RESUMO

Venous catheters have become essential devices in the management of hospitalized patients. Indeed, according to several studies, 60 to 70 % of hospitalized patients require venous access during their stay. However, there is a wide range of venous catheters, and it is important to understand their main differences and indications. The main factors to consider for the choice are the degree of urgency, the expected duration of catheter maintenance, the number of infusions to be administ red, and the toxicity of the medication to the vascular endothelium.


Les cathéters veineux sont devenus des dispositifs indispensables à la prise en charge des patients hospitalisés. En effet, selon plusieurs études, 60 à 70 % des patients hospitalisés nécessitent un accès veineux durant leur séjour. Cependant, il existe tout un panel de cathéters veineux dont il convient de connaître les principales différences ainsi que les indications. Les éléments importants à prendre en compte pour le choix sont le degré d'urgence, la durée prévisionnelle de maintien du cathéter, le nombre de perfusions à administrer et la toxicité du médicament pour l'endothélium vasculaire.

RESUMO

The Swiss Health Care Atlas (ASSS) is a free platform accessible to healthcare professionals and the general public. It provides a detailed overview of healthcare practices and the use of pharmaceutical and technological resources in both acute and outpatient care settings throughout Switzerland. It serves as an interactive tool for visualizing medical services and practices in Switzerland, along with regional variations. The ASSS allows the identification of potential under and overutilization of healthcare services, making it a valuable planning and management tool for healthcare and political authorities. This article reviews healthcare practices related to general internal medicine and guides the reader in interpreting a selection of ASSS indicators.


L'Atlas suisse des services de santé (ASSS) est une plateforme gratuite accessible aux professionnels de santé, mais aussi au grand public. Il permet un aperçu détaillé des pratiques de soins et de l'utilisation des ressources pharmacologiques et technologiques, en milieux hospitalier et/ou ambulatoire, sur le territoire helvétique. Il s'agit d'un moyen interactif permettant de visualiser certaines prestations médicales, ainsi que leur variation régionale. L'ASSS permet d'identifier une potentielle sous/surutilisation des services de soins et constitue ainsi un outil de planification et de pilotage pour les autorités sanitaires et politiques. Cet article passe en revue les pratiques de soins liées à la médecine interne générale et guide le lecteur dans l'interprétation d'une sélection d'indicateurs de l'ASSS.

According to PubMed statistics when writing this review, the year 2022 is expected to mark the first dip in the number of articles published in relation to the Covid-19 pandemic. This review, without any mention to Sars-CoV-2, highlight this transition and addresses many topics in internal medicine: gastroenterology, cardiology, endocrinology, respiratory medicine, infectious diseases and venous access. Each year, the chief residents of the internal medicine ward in Lausanne university hospital (CHUV) in Switzerland meet up to share their readings: here is a selection of ten articles that have caught our attention, summarized and commented for you, which should change our daily practice.


D'après les statistiques PubMed au moment de la rédaction de cette revue, l'année 2022 devrait marquer le premier infléchissement du nombre d'articles publiés en relation avec la pandémie de Covid-19. Cette revue d'articles, sans écho au Sars-CoV-2, souligne cette transition et aborde de nombreux sujets de la médecine interne : gastroentérologie, cardiologie, endocrinologie, pneumologie, infectiologie et accès veineux. Chaque année, les cheffes et chefs de clinique du Service de médecine interne du CHUV se réunissent pour partager leurs lectures : voici une sélection de dix articles ayant retenu notre attention, revus et commentés pour vous, et qui devraient faire évoluer notre pratique quotidienne.

L'année 2021, malgré la pandémie de Covid-19, a vu de nombreux progrès en médecine interne générale, avec de multiples implications pour notre pratique quotidienne. D'une durée diminuée d'antibiothérapie pour le traitement de la pneumonie communautaire à de nouvelles indications au traitement de colchicine, en passant par des précisions sur les cibles de transfusion érythrocytaire, ainsi qu'une remise en question de l'intérêt de la vitamine D à haute dose dans la prévention des chutes chez la personne âgée, et pour finir des espoirs déçus de nouvelle indication à la substitution d'albumine dans la cirrhose, les nouveautés abondent dans la littérature. RESUMO

Midline long peripheral venous catheters are an interesting alternative to central venous catheters and PICC-lines when the placement of a peripheral venous catheter is impossible or when the patient requires prolonged intravenous treatment. Midline catheters can be inserted at the patient's bedside and require no radiological verification after insertion. They can be kept in place for up to 14 days and allow for repeated blood sampling. The mechanical, infectious, or thrombotic complications and safety profile do not differ from other venous catheters, notably PICC-line.


