RESUMO

Containing a pandemic requires that individuals adhere to measures such as wearing face-masks and getting vaccinated. Therefore, identifying predictors and motives for both behaviors is of importance. Here, we study the decisions made by a cross-national sample in randomized hypothetical scenarios during the COVID-19 pandemic. Our results show that mask-wearing was predicted by empathic tendencies, germ aversion, and higher age, whilst belief in misinformation and presentation of an interaction partner as a family member lowered the safety standards. The main motives associated with taking the mask off included: rationalization, facilitating interaction, and comfort. Vaccination intention was positively predicted by empathy, and negatively predicted by belief in misinformation and higher costs of the vaccine. We found no effect of immunization status of the surrounding social group. The most common motive for vaccination was protection of oneself and others, whereas undecided and anti-vaccine groups reported doubts about the effectiveness and fear of side effects. Together, we identify social and psychological predictors and motives of mask-wearing behavior and vaccination intention. The results highlight the importance of social context for mask-wearing, easy access to vaccines, empathy, and trust in publicly distributed information.

INTRODUCTION: Social networking sites such as Instagram have provided young people with unprecedented opportunities for social comparison, and such behaviour can have implications for identity development. Although initial evidence suggests that there may be developmental differences in terms of how such behaviour informs identity development during adolescence and emerging adulthood, all previous research has been conducted in highly individualistic cultural contexts (i.e., the UK and the US). METHOD: To shed further light on these possible developmental differences and to determine whether results replicate amongst young people from more collectivist cultural contexts, cross-sectional survey data were collected from 1,085 (M age = 18.87, SD = 2.57; Female = 77.8%) adolescents and emerging adults in Romania and Serbia between December 2019 and March 2020. The relationships between social comparisons of ability and opinion on Instagram and three key identity processes (i.e., commitment, in-depth exploration, and reconsideration of commitment) were then examined. RESULT: Hierarchical multiple regression analyses identified significant age differences in terms of how social comparisons of ability and opinion on Instagram associated with identity commitment and in-depth exploration. Furthermore, possible cultural differences were identified in terms of how social comparisons of opinion on Instagram associated with the identity processes. CONCLUSION: Overall, results suggest that whilst social comparisons on Instagram can elicit self-focus and prompt further exploration, developmental and cultural factors may influence how such behaviour informs identity development during adolescence and emerging adulthood.

Perforation of the pharynx is a rare occurrence but has the potential to cause mediastinitis and has an attendant mortality risk. Though numerous mechanisms have been described, we report a unique case of a young woman who presented with a sore throat, odynophagia and subcutaneous emphysema, a short time after performing fellatio. A contrast swallow confirmed hypopharyngeal perforation. She was managed expectantly with nasogastric feeding and empirical antibiotics. The perforation took 4 weeks to heal, but there were no residual swallowing problems at 3-month follow-up. We will explore the incidence and causes of pharyngeal perforation and discuss the options for and risks of surgical repair. This case highlights that non-surgical management of such injuries can be both safe and feasible, and reinforces the importance of ensuring confidentiality and the need for vigilance regarding potential non-consensual injury.

Research on the negative psycho-emotional implications of social comparisons on social network sites such as Instagram has rapidly accumulated in recent years. However, little research has considered the extent to which such comparisons can elicit positive motivational outcomes for adolescent users, specifically inspiration. Furthermore, little is known about whether it matters whom young people compare themselves to on Instagram (i.e., network composition) and how this may modulate the emotional outcomes of Instagram social comparisons. The present study thus sought to determine how adolescents' Instagram comparisons of ability associate with inspiration through the mechanism of benign and malicious envy. We further examined whether two key aspects of network composition-perceived similarity and the amount of strangers followed-moderated these relationships. Results from a paper survey among n = 266 British adolescents confirm the hypothesis that those adolescents who compare more strongly on Instagram also report more inspiration from Instagram use. While benign envy positively mediated this relationship, malicious envy worked in the opposite direction, indicating the need to distinguish these two types of envy in future research. In addition, while the amount of strangers followed did not significantly affect the relationships between social comparison, envy, and inspiration, higher perceived network homophily positively moderated the relationship between social comparison and inspiration by eliciting more benign and less malicious envy. Results overall suggest that social comparisons on Instagram may be more inspiring when adolescents compare themselves to similar others and avoid unachievable false role models in their online networks.

Chronic rhinosinusitis has a significant impact on health-related and generic quality-of-life, has a high cost burden to both society and patients, and may be associated with absenteeism, loss of productivity and poor respiratory function. Though there is a paucity of level 1 evidence, endoscopic sinus surgery may be considered in medically refractory patients and a variety of objective and subjective outcome measures exist to assess the effectiveness of intervention. We outline the outcome measurements available and review in-depth the published outcomes to date. Furthermore we discuss the literature that indicates that endoscopic sinus surgery can have a positive effect on respiratory function in asthma. How patient selection, timing and extent of surgery, and post-operative care interventions may optimise surgical outcomes is explored.

The T-score discordance among skeletal sites is an important aspect of dual-energy X-ray absorptiometry (DXA) measurements. In the spine, large T-score differences between vertebrae are frequently seen in elderly patients owing to degenerative disease. However, it is unclear how often such differences occur in younger adults with healthy spines. The T-scores for individual lumbar vertebrae were compared for 2391 female singletons (18-79 yr) recruited to the Twins UK Adult Twin Register. Women were divided into 6 age bands and 5 bands by body weight, respectively, and the T-score differences between the pairs of vertebrae were examined using correlation coefficients and the standard error of the estimate (SEE) from linear regression analysis. Correlations between the T-scores for adjacent lumbar vertebrae were r = 0.92 decreasing to r = 0.79 between L1 and L4. When plotted as a function of age, r-values were constant for the 5 younger age bands, but decreased in the oldest group. In contrast, the T-score SEE values increased progressively with age from 0.4 to 0.5 for the younger groups to 0.7 for the oldest. Similar trends were seen when women were divided according to body weight. Both increasing age and higher body weight were statistically significantly associated with a higher T-score SEE. The incidence of large T-score differences between vertebrae varies with age and body weight, but is common even among younger women. Clinically significant T-score differences can occur in the absence of osteoarthritis, and visual assessment of spine DXA scans for evidence of degenerative disease is advised before vertebrae are omitted from scan analyses.

RESUMO

Balloon sinuplasty has been licensed for the treatment of chronic rhinosinusitis since 2006. Balloon dilation enlarges the sinus ostium by effecting tiny fractures of the surrounding bone and compressing soft tissue such that no mucosa is removed. We believe this to be the ideal technique for achieving drainage of an acutely infected frontal sinus when surgical intervention is required. We report the application of this new technique in a 32-year-old female with acute frontal sinusitis. The frontal sinus was successfully cannulated and drained. Post-operative irrigation was achieved using a modified Vortex catheter.
