Background: Older patients admitted to hospitals have a greater impact on the healthcare system as the population ages. The relationship between the recovery of functional impairments and frailty status in geriatric care units is still not clear. Simple screening technologies are required in order to operationalize frailty management in this susceptible population due to these restrictions. Aim: The study aims to screen hospitalized older adults for frailty and associated co-morbid conditions in western Gujarat, India. Materials and Methods: This is an institutionally based cross-sectional study conducted on the elderly patients (aged 60 years or more) admitted at the tertiary-level government hospital of Jamnagar District of Gujarat State during the period of October 22 to December 22. The assessment was done with a structured questionnaire for FRAIL screen, the Rapid Cognitive Screen (RCS), Charles Co-morbidity Index, Geriatric Depression Scale-5 (GDS-5), and Short Form-12 (SF-12) Health Survey. Results: The overall findings of this study reveal that of 124 participants 34 (27%) were frail, 52 (42%) were found to be a likelihood of depression by the GDS, and 29 (23%) were having dementia by RCS, respectively. In our study, we also found a statistically significant association between frailty and dementia (P value < 0.001). Conclusion: The present study implies the prevalence of frailty among old age elderly patients and its association with various socio-demographic and co-morbid conditions of the participants. Early identification of frailty and co-morbid conditions can help to prevent adverse health outcomes.

Nitrogen (N) and phosphorus (P) are vital for crop growth. However, most agricultural systems have limited inherent ability to supply N and P to crops. Biochars (BCs) are strongly advocated in agrosystems and are known to improve the availability of N and P in crops through different chemical transformations. Herein, a soil-biochar incubation experiment was carried out to investigate the transformations of N and P in two different textured soils, namely clay loam and loamy sand, on mixing with rice straw biochar (RSB) and acacia wood biochar (ACB) at each level (0, 0.5, and 1.0% w/w). Ammonium N (NH4-N) decreased continuously with the increasing incubation period. The ammonium N content disappeared rapidly in both the soils incubated with biochars compared to the unamended soil. RSB increased the nitrate N (NO3-N) content significantly compared to ACB for the entire study period in both texturally divergent soils. The nitrate N content increased with the enhanced biochar addition rate in clay loam soil until 15 days after incubation; however, it was reduced for the biochar addition rate of 1% compared to 0.5% at 30 and 60 days after incubation in loamy sand soil. With ACB, the net increase in nitrate N content with the biochar addition rate of 1% remained higher than the 0.5% rate for 60 days in clay loam and 30 days in loamy sand soil. The phosphorus content remained consistently higher in both the soils amended with two types of biochars till the completion of the experiment.

In the plains of western North India, traditional rice and wheat cropping systems (RWCS) consume a significant amount of energy and carbon. In order to assess the long-term energy budgets, ecological footprint, and greenhouse gas (GHG) pollutants from RWCS with residual management techniques, field research was conducted which consisted of fourteen treatments that combined various tillage techniques, fertilization methods, and whether or not straw return was present in randomized block design. By altering the formation of aggregates and the distribution of carbon within them, tillage techniques can affect the dynamics of organic carbon in soil and soil microbial activity. The stability of large macro-aggregates (> 2 mm), small macro-aggregates (2.0-2.25 mm), and micro-aggregates in the topsoil were improved by 35.18%, 33.52%, and 25.10%, respectively, over conventional tillage (0-20 cm) using tillage strategies for conservation methods (no-till in conjunction with straw return and organic fertilizers). The subsoil (20-40 cm) displayed the same pattern. In contrast to conventional tilling with no straw returns, macro-aggregates of all sizes and micro-aggregates increased by 24.52%, 28.48%, and 18.12%, respectively, when conservation tillage with organic and chemical fertilizers was used. The straw return (aggregate-associated C) also resulted in a significant increase in aggregate-associated carbon. When zero tillage was paired with straw return, chemical, and organic fertilizers, the topsoil's overall aggregate-associated C across all aggregate proportions increased. Conversely, conventional tillage, in contrast to conservation tillage, included straw return as well as chemical and organic fertilizers and had high aggregate-associated C in the subsurface. This study finds that tillage techniques could change the dynamics of microbial biomass in soils and organic soil carbon by altering the aggregate and distribution of C therein.

