OBJECTIVE: To evaluate the pulp chamber temperature rise (PCTR) in light-cure bonding of brackets with and without primer, in intact and restored mandibular central incisors (M1), maxillary first premolars (Mx4), and mandibular third molars (M8). MATERIAL AND METHODS: Ninety human teeth were included: M1 (n=30), Mx4 (n=30), and M8 (n=30). Light-cure bonding of brackets was performed in intact (n=60) and restored (n=30) teeth, with primer (n=60) or without (n=30) primer. PCTR was defined as the difference between initial (T0) and peak temperatures (T1), recorded with a thermocouple during light-cure bonding. Differences on PCTR between bonding techniques (primer vs. no primer), teeth types (M1 vs. Mx4 vs. M8), and teeth condition (intact vs. restored) were estimated by ANCOVA, with α=5%.Results: PCTR was significantly higher with the use of primer (2.05 ± 0.08oC) than without primer (1.65 ± 0.14oC) (p=0.02), and in M1 (2.23 ± 0.22oC) compared to Mx4 (1.56 ± 0.14oC) (p<0.01). There was no difference in the PCTR in M8 (1.77 ± 0.28oC) compared to M1 or Mx4 (p>0.05), and no difference between intact (1.78 ± 0.14oC) and restored (1.92 ± 0.08oC) teeth (p=0.38). There was no influence of dentin enamel thickness in the PCTR (p=0.19). CONCLUSION: PCTR was higher in light-cure bonding of brackets with primer, especially in M1. Light-cure bonding seems less invasive without primer.

RESUMO Objetivo: Comparar o aumento na temperatura da câmara pulpar (ATCP) na colagem de braquetes com fotoativação usando ou não primer, em dentes hígidos versus dentes restaurados, em incisivos centrais inferiores (ICI), primeiros pré-molares superiores (1PMS) e terceiros molares inferiores (3MI). Métodos: Foram incluídos noventa dentes humanos: ICI (n=30), 1PMS (n=30) e 3MI (n=30). A colagem de braquetes com fotoativação foi realizada em dentes hígidos (n=60) ou dentes restaurados (n=30), com primer (n=60) ou sem (n=30) primer. O ATCP foi definido como a diferença entre as temperaturas inicial (T0) e máxima (T1) registradas com um termômetro durante a colagem com fotoativação. As diferenças no ATCP entre as técnicas de colagem (com primer vs. sem primer), diferentes dentes (ICIvs.1PMSvs.3MI) e condição dos dentes (hígidos vs. restaurados) foram estimadas por ANCOVA, com α=5%. Resultados: O ATCP foi significativamente maior com o uso de primer (2,05 ± 0,08oC) do que sem primer (1,65 ± 0,14oC) (p=0,02), e no ICI (2,23 ± 0,22oC) do que no 1PMS (1,56 ± 0,14oC) (p<0,01). Não houve diferença no ATCP do 3MI (1,77 ± 0,28oC) em comparação com ICI ou 1PMS (p>0,05), e não houve diferença entre dentes hígidos (1,78 ± 0,14oC) e restaurados (1,92 ± 0,08oC) (p=0,38). Não houve influência da espessura da dentina e do esmalte no ATCP (p=0,19). Conclusão: O ATCP foi maior na colagem de braquetes com fotoativação usando primer, principalmente no ICI. A colagem com fotoativação parece ser menos invasiva sem primer.

Resumo Os benefícios estéticos estão entre os principais objetivos do tratamento ortodôntico. Por isso, as extrações dentárias têm sido evitadas como protocolo de tratamento ortodôntico, pois podem prejudicar o perfil facial. No presente artigo, serão discutidos aspectos como a magnitude da resposta do perfil de tecidos moles em decorrência das mudanças na posição dos incisivos e os efeitos de diferentes sequências de extrações de pré-molares. Um caso clínico ilustrará o tema, com resultados estéticos e oclusais favoráveis e estáveis cinco anos após o tratamento ortodôntico com extrações de segundos pré-molares.