Lorsque la pose d'un cathéter veineux périphérique est impossible ou que le patient nécessite un traitement intraveineux prolongé, les cathéters veineux périphériques longs, de type Midline, représentent une alternative intéressante aux cathéters veineux centraux et PICC-line (Peripherally Inserted Central Catheters). Les Midlines peuvent être mis en place au lit du patient, sans nécessiter un contrôle radiologique après la pose. Ils peuvent rester en place jusqu'à 14 jours et permettent d'effectuer des prélèvements sanguins répétés. Les complications mécaniques, infectieuses ou thrombotiques, et le profil de sécurité ne diffèrent pas des autres cathéters veineux, notamment des PICC-lines.

Point-of-Care Ultrasonography (POCUS) is an emerging important diagnostic tool in internal medicine wards. The concept is to focus on specific or binary questions to rule in disease and has a proven additional benefit for procedural guidance in terms of patients' safety. Its advantages lie in its rapid and non-invasive use at the patient's bedside. It does not replace the traditional diagnostic procedures or specialists' investigations. Since medical decisions can be guided by ultrasounds results, it's important to remember that the diagnostic performance is largely operator dependent. Implementation of POCUS in internal medicine wards is still challenging regarding the small number of ultrasound experts to provide education programs and the lack of well-established learning objectives and training programs.


L'ultrasonographie ciblée (POCUS) a gagné en importance dans la pratique hospitalière comme aide pour l'orientation diagnostique. Devenue incontournable pour la réalisation de procédures invasives de façon sécuritaire, ses atouts résident dans sa capacité de répondre rapidement, au lit du malade, de façon binaire (oui/non), à des questions spécifiques. Il reste important de se souvenir que son recours ne remplace pas les démarches diagnostiques traditionnelles, ni l'examen du spécialiste, et que la qualité d'un examen et des conclusions en découlant est hautement dépendante du niveau de formation et des compétences de l'utilisateur. L'implémentation sécuritaire du POCUS en médecine interne reste encore un défi au niveau de la formation des cliniciens, notamment en termes de cursus, encore non clairement défini, et du faible nombre de formateurs.

Thiamine-responsive megaloblastic anaemia (TRMA) is a syndrome associated with megaloblastic anaemia, diabetes mellitus and sensorineural deafness, due to mutations in the SLC19A2 gene, which codes for a thiamine carrier protein. Oral thiamine supplementation is the main treatment. We report the case of a 25-year-old woman known for TRMA, who presented with pancytopenia (haemoglobin 7.6 g/dL, leucocytes 2.9×109/L, thrombocytes 6×109/L) revealed by dyspnoea. Investigations excluded coagulopathy, a recent viral infection, vitamin and iron deficiencies, and a malignant process. We later found out that thiamine treatment had been discontinued 5 weeks before, due to prescription error. Parenteral thiamine administration resulted in the recovery of haematopoiesis within 3 weeks. Pancytopenia is uncommon in patients with TRMA. Pre-existing medullary impairment caused by the patient's daily antipsychotic medications or the natural course of the syndrome may explain the severity of the laboratory findings in our patient.

Medically unexplained symptoms (MUS) are common among adolescents and are frequently encountered in primary care. Our aim was to explore how these adolescents and their parents experience the condition and its impact on their daily lives and to provide recommendations for health professionals. Using a qualitative approach, six focus groups and two individual interviews were conducted. These involved a total of ten adolescents with different types of MUS and sixteen parents. The respondents were recruited in a university hospital in Switzerland. A thematic analysis was conducted according to the Grounded Theory. The analysis of the data highlighted four core themes: disbelief, being different, concealing symptoms, and priority to adolescent's health. Transcending these themes was a core issue regarding the discrepancy between the strategies that adolescents and their parents use to cope with the symptoms. Health professionals should be made aware of the emotional needs of these patients and their families.
