In this study, we present the design, synthesis, and cytotoxic evaluation of a series of benzimidazole N-acylhydrazones against strains of T. cruzi (Y and Tulahuen) and Leishmania species (L. amazonensis and L. infantum). Compound (E)-N'-((5-Nitrofuran-2-yl)methylene)-1H-benzo[d]imidazole-2-carbohydrazide demonstrated significant activity against both trypomastigote and amastigote forms (Tulahuen strain), with an IC50/120 h of 0.033 µM and a selectivity index (SI) of 7680. This represents a potency 46 times greater than that of benznidazole (IC50/120 h = 1.520 µM, SI = 1390). Another compound (E)-N'-(2-Hydroxybenzylidene)-1H-benzo[d]imidazole-2-carbohydrazide showed promising activity against both trypomastigote and amastigote forms (Tulahuen strain), with an IC50/120 h of 3.600 µM and an SI of 14.70. However, its efficacy against L. infantum and L. amazonensis was comparatively lower. These findings provide valuable insights for the development of more effective treatments against Trypanosoma cruzi.

Although hydrochar and biochar have been used as soil conditioners, there is not a clear understanding of how their properties changes due to aging impacts their colloidal particles behavior on the soil system. From this premise, we produced hydrochar and biochar from the same feedstock (cashew bagasse) and aged with different chemical methods: (i) using hydrogen peroxide, (ii) a mixture of nitric and sulfuric acids, and (iii) hot water. It was analyzed the effects of aging on the stability of the carbonaceous materials (CMs) colloids in aqueous medium with different ionic strength (single systems), as well as the stability of the natural-soil colloid when interacting with biochar and hydrochar colloids (binary systems). A chemical composition (C, H, N, and O content) change in CMs due to the chemically induced aging was observed along with minor structural modifications. Chemical aging could increase the amount of oxygen functional groups for both biochar and hydrochar, though in a different level depending on the methodology applied. In this sense, hydrochar was more susceptive to chemical oxidation than biochar. The effectiveness of chemical aging treatments for biochar increased in the order of water < acid < hydrogen peroxide, whereas for hydrochar the order was water < hydrogen peroxide < acid. While the increase in surface oxidation improved the biochar colloidal stability in water medium at different ionic strengths (single systems), the stability and critical coagulation concentration (CCC) slightly changed for hydrochar. Natural-soil clay (NSC) interactions with oxidized carbonaceous material colloids (binary systems) enhanced NSC stability, which is less likely to aggregate. Therefore, the aging of carbonaceous materials modifies the interaction and dynamics of soil small particles, requiring far more attention to the environmental risks due to their application over time.

This study aimed to clarify the type of photosensitization induced by C. serpens and to verify if the plant remains toxic after being collected and stored. Eight crossbred sheep, aged between 6 and 36 months, were divided into three groups (G1 to G3). Over 30 days, daily, G1 received an exclusive diet of C. serpens, and G2 and G3 received 10 g/kg/BW and 20 g/kg/BW, respectively. Two other sheep were used as controls (CG). Before administration, the plant had been harvested every 15 days. Liver biopsies and blood samples were taken from all sheep on day zero and weekly. All sheep that received the plant developed clinical signs of photosensitization, and no changes were observed in the serum activities of AST and GGT. On day 30, all sheep except Ov1 from G1 and Ov7 were euthanized and necropsied. All sheep that received the plant developed clinical signs. Macroscopic or histologic lesions were not observed in the liver. Ov 1 recovered 13 days after the end of ingestion. These results demonstrated that C. serpens causes primary photosensitization. It is advisable to avoid grazing on pastures invaded by the plant or to remove them from the pastures immediately after observing the first signs.