The amount of available bone in the lower incisor region is critical for periodontal preservation when planning large anteroposterior dental movements. The aims of this study were to evaluate bone limits of the lower incisors in the mandibular symphysis and to verify whether they are influenced by facial growth patterns, lower incisor inclinations, skeletal anteroposterior relationships, or patient age. Tomographic images of 40 orthodontically untreated patients were evaluated and measurements of width and height of the mandibular symphysis, thickness on the lingual and labial sides of the alveolar bone, and thickness of the entire alveolar bone were performed in sagittal view. The following cephalometric measurements were also evaluated: growth pattern (FHI), lower incisor inclination (IMPA), and skeletal anteroposterior relationships (AO-BO). Pearson's correlation test was used to assess associations among bone measurements, cephalometric measurements, and patients' ages. Weak to moderate positive correlations between FHI and bone measurements on the labial side of the incisors and total alveolar width were found. The height of the symphysis had a moderate negative correlation with FHI. It was concluded that patient age, FHI, and IMPA influenced bone limits of the lower incisors in the mandibular symphysis, while AO-BO had no influence.

INTRODUCTION: The aim of this in vitro study was to compare the temperature rises in the pulp chamber and time spent with different techniques for orthodontic resin adhesive removal. METHODS AND MATERIALS: Adhesive removal was performed in 20 extracted human maxillary second premolars with five techniques: high-speed tungsten carbide burs with water-cooling (BurH-cool) and without cooling (BurH), low-speed carbide burs (BurL), low-speed aluminum-oxide discs (DiscL), and low-speed fiberglass burs (BurFGL). Pulp chamber temperature was measured with a thermocouple probe and time spent was recorded with a digital stopwatch. Comparisons of temperature rise and time between the techniques were performed with Analysis of variance and Tukey's Honestly test. Correlation between variables was investigated with Pearson's correlation coefficient. RESULTS: Temperature rise and time were statistically different between techniques and showed a positive correlation between them (r=0.826) (P<0.01). BurH-cool provoked the lowest temperature rise and BurFGL the highest (P<0.01). Temperature rises were higher with DiscL than with BurH and BurL (P<0.01), which showed no statistical differences between them (P>0.05). The fastest technique was BurH-cool followed by BurL, BurH, DiscL and BurFGL (P<0.01). CONCLUSION: BurH-cool, BurH and BurL are safe adhesive removal techniques, whereas DiscL and BurFGL may damage pulp tissues. Time spent on adhesive removal has direct effect on temperature rise in the pulp chamber.

BACKGROUND: The aim of this study is to evaluate the change in pulp chamber temperature during the stripping technique. METHODS: Seventy-eight proximal surfaces of 39 extracted human teeth were stripped by two techniques: double-sided perforated stripping disk (PSD) and handheld stripper (HS). The teeth were divided into three groups: incisors (group 1), premolars (2), and molars (3). A J type thermocouple was inserted into the pulp chamber for temperature evaluation during the stripping procedure. RESULTS: Temperature rise was observed in all groups. The average temperature increase for the incisors was 2.58°C (±0.27°C) with PSD and 1.24°C (±0.3°C) with HS; for the premolars, 2.64°C (±0.29°C) with PSD and 0.96°C (±0.39°C) with HS; and for the molars, 2.48°C (±0.38°C) with PSD and 0.92°C (±0.18°C) with HS. There was significant difference (p<0.001) in pulp temperature variation among the stripping techniques evaluated. Greater variations in the temperature were observed for the stripping technique with PSD for all groups (3.1°C in incisors and premolars, 3.2°C in molars). Stripping performed with HS had minor differences in pulp temperature (1.7°C in incisors, 1.9°C in premolars, and 1.2°C in molars) than those in PSD group. However, the temperature variation was less than the critical threshold (5.5°C) in all groups. The results for teeth group comparison showed no significant difference in the temperature variation. CONCLUSIONS: The stripping technique with PSD produced significant increase in pulp temperature, with no differences between the types of teeth. However, it may not be clinically relevant, and both stripping techniques can be used safely.

RESUMO

Triple X syndrome (47,XXX) is a numerical chromosomal alteration that affects 1/1,000 women, in which the woman is born with an extra X chromosome. Some oral changes have been reported in the literature, as hypodontia, influence on deposition of crown enamel and discrepancies in cephalometric measurements. Other systemic complications may lead to oral abnormalities similar to those seen in triple X patients, such as congenital hypothyroidism (CH). This paper reports a triple X syndrome case associated with CH later treated. Besides delay in cognitive and intellectual development, the patient had changes in teeth development and in cephalometric measurements with deficiencies in the maxilla and mandible. This is the first report of a triple X syndrome associated with CH. Both conditions may result in changes in dentofacial development.

