RESUMO

Flank pain is an exceptionally common presenting symptom in the emergency and primary care setting; however, most clinicians may not include a differential diagnosis of renal infarct (RI) due to the reported low incidence of this condition. Delayed diagnosis or treatment intervention for RI can have dire consequences for the patient including hypertension and longstanding renal impairment. In this report, we review a case of a previously healthy 39-year-old male presenting with flank pain, which after extensive workup, was revealed to be caused by renal infarction from a renal artery dissection secondary to segmental arterial mediolysis (SAM).

Daptomycin is an antibiotic used for resistant Gram-positive organisms and has the rare side effect of inducing acute eosinophilic pneumonia (AEP). This condition can be fatal due to respiratory failure if not treated, as eosinophils migrate to the lungs and inflammatory cascades cause epithelial injury. Daptomycin-induced AEP can be misdiagnosed as bacterial pneumonia or malignancy, which may lead to unnecessary testing or treatments. Diagnostic criteria include dyspnea, fever, recent daptomycin exposure, infiltrates on imaging, eosinophils on bronchoalveolar lavage or peripheral eosinophilia, and clinical improvement with medication discontinuation. We present a unique case of daptomycin-induced eosinophilic pneumonia in a 72-year-old male with the chief complaint of dyspnea and initial concerns for lung cancer after a spiculated nodule was seen on imaging. Prior to undergoing a lung biopsy, repeat imaging showed a decrease in the suspicious nodule, reducing the likelihood of malignancy and prompting a re-evaluation of the history of the present illness and medication list. Daptomycin was stopped, and the patient's symptoms and imaging improved. This case illustrates the importance of early recognition and appropriate treatment of AEP, which allows for complete clinical recovery.

Pediatric obesity is a global concern with distressing comorbid conditions, including mood disturbance, cardiovascular changes, endocrine imbalance, liver disease, sleep apnea, and orthopedic conditions. The primary treatment of this condition includes physical activity. Participating in organized sports has been shown to reduce weight and the complications of pediatric obesity more effectively than individual exercise.

Decades of research in social identity have shown that people instinctively hold positive attitudes towards ingroup members and negative attitudes towards outgroup members. However, it remains unclear how people respond to individuals explicitly identified with both one's ingroup and outgroup. We propose that when people are exposed to dual-identified individuals and groups (e.g., Muslim-Americans explicitly identifying with both their Muslim and American identities), intergroup attitudes will improve, driven more by the ingroup component (American), despite the presence of the outgroup component (Muslim). Moreover, we suggest exposure to dual-identification can also improve attitudes toward the broader outgroup (Muslims more generally), a phenomenon called the gateway-group effect. To test these hypotheses, we created a new measure of dual-identification and conducted three studies involving both Muslim-Americans and Mexican-Americans. Results confirmed that exposure to explicitly dual-identified groups improved attitudes towards the dual-identified group (e.g., Mexican-Americans) as well as toward the respective outgroup (e.g., Mexicans).

Introduction The coronavirus disease 2019 (COVID-19) pandemic has resulted in rapid healthcare system adaptations, including the acceptance of telemedicine in primary care. In the case of knee ailments, among the most common problems encountered in primary care, telemedicine provides a literal window to observe the patient performing functional activities. Despite its potential, there is a lack of standardized protocols for data collection. The purpose of this article is to provide a step-by-step protocol to aid in performing a telemedicine examination of the knee. Methods This article provides a step-by-step guide for a telehealth examination of the knee. Results A step-by-step examination of how to structure a telemedicine evaluation of the knee. A glossary of images of each maneuver has been included to demonstrate the components of the examination. Additionally, a table of questions and possible answers were included to help guide the provider through a knee examination. Conclusion This article provides a structured and efficient means of extracting clinically relevant information during telemedicine examinations of the knee.

Question-and-answer (Q&A) sessions following research talks provide key opportunities for the audience to engage in scientific discourse. Gender inequities persist in academia, where women are underrepresented as faculty and their contributions are less valued than men's. In the present research, we tested how this gender difference translates to face-to-face Q&A-session participation and its psychological correlates. Across two studies examining participation in three conferences, men disproportionately participated in Q&A sessions in a live, recorded conference (N = 189 Q&A interactions), and women were less comfortable participating in Q&A sessions and more likely to fear backlash for their participation (N = 234 conference attendees). Additionally, women were more likely to hold back questions because of anxiety, whereas men were more likely to hold back questions to make space for others to participate. To the extent that men engage more than women in Q&A sessions, men may continue to have more influence over the direction of science.

Objective To determine whether there is a correlation between pain and the amount of fluid present at the distal insertion of the iliotibial band (ITB) in runners, as measured by USG. Method Our retrospective cross-sectional study evaluated 100 male and female runners prior to the start of a race. A valid and reliable questionnaire collected demographic, pain, and training data. If a runner reported knee pain, a numeric pain rating scale was used to record the degree of pain. Participants then underwent USG on both knees to determine the presence or absence of fluid at the distal insertion of the ITB. Result We found no statistically significant correlations of fluid measurements with pain score, running experience in years, or age. In addition, we found no other differences in fluid measurements between those with and without knee pain or between the sexes. Conclusions Our findings indicate that the presence or absence of fluid at the distal insertion of the ITB does not correlate with knee pain in runners, regardless of age, running experience, or sex.

RESUMO

We all keep secrets, and often to the detriment of our well-being. But what happens when we reveal a secret? This review integrates research on revealing secrets, from the perspective of both the revealer and the confidant. First, revealing secrets must be differentiated from other forms of social disclosure. Second, the decision of whether to share a secret is complex, and the benefits depend on the extent to which sharing elicits social support and insight. On the other side of the revelation, recent research demonstrates antecedents (e.g., certain personality traits) and outcomes (e.g., increased relational closeness) of being a confidant. Occasionally, people reveal others' secrets, the likelihood of which depends on factors such as the perceived immorality of the secret. While many open questions remain when it comes to revealing secrets, we highlight a growing understanding of the processes of secret sharing, both for those who confide and those who are confided in.

Here we describe an in vitro primary culture system for Caenorhabditis elegans germline stem cells. This culture system was used to identify a bacterial folate as a positive regulator of germ cell proliferation. Folates are a family of B-complex vitamins that function in one-carbon metabolism to allow the de novo synthesis of amino acids and nucleosides. We show that germ cell proliferation is stimulated by the folate 10-formyl-tetrahydrofolate-Glun both in vitro and in animals. Other folates that can act as vitamins to rescue folate deficiency lack this germ cell stimulatory activity. The bacterial folate precursor dihydropteroate also promotes germ cell proliferation in vitro and in vivo, despite its inability to promote one-carbon metabolism. The folate receptor homolog FOLR-1 is required for the stimulation of germ cells by 10-formyl-tetrahydrofolate-Glun and dihydropteroate. This work defines a folate and folate-related compound as exogenous signals to modulate germ cell proliferation.