There is evidence that harm to the mental health of healthcare workers has occurred during the pandemic caused by COVID-19. The burnout syndrome is a form of exhaustion that occurs in occupational settings and is a condition caused by long-term stress in the workplace. The objectives of this systematic review of observational studies were to present data from research into the prevalence of burnout syndrome in healthcare workers during the COVID-19 pandemic and observe its prevalence among frontline workers. The search was conducted on the MEDLINE, LILACS, and Embase databases from 2019 to May of 2021 and returned 538 publications, which underwent a two-stage process of selection by independent peers, resulting in selection of a sample of 29 articles. Data were then extracted and synthesized for presentation in narrative form. Cross-sectional designs were more prevalent (n = 26) than longitudinal studies (n = 3). The sample included research from 19 different countries, with one Brazilian study. A wide range of different instruments were administered by study authors to assess burnout syndrome, the most common of which was the Maslach Burnout Inventory (n = 13). The prevalence of burnout syndrome in the studies varied from 76 to 14.7%. Data on the relationship between development of burnout syndrome and working on the frontline were controversial. The lack of standardization of burnout syndrome assessment was a source of considerable difficulty, compromising comparability of the results, and should therefore be targeted for improvement by researchers. We suggest that more investigations should be conducted into prevalence and the associated factors of risk and protection.

OBJECTIVES: To evaluate in vitro two high concentration self-mix bleaching gels (35% or 37.5%) with different application tips (with or without an applicator brush) during in-office bleaching. MATERIALS AND METHODS: Healthy premolars were randomly assigned to five groups (n = 8): no treatment; 35% HP without applicator brush, 35% HP with applicator brush, 37.5% HP without applicator brush, and 37.5% HP with applicator brush. After the procedures, the concentration of HP transferred into the pulp chamber was evaluated using UV-Vis. The amount of gel used in each group was measured on a precision analytical balance. Color change (ΔEab, ΔE00, and ΔWID) was evaluated with a digital spectrophotometer. Initial concentration was measured by titration with potassium permanganate. The pH was evaluated using a digital pH meter. The data from each test were submitted to nonparametric tests (α = 0.05). RESULTS: Using a tip with an applicator brush expended less gel and left a lower amount of HP inside the pulp chamber compared to the tip without a brush for both bleaching gels (p < 0.0003), although no significant difference in color change was observed (p < 0.05). The 37.5% HP showed a more stable and less acidic pH and a lower amount of HP in the pulp chamber than the 35% HP (p < 0.00001). CONCLUSION: The HP penetration into the pulp chamber was lower when using an applicator with a brush tip than when using one with a conventional tip. As for the color, both tips were considered to lighten teeth. CLINICAL SIGNIFICANCE: For the application of a self-mixing high concentration in-office bleaching gel, a brush tip should be recommended because its use diminishes the penetration of HP into the pulp chamber and wastes less bleaching gel.

Boron-doped diamond (BDD) electrodes are regarded as the most promising catalytic materials that are highly efficient and suitable for application in advanced electrochemical oxidation processes targeted at the removal of recalcitrant contaminants in different water matrices. Improving the synthesis of these electrodes through the enhancement of their morphology, structure and stability has become the goal of the material scientists. The present work reports the use of an ultranano-diamond electrode with a highly porous structure (B-UNCDWS/TDNT/Ti) for the treatment of water containing carbaryl. The application of the proposed electrode at current density of 75 mA cm-2 led to the complete removal of the pollutant (carbaryl) from the synthetic medium in 30 min of electrolysis with an electric energy per order of 4.01 kWh m-3 order-1. The results obtained from the time-course analysis of the carboxylic acids and nitrogen-based ions present in the solution showed that the concentrations of nitrogen-based ions were within the established maximum levels for human consumption. Under optimal operating conditions, the proposed electrode was successfully employed for the complete removal of carbaryl in real water. Thus, the findings of this study show that the unique, easy-to-prepare BDD-based electrode proposed in this study is a highly efficient tool which has excellent application potential for the removal of recalcitrant pollutants in water.

