[The ways of prophylaxis of the gastro-intestinal hemorrhage recurrence].

Through three-years period 57 patients, suffering gastro-intestinal hemorrhage (GIH) of the ulcer genesis, were treated in the clinic. Among them were 37 (64.9%) men and 20 (35.1%) women. The patients have had (56.3 +/- 4.7) yrs old at average. The treatment programm for the patients have included a local arrest of GIH, medicinal therapy, directed on restoration of the circulating blood volume, gastric secretion inhibition, rising of the blood coagulation property. In cases of a middle and severe blood loss the blood preparations were transfused--the erythrocytic mass and freshly frozen plasm. Three patients were operated in the high period of GIH. The GIH recurrence was absent. In 7 - 8 days, if a persistent hemostasis was achieved, the patients were transferred to therapeutic department for the conservative therapy prolongation.

[Towards universal nomenclature for urgent surgical care].

In a modern professional literature the diseases, which undoubtedly threaten the patient's health and life, are called an urgent, special, emergent, fixed-date, etc. Not rare these terms are used simultaneously. Such a plurality of names of a quite dangerous state causes sometimes in these conditions uncertainty to seek help of a specialists and loss of a time. Modern dictionaries of a foreign languages words, of a foreign languages words in Ukrainian language, medical, big explanatory dictionary of a modern Ukrainian language definitely explains, that these terms are synonyms. All of them mean unconditional, timing. And every expression may be used in this context. The above mentioned suggestions and thoughts do not promote a secure fixing in the citizens consciousness the undoubtedness, the disease consequences danger, a threat to health and life. To deposit this in their awareness it is possible not by amorphous depiction, but using a singular, brief, firm term - an urgent.

[Experience of diagnosis and treatment of phlegmon in the diabetic foot syndrome].

In the clinic in 2007-2010 yrs 123 patients were treated for diabetic foot syndrome (DFS). In 29 (34.1%) of them the foot phlegmon was diagnosed, including 9 (31%), suffering osteomyelitis of 2-3 metatarsal bones. Neuropathic form of DFS was diagnosed in 20 (69%) and angiopathic one - in 9 (31%) patients. In a day of admittance to hospital 26 (89.7%) patients were operated, in a one day, after conduction of purposeful preoperative preparation - 3 (10.3%). Incomplete sutures were put on, the flow cleansing of wounds, using antiseptic solutions, were used. Wounds were treated in accordance to course of the wound process phases. Besides antibacterial and desintoxication therapy conduction, peripheral vasodilators, hypoxants, and in neuropathic form of DFS the group B vitamins and preparations of alpha-lipoic acid, were prescribed. All the patients have recovered. A stationary treatment term of patients, suffering neuropathic form of DFS, have constituted at average (16.3 +/- 4.2) days, and in angiopathic one - (19.4 +/- 3.6) days. The foot support ability was secured in 96.6% of patients.

[Syndrome of acute coronary insufficiency in ulcerative gastro-intestinal hemorrhage].

In 2004- 2007 in the clinic 249 patients, in whom the ulcer disease gastro-intestinal hemorrhage (GIH) occurred, were observed. Of 107 elderly patients 43 (40.1%) were hospitalized lately than 24 hours after GIH beginning. Gastroduodenofibroscopy was conducted for the hemorrhage origin investigation and the local hemostasis state determination. In a hospitalization day in 19 (17.7%) patients myocardial infarction on the GIH background was diagnosed. GIH and myocardial infarction were treated simultaneously. Timely elimination of the circulating blood volume deficiency under constant control of arterial and central venous pressure, the hourly diuresis and ECG dynamics was considered the main element of a treatment program. After infusion-transfusion therapy conduction the acute coronary insufficiency syndrome was eliminated on the 3-5th day.
