Since 1987, the World Health Organization (WHO) carried out global surveys on polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and polychlorinated biphenyls (PCBs) in human milk. This study presents a review of the three most recent surveys from 2000 to 2010, including DDT. The objective was to identify global quantitative differences and provide baseline information for 52 countries or provide time-trends for countries with previous data. Individual human milk samples were collected following a WHO-designed procedure and combined to form a national pooled sample. Here, we report global levels for PCDDs, PCDFs, PCBs and the sum of o,p'-DDT, p,p'-DDT, o,p'-DDE, p,p'-DDE, o,p'-DDD and p,p'-DDD (ΣDDTs). A concise risk-benefit evaluation related to human milk contamination with these persistent organic pollutants (POPs) was also done. Large global and regional differences were observed. Levels of PCDDs and PCDFs were highest in India and some European and African countries. PCB levels were highest in East and West Europe. The highest levels of ΣDDTs were found in less industrialized countries. A temporal downward trend for PCDDs, PCDFs and PCBs is indicated. A risk-benefit assessment indicates that human milk levels of PCDDs, PCDFs and PCBs are still significantly above those considered toxicologically safe, while ΣDDTs are below or around those considered safe. With respect to potential adverse health effects, a more dominant role of in utero exposure versus lactational exposure is indicated. If potential adverse effects are balanced against positive health aspects for (breastfed) infants, the advantages of breastfeeding far outweigh the possible disadvantages. Our observations provide a strong argument to plea for further global source-directed measures to reduce human exposure further to dioxin-like compounds.

The Stockholm Convention on Persistent Organic Pollutants (POPs) aims to protect human health and the environment from POPs through a range of measures aimed at reducing and ultimately eliminating their releases into the environment and subsequent human exposure. Article 16 of the Convention sets the basis for a mechanism to assess the success of the activities undertaken worldwide to implement the Convention. One of major pillars for the evaluation of the effectiveness of the Convention is monitoring data obtained through the Global Monitoring Plan (GMP) for POPs. The implementation of the GMP over the last eleven years, since the entry into force of the Convention, shows how a global treaty such as the Stockholm Convention streamlined existing monitoring efforts and triggered harmonization and further development of a global monitoring network. In its initial stages, long term POPs monitoring programmes were available only in some parts of the globe. Over more than a decade of generation of harmonized, comparable monitoring data on 23 chemicals of global concern, a rich and extremely valuable dataset has been generated in the frame of the GMP. Long-term monitoring programmes have enlarged the scope of their activities to cover newly listed chemicals, and new programmes have emerged. Monitoring data are broadly shared through the GMP data warehouse, the Convention's clearing-house mechanism, and through other appropriate global tools. Through its global reach, the GMP contributes to the global chemicals and waste policy agenda, supports and triggers further research initiatives, and provides information to the general public at large.

In Oman, DDT was sprayed indoors during an intensive malaria eradication program between 1976 and 1992. DDT can remain for years after spraying and is associated with potential health risk. This raises the concern for human exposure in areas where DDT was used for indoor spraying. Twelve houses in three regions with a different history of DDT indoor spraying were chosen for a sampling campaign in 2005 to determine p,p'-dichlorodiphenyltrichloroethane (p,p'-DDT), p,p'-dichlorodiphenyldichloroethylene (p,p'-DDE) and p,p'-dichlorodiphenyldichloroethane (p,p'-DDD) levels in indoor air, dust, and outdoor soil. Although DDT was only sprayed indoor, p,p'-DDT, p,p'-DDE and p,p'-DDD were also found in outdoor soil. The results indicate that release and exposure continue for years after cessation of spraying. The predicted cancer risk based on concentrations determined in 2005, indicate that there was still a significant cancer risk up to 13 to 16years after indoor DDT spraying. A novel approach, based on region-specific half-lives, was used to predict concentrations in 2015 and showed that more than 21years after spraying, cancer risk for exposure to indoor air, dust, and outdoor soil are acceptable in Oman for adults and young children. The model can be used for other locations and countries to predict prospective exposure of contaminants based on indoor experimental measurements and knowledge about the spraying time-schedule to extrapolate region-specific half-lives and predict effects on the human population years after spraying.