Limited information is available regarding the role of anaerobic metabolism capacity on GOLD 1 and 2 COPD patients during upper limb exercise. We aimed to compare the upper limb anaerobic power capacity, blood lactate concentration, cardiovascular and respiratory responses, in male COPD patients versus healthy subjects during the 30-s Wingate anaerobic test (WAnT). The rate of fatigue and time constant of the power output decay (τ, tau) were also calculated and a regression analysis model was built to assess the predictors of τ in these patients. Twenty-four male COPD patients (post-bronchodilator FEV1 73.2 ± 15.3% of predicted) and 17 healthy subjects (FEV1 103.5 ± 10.1% of predicted) underwent the WAnT. Measurements were performed at rest, at the end of the WAnT, and during 3' and 5' of recovery time. Peak power (p = 0.04), low power (p = 0.002), and mean power output (p = 0.008) were significantly lower in COPD patients than in healthy subjects. Power output decreased exponentially in both groups, but at a significantly faster rate (p = 0.007) in COPD patients. The time constant of power decay was associated with resistance (in ohms) and fat-free mass (r2 = 0.604, adjusted r2 = 0.555, and p = 0.002). Blood lactate concentration was significantly higher in healthy subjects at the end of the test, as well as during 3' and 5' of recovery time (p < 0.01). Compared with healthy subjects, COPD patients with GOLD 1 and 2 presented lower upper limb anaerobic capacity and a faster rate of power output decrease during a maximal intensity exercise. Also, the WAnT proved to be a valid tool to measure the upper limb anaerobic capacity in these patients.

OBJECTIVES: This split-mouth randomized, single-blind clinical trial evaluated the gingival irritation (GI) of at-home bleaching with individual trays of different cutouts, as well as the tooth sensitivity (TS) and color change. MATERIALS AND METHODS: One hundred and twenty patients were randomized as to which side would receive the type of bleaching tray cutout: scalloped (in the gingival margin) and nonscalloped (extended from the gingival margin). The at-home bleaching was performed for 30 min with 10% hydrogen peroxide (HP) for 2 weeks. The absolute risk and intensity of GI and TS were assessed with a visual analog scale. Color change was assessed using a digital spectrophotometer and a color guide (α = 0.05). RESULTS: The proportion of patients who experienced GI was 57.5% (odds ratio 95% CI = 1.1 [0.7 to 1.8]), with no significant difference between groups (p = 0.66). The proportion of patients who experienced TS was 64.1% (odds ratio 95% CI = 1.0 [0.6 to 1.6]), with no significant difference between groups (p = 1.0). There is equivalence of scalloped and noscalloped groups for GI intensity (p < 0.01). Significant whitening was detected for both groups. Although some differences were observed between groups (CIELab and CIEDE00; p < 0.02), these were below of the considered clinically noticeable. CONCLUSIONS: The different cutouts of trays proved to be equivalent when regarding gengival irritation and tooth sensitivity when 10% HP for at-home bleaching was used. Significant color change was observed in both groups. However, significant differences detected between groups are not considered clinically noticeable. TRIAL REGISTRATION: Brazilian Clinical Trials Registry (RBR-2s34685). CLINICAL RELEVANCE: Scalloped or not, the individual trays for at-home bleaching could be considered a clinician's decision.

