RESUMO

The Academic Life in Emergency Medicine (ALiEM) Approved Instructional Resources (AIR) Series was created in 2014 to address the Free Open Access Medical Education (FOAM) movement's decentralized nature and lack of inherent peer review. The AIR series provides a topic-based, curated list of online educational content vetted by academic emergency medicine (EM) faculty that meets individualized interactive instruction criteria for EM trainees. Relevant FOAM resources were identified from the top 50 FOAM websites using the Social Media Index and then scored by EM faculty using a validated instrument to identify the highest quality posts related to a topic. This article reviews FOAM resources pertaining to EM procedures that were labeled as an "Approved Instructional Resource" or "Honorable Mention" using the AIR series methodology.

RESUMO

The report describes a case of cutaneous blastomycosis. The patient was a healthy elderly male with a history of rectal adenocarcinoma, who presented with an ulcerating wound on his left buttock. Fungal culture was positive for Blastomyces dermatitidis, and the patient was treated with itraconazole followed by voriconazole for three months, which led to clinical resolution of the infection. This highlights an atypical case of blastomycosis, which presented as an isolated skin condition, without pulmonary or hematogenous complications. There are many challenges to diagnosing blastomycosis due to its wide range of symptoms, which can mimic other conditions, making it difficult to diagnose. Accurate diagnosis of blastomycosis is crucial to provide effective treatment and prevent potential complications, such as the infection spreading to other parts of the body and causing systemic symptoms. The report concludes by emphasizing the importance of a high index of suspicion for the diagnosis of cutaneous blastomycosis.

The diagnosis of secondary syphilis can be challenging due to its various clinical and histopathological presentations. A late or incorrect diagnosis can result in disease progression with consequent morbidity or mortality. Due to the importance of a correct diagnosis and specific treatment, it is of the utmost importance for healthcare providers to consider the various manifestations of syphilis. We describe an atypical presentation of secondary syphilis in an African American man infected with HIV and chronic psoriasis, who presented with two months of diffuse maculopapular rash and new visual changes, found to have acute syphilitic placoid chorioretinitis (ASPCC) and a psoriasis vulgaris flare.

Patients with advanced-stage gastroenteropancreatic neuroendocrine tumors (GEP-NETs) have a poor overall prognosis despite chemotherapy and radiotherapy (e.g., peptide receptor radionuclide therapy (PRRT)). Better treatment options are needed to improve disease regression and patient survival. The purpose of this study was to examine a new treatment strategy by combining PI3K/mTOR dual inhibition and radiotherapy. First, we assessed the efficacy of two PI3K/mTOR dual inhibitors, PF-04691502 and PKI-402, to inhibit pAkt and increase apoptosis in NET cell lines (BON and QGP-1) and patient-derived tumor spheroids as single agents or combined with radiotherapy (XRT). Treatment with PF-04691502 decreased pAkt (Ser473) expression for up to 72 h compared with the control; in contrast, decreased pAkt expression was noted for less than 24 h with PKI-402. Simultaneous treatment with PF-04691502 and XRT did not induce apoptosis in NET cells; however, the addition of PF-04691502 48 h after XRT significantly increased apoptosis compared to PF-04691502 or XRT treatment alone. Our results demonstrate that schedule-dependent administration of a PI3K/mTOR inhibitor, combined with XRT, can enhance cytotoxicity by promoting the radiosensitivity of NET cells. Moreover, our findings suggest that radiotherapy, in combination with timed PI3K/mTOR inhibition, may be a promising therapeutic regimen for patients with GEP-NET.

The microscopic origin of mechanical enhancement in polymer nanocomposite (PNC) melts is investigated through the combination of rheology and small-angle neutron scattering. It is shown that in the absence of an extensive particle network, the molecular deformation of polymer chains dominates the stress response on intermediate time scales. Quantitative analyses of small-angle neutron scattering spectra, however, reveal no enhanced structural anisotropy in the PNCs, compared with the pristine polymers under the same deformation conditions. These results demonstrate that the mechanical reinforcement of PNCs is not due to molecular overstraining, but instead a redistribution of strain field in the polymer matrix, akin to the classical picture of hydrodynamic effect of nanoparticles.

This Letter investigates the external deformation on modifying the polymer-nanoparticle (NP) and NP-NP interactions as well as their influences on the macroscopic properties of polymer nanocomposites (PNCs). Specifically, the applied uniaxial extension brings together the NPs along the transverse stretching direction and catalyzes the percolation transition from the initially well-dispersed NPs in the PNCs. The percolated NP network thus increases the strength of PNCs and leads to a strong surge in the elongation viscosity at very low strain rates, exhibiting the unexpected nonmonotonic strain rate dependence on the strain hardening. These results reveal explicitly the unconventional roles of the external deformation on modulating the relative strength of the polymer-NP and the NP-NP interactions, enabling advanced structures and properties control of PNCs.