Background: The sustainability of paddy production systems in South Asia has recently been affected by a decline in soil health and excessive water usage. As a response to the global energy crisis, escalating costs of synthetic fertilisers, and growing environmental concerns, the utilization of organic plant-nutrient sources has gained considerable attention. Emerging adaptation technologies, including conservation tillage and innovative approaches to fertilizer management, present practical choices that can significantly contribute to the long-term preservation of soil fertility. Methods: The two year-long field experiment was completed in sandy loam soil during rainy (Kharif) seasons in 2019 and 2020 at the crop research centre farm of Sardar Vallabhbhai Patel University of Agricultural & Technology, Meerut, Uttar Pradesh to analyze the impacts of different tillage establishment of the crop and its methodologies as well as integrated nutritional management approaches on rice growth, yield, productivity of water, nutrient uptake, and fertility status of soil under a rice-wheat rotation system. The experiment was set up in a factorial randomized block design and replicated three times in a semi-arid subtropical environment. Results: The conventionally transplanted rice puddled (CT-TPR) grew substantially better taller plants, and higher dry matter buildup leads to increased yields than transplanted rice under raised wide bed (WBed-TPR). WBed-TPR plots had more tillers, LAI, CGR, RGR, and yield characteristics of the rice in two year study. CT-TPR increased grain yield by 4.39 and 4.03% over WBed-TPR in 2019 and 2020, while WBed-TPR produced the highest water productivity (0.44 kg m-3) than CT-TPR, respectively. The 100% RDF+ ZnSO4 25 kg ha-1 + FYM (5 t ha-1) + PSB (5 kg ha-1) + Azotobacter 20 kg ha-1 (N6) treatment outperformed the other fertiliser management practices in terms of crop growth parameters, yields of grain (4,903 and 5,018 kg ha-1), nutrient uptake and NPK availability, organic soil carbon. Among the fertilizer management practices, with the direct applications of the recommended dose of fertilizer (RDF), farm yard manure (FYM), phosphate solubilizing bacteria (PSB), Azatobactor and zinc worked synergistically and increased grain yields by 53.4, 51.3, 47.9 and 46.2% over their respective control treatment. Conclusions: To enhance rice productivity and promote soil health, the study suggests that adopting conservation tillage-based establishment practices and implementing effective fertilizer management techniques could serve as practical alternatives. It is concluded that the rice yield was improved by the inclusive use of inorganic fertiliser and organic manure (FYM). Additionally, the study observed that the combination of conventional puddled transplanted rice (CT-TPR) and N6 nitrogen application resulted in enhanced rice crop productivity and improved soil health.

A continuous indirect electro-oxidation (EO) process was developed using graphite electrode to investigate the treatability of reactive turquoise blue RTB21 dye wastewater under specific operating conditions of initial pH, current density, hydraulic retention time (HRT), and electrolyte (NaCl) concentration. The experiments were performed in accordance with the central composite design (CCD), and the findings were used to create a model utilizing artificial neural networks (ANNs). According to the predicted findings of the ANN model, the MSE values for colour and COD removal efficiencies were estimated to be 0.748 and 0.870, respectively, while the R2 values were 0.9999 and 0.9998, respectively. The Multi-objective optimization using genetic algorithm (MOGA) over the ANN model maximizes the multiple responses: colour and COD removal efficiency (%). The MOGA generates a non-dominated Pareto front, which provides an insight into the process's optimum operating conditions.