The present work investigates the electrocatalytic performance of two different morphologies of boron doped-diamond film electrode (microcrystalline diamond - MCD, and nanocrystalline diamond - NCD) used in electrochemical oxidation for the removal of the antibiotic ciprofloxacin (CIP). A thorough study was conducted regarding the formation of the MCD and NCD films through the adjustment of methane in CH4/H2 gas mixture, and the two films were compared in terms of crystalline structure, apparent doping level, and electrochemical properties. The physicochemical results showed that the NCD film had higher sp2 carbon content and greater doping level; this contributed to improvements in its surface roughness, as well as its specific capacitance and charge transfer, which consequently enhanced its electrocatalytic activity in comparison with the MCD. The results obtained from CIP removal and mineralization assays performed in sulfate medium also showed that the NCD was more efficient than the MCD under all the current densities investigated. The effects of CIP concentration and the evolution of the final by-products, including short-chain carboxylic acids and inorganic ions, were also investigated. The electrochemical performance of the NCD was evaluated in different aqueous matrices, including chloride medium, real wastewater and simulated urine. The application of the NCD led to complete or almost complete CIP degradation, regardless of the medium employed. The kinetic constant rates obtained under the different media investigated were as follows: synthetic urine (0.0416 min-1 - R2 = 0.991) < real wastewater (0.0923 min-1 R2 = 0.997) < synthetic matrix containing chloride (0.1992 min-1 - R2 = 0.995); this shows that the pollutant degradation was affected by the type of aqueous matrix and the oxidants that were electrogenerated in situ. The results obtained from the analysis of electrical energy per order (EE/O) showed that the treatment of simulated urine spkiked with required the highest energy consumption, followed by the real effluent and synthetic matrix containing chloride. The present study proves the viability of electrocatalytic nanostructured materials to the treatment of antibiotics in complex matrices.

Hydrochar is a carbon-based material that can be used as soil amendment. Since the physical-chemical properties of hydrochar are mainly assigned to process parameters, we aimed at evaluating the organic fraction of different hydrochars through 13C-NMR and off-line TMAH-GC/MS. Four hydrochars produced with sugarcane bagasse, vinasse and sulfuric or phosphoric acids were analyzed to elucidate the main molecular features. Germination and initial growth of maize seedlings were assessed using hydrochar water-soluble fraction to evaluate their potential use as growth promoters. The hydrochars prepared with phosphoric acid showed larger amounts of bioavailable lignin-derived structures. Although no differences were shown about the percentage of maize seeds germination, the hydrochar produced with phosphoric acid promoted a better seedling growth. For this sample, the greatest relative percentage of benzene derivatives and phenolic compounds were associated to hormone-like effects, responsible for stimulating shoot and root elongation. The reactions parameters proved to be determinant for the organic composition of hydrochar, exerting a strict influence on molecular features and plant growth response.

This study aimed to describe the first reports of outbreaks of hepatogenous photosensitization in cattle, sheep, and horses caused by spontaneous ingestion of Chamaecrista serpens, as well as to reproduce poisoning in sheep experimentally. Eleven photodermatitis outbreaks of unknown cause occurred in cattle, sheep and horses on nine farms in the semiarid region of Bahia, northeastern Brazil, between July 2017 and July 2020. Cutaneous lesions of photosensitization initiated until one week after the animals were introduced in paddocks invaded by the plant at the beginning of the rainy season. The photosensitive skin lesions were progressive and consisted of hyperemia, edema, ulcerative-crusted lesions with necrosis, especially in non-pigmented skin areas. The lesions in young animals were more severe. The animals avoided the sun and exhibited hyporexia, weight loss, restlessness, irritability, and severe itching. An experimental study was made using seven sheep, and resulted in photodermatitis, similar to that observed in the natural poisoning, seven days after the beginning of plant ingestion. Two sheep were reserved for the control group. Serum biochemistry changes indicated liver injury caused by the plant. Skin biopsies and liver biopsy guided by ultrasound were performed. The one sheep that had more pronounced skin lesions was euthanized and necropsied. At the necropsy, the liver was enlarged, diffusely pale, and firm, with an evident lobular pattern and an empty gallbladder. Histopathology revealed similar skin and liver lesions in samples from biopsies and the necropsy. There was a marked disorganization of the cords of hepatocytes associated with degenerative necrotic changes on the liver. The cutaneous injuries included orthokeratotic hyperkeratosis, hypergranulosis, acanthosis, and extensive areas of epidermic necrosis and ulceration. Three sheep were protected from sunlight and the lesions regressed within 45 days after the plant's consumption ceased. In conclusion, C. serpens causes hepatogenous photosensitization in ruminants and horses, and should be included in the list of differential diagnoses in cases of photosensitive dermatitis.

