RESUMO

A man in his early 20s, originally from north-central Africa, presented with a high incidental serum creatinine level. A non-contrast CT scan was taken which was suggestive of urinary schistosomiasis. The diagnosis was confirmed on obtaining biopsies from the bladder. Due to bilateral hydronephrosis, an attempt at bilateral renal decompression was made. This failed to improve renal function. He eventually underwent a right nephrectomy. He is in end-stage renal failure, undergoing long-term dialysis, and has been placed on the waiting list for renal transplant.

RESUMO

Solitary Fibrous Tumours(SFT), previously known as haemangiopericytoma, are rarely encountered in surgery. They arise from mesenchyme tissue and can occur at several sites in the body - head and neck, extremities, thorax, abdomen and retroperitoneal space. In the thorax, where they arise from the pleura, and abdomen they may attain a large size before giving rise to symptoms. Most SFT behave in a benign manner. However a number of them recur locally or metastasize. Recurrences can occur several years after excision of the primary tumour. Complete surgical excision remains the primary modality of treatment. But, in sites where complete excision is not possible, other modalities have been tried with varying success. Here, we describe a SFT of the mesentery of the small intestine, an uncommon manifestation of the tumour, recurring after a period of 19 years in a 55 year old female, and presenting to the Emergency Department as an acute abdomen caused by acute intestinal obstruction. Surgical excision of the tumour was performed together with primary anastomosis of the small intestine. KEY WORDS: Acute abdomen, Late recurrence, Mesentery, Solitary Fibrous tumour.

As a source of biologically active compounds, buckwheat has beneficial effects in nutrition due to its high content of flavonoids, particularly rutin. Aim of our study was to examine effects of buckwheat on plasma lipid status and phospholipids fatty acids composition, histological and parameters of oxidative stress in Wistar rats fed a high-fat diet. This study showed that buckwheat leaf and flower (BLF) mixture supplementation significantly reduce weight gain, plasma lipid concentrations and atherogenic index in rats fed a high-fat diet. Treatment of the high-fat group of animals with buckwheat significantly increased percentage of n-6 fatty acids as well as eicosapentaenoic acid (EPA) and decreased percentage of saturated fatty acids (SFA) and oleic acid. Buckwheat antioxidant effects diminished negative influence of high-fat diet in hyperlipidemic rats, while pathohistological analysis of liver confirmed changes after high-fat consumption. Our results showed hypolipidemic, antiatherogenic and antioxidative features of buckwheat leaf and flower mixture, and these parts of the plant with the highest rutin content could be beneficial in prevention and curing of hyperlipidemia.

INTRODUCTION: Cystadenofibromas are tumors of the ovary which originate from the surface coelomic epithelium. Benign mucinous cystadenofibroma is a very rare form of these tumors, which consists of dominant stromal component of the connective tissue and one or more cysts. CASE REPORT: The case of a 62-year-old female with tumor of right ovary is reported in this paper. Histologically, tumor of the ovary had multilocular cystic formation, lined by a single-layer of mucoproductive cylindrical epithelium - endocervical type. In one area of tumor, the stromal component was abundant and made from partially hyalinised dense connective tissue. Mucinous cystadenofibroma was diagnosed on the basis of histological examination. Since the mucinous type of cystadenofibroma or adenofibroma is rare, this case has been chosen to be presented. CONCLUSION: Mucinous cystadenofibromas are differentially-diagnostically very similar to different malignant tumors and it is extremely important to make correct diagnosis of these neoplasms.

INTRODUCTION: Chronic necrotizing pulmonary aspergillosis (CNPA) is a cavitary, infectious process of lung parenchyma with slow progressive course. Vascular invasion and dissemination to other organs are unusual. CASE REPORT: We presented a 25-year old man with bilineal acute leukaemia who developed pulmonary and systemic symptoms. Chest CT showed nodular consolidations and cavitary lesions in both lungs. Bronchial biopsy revealed necrotic hyphae but it was negative for Aspergillus by culture. Serum was positive for antibodies to Aspergillus, but it was negative for antigens. A thoracoscopic lung biopsy of the upper left lobe revealed necrosis of lung tissue, with acute and chronic inflammation of the cavity wall and the presence of hyphae consistent with Aspergillus species. CONCLUSION: Although confirmation of the diagnosis is difficult, a combination of characteristic clinical, radiological and histological findings and either serological results positive for Aspergillus or the isolation of Aspergillus from respiratory samples are highly indicative of CNPA.

INTRODUCTION: Cavernous hemangiomas are benign vascular tumours rarely described in the lungs. Symptoms include respiratory distress, cardiac failure and massive haemoptysis, but they are mostly asymptomatic. CASE REPORT: A 67-year-old woman was referred to our institute and treated for pneumonia. A computed tomography scan of the thorax showed an infiltrative mass about 46mm in its greatest dimension in the right upper lobe. The mass was in contact with the mediastinal pleura. Since bronchial biopsy and FNAC did not reveal the aetiology of the lesion, the video-assisted thoracic surgery with right anterolateral thoracotomy and enucleation was performed. The intraoperative and postoperative pathohistological study showed cavernous hemangioma and this diagnosis was confirmed on immunohistochemical staining. CONCLUSION: In spite of its benign behaviour and mostly asymptomatic clinical course it is necessary to consider cavernous hemangioma in the differential diagnosis of other pulmonary lesions. The definitive diagnosis can be made only by histopathological and immunohistochemical examination.