BACKGROUND: Both hypertension (HTN) and tobacco addiction pose a threat to the health, environment, and socioeconomic status (SES) of the people. When tobacco use disorder exists in people with HTN, it hastens the disease progress and causes early complications. The present study aimed to study the knowledge and practice of tobacco addiction in patients with HTN and find out the correlates of knowledge and practice of tobacco addiction. METHODS: A cross-sectional study was conducted for a period of one year in the Jamnagar District of Western Gujarat, India. Out of total 400 samples, 50% were collected from the five selected Community Health Centers (CHCs) by random sampling and the rest from non-communicable disease (NCD) clinics at the tertiary care hospital of the district. FINDINGS: Most of the patients were in their fifties or above (67.0%), women (57.0%), and married (86.5%). Only 12.0% had awareness about the hazards of tobacco addiction and the prevalence of tobacco addiction was 11%. It was found that use of smokeless tobacco (SLT) among samples was the highest (72.7%) followed by dual consumption, i.e., SLT and smoking (20.5%). The frequency of consumption was ≥ 5 times/day in 54.5%, 70.4% were addicted for > 10 years, and only 15.9% had ever tried to quit tobacco while only 11.4% had successfully quit it. It was seen that those who were aged < 50 years, men, literates, employed, and those belonging to higher SES and urban residence had better knowledge of the health hazards. This was found to have significant statistical association. CONCLUSION: The results provide valuable insight into the tobacco addiction in patients with HTN on which non-pharmacological treatment of HTN can be based.

INTRODUCTION: Hypertension poses a global challenge in terms of morbidity and mortality. Worldwide prevalence of hypertension is over 40%. Management of hypertension targets blood pressure control to prevent disease complications. Though stress and hypertension are closely related, stress management is often overlooked in the treatment of hypertension. AIMS: (1) To estimate the prevalence of stress in hypertensive patients and (2) to study the associated risk factors of stress and its implications in disease management. MATERIALS AND METHODS: It was a hospital-based, cross-sectional study done in Western India for 1 year. Data were collected from 400 hypertensive patients attending the selected health institutions using a pretested questionnaire. Chi-square tests were done using Medcalc 10.4.8.0. RESULTS: The prevalence of stress in hypertensive patients was found to be 84.3%. Only 2.4% of these patients sought help from any health professional for stress. The most common stressors found in the patients were financial dependence on others, living in rented house, having a daughter of marriageable age because of associated dowry, death of a loved one, sleep-related problem, and owing a debt among others. Significant statistical association (P < 0.05) of stress was observed with the type of family and socioeconomic status. A highly significant association (P < 0.001) of stress with religion and residential area (whether urban non-slum, slum, or rural) was observed. Stress in individuals leads to poorer compliance with treatment and blood pressure control. CONCLUSION: Coexistent stress should be diagnosed and managed in patients of hypertension for proper disease management and control.

Genetic diversity was assessed among 53 Indian garlic accessions using SSR markers. Initially, 24 SSR primer pairs were used for screening three selected garlic accessions. Out of 24 SSR primer pairs, 10 primer pairs which consistently showed good amplification and polymorphism were selected for DNA profiling. SSR primer pairs showed PIC values ranging from 0.30 to 0.99. Based on AMOVA we found that the greater part of the genetic diversity was expected due to intra population with 84% variation and only 16% of variation was due to among populations suggesting presence of genetic structure. The results of cluster analysis and principal component analysis largely correspond to each other. Population structure analysis revealed genetic differentiation of accessions. The results of present study revealed existence of significant variability in Indian garlic germplasm.

Existing vital sign monitoring systems in the neonatal intensive care unit (NICU) require multiple wires connected to rigid sensors with strongly adherent interfaces to the skin. We introduce a pair of ultrathin, soft, skin-like electronic devices whose coordinated, wireless operation reproduces the functionality of these traditional technologies but bypasses their intrinsic limitations. The enabling advances in engineering science include designs that support wireless, battery-free operation; real-time, in-sensor data analytics; time-synchronized, continuous data streaming; soft mechanics and gentle adhesive interfaces to the skin; and compatibility with visual inspection and with medical imaging techniques used in the NICU. Preliminary studies on neonates admitted to operating NICUs demonstrate performance comparable to the most advanced clinical-standard monitoring systems.