The remediation of a real textile wastewater aiming its reuse in the textile industry was carried out by integrating two processes: (i) a chemical or electrochemical advanced oxidation process (AOP or EAOP) based on Fenton's reaction for organics degradation, and (ii) a cation exchange process using marine macroalgae for removal of the iron acting in the Fenton's reaction based processes. Four AOPs/EAOPs at acidic pH 2.8 were tested: Fenton, photo-Fenton with ultraviolet A (UVA) radiation (PF/UVA), electro-Fenton (EF) and photoelectro-Fenton with UVA radiation (PEF/UVA). These processes provided very high color removals. After a running time of 45 min, the color removals were 68-95% for the Fenton process, 76-94% for the EF process, 80-98% for the PF/UVA process and 85-100% for the PEF/UVA process. In contrast, the mineralization was negligible for all the processes, indicating the generation/presence of persistent colorless compounds. The PF process was selected as first treatment stage due to its ability for color removal and related lower costs. A set of six marine macroalgae (Gracilaria caudata, Gracilaria cervicornis, Ascophyllum nodosum, Fucus spiralis, Laminaria hyperborea and Pelvetia canaliculata) were tested for iron uptake. Laminaria hyperborea showed the highest ion exchange capacity and affinity for iron species. Its application allowed the removal of all the iron acting in the PF process (3.4 mg/L). The textile wastewater resulting from the application of PF process followed by cation exchange with Laminaria hyperborea was successfully reused in scouring, bleaching and dyeing processes.

OBJECTIVE: The aim of this study was to evaluate the acute effects of time-restricted feeding in obese women living in social vulnerability who were placed on diets with the same energy deficit. METHODS: Fifty-eight obese women (19-44 y of age) were randomized to a group with a hypoenergetic diet and 12 h of fasting daily or to a group with only a hypoenergetic diet for 21 d, with body weight and waist circumference monitoring up to 81 d of intervention. The determination of the individual's energy content of the diets was based on their resting metabolic rate (by indirect calorimetry) and physical activity level (by triaxial accelerometers). Body composition, temperature, blood pressure, appetite, adhesion difficulty, thyroid axis hormones, leptin, glucose concentration, and insulin were measured before and after 21 d of intervention. A mixed analysis of variance test was performed. RESULTS: The women had a mean age of 31 y and mean body mass index of 33 kg/m². Significant interaction between group × time was observed only in axillary temperature (0.44°C; 95% confidence interval [CI], 0.17-0.74°C; P < 0.01), which increased in the experimental group and in body fat (-0.75%; 95% CI, -1.43% to -0.07%; P = 0.02) decreased in the experimental group. Also, there was a significant decrease in waist circumference in the time-restricted feeding group after 81 d. There were no differences in hormonal profile, resting metabolic rate, reported appetite, or adherence difficulty. CONCLUSION: Time-restricted feeding may be considered an alternative strategy for treating obesity in socially vulnerable women.

BACKGROUND: Advanced heart failure (HF) therapies, such as heart transplantation, are resource intensive and costly. In Brazil, only one-fifth of the estimated population need is fulfilled. We examined cost expenditures of heart transplants in a public institution in Brazil. METHODS AND RESULTS: We used microcosting analysis (time-driven activity-based costing) to examine total costs and individual cost components related to the index transplant hospital admission of all consecutive heart transplant recipients at a single center from July 2015 to June 2017. Average total cost for the 27 patients included was US$ 74,341 which exceeds the reimbursement value per patient by 60%. Major cost drivers were hospital structure and personnel, similarly to what is observed in the United States (US) and other developed countries. Total costs for index transplant admission were ∼50% lower than in the US, but approximate to values reported in some European countries. Costs of heart transplantation in Brazil were lower than those reported for developed countries, and higher than national reimbursement values. CONCLUSIONS: Advanced microcosting methodologies represent an important quality contribution to economic studies in health care and may provide insights for transplant-related health care policies in developing countries.

