RESUMO

La angiomatosis bacilar (AB) es una enfermedad infec-ciosa poco frecuente, causada por bacterias del género Bartonella spp. transmitidas por vectores como pulgas, piojos y mosquitos. En el ser humano provoca diferentes síndromes clínicos. En pacientes con infección por el virus de inmunodeficiencia humana (VIH) con recuento de LT CD4 + <100 cél/µL se asocia a lesiones angiomatosas con neovascularización que comprometen la piel y, en menor medida, mucosas, hígado, bazo y huesos.El sarcoma de Kaposi (SK) es una neoplasia caracteriza-da por hiperplasia vascular multifocal de origen endotelial relacionada con el herpes virus humano 8. También puede afectar piel, mucosas y vísceras, siendo la variante epidé-mica una enfermedad marcadora de la infección avanzada por VIH. El principal diagnóstico diferencial clínico para las lesiones cutáneas y mucosas del SK es la AB.Presentamos un paciente con enfermedad VIH/sida que desarrolló AB y SK en forma concomitante en la misma lesión cutánea


Bacillary angiomatosis (BA) is a rare infectious disease, caused by bacteria of the genus Bartonella spp, transmitted by vectors such as fleas, lice and mosquitoes. It causes different clinical syndromes in humans. In patients with human immunodeficiency virus (HIV) infection with an LT CD4 + <100 cell/µL count, it is associated with the development of angiomatous lesions with neovascularization involving the skin and, with less frequency, mucous membranes, liver, spleen and bones. Kaposi's sarcoma (KS) is a neoplasm characterized by multifocal vascular hyperplasia of endothelial origin related to human herpes virus 8. It can also compromiso the skin, mucous membranes and viscera, with the epidemic variant being a marker disease of advanced HIV infection. The main clinical differential diagnosis for KS skin and mucosal lesions is the BA.Herein we present a patient with HIV/AIDS disease that developed BA and KS concomitantly in the same skin lesion

BACKGROUND: The inFLUenza Patient Reported Outcome (FLU-PRO) measure is a daily diary assessing signs/symptoms of influenza across six body systems: Nose, Throat, Eyes, Chest/Respiratory, Gastrointestinal, Body/Systemic, developed and tested in adults with influenza. OBJECTIVES: This study tested the reliability, validity, and responsiveness of FLU-PRO scores in adults with influenza-like illness (ILI). METHODS: Data from the prospective, observational study used to develop and test the FLU-PRO in influenza virus positive patients were analyzed. Adults (≥18 years) presenting with influenza symptoms in outpatient settings in the US, UK, Mexico, and South America were enrolled, tested for influenza virus, and asked to complete the 37-item draft FLU-PRO daily for up to 14-days. Analyses were performed on data from patients testing negative. Reliability of the final, 32-item FLU-PRO was estimated using Cronbach's alpha (α; Day 1) and intraclass correlation coefficients (ICC; 2-day reproducibility). Convergent and known-groups validity were assessed using patient global assessments of influenza severity (PGA). Patient report of return to usual health was used to assess responsiveness (Day 1-7). RESULTS: The analytical sample included 220 ILI patients (mean age = 39.3, 64.1% female, 88.6% white). Sixty-one (28%) were hospitalized at some point in their illness. Internal consistency reliability (α) of FLU-PRO Total score was 0.90 and ranged from 0.72-0.86 for domain scores. Reproducibility (Day 1-2) was 0.64 for Total, ranging from 0.46-0.78 for domain scores. Day 1 FLU-PRO scores correlated (≥0.30) with the PGA (except Gastrointestinal) and were significantly different across PGA severity groups (Total: F = 81.7, p<0.001; subscales: F = 6.9-62.2; p<0.01). Mean score improvements Day 1-7 were significantly greater in patients reporting return to usual health compared with those who did not (p<0.05, Total and subscales, except Gastrointestinal and Eyes). CONCLUSIONS: Results suggest FLU-PRO scores are reliable, valid, and responsive in adults with influenza-like illness.

Desde la pandemia de Influenza del año 2009, se realizaron campañas de vacunación, aumentaron las consultas médicas y el uso de medicación. Los datos recolectados durante las últimas temporadas nos permitieron describir incidencia de casos de influenza, comparar la incidencia de tipos y subtipos, y la frecuencia de vacunación. Para ello se realizó un estudio prospectivo, observacional de cohorte, sobre la población de pacientes que consultaron al Hospital J. M. Ramos Mejía desde octubre/2010 a septiembre/2013. Participaron 336 pacientes de los cuales 147 tuvieron resultado positivo para Influenza. La incidencia de casos de Influenza fue 43,7% sobre los pacientes con síntomas gri-pales durante el periodo evaluado. Se observó que el odds de presen-tar infección por influenza fue 3,8 veces de los pacientes no vacunados que los que recibieron la vacuna antigripal. Estos resultados permitie-ron observar una variación estacional de los subtipos virales de Influen-za, frecuencia de vacunación y complicaciones de nuestra población


Since the 2009 pandemic Influenza, vaccination campaigns were conducted, and medical visits and medication use increased. The data collected during recent flu seasons allowed us to describe Influenza type and subtype incidence, and to compare the incidence and frequency of vaccination. A prospective, observational cohort study was conducted of the patient population attended in the J. M. Ramos Mejia Hospital from October / 2010 to September / 2013. It involved 336 patients of which 147 were positive for influenza. The incidence of influenza was of 43.7% over the symptomatic subjects during the study period. It was observed that the odds of presenting influenza infection was 3.8 times in unvaccinated patients than those who received flu vaccine. These results show a seasonal variation of influenza viral subtypes, the vaccination frequency and the medical complications