Pseudouridine (Ψ) is the most abundant post-transcriptionally modified ribonucleoside. Different Ψ modifications correlate with stress responses and are postulated to coordinate the distinct biological responses to a diverse panel of cellular stresses. With the help of different guide RNAs, the dyskerin complex pseudouridylates ribosomal RNA, small nuclear RNA and selective messenger RNAs. To monitor Ψ levels quantitatively, a previously reported high performance liquid chromatography method coupled with ultraviolet detection (HPLC-UV) was modified to determine total Ψ levels in different cellular RNA fractions. Our method was validated to be accurate and precise within the linear range of 0.06-15.36 pmol/µL and to have absolute Ψ quantification levels as low as 3.07 pmol. Using our optimized HPLC assay, we found that 1.20% and 1.94% of all ribonucleosides in nuclear-enriched RNA and small non-coding RNA pools from the HEK293 cell line, and 1.77% and 0.98% of ribonucleosides in 18S and 28S rRNA isolated from the HeLa cell line, were pseudouridylated. Upon knockdown of dyskerin expression, a consistent and significant reduction in total Ψ levels in nuclear-enriched RNA pools was observed. Our assay provides a fast and accurate quantification method to measure changes in Ψ levels of different RNA pools without sample derivatization.

This paper presents an energy-efficient and high-throughput architecture for Sparse Distributed Memory (SDM)-a computational model of the human brain [1]. The proposed SDM architecture is based on the recently proposed in-memory computing kernel for machine learning applications called Compute Memory (CM) [2], [3]. CM achieves energy and throughput efficiencies by deeply embedding computation into the memory array. SDM-specific techniques such as hierarchical binary decision (HBD) are employed to reduce the delay and energy further. The CM-based SDM (CM-SDM) is a mixed-signal circuit, and hence circuit-aware behavioral, energy, and delay models in a 65 nm CMOS process are developed in order to predict system performance of SDM architectures in the auto- and hetero-associative modes. The delay and energy models indicate that CM-SDM, in general, can achieve up to 25 × and 12 × delay and energy reduction, respectively, over conventional SDM. When classifying 16 × 16 binary images with high noise levels (input bad pixel ratios: 15%-25%) into nine classes, all SDM architectures are able to generate output bad pixel ratios (Bo) ≤ 2%. The CM-SDM exhibits negligible loss in accuracy, i.e., its Bo degradation is within 0.4% as compared to that of the conventional SDM.

UNLABELLED: The anaerobic intestinal spirochaete Brachyspira pilosicoli colonizes the large intestine of various species of mammals and birds, where it may induce colitis. Strains of the spirochaete have also been isolated from the bloodstream of immunocompromised human patients and have been seen in liver sections, and a similar systemic spread was recently observed in experimentally infected chickens. Some other spirochaete species that may be present in blood attach to and aggregate erythrocytes, and this is believed to contribute to disease severity. The aim of the current study was to determine whether B. pilosicoli strains have the capacity to attach to and aggregate erythrocytes. Initially, four strains of B. pilosicoli were incubated with erythrocytes from sheep, cows, pigs, dogs, humans, chickens and geese, and were observed by phase-contrast microscopy. Only strain WesB attached, and this was only with erythrocytes from chickens and geese. Subsequently, six other strains of B. pilosicoli were tested just with goose erythrocytes, and five attached to and caused aggregation of the erythrocytes. Scanning and transmission electron microscopy demonstrated that spirochaetes abutted and apparently firmly attached to the erythrocyte membranes. Aggregation of erythrocytes by B. pilosicoli may contribute to disease severity in species that develop a spirochaetaemia. SIGNIFICANCE AND IMPACT OF THE STUDY: The intestinal spirochaete Brachyspira pilosicoli has been isolated from the bloodstream of immunocompromised human patients, and spread to the liver has been reported in humans and in experimentally infected chickens. In this study, B. pilosicoli was shown to undergo attachment by one cell end to chicken and goose erythrocytes in vitro and to aggregate them. This activity has the potential to contribute to disease severity in avian and possibly other species that develop a spirochaetaemia and systemic spread. Avian erythrocytes may be useful for studying the mechanisms by which B. pilosicoli attaches to cells.