Snakebites are included in the group of emergencies for domestic animals, and these consultations demand technical knowledge as well as careful clinical evaluation of patients. Because of the importance of this theme and the higher prevalence of snakebites caused by snakes of the genus Bothrops in Rio Grande do Sul state, Brazil, this study aimed to address the epidemiological and clinical aspects of these accidents in dogs in the state, in addition to establishing their incidence. Clinical records of dogs, with a diagnosis of ophidian bothropic accident, assisted at the Toxicological Information Center of Rio Grande do Sul (CIT-RS) were revised from 2014 to 2016. These data were collected at the Toxicological Information Center, Statistics and Evaluation Center, State Secretary of Health. Data on the incidence of accidents and epidemiological and clinical information were obtained for each case. A total of 53 records were revised. In the records that included animal sex (n=49), 53% were female and 46.9% were male. As for animal age (n=47), the dogs ranged from one to 14 years, and most of them (46.9%) were in the 1-4-year age group. Over half of the cases occurred in the rural area (60.7%), and the head and neck were the main venom inoculation sites (76.3%). Diseases were characterized by clinical courses varying from hyperacute (<30 min to 6h), acute (6-24h), to subacute (4-5 days). Severe accidents accounted for 40% of the cases, with edema as the most frequent clinical sign (88.7%) followed by hemorrhagic manifestations (41.5%). Associations of epidemiological and clinical aspects coupled with history of exposure are important characteristics to assist with suspicion and definitive diagnosis of bothropic accidents in dogs.(AU)


Os acidentes ofídicos se enquadram no grupo de emergências para animais domésticos e esses atendimentos demandam conhecimento técnico, aliado a uma criteriosa avaliação clínica do paciente. Devido à importância do tema e a maior frequência de acidentes ofídicos provocados por serpentes do gênero Bothrops no Rio Grande do Sul, esse estudo teve como objetivo abordar os aspectos epidemiológicos e clínicos desses acidentes em cães nesse estado, além de estabelecer a frequência em que ocorrem. Foram revisados os protocolos de atendimentos clínicos de cães realizados pelo Centro de Informações Toxicológicas do Rio Grande do Sul (CIT-RS) com diagnóstico de acidente ofídico botrópico, entre o período de 2014 e 2016. Esses dados fazem parte do Centro de informação Toxicológica, Secretaria da Saúde, RS, Núcleo de Estatística e Avaliação. Foram obtidos dados relacionados à frequência dos acidentes e informações referentes à epidemiologia e à clínica de cada caso. Totalizaram-se 53 registros de atendimentos. Dos protocolos que informaram o sexo (n=49), 53% corresponderam a fêmeas e 46,9%, a machos. Em relação à idade (n=47), houve uma variação entre um e 14 anos, com maior concentração dos casos entre 1-4 anos (46,9%). Mais da metade dos atendimentos ocorreu em zona rural (60,7%) e a cabeça e o pescoço foram os principais locais de inoculação do veneno botrópico (76,3%). Observou-se uma doença com um curso que variou de hiperagudo (<30 min à 6h), agudo (6-24h) a subagudo (4-5 dias). Acidentes graves representaram 40% dos casos, no qual edema foi o sinal clínico mais frequente (88,7%), seguido por manifestações hemorrágicas (41,5%). A associação dos aspectos epidemiológicos e clínicos, aliados ao histórico de exposição, constituem características importantes que auxiliam na suspeita e no diagnóstico definitivo do acidente botrópico em animais.(AU)

