RESUMO

The Thai Diagnostic Autism Scale (TDAS) was developed for autism spectrum disorder (ASD) diagnosis in Thai children aged 1-5 years. Previous studies have indicated its good performance; however, additional health resources and healthcare providers are necessary for evaluation. Therefore, this study aimed to assess the cost-effectiveness of TDAS compared to clinical diagnosis (ClinDx) for ASD diagnosis in Thai children aged 1-5 years from a societal perspective. The analysis employed a hybrid model consisting of a decision tree model for a diagnostic phase with a state transition model for a follow-up phase. A literature review was conducted to determine TDAS performance and the relative risk of death in patients with ASD. Direct medical costs were assessed through a retrospective medical records review, and a cross-sectional survey was conducted to determine direct nonmedical costs, ASD severities, and utility values. The cost of TDAS was derived from a healthcare provider interview (n = 10). The incremental cost-effectiveness ratio (ICER) compared the total lifetime cost and quality-adjusted life years (QALY) between TDAS and ClinDx. We found that TDAS could improve QALY by 1.96 but increased total lifetime cost by 5577 USD, resulting in an ICER of 2852 USD/QALY. Sensitivity analysis indicated an 81.16% chance that TDAS is cost-effective. The probabilities of different ASD severities were key influencing factors of the findings. In conclusion, TDAS is the cost-effective option for ASD diagnosis in Thai children aged 1-5 years compared to ClinDx, despite some uncertainties around inputs. Further monitoring and evaluation are warranted if TDAS is to be implemented nationwide.

BACKGROUND: Presently, the internet plays a big role in daily life, especially for adolescents. In this age group, they are more concerned about their face and body shape. Despite the numerous studies on the effect traditional media has on body image, very few have focused on the effect of newer forms of media (e.g. online media). And almost none have looked at the relationship between time spent online and body image. AIM: To study the associations between time spent on the internet, body image satisfaction and eating behaviors of students grades 7 to 12 in the Thai educational system. METHODS: The sample group included 620 students, who were selected using simple random sampling from 6 secondary schools in Bangkok. Data were collected using the Media and Internet use behavior questionnaires, The Body-Esteem Scale for Adolescents and Adults: Thai version (BESAA), Drive for Muscularity Scale (DMS: males only), The Rosenberg Self-Esteem Scale: Thai version, Eating Attitude Test-26: Thai version (EAT-26) and the eating behaviors at risk of obesity questionnaire. RESULTS: Mean (sd) age of the sample was 15.7 (1.9) years, 246 participants (39.7%) were male and 374 (60.3%) were female. Using the internet and social networks for content related to body image and eating behaviors, was negatively associated with body image satisfaction but positively associated with inappropriate eating attitudes/behaviors, binging, purging, use of laxatives/diuretics and drive for muscularity with respect to behaviors and attitudes, and was associated with eating behaviors that carried a risk for obesity. CONCLUSIONS: Time spent on internet, especially engaged in activities related to self-image, and eating attitudes and behaviors, were associated with a decrease in body image satisfaction and problematic eating behaviors.

This case report highlights the importance of pharmacogenetic testing in the treatment of attention deficit hyperactive disorder (ADHD). A 6-year-old boy diagnosed with ADHD was prescribed methylphenidate 5 mg twice daily (7 am and noon) and the family was compliant with administration of this medication. On the first day of treatment, the patient had an adverse reaction, becoming disobedient, more mischievous, erratic, resistant to discipline, would not go to sleep until midnight, and had a poor appetite. The All-In-One PGX (All-In-One Pharmacogenetics for Antipsychotics test for CYP2D6, CYP2C19, and CYP2C9) was performed using microarray-based and real-time polymerase chain reaction techniques. The genotype of our patient was identified to be CYP2D6*2/*10, with isoforms of the enzyme consistent with a predicted cytochrome P450 2D6 intermediate metabolizer phenotype. Consequently, the physician adjusted the methylphenidate dose to 2.5 mg once daily in the morning. At this dosage, the patient had a good response without any further adverse reactions. Pharmacogenetic testing should be included in the management plan for ADHD. In this case, cooperation between the medical team and the patients' relatives was key to successful treatment.
