9X) on their non-dominant wrist for seven consecutive days during free-living conditions. Raw acceleration data from the devices were summarized into average time (min/day) spent in SED and moderate-to-vigorous PA (MVPA). Additionally, using the Cosemed K5 indirect calorimetry system (K5) as a criterion measure, the validity of the MotionSense HRV was examined in simulated free-living conditions. Pearson correlations, mean absolute percent errors (MAPE), Bland-Altman (BA) plots, and equivalence tests were used to examine the validity of the MotionSense HRV against criterion measures. The correlations between the MotionSense HRV and GT9X were high and the MAPE were low for both the SED (r = 0.99, MAPE = 2.4%) and MVPA (r = 0.97, MAPE = 9.1%) estimates under free-living conditions. BA plots illustrated that there was no systematic bias between the MotionSense HRV and criterion measures. The estimates of SED and MVPA from the MotionSense HRV were significantly equivalent to those from the GT9X; the equivalence zones were set at 16.5% for SED and 29% for MVPA. The estimates of SED and PA from the MotionSense HRV were less comparable when compared with those from the K5. The MotionSense HRV yielded comparable estimates for SED and PA when compared with the GT9X accelerometer under free-living conditions. We confirmed the promising application of the MotionSense HRV for monitoring PA patterns for practical and research purposes.

The purpose of this study was to examine the effects of after-school sedentary screen time on children's brain activation in reward and cognitive control regions in response to pictures of high- and low-calorie foods. Thirty-two children participated in a randomized crossover study with counterbalanced treatment conditions. Conditions took place on separate days after school and included three hours of active or sedentary play. After each condition, neural activation was assessed using functional magnetic resonance imaging (fMRI) while participants completed a go/no-go task involving pictures of high- and low-calorie foods. General response inhibition was also measured using the Stroop task. Hunger was measured upon arrival to the testing facility and just prior to fMRI scans. Mixed effects models were used to evaluate main effects and interactions. Significant stimulus by condition interactions were found in the right superior parietal cortex, and left anterior cingulate cortex (Ps ≤ 0.05). High-calorie pictures elicited significantly more activation bilaterally in the orbitofrontal cortex compared to low-calorie pictures (Ps ≤ 0.05). Stroop task performance diminished significantly following the sedentary condition compared to the active (P ≤ 0.05). Subjective feelings of hunger were not different between conditions at any point. Sedentary screen time was associated with significantly decreased response inhibition and a reversed brain activation pattern to pictures of high- and low-calorie foods compared to active play, in areas of the brain important to the modulation of food intake. Decreased attention, and impulse control following sedentary screen time may contribute to disinhibited eating that can lead to overweight and obesity.

BACKGROUND: Runners often experience anterior knee pain and this pain is associated with altered running neuromechanics. The purpose of this study was to examine potential therapeutic benefits (reduced pain and restored running neuromechanics) of simultaneously applied ice and transcutaneous electrical nerve stimulation on experimentally-induced anterior knee pain. METHODS: Nineteen healthy subjects completed a sham and treatment data collection session. For both sessions, hypertonic saline was infused into the infrapatellar fat pad for approximately 80 min to induce experimental anterior knee pain. Perceived pain levels were measured every two minutes and running neuromechanics were recorded at four time points: pre-pain, pain before treatment, pain immediately post-treatment, and pain 20 min post-treatment. FINDINGS: The saline infusion significantly increased perceived knee pain from 0 to 2.8 cm. The ice/transcutaneous electrical nerve stimulation treatment significantly reduced perceived knee pain by 35%, six minutes after the treatment initiation. Perceived knee pain remained reduced until eight minutes after the treatment termination. The knee pain significantly decreased peak gluteus medius, vastus lateralis, and vastus medialis activation during running, each by an average of 17% plus/minus 6%; however, none of these decreases were resolved via the therapeutic treatment. Neither the knee pain nor the therapeutic treatment significantly affected peak gluteus maximus activation or peak hip adduction angle. INTERPRETATION: The experimental pain model effectively produced anterior knee pain and decreased muscle activation during running. The simultaneous ice/transcutaneous electrical nerve stimulation treatment effectively decreased anterior knee pain, but did not restore running neuromechanics that were altered due to the pain.

Forehand groundstroke effectiveness is important for tennis success. Ball topspin angular velocity (TAV) and accuracy are important for forehand groundstroke effectiveness, and have been extensively studied, previously; despite previous, quality studies, it was unclear whether certain racquet kinematics relate to ball TAV and shot accuracy during the forehand groundstroke. This study evaluated potential relationships between (1) ball TAV and (2) forehand accuracy, and five measures of racquet kinematics: racquet head impact angle (i.e., closed or open face), horizontal and vertical racquet head velocity before impact, racquet head trajectory (resultant velocity direction, relative to horizontal) before impact, and hitting zone length (quasi-linear displacement, immediately before and after impact). Thirteen collegiate-level tennis players hit forehand groundstrokes in a biomechanics laboratory, where racquet kinematics and ball TAV were measured, and on a tennis court, to assess accuracy. Correlational statistics were used to evaluate potential relationships between racquet kinematics, and ball TAV (mixed model) and forehand accuracy (between-subjects model; α = 0.05). We observed an average (1) racquet head impact angle, (2) racquet head trajectory before impact, relative to horizontal, (3) racquet head horizontal velocity before impact, (4) racquet head vertical velocity before impact, and (5) hitting zone length of 80.4 ± 3.6˚, 18.6 ± 4.3˚, 15.4 ± 1.4 m·s-1, 6.6 ± 2.2 m·s-1, and 79.8 ± 8.6 mm, respectively; and an average ball TAV of 969 ± 375 revolutions per minute. Only racquet head impact angle and racquet head vertical velocity, before impact, significantly correlated with ball TAV (p < 0.01). None of the observed racquet kinematics significantly correlated to the measures of forehand accuracy. These results confirmed mechanical logic and indicate that increased ball TAV is associated with a more closed racquet head impact angle (ranging from 70 to 85˚, relative to the ground) and increased racquet head vertical velocity before impact.

INTRODUCTION: Regular exercise protects against degenerative joint disorders, yet the mechanisms that underlie these benefits are poorly understood. Chronic, low-grade inflammation is widely implicated in the onset and progression of degenerative joint disease. PURPOSE: To examine the effect of running on knee intra-articular and circulating markers of inflammation and cartilage turnover in healthy men and women. METHODS: Six recreational runners completed a running (30 min) and control (unloaded for 30 min) session in a counterbalanced order. Synovial fluid (SF) and serum samples were taken before and after each session. Cytokine concentration was measured in SF and serum using a multiplexed cytokine magnetic bead array. Ground reaction forces were measured during the run. RESULTS: There were no changes in serum or SF cytokine concentration in the control condition. The cytokine GM-CSF decreased from 10.7 ± 9.8 to 6.2 ± 5.9 pg/ml pre- to post-run (p = 0.03). IL-15 showed a trend for decreasing concentration pre- (6.7 ± 7.5 pg/ml) to post-run (4.3 ± 2.7 pg/ml) (p = 0.06). Changes in IL-15 concentration negatively correlated with the mean number of foot strikes during the run (r 2 = 0.67; p = 0.047). The control condition induced a decrease in serum COMP and an increase in SF COMP, while conversely the run induced an increase in serum COMP and a decrease in SF COMP. Changes in serum and SF COMP pre- to post-intervention were inversely correlated (r 2 = 0.47; p = 0.01). CONCLUSIONS: Running appears to decrease knee intra-articular pro-inflammatory cytokine concentration and facilitates the movement of COMP from the joint space to the serum.
