[Prevention of post-burn scarring esophageal strictures, strategies of surgical treatment]. / Preduprezhdenie obrazovaniia posleozhogovoi rubtsovoi striktury pishchevoda, taktika khirurgicheskogo lecheniia.

Experience of treatment of 52 patients with oesophageal chemical burn in acute and early postburn period was analyzed, as well as in 649--with oesophageal postburn cicatricial stricture (OPCS). In 18 patients perforation of esophagus had occurred during performance of bougienage. Rational complex of diagnostic and therapeutic measures in such patients was determined. The results of bougienage in patients with OPCS were summarized. Causes of the oesophageal perforation occurrence during bougienage were analyzed. Tactics of surgical treatment of esophageal perforation was determined. Analysis of the treatment results and of the main causes, which conditioned upon the postoperative complications occurrence, was conducted.

[Features of diagnosis and treatment of endotoxicosis syndrome in acute chemical poisoning]. / Osobennosti diagnostiki i lecheniia sindroma éndotoksikoza pri ostrykh otravleniiakh khimicheskoi étiologii.

Experience gained in the treatment of more than 750 patients with acute poisoning complicated by endotoxicosis showed that the entire complex of typical changes in endotoxicosis markers and homeostasis parameters (hematological, immunological, hemorheological, LPO/AOD) should be taken into consideration during the diagnostic studies. These shifts should be timely corrected by the basic efferent methods of artificial detoxication (hemosorption, hemodiafiltration, hemofiltration) with obligatory physio- and chemohemotherapy (laser-UV hemotherapy, infusion of sodium hypochlorite).

[Possibilities and prospects of regional anesthesia in the current anesthesiological practice]. / Vozmozhnosti i perspektivy regionarnoi anestezii v sovremennoi anesteziologicheskoi praktike.

Presents a modern concept of conduction blocking, permitting revision of indication for it. Blocking is regarded as the basic component of present-day anesthesiologic care responsible for analgesia and regional muscle relaxation. Use of objective methods of verification of the elements of nervous formations improves the efficacy of and extends the range of indications for regional blocking solves the problem of regulation, and rational combination of this modality with selectively acting drugs provides the optimal realization of balanced anesthesia.

[A comparative analysis of the efficacy of trimecaine, lidocaine and bupivacaine for a prolonged brachial plexus block]. / Sravnitel'naia otsenka éffektivnosti trimekaina, lidokaina i bupivakaina dlia prodlennoi blokady plechevogo spleteniia.

Local anesthetic effects of trimecaine, lidocaine and bupivacaine have been compared in high axillary brachial plexus blockade in 110 patients operated on the lower limb. The clinical pattern of conductive anesthesia and its efficacy (90 to 95.5%) did not depend on the kind of the anesthetic used. The rate of analgesia development in this technique is determined by physicochemical properties of the drug. Trimecaine effect is of minimum duration (130.0 +/- 1.3 min), while bupivacaine effect is of maximum duration (375.7 +/- 22.7 min). Lidocaine has an intermediate position (161.4 +/- 2.6 min).

[High conduction block of the brachial plexus using axillary approach]. / Vysokaia provodnikovaia blokada plechevogo spleteniia iz podmyshechnogo dostupa.

An original technique of the brachial plexus block from the axillary approach is presented. The recommended technique suggests introduction of a rigid catheter into the paraneural space of the plexus up to the level of the sternoclavicular triangle. This provides for a high efficacy of the anesthesia and prevents development of complications. The methodology was elaborated on the basis of anatomic (n = 20) and clinicoradiological (n = 29) studies. The clinical evidence was obtained from 115 anesthesias for long-term reconstructive and plastic operations on the upper limb. A protocol for conduction anesthesia maintenance by repeated administration of lidocaine, trimecaine, and bupivacaine solutions is offered.