Orthodontic appliances predispose to the accumulation of plaque due to the great number ofretentive sites, which might lead to enamel demineralization adjacent to the accessories. Aim: Toassess the effectiveness of a compomer for orthodontic bonding in preventing the formation ofwhite spots around orthodontic brackets. Methods: Forty extracted human premolars weredivided into two groups: control group (CG), in which conventional resin Transbond™ XT LightCure (3M Unitek™) was used to bond the brackets; and experimental group (EG), in which thecompomer Transbond™ Plus Color Change (3M Unitek™) was used. pH cycling was performedfor 17 days to induce the demineralization process. Enamel on the buccal face was photographedunder a stereomicroscope (at 10x magnification) before (t0) and after (t1) pH cycling. The imageswere used to compare demineralization between the groups by using a visual scale. Results: Astatistically significant difference between control and experimental groups was found (p=0.004)showing that the compomer was more efficient than the conventional resin in preventing whitespots. Conclusions: The compomer Transbond Plus Color Change was capable of inhibitingenamel demineralization adjacent to the bonding area of brackets. However, the inhibition halodid not exceed 1 mm.

INTRODUCTION: The purposes of this study were to evaluate and compare the immediate effects of rapid maxillary expansion (RME) in the transverse plane with Haas-type and hyrax-type expanders by using cone-beam computed tomography. METHODS: A sample of 33 subjects (mean age, 10.7 years; range, 7.2-14.5 years) with transverse maxillary deficiency were randomly divided into 2 groups: Haas (n = 18) and hyrax (n = 15). All patients had RME with an initial activation of 4 quarter turns followed by 2 quarter turns per day until the expansion reached 8 mm. Cone-beam computed tomography scans were taken before expansion and at the end of the RME phase. Maxillary transversal measurements were compared by using the mixed analysis of variance (ANOVA) model and the Tukey-Kramer method. RESULTS: RME increased all maxillary transverse dimensions (P <0.0001). There was less expansion at skeletal than dental levels. The hyrax group had greater statistically significant orthopedic effects and less tipping tendency of the maxillary first molars compared with the Haas group. CONCLUSIONS: Both appliances were efficient in correcting a transverse maxillary deficiency. The pure skeletal expansion was greater than actual dental expansion. The hyrax-type expander produced greater orthopedic effects than did the Haas-type expander, but this effect was less than 0.5 mm per side and might not be clinically significant.

OBJECTIVE: To measure space closure during the retraction of upper permanent canines with self-ligating and conventional brackets. MATERIALS AND METHODS: Fifteen patients who required maxillary canine retraction into first premolar extraction sites as part of their orthodontic treatment completed this study. In a random split-mouth design, the retraction of upper canines was performed using an elastomeric chain with 150 g of force. The evaluations were performed in dental casts (T0, initial; T1, 4 weeks; T2, 8 weeks; T3, 12 weeks). The amount of movement and the rotation of the canines as well as anchorage loss of the upper first molars were evaluated. RESULTS: There was no difference between self-ligating and conventional brackets regarding the distal movement of upper canines and mesial movement of first molars (P > .05). Rotation of the upper canines was minimized with self-ligating brackets (P < .05). CONCLUSION: Distal movement of the upper canines and anchorage loss of the first molars were similar with both conventional and self-ligating brackets. Rotation of the upper canines during sliding mechanics was minimized with self-ligating brackets.

PURPOSE: This retrospective study aimed at assessing patients treated with extraction of premolars through cephalometry in lateral teleradiography, comparing the effect of different tooth extraction protocols on the facial profile. METHODS: For this study, 87 patients (31 boys and 56 girls) were selected from the private practices of three dentists certified by the Brazilian Board of Orthodontics and Facial Orthopedics. These patients were treated with fixed edgewise appliance and divided into three groups, according to the sequence in which premolars were extracted "Group 40" comprised 22 patients treated with extractions of the first two superior premolars, adopted as the control group; "Group 44" comprised 43 patients treated with extractions of the four first premolars; and "Group 45" comprised 22 patients treated with extractions of first superior premolars and second inferior premolars. The Holdaway analysis was used to quantify and compare the group profiles before and after treatment. RESULTS: Student's t-tests showed a profile improvement in all groups. When groups were compared, ANOVA showed that only the Inferior sulcus to the H line showed differences in incremental changes resulting from the treatment. Group 44 showed a significantly higher difference than Group 40. CONCLUSION: We conclude that the protocols produce similar facial appearances.


