RESUMO

Oral cancer is detected in 270,000 people per annum or around 3% of all malignancies. It ranks sixth among males and twelve among females in terms of prevalence. Mostly, all oral squamous cell carcinomas (OSCC) are responsible for 92-95% of cases. Oral carcinoma starts five years before the average age of PMD patients, who range in age from 5th to 6th decade of life. Thus, the goal of this study is to identify any relevant risk factors and assess the incidence of oral possibly malignant disorders in both men and women living in rural Bhojpur District, Bihar. A cross-sectional study has been carried out at the Sadar Hospital in Bhojpur by surveying volunteers at various oral health screening and treatment camps held from January 2022 to July 2022. A pre-validated questionnaire adapted and modified from Kumar S et al. was utilized to collect data in a qualitative interview. It was discovered that (17%) of all people had an oral possibly malignant disease. OSMF was the lesion with the greatest prevalence (7%) and lichen planus having the smallest prevalence (1%). Malignancy frequently develops from oral potential malignant diseases. Diabetes and BMI were inversely related, which is expected provided the socioeconomic position.

Objectives: To investigate the outcome of the loading direction and implant tilting on the micromotion and displacement of immediately placed implants with finite element analysis (FEA). Materials and Method: Eight blocks of synthetic bone were created. Eight screw-type implants were inserted, four axially and four slanted, each measuring 11 mm in length and 4.5 mm in diameter. The axial implants and the tilted implants were distally inclined by 30°. The top of the abutment was subjected to 180 N vertical and mesiodistal oblique (45° angle) loads, and the displacement of the abutment was measured. The abutment displacement and micromotion were estimated, and nonlinear finite element models simulating the in vitro experiment were built. In vitro studies and FEA data on abutment displacement were compared, and the reliability of the finite element model was assessed. Result: Under oblique stress, abutment displacement was larger than under axial loading, and it was also greater for tilted implants than for axial implants. The consistency of the in vitro and FEA data was satisfactory. Under vertical stress, the highest micromotion values in the axial and tilted implants were extremely near. Conclusion: Under mesiodistal oblique stress, tilted implants may have a smaller maximum amount of micromotion than axial implants. The loading direction had a significant impact on the highest micromotion values. The abutment displacement values were not reflected in the maximum micromotion measurements.

CONTEXT: The adverse effects associated with traditional adjunctive chemical agents in the management of gingivitis has stimulated research into search for alternate molecules including herbal products. Aims-To compare the effects of oral curcumin gel as an adjunct to scaling and root planing (SRP) with the effects achieved by SRP alone by assessing their effects on plaque, gingival inflammation and gingival bleeding. MATERIALS AND METHODS: Sixty patients in the age group of 20-60 years were recruited employing inclusion and exclusion criteria. Study population was randomly divided into two groups. Group A-test group was instructed to use twice daily the experimental curcumin gel after initial SRP. Group B-control group received only SRP. Parameters recorded were plaque index (PI), gingival index (GI) and sulcus bleeding index (SBI) on day 0 (baseline), days 7, 14, and 21 along with subjective assessment of any reported untoward side effect. Data was analyzed statistically. RESULTS: The PI, GI and SBI decreased in both the test group and control group from day 0 to day 21 consecutively. Intergroup comparison shows that all the parameters were statistically significant in Group A (P < 0.001) as compared to Group B. CONCLUSION: Curcumin gel has significant antiplaque and anti-inflammatory effect in the treatment of gingivitis and can be effectively employed as an adjunct to SRP.

BACKGROUND: Nutrition has a significant impact on the pathophysiology of periodontal disease. Both micro- and macronutrients have an impact on periodontal health. This study aimed at the evaluation of the effects of a diet low in carbohydrate and rich in omega-3 fatty acids, ascorbic acid, antioxidants, and fiber on clinical outcomes in patients with gingival inflammation for four weeks. MATERIALS AND METHODS: Overall, 54 systemically healthy subjects were enrolled in this study. The clinical trial consisted of two groups: Group A (test group) (n = 27) was instructed to consume a diet comprising low carbohydrates, rich in omega-3 fatty acids, ascorbic acid, antioxidants, and fibers for the next four weeks; in Group B (control group) (n = 27), no alteration in dietary behavior was done, and these subjects were instructed to have their daily routine diet. Clinical parameters measured were plaque index (PI), gingival bleeding index (GI), probing depths (PD), clinical attachment level (CAL), and bleeding on probing (BOP) at one week without any dietary changes (baseline) for both the groups, followed by a one-week adaptation period; then, the parameters were checked on a weekly basis for the next four weeks. RESULTS: Primary clinical outcome BOP and secondary outcome GI showed significant changes in the test group compared with the control group (P < 0.05). However, no significant changes were seen in the plaque scores in the test group (P > 0.05). The degree of diet compliance on the clinical parameters (PI, GI, and BOP) was assessed by using regression analysis. CONCLUSION: Dietary recommendations can be beneficial in managing gingival and periodontal inflammation. Nutritional interventional studies as monotherapy are required to evaluate the clinical significance of diet in periodontal therapy.