Os carrapatos causam perdas econômicas à indústria pecuária no Brasil. Dessa forma, o uso de fitoterápicos como acaricidas mostrou eficácia no controle desses parasitas. Neste estudo, foi avaliada a eficácia in vitro dos seguintes fármacos: dimetil sulfóxido (DMSO), etanol, metanol, propilenoglicol e monolaurato de sorbitan polietilenoglicol em diluições de 100%, 20%, 10%, 5%, 1% e 0,5%. O efeito tóxico destes solventes em fêmeas e larvas ingurgitadas de Rhipicephalus microplus foi analisado. O teste de biocarrapaticidograma, foi utilizado para definir a eficiência reprodutiva das teleóginas de R. microplus e a eficiência dos produtos testados. Para análise da susceptibilidade das larvas foi utilizado o teste por imersão de larvas em seringa modificado. O uso de propilenoglicol e DMSO em fêmeas ingurgitadas e o uso de DMSO, etanol, metanol e propilenoglicol em larvas, todos à concentração de 0,5%, apresentaram uma menor taxa de mortalidade, podendo ter seu uso indicado em baixas concentrações como solventes de acaricidas utilizados no controle do R. microplus.(AU)


Cattle ticks cause economic losses to the cattle industry in Brazil. The use of phytotherapics as acaricides has shown efficacy of the control of these parasites. In the present study, the in vitro efficacy of the following drugs was evaluated: dimethyl sulfoxide (DMSO), ethanol, methanol, propylene glycol and polyethylene glycol sorbitan monolaurate at dilutions 100%, 20%, 10%, 5%, 1% e 0.5%. The toxic effect of these solvents on engorged female and larvae of Rhipicephalus microplus was analyzed. The immersion test was used to define the reproductive efficiency of engorged R. microplus females and efficiency of the drugs. The immersion test larvae modified syringe was used to analyze of the susceptibility of tick larvae. Propylene glycol and DMSO was used on engorged females and DMSO, ethanol, methanol and propylene glycol, all at concentration of 0.5%, were use on larvae the, showed the lowest mortality rate; this indicate that its use at low concentrations as solvent acaricide in the control of R. microplus could be recommended.(AU)

Background In patients with heart failure, inflammation has been associated with worse functional capacity, but it is uncertain whether it could affect their response to exercise training. We evaluated whether inflammatory biomarkers are related to differential effect of exercise on the peak oxygen uptake (V˙O2) among patients with heart failure. Design Open, parallel group, randomized controlled trial. Methods Patients with heart failure and ejection fraction ≤0.4 were randomized into exercise training or control for 12 weeks. Patients were classified according to: 1) inflammatory biomarkers blood levels, defined as 'low' if both interleukin-6 and tumor necrosis factor-alpha blood levels were below median, and 'high' otherwise; and 2) galectin-3 blood levels, which also reflect pro-fibrotic processes. Results Forty-four participants (50 ± 7 years old, 55% men, 25% ischemic) were allocated to exercise training ( n = 28) or control ( n = 16). Exercise significantly improved peak V˙O2 among participants with 'low' inflammatory biomarkers (3.5 ± 0.9 vs. -0.7 ± 1.1 ml/kg per min, p = 0.006), as compared with control, but not among those with 'high' inflammatory biomarkers (0.4 ± 0.6 vs. -0.2 ± 0.7 ml/kg per min, p = 0.54, p for interaction = 0.009). Similarly, exercise improved peak V˙O2 among participants with below median (2.4 ± 0.8 vs. -0.3 ± 0.9 ml/kg per min, p = 0.032), but not among those with above median galectin-3 blood levels (0.3 ± 0.7 vs. -0.7 ± 1.0 ml/kg per min, p = 0.41, p for interaction = 0.053). Conclusion In patients with heart failure, levels of biomarkers that reflect pro-inflammatory and pro-fibrotic processes were associated with differential effect of exercise on functional capacity. Further studies should evaluate whether exercise training can improve clinical outcomes in patients with heart failure and low levels of these biomarkers.