OBJETIVO: Este trabalho retrospectivo tem como proposta avaliar pacientes tratados com exodontias de pré-molares através de cefalometria em telerradiografia de perfil, comparando o efeito de diferentes protocolos de exodontias no perfil. METODOLOGIA: Oitenta e sete pacientes (31 meninos e 56 meninas) foram selecionados da clínica particular de três ortodontistas diplomados pelo Board Brasileiro de Ortodontia e Ortopedia Facial. Estes pacientes foram tratados com aparelho fixo Edgewise foram divididos em três grupos, compreendendo a sequência de exodontia de pré-molares empregada Grupo 40 compreendendo 22 pacientes tratados com exodontia de primeiros pré-molares superiores, Grupo 44 compreendendo 43 pacientes tratados com exodontia de quatro primeiros pré-molares e Grupo 45 compreendendo 22 pacientes tratados com exodontia de primeiros pré-molares superiores e segundos inferiores. A Análise de Holdaway foi utilizada para quantificar e comparar os perfis antes e após o tratamento dos grupos. RESULTADOS: O teste t-Student mostrou melhora no perfil em todos os grupos. Quando comparados entre si, ANOVA revelou que apenas a medida sulco lábio inferior em relação à linha H apresentou diferenças nas mudanças incrementais proporcionadas pelo tratamento, em que o grupo 44 apresentou uma diferença significativamente superior ao grupo 40. CONCLUSÃO: A partir dos resultados concluiu-se que os protocolos produzem aparências faciais semelhantes.

Este trabalho tem por objetivo apresentar uma abordagem alternativa para o tratamento ortodôntico das más oclusões de Classe II. Através de uma revisão da literatura, verificou-se que a extração de segundos molares superiores demonstrou ser uma alternativa viável para o tratamento desse tipo de má oclusão. Essa opção terapêutica possibilita maior rapidez na distalização dos primeiros molares com menor necessidade de cooperação por parte do paciente. Porém, a análise do grau de formação, posição intraóssea e morfologia do terceiro molar deve ser cuidadosamente realizada para proporcionar o correto posicionamento do mesmo no lugar do segundo molar extraído. Dois casos clínicos apresentarão a sequência do diagnóstico e tratamento com essa mecânica, exibindo resultados adequados dos pontos de vista funcional e estético.


The purpose of this article is to present an alternative approach to the orthodontic treatment of Angle Class II malocclusion. According to a literature review it was observed that the extraction of upper second molars has proven to be a viable alternative for the treatment of this type of malocclusion. This therapeutic option enables faster first molar retraction and requires less patient compliance. However, the level of development, intraosseous position and morphology of the third molar should be carefully evaluated to ensure its correct positioning in place of the extracted second molar. Two clinical case reports will demonstrate that the sequence of diagnosis and treatment used with this mechanics yields satisfactory functional and aesthetic results.

Objetivos: Avaliar o grau de abertura bucal pós-operatória e fatores que possam influenciá-la, em pacientes submetidos à remoção de terceiros molares inferiores e superiores no ambulatório de Cirurgia e Traumatologia Bucomaxilofacial da Universidade Federal de Santa Maria, Rio Grande do Sul, Brasil. Método: Foram examinados pacientes submetidos à exodontia de terceiros molares inferiores e superiores no ambulatório de Cirurgia e Traumatologia Bucomaxilofacial da Universidade Federal de Santa Maria. A abertura bucal dos pacientes foi medida em milímetros minutos antes, dois e sete dias após a exodontia, com o uso de uma régua. Resultados: O trismo é uma ocorrência freqüente nas cirurgias de terceiros molares inclusos, ocorrendo em 56,5% dos pacientes no período de dois dias após a cirurgia, havendo uma redução na reavaliação após sete dias. Conclusão: Conclui-se que o trismo é uma ocorrência freqüente nas cirurgias de terceiros molares inclusos e que não foram encontradas relação significante entre o trauma da cirurgia e a presença de trismo.
