RESUMO

AIMS: To describe cocaine treatment demand in 10 western European countries and to examine the size, direction and temporality of recent trends in the proportion of cocaine users among all clients entering treatment. DESIGN: Aggregated data collected through the European Union standardized treatment demand monitoring system (TDI) between 2011 and 2018 were used. SETTING: Belgium, England, France, Germany, Ireland, Italy, Luxembourg, Spain, Switzerland and the Netherlands. PARTICIPANTS: In total, more than 700 000 cocaine treatment records were analysed. Clients in treatment for cocaine as primary drug were predominantly male (85%), with an average age of 35 years. MEASUREMENTS: Number of treatment episodes for substance use and for cocaine as primary or secondary drug were collected year- and country-wise. When available, powder cocaine and crack and patients with and without previous treatment were differentiated. FINDINGS: Among the participating countries the share of cocaine as primary drug in treatment demand ranged between 4.7% [95% confidence interval (CI) = 4.6-4.9%] in Germany and 43.1% in Spain (95% CI = 42.6-43.5%). The general trend analysis showed a decreasing proportion of cocaine-related treatment entrants between 2011 and 2014 among all subgroups followed by a strong increase in 2015. The increase appeared stronger than for powder cocaine. Seven of 10 countries observed a recent significant increase in the proportion of treatment entrants reporting cocaine as the primary substance: Belgium [annual percentage change (APC) = 9.6%, P < 0.01], England (APC = 14.9%, P < 0.05), France (APC = 21.8%, P < 0.01), Ireland (APC = 28.2%, P < 0.01), Italy (APC = 7.8%, P < 0.01), Spain (APC = 7.0%, P < 0.05) and Switzerland (APC = 12.0%, P < 0.05). Trends were similar when looking at cocaine reported as primary or adjunctive substance. CONCLUSIONS: Despite substantial country-specific variation regarding cocaine prevalence and treatment demand, there has been an overall significant increase since 2015 in the share of cocaine-related treatment demand in western Europe.

Alcohol consumption in France in 2017. Alcohol consumption was still a very common habit in France: 86.5% of 18-75 years old had drank alcohol during the last 12 months and 40.0% did at least once a week. One quarter of adults stated they consumed above the low-risk alcohol drinking guidelines (no more than 10 drinks per week and 2 drinks per day and alcohol-free days every week). Regardless of the indicator, women consumed less often than men. Besides, alcohol consumption levels and patterns and reasons for use were closely related to the age: the youths drank less often but more intensively than the elder. First alcohol uses are seen in middle school and some regular uses are already established in high school. Overall, pure alcohol intake per capita has been divided by more than two since the sixties but the decline observed every year tends to slow down.

En 2017, la consommation d'alcool restait une pratique très courante en France : 86,5 % des 18 à 75 ans ont bu de l'alcool au cours des 12 derniers mois et 40,0 % au moins une fois par semaine. Un quart des adultes déclaraient dépasser les repères de consommation à moindre risque (maximum 10 verres par semaine, maximum 2 verres par jour et des jours sans consommation dans la semaine). Quel que soit l'indicateur de consommation, les femmes sont moins fréquemment consommatrices que les hommes. De plus, les niveaux, modes et motifs de consommation sont très liés à l'âge : les plus jeunes consomment moins souvent mais plus intensément que les plus âgés. Les premières expérimentations se font dès le collège, et certains usages réguliers sont déjà en place au lycée. Au global, la quantité d'alcool pur consommé par habitant a été divisé par plus de 2 depuis le début des années 1960, mais la diminution observée presque chaque année tend à se ralentir.

RESUMO

Cannabis is the most consumed illicit drug in France. Its use concerns especially men, in all social groups. It is mainly consumed by young adults and teenagers. Over the last ten years the consumption of cannabis strongly increased amongst French young people, the rise has been even stronger for regular use. Prevalence use in France has now reached the highest European levels. Cannabis use can lead to a treatment demand and a research will soon allow to screen problematic users in general population in France. The part of the cannabis in the activity of specialised centres for drug addicts progresses. Due to a lack of indicators, the sanitary impact is not well-informed; nevertheless there are presumptions that cannabis is involved in road accidents, cancers and psychiatric pathologies. The cannabis constitutes the main part of the arrests for drug use.

RESUMO

Today, we can draw a picture of addiction behaviours linked to psychoactive products by distinguishing "use" and "problematic use". These are operational terms that imperfectly cover the established concepts of "use", "abuse" and "dependency". Alcohol and tobacco occupy the front place in the field of addictions in France, not only by the levels of consumption but also by the related health harms induced. We can also note a consumption of medicines without prescription among young girls (17.4% of those aged 14 to 18) and a "potentially problematic use" of cannabis among young men (14% of those aged 18). The consumption of other drugs is very marginal and most of health harms induced is due to problematic consumption of opiate drugs and cocaine.