BACKGROUND: Nasoethmoidal meningocele is considered an uncommon type of cephalocele, and congenital cystic adenomatoid malformation (CCAM) is a rare lung disorder characterized by overgrowth of the terminal bronchioles. CASE: We report the unusual association between a nasoethmoidal meningocele and CCAM type II in a fetus exposed to valproic acid and misoprostol. The mother was an 18-year-old woman on her first pregnancy. She had a history of absence seizures since she was 5 years old. She took valproic acid from the beginning of the gestation until the end of the third month. At the end of the third month, she attempted interruption of her pregnancy using misoprostol. The fetal nasoethmoidal meningocele and CCAM type II were identified through morphological ultrasound examination and magnetic resonance imaging. A genome-wide study detected one copy number variation classified as rare, entirely contained into the SPATA5 gene. However, it does not seem to be associated to the clinical findings of the patient. CONCLUSION: To our knowledge, there is only one case reported in the literature showing the same association between a nasoethmoidal meningocele and CCAM. Thus, the malformations observed in our patient may be related to the gestational exposures. Also, we cannot rule out that the patient may present the same condition characterized by a cephalocele and CCAM described by some authors, or even an undescribed entity, because some hallmark features, such as laryngeal atresia and limb defects, were not observed in our case. Further reports will be very important to better understand the associations described in our study.

The aim of this study was to examine the Salmonella serovars and antimicrobial resistance within an animal-based agriculture river system. The study area consisted of a 1,345 ha upper part of Pinhal catchment. A total of 384 samples were collected in four years of monitoring. Salmonella was isolated from 241 samples (62.7%), resulting in 324 isolates. The highest number of Salmonella sp. occurred in samples associated with sites with high stoking density animal unit per hectare. It was possible to demonstrate the variability of serovars in the study area: 30 different serovars were found and at least 11 per monitoring site. Thirty-three potentially related isolates were genotyped by PFGE, one major clone was observed in serovar Typhimurium, which occurred in animal feces (swine and bovine), and different sites and samplings proving the cross-contamination and persistence of this specific clone. Among 180 isolates submitted to an antimicrobial susceptibility test, 50.5% were susceptible to all 21 antimicrobials tested and 54 different profiles were found. In the current study, 49.5% of the tested isolates were resistant to at least one antimicrobial, and multi-resistance occurred in 18% of isolates. Results indicate a close interaction between animal-based agriculture, Salmonella, and antimicrobial resistance.

High blood pressure (BP) increases the risk of cardiovascular diseases, and its control is a clinical challenge. Regular exercise lowers BP in patients with mild-to-moderate hypertension. No data are available on the effects of heated water-based exercise in hypertensive patients. Our objective was to evaluate the effects of heated water-based exercise on BP in patients with resistant hypertension. We tested the effects of 60-min heated water-based exercise training three times per week in 16 patients with resistant hypertension (age 55±6 years). The protocol included walking and callisthenic exercises. All patients underwent 24-h ambulatory blood pressure monitoring (ABPM) before and after a 2-week exercise program in a heated pool. Systolic office BP was reduced from 162 to 144 mmHg (P<0.004) after heated-water training. After the heated-water exercise training during 24-h ABPM, systolic BP decreased from 135 to 123 mmHg (P=0.02), diastolic BP decreased from 83 to 74 mmHg (P=0.001), daytime systolic BP decreased from 141 to 125 mmHg (P=0.02), diastolic BP decreased from 87 to 77 mmHg (P=0.009), night-time systolic BP decreased from 128 to 118 mmHg (P=0.06), and diastolic BP decreased from 77 to 69 mmHg (P=0.01). In addition, BP cardiovascular load was reduced significantly during the 24-h daytime and night-time period after the heated water-based exercise. Heated water-based exercise reduced office BP and 24-h daytime and night-time ABPM levels. These effects suggest that heated water-based exercise may have a potential as a new therapeutic approach to resistant hypertensive patients.