OBJECTIVE: Maintenance of meticulous oral health practices is critical for patients who are under orthodontic treatment as failure to do so can result in deterioration of periodontal health. Thus, the present study was commenced to assess dental negligence and oral health status among patients undergoing orthodontic treatment using dental neglect scale (DNS) questionnaire. MATERIALS AND METHODS: The present cross-sectional study was planned and carried out among the 40 patients undergoing fixed orthodontic treatment. The study comprised of two questionnaires, one was close-ended questionnaire which consisted of questions regarding patient practice in maintenance of oral health and other questionnaire comprised of DNS followed by examination of oral hygiene status using Oral Hygiene Index Simplified. Data so obtained were subjected to analysis using SPSS version 20 and Chi-square test was used to statistically analyze data with P < 0.05 regarded as a statistically significant value. RESULTS: The present study revealed that 63% among the studied orthodontic patients brushed once daily, 26% brushed twice daily, and 11% brushed thrice. About one-fourth was using brush with soft bristles and only 9% among the respondents used interdental aids. Data revealed positive correlation between DNS and oral hygiene index-simplified score with P < 0.05. CONCLUSION: The present study found that less frequency of brushing, rinsing mouth, and eating sticky and hard food can be attributed to self-neglect of the orthodontic patients.

BACKGROUND: Use of smokeless tobacco in the form of moist snuff placed in the oral cavity is popular in rural India. The aim of the present cross-sectional study was to determine the effect of snuff on periodontitis by assessing interleukin (IL)-1 ß and IL-8 levels in gingival crevicular fluid. MATERIALS AND METHODS: A total of 60 subjects were selected for this study. 40 subjects presented with periodontitis, which included 20 snuff users (SP) and 20 nonsnuff users (NS). 20 periodontally healthy patients formed the controls (healthy control: HC). The clinical parameters recorded were gingival index (GI), plaque index, calculus index, bleeding on probing (BOP), probing depth (PD), recession (RC), and clinical attachment level (CAL). The IL-1 ß and IL-8 levels were assessed through enzyme-linked immunosorbent assay (Quantikine(®)). Analysis of variance (ANOVA), post-hoc Tukey's, Kruskal-Walli's ANOVA and Mann-Whitney test was used for comparison among groups and P > 0.05 was considered statistically significant. RESULTS: No significant difference was seen in levels of IL-1 ß and IL-8 between SP and NS groups (P = 0.16, 0.97). However, both the periodontitis groups (SP and NS) had increased IL-ß levels when compared to HC group (P = 0.01, 0.001). The snuff users showed significant increase in GI, BOP, RC, and CAL when compared with NS (P = 0.002, 0.001, 0.012, 0.002) whereas NS group had significant increase in PD (P = 0.003). CONCLUSION: Within the limitations of this study, use of snuff does not affect the host inflammatory response associated with periodontitis and leads to RC and increased CAL due to local irritant effect.

RESUMO

The use of probiotics is based on the concept that adding the right live microbes to the complex human system can result in physiological benefits. The benefit of fermented milk in human diet has been acknowledged since Vedic times; however, the scientific interest in this field was evoked by Ellie Metchinkoff who recommended that people should consume fermented milk containing lactobacilli to prolong their lives, as accelerated aging is due to autointoxication caused by the toxins produced by the gut microflora. They have been used to improve gastrointestinal health and their attractiveness has evinced interest to study their role in the promotion of oral health also. Studies have been widely carried out to establish the role of intestinal lactobacilli as probiotic to treat various gastrointestinal disorders, but only limited studies are available on the oral use of probiotics. The probiotic products usually contain lactobacilli and bifidobacteria, and their demand in the market is growing day by day. This paper provides an overview of various studies in the literature that emphasize on the role of probiotics to combat oral diseases and encourages more research in this field.

UNLABELLED: Any suture material, absorbable or nonabsorbable, elicits a kind of inflammatory reaction within the tissue. Nonabsorbable black silk suture and absorbable polyglycolic acid suture were compared clinically and histologically on various parameters. MATERIALS AND METHODS: This study consisted of 50 patients requiring minor surgical procedure, who were referred to the Department of Oral and Maxillofacial Surgery. Patients were selected randomly and sutures were placed in the oral cavity 7 days preoperatively. Polyglycolic acid was placed on one side and black silk suture material on the other. Seven days later, prior to surgical procedure the sutures will be assessed. After the surgical procedure the sutures will be placed postoperatively in the same way for 7 days, after which the sutures will be assessed clinically and histologically. RESULTS: The results of this study showed that all the sutures were retained in case of polyglycolic acid suture whereas four cases were not retained in case of black silk suture. As far as polyglycolic acid suture is concerned 25 cases were mild, 18 cases moderate and seven cases were severe. Black silk showed 20 mild cases, 21 moderate cases and six severe cases. The histological results showed that 33 cases showed mild, 14 cases moderate and three cases severe in case of polyglycolic acid suture. Whereas in case of black silk suture 41 cases were mild. Seven cases were moderate and two cases were severe. Black silk showed milder response than polyglycolic acid suture histologically. CONCLUSION: The polyglycolic acid suture was more superior because in all 50 patients the suture was retained. It had less tissue reaction, better handling characteristics and knotting capacity.
