Background: Phytochemicals are considered the reliable source for the treatment of infection including malaria. Especially, phenols are known as potentially toxic to the growth and development of pathogens, among which flavonoids are the most extensively studied and play more intensive roles in ethnopharmacology. The immunological effect and role of T. diversifolia flavonoids-rich extract in treatment of malaria have therefore been examined in this study. Methods: In vitro test against Plasmodium falciparum and 4-day suppressive and Rane's tests against Plasmodium berghei in mice were used to evaluate the antimalarial activities. TNF-α and INF-γ levels, phagocytic tests, and production of oxygen and nitrogen radical were assessed to appreciate the immunomodulatory activity. One-way analysis of variance followed by post hoc Student's t tests was used for data analysis. Results: T. diversifolia flavonoids-rich extract at the concentrations ranging from 0.0004 mg/ml significantly (p < 0.05) inhibited in a concentration-dependent manner the growth of trophozoite up to 100% inhibition with 0.025 mg/ml at 24 and 48 hrs. Moreover, T. diversifolia flavonoids-rich extract reduced the level of parasitemia and improved in a dose-dependent manner the survival time of infected mice significantly (p < 0.05) compared to their control in 4-day suppressive test as well as in Rane's test. Additionally, T. diversifolia flavonoids-rich extract increased the TNF-α and INF-γ levels in rats infected by P. berghei. Furthermore, the flavonoid-rich extract enhanced weight of spleen in the rats, the metabolic and phagocytic activities of the peritoneal cells, and the concentration of nitric oxide and oxygen radicals in methylprednisolone-immunocompromised rats compared to the control (p < 0.05). Conclusion: The study has revealed that T. diversifolia flavonoids-rich extract through its antiplasmodial and phagocytic activities is a promising treatment of malaria.

The diagnosis of onchocerciasis in endemic areas has been demanding given the need to replace the invasive skin snip method with a more sensitive and specific rapid point-of-contact tool. Filarial antigen detection tests are better alternative methods in diagnosing Onchocercal infections, as they detect infections and could be used to monitor transmission in endemic areas following mass drug administration. With the shift in paradigme from control to elimination, a rapid point- of-contact tool is required to support elimination programs. This was a cross-sectional, community-based study conducted in 50 villages selected from six health districts using a systematic sampling technique. Individuals ≥17 years who had lived in the community for a duration of 5 years and above provided blood specimens for IgG4 antibodies testing against O. volvulus antigens. Data were analyzed using SPSS v.20 and expectation maximization to classify optical densities for positive and negative samples from ELISA results. The kappa statistics was used to measure the level of agreement between the two tests. In a total of 5001 participants which were recruited for the study, 4416 (88.3 %) participant samples passed the plate quality control criteria and were considered for the test comparison analysis. Out of the 4416 participants, 292 (6.6 %) tested positive with Ov16 RDT and 310 (7.0 %) with Ov16 ELISA. All those who tested positive with the rapid test agreed positive with ELISA. The overall percentage agreement was 99.2 %, the Kappa score of 0.936. The results obtained indicate an excellent agreement between ELISA and RDT as measured by kappa (0.936) which was statistically significant (P<0.001). Our experience with the Ov16 ELISA biplex rapid test was favorable. However, the Ov16 RDT test may be more appropriate to use in remote areas for the point diagnosis of onchocerciasis in view towards achieving elimination in Africa.

BACKGROUND: Malaria is a growing problem in Africa, with prevalence varies from areas to areas due to several factors including the altitude. This study aimed to investigate the malaria distribution and its relationship with level of some blood parameters and plasma myeloperoxidase (MPO) in population of three localities with different altitudes. METHODS: A total of 150 participants were recruited in each locality and facial body temperature of each was measured using a forehead digital thermometer. Blood samples were collected and used diagnose malaria parasite using the rapid test followed by Giemsa stain microscopy and have the full blood count and MPO level using a colorimetric method. RESULTS: The overall prevalence of falciparum malaria was 34.7%, with no difference between the three communities, but Bambili of high altitude had the highest prevalence (70.7%). A majority of the infected persons had mild malaria, with most cases being asymptomatic (temperature < 37.5 ºC). Patients had significant increase of geometric mean malaria parasite density (GMPD) in Bambili (1755 ± 216 parasites/µL) and Bamenda (1060 ± 2515 parasites/µL of blood) than patients in Santa (737 ± 799 parasites/µL). There was a significant risk to have malaria infection in Bambili (OR = 33.367, p = 0.021) than in Santa (OR = 2.309, p = 0.362). Bambili' participants of 6-10 years showed a high prevalence of malaria (85.7%). GMPD was significantly different between males (p = 0.010) as well as females (p = 0.000). Participants from Santa (11.2 ± 3.2 g/dL) and Bambili (12.6 ± 2.4 g/dL) had a high haemoglobin concentration than those from Bamenda (10.6 ± 2.1 g/dL). There was a significant difference in the WBC counts and platelet counts among infected participants in the study areas. MPO level had an increasing trend among infected participants in Santa (2.378 ± 0.250), Bambili (2.582 ± 0.482) and Bamenda (2.635 ± 0.466). CONCLUSION: The results of the present study demonstrated that altitudinal variations significant impact the risk of population to have malaria with high parasitaemia and may contribute to the malaria prevalence and severity by affecting the haemoglobin concentration, WBC and platelet level and plasma MPO in population.