Dyskerin (encoded by the DKC1 locus) is the pseudouridine synthase responsible for the modification of noncoding RNA. Dyskerin is also an obligate member of the telomerase enzyme, and participates in the biogenesis of telomerase. Genetic lesions at the DKC1 locus are associated with X-linked dyskeratosis congenita (X-DC) and the Hoyeraal-Hreidarsson Syndrome (HHS). Both syndromes have been linked to deficient telomere maintenance, but little is known about the RNA modification activities of dyskerin in X-DC and HHS cells. To evaluate whether X-DC-associated dyskerin mutations affect the modification or function of ribosomal RNA, we studied five telomerase-rescued X-DC cells (X-DC(T) ). Our data revealed a small reproducible loss of pseudouridines in mature rRNA in two X-DC variants. However, we found no difference in protein synthesis between telomerized wild-type (WT(T) ) and X-DC(T) cells, with an internal ribosomal entry site translation assay, or by measuring total protein synthesis in live cells. X-DC(T) cells and WT(T) cells also exhibited similar tolerances to ionizing radiation and endoplasmic reticulum stress. Despite the loss in rRNA pseudouridine modification, functional perturbations from these changes are secondary to the telomere maintenance defects of X-DC. Our data show that telomere dysfunction is the primary and unifying etiology of X-DC.

BACKGROUND: The pre-marital sex and live-in relationship among young people are increasing at an alarming rate. Remote consequences of such high risk behaviors are increase in the incidence of STDs (including HIV), unsafe and illegal abortion, adolescent pregnancy and motherhood, single mother child/abandoned child, juvenile delinquency and many more. OBJECTIVES: The aim of this study was to investigate the high-risk sexual behaviors in depth, influenced by various factors including age at sexual debut, type of partners, consistent condom usage, hostel stay, socioeconomic class, etc. among college-going male youth. MATERIALS AND METHODS: The study was conducted in Jamnagar among undergraduate (18-24 years) male college students. A total of 450 students were randomly selected from three colleges of Jamnagar. RESULTS: Out of all 450 participants, 49.11% were in the age group of 18-20 years. Among study subjects, 13.78% had one or more pre-marital sexual exposures. In students with positive pre-marital sexual history, the various sex partners were girlfriends (95.16%), commercial sex workers (14.5%), homosexuals (6.45%), and multiple sex partners (33.88%). Among students, 62.9% were using condom consistently. Three-fifth of the ones indulged in premarital sex, were in the age group of 16-20 at the time of sexual debut. CONCLUSIONS: Most of the students were quite young (16-18 years) at the time of first pre-marital sexual exposure. Consistent condom usage was not uniform. The students staying at hostels, indulged in premarital sex, were found to have multiple sex partners.

Telomerase is a specialized reverse transcriptase responsible for the de novo synthesis of telomeric DNA repeats. In addition to its established reverse transcriptase and terminal transferase activities, recent reports have revealed unexpected cellular activities of telomerase, including RNA-dependent RNA polymerization. This telomerase characteristic, distinct from other reverse transcriptases, indicates that clinically relevant reverse transcriptase inhibitors might have unexpected telomerase inhibition profiles. This is particularly important for the newer generation of RT inhibitors designed for anti-HIV therapy, which have reported higher safety margins than older agents. Using an in vitro primer extension assay, we tested the effects of clinically relevant HIV reverse transcriptase inhibitors on cellular telomerase activity. We observed that all commonly used nucleoside reverse transcriptase inhibitors (NRTIs), including zidovudine, stavudine, tenofovir, didanosine and abacavir, inhibit telomerase effectively in vitro. Truncated telomere synthesis was consistent with the expected mode of inhibition by all tested NRTIs. Through dose-response experiments, we established relative inhibitory potencies of NRTIs on in vitro telomerase activity as compared to the inhibitory potencies of the corresponding dideoxynucleotide triphosphates. In contrast to NRTIs, the non-nucleoside reverse transcriptase inhibitors (NNRTIs) nevirapine and efavirenz did not inhibit the primer extension activity of telomerase, even at millimolar concentrations. Long-term, continuous treatment of human HT29 cells with select NRTIs resulted in an accelerated loss of telomere repeats. All tested NRTIs exhibited the same rank order of inhibitory potencies on telomerase and HIV RT, which, according to published data, were orders-of-magnitude more sensitive than other DNA polymerases, including the susceptible mitochondria-specific DNA polymerase gamma. We concluded that telomerase activity could be inhibited by common NRTIs, including currently recommended RTI agents tenofovir and abacavir, which warrants large-scale clinical and epidemiological investigation of the off-target effects of long-term highly active antiretroviral therapy (HAART) with these agents.

