RESUMO

Our aim was to survey the opinions of Italian radiation and ENT oncologists regarding the role of postoperative radiotherapy (PRT) and the appropriate dose to be given to patients with remnant larynx (RL) after open partial laryngectomy (OPL). The radio-oncologists (ROs) of the Italian Radiation-Oncologist Association (AIRO) and the ENTs of the Head-Neck Oncology Society (AIOCC-IHNS) were contacted through a SurveyMonkey online interface questionnaire. There were 148 usable responses. The majority of ROs recommended PRT in the case of positive/close margins (R(+)/R(close)) or in the case of initial involvement of thyroid cartilage (pT3(tci)). In the same cases, ENTs prefer a "watch and wait" policy (w&w). Both disciplines recommended w&w in the case of negative margins (R(-)). Finally, the majority of RO s recommended irradiating RL with 62-66 Gy in R(+), with 56-66 Gy (61.4%) in R(close) and with 56-60 Gy (34%) in pT3(tci). In Conclusion, OPL raises new considerations about PRT.

The authors report the results of a census among the Piedmont region radiation therapy departments during the period 1980-1991 concerning non antineoplastic radiotherapy. Eight out of eleven centers respond to the questionnaire. During the period considered 36,480 patients were treated, with an annual mean of 4056; the number of treated patients varies from 1.2% to over 71.3% in the different centers when compared to the number of neoplastic patients. Arthrosis, osseous angioma, cheloides and verruca were the most treated diseases. Plesioroentgentherapy and roentgentherapy were normally employed; telecesium and telecobalt therapy were also used; only two centers used electron beams or brachytherapy. The doses of radiotherapy were not uniform; also patients' information and follow-up criteria were quite different in the various centers. The authors conclude with a guidelines for future radiotherapy in benign diseases.

Authors present results obtained by hyperthermic treatment (alone or associated with radiation therapy) on 50 patients (with a total of 66 sites) treated between September 1983 and August 1986. All patients had recurrent or metastatic lesions of pretreated malignant tumors. Among these there were 19 breast adenocarcinomas, 33 squamous cell carcinomas of head and neck, 9 melanomas and 5 subcutaneous metastases of adenocarcinoma of the cervix, rectum and colon. The clinical protocol was: hyperthermia alone (43 degrees -45 degrees C for 30 minutes, bi-weekly, for 4-5 weeks) or hyperthermia plus radiation therapy (4 Gy/fraction) for a total of 8-10 seances, in accordance with previous given doses of irradiation. In summary, complete response rates were 21.2% for head and neck tumors, and 51.5% for cutaneous or subcutaneous lesions. Results have also been analysed according to different prognostic variables related to tumor and treatment characteristics. These data are discussed.

RESUMO

A computerized system with automatic treatment parameters control for radiological hyperthermia, called Sapic SV03, is presented. The system has been planned and built by the "Sezione Avionica ed Equipaggiamenti" of the Aeritalia in cooperation with the Radiotherapy Department of the University of Turin. The device is supplied with a multifrequency generator system (915, 433, 2-30 MHz) connected with many kinds of applicators, with a fiber optic system for temperature control and a previsional thermometry system. In this paper the authors presented three new applicators. The first one is a concave parallel microstrip applicator at 433 MHz, with a size 16 x 9 cm; the heating pattern is homogenous until 4 cm of depth. This antenna can be used for the treatment of chest wall recurrences of breast cancer. The second is a 27 MHz inductive ring and the third applicator is a pyramidal antenna ("TEM line") that operates at a frequency around 27 MHz.

RESUMO

The system has been planned and built by the "Sezione Avionica ed Equipaggiamenti" of the Aeritalia in cooperation with the Radiotherapy Department of the University of Turin. The device is supplied with a multifrequency generator system (915, 433, 2-30 MHz) connected with many kinds of applicators (microstrip antennas, high constant dielectric material loaded antennas, waveguide antennas), with a fiber optic system for temperature control and with a previsional thermometry system.

All patients had recurrent lesion of pretreated malignant tumors. Among these there were brest adenocarcinomas, squamous cell carcinomas of head and neck, melanomas and soft tissues sarcomas. The clinical protocol was: hyperthermia alone (43 degrees-45 degrees C during 30 minutes) twice weekly (for a total of 8-10 treatments) or hyperthermia plus radiation therapy (4 Gy/fraction) for a total of 6-8 seances. In summary the response rate (CR + PR) was 80.4% (RT + HT 90% - HT alone 67.5%); the complete response rate was 24.5%.
