RESUMO

To assess point-of-care ultrasound (POCUS) in 2024, we should start by defining its expanded scope and integration into general and specialty practice. Clinicians should abide by the evolving evidence for POCUS utilization and patient outcomes different from mortality and morbidity, especially as there are notable advancements in handheld ultrasound technology with a clear shift from capability to portability. To reduce diagnostic errors, POCUS practitioners need a holistic framework that accounts for known and new applications. Defining the POCUS scope of practices, proper training, and innovation, like artificial intelligence, can play a bigger role in mitigating diagnostic errors as we move forward.

BACKGROUND: Pneumorrhachis is the presence of air within the spinal canal and is most often traumatic or iatrogenic in etiology. Rarely, a small amount of pneumorrhachis can be seen with spontaneous pneumomediastinum. Here we describe a case of asymptomatic longitudinally extensive pneumorrhachis associated with spontaneous pneumomediastinum. CASE DESCRIPTION: A man in his mid-20s presented to the hospital with subcutaneous emphysema after a choking episode. On imaging of his neck and chest he was noted to have extensive pneumorrhachis with anterior displacement of the spinal cord. Out of concern for further accumulation of air, he was monitored in an intensive care setting for 48 hours but remained asymptomatic. He was discharged home after ruling out esophageal rupture as a cause for his pneumomediastinum. On follow-up 1 month after discharge he was doing well without symptoms. CONCLUSIONS: In cases of spontaneous pneumomediastinum, air can be entrained within the spinal canal. Special attention should be paid to any patient with pneumomediastinum with neurologic symptoms, as this could be due to pneumorrhachis.

RESUMO

We present an acute apixaban overdose without reported coingestants; it is the first such case report associated with multiple serum drug levels to assist in determining overdose kinetics. A 62 year old female presented to an emergency department (ED) 2 hours after ingesting sixty 5 mg tablets (5mg/kg) of her spouse's apixaban medication. She denied coingestants, and did not take her prescribed medications that day. Her vital signs were normal and she denied symptoms. Chemistry and hematology labs were unremarkable. Plasma apixaban concentrations were 2765.6 ng/ml at 14 hours post ingestion with a non-linear half life. There was no utilization of blood products or factor replacement. There was never any bleeding, and her hemoglobin did not decrease. This case demonstrates that a single ingestion of apixaban can occur without any complications occurring.

RESUMO

Cyanide poisoning occurs in many smoke inhalation victims. The newest FDA-approved treatment for acute cyanide intoxication is hydroxocobalamin (Cyanokit). However, hydroxocobalamin exhibits chemical properties that can disrupt several clinical laboratory tests. Knowledge of these effects on laboratory tests can be useful in assisting laboratory technicians and clinicians in managing these patients. This article briefly discusses acute cyanide poisoning and treatment, and summarizes laboratory interferences that have been reported with the use of hydroxocobalamin.

The objective of this article is to provide updated treatment options for bioterrorism agents. This updated synopsis includes recent clinical cases and treatment recommendations that have arisen in the last 5 years. The decontamination, treatment, and disposition of these biologic and chemical agents are presented alphabetically by agent type: biologic, chemical, and radiologic/nuclear. The information provided outlines only new treatment options since 2003.