Aqueous solution containing two additives, silver nitrate (AgNO(3)) and sodium salt of N-cholyl amino acid were irradiated by sunlight for the synthesis of spherical shaped AgNPs without the need for an additional stabilizer or capping agent. Variations of N-cholyl amino acid concentration provided good control over the morphology of the AgNPs, while the carboxylate group of bile salt reduced the Ag(+) ions and the amide group binds strongly to the surface of the NPs. The optical properties, morphology of the AgNPs were characterized by UV-visible, transmission electron microscopy (TEM) and dynamic light scattering (DLS) studies. The interaction of N-cholyl amino acid on the AgNPs surface was studied using cyclic voltammetry and FT-IR techniques. The reduction process was completed within 5 min and the synthesized AgNPs were stable for more than 6 months. The possible mechanism of N-cholyl amino acid on the reduction and stabilization of AgNPs is also discussed. The antimicrobial activity of N-cholyl amino acid capped AgNPs against Escherichia coli, Staphylococcus aureus and Pseudomonas aeroginosa using Mueller Hinton broth and the antifungal activity against Candida albicans, Candida krusei and Candida tropicalis using RPMI broth were determined by MIC studies as per CLSI guidelines.

X-linked dyskeratosis congenita (X-DC) is caused by mutations in the housekeeping nucleolar protein dyskerin. Amino acid changes associated with X-DC are remarkably heterogeneous. Peripheral mononuclear blood cells and fibroblasts isolated from X-DC patients harbor lower steady-state telomerase RNA (TER) levels and shorter telomeres than healthy age-matched controls. Previously, we showed that retroviral expression of recombinant TER, together with expression of recombinant telomerase reverse transcriptase, restored telomere maintenance and proliferative capacity in X-DC patient cells. Using rare X-DC isoforms (ΔL37 and A386T dyskerin), we showed that telomere maintenance defects observed in X-DC are solely due to decreased steady-state levels of TER. Disease-associated reductions in steady-state TER levels cause deficiencies in telomere maintenance. Here, we confirm these findings in other primary X-DC patient cell lines coding for the most common (A353V dyskerin) and more clinically severe (K314R and A353V dyskerin) X-DC isoforms. Using cell lines derived from these patients, we also examined the steady-state levels of other hinge-ACA motif RNAs and did not find differences in their in vivo accumulations. We show, for the first time, that purified telomerase holoenzyme complexes from different X-DC cells have normal catalytic activity. Our data confirm that dyskerin promotes TER stability in vivo, endorsing the development of TER supplementation strategies for the treatment of X-DC.

INTRODUCTION: More than one third of reported cases of HIV/AIDS in India are among youth and 60 percent of these reside in rural areas. Assessment of the awareness of HIV/AIDS in the youth is important for determining the impact of previous and current awareness programs as well as the need for interventions. This study aimed to assess the knowledge of rural youth regarding HIV/AIDS and to explore the epidemiological determinants of awareness among them. METHODOLOGY: A community-based cross-sectional study was conducted among youths aged 15-24 years in rural areas of the Saurashtra region of Gujarat, India. A cluster sampling design was used, surveying 50 subjects from each of 30 clusters. Data was collected through house-to-house visits using a semi-structured questionnaire. Proportions and logistic regression were used for analysis. RESULTS: Out of a total of 1,237 subjects who participated in survey, 60% knew something about HIV. Of those who had heard of HIV, more than 90% subjects knew the modes of transmission and more than 80% were aware of modes of prevention of HIV/AIDS. One fifth of the subjects had misconceptions in relation to HIV/AIDS. On applying multiple logistic regression, age, education, occupation, and mass media exposure were found to be the major determinants of their knowledge with regard to HIV/AIDS. CONCLUSIONS: Basic knowledge of HIV/AIDS is still lacking in two fifths of the rural youth. Literacy and media exposure are factors that determine awareness of HIV among them and can be helpful to raise their knowledge regarding this scourge.
