RESUMO

Information on paracoccidioidomycosis (PCM) in Argentina is fragmented and has historically been based on estimates, supported only by a series of a few reported cases. Considering the lack of global information, a national multicentric study in order to carry out a more comprehensive analysis was warranted. We present a data analysis including demographic and clinical aspects of a historical series of 466 cases recorded over 10 years (2012-2021). Patients were aged from 1 to 89 years. The general male: female (M:F) ratio was 9.5:1 with significant variation according to the age group. Interestingly, the age range 21-30 shows an M:F ratio of 2:1. Most of the cases (86%) were registered in northeast Argentina (NEA), showing hyperendemic areas in Chaco province with more than 2 cases per 10,000 inhabitants. The chronic clinical form occurred in 85.6% of cases and the acute/subacute form occurred in 14.4% of cases, but most of these juvenile type cases occurred in northwestern Argentina (NWA). In NEA, the incidence of the chronic form was 90.6%; in NWA, the acute/subacute form exceeded 37%. Diagnosis by microscopy showed 96% positivity but antibody detection displays 17% of false negatives. Tuberculosis was the most frequent comorbidity, but a diverse spectrum of bacterial, fungal, viral, parasitic, and other non-infectious comorbidities was recorded. This national multicenter registry was launched in order to better understand the current status of PCM in Argentina and shows the two endemic zones with a highly diverse epidemiology.

Resumen Evaluamos las concordancias interlaboratorio, esencial y categórica entre el panel Sensititre YeastOne™ y los métodos de referencia correspondientes al M27 4.a ed. (Clinical and Laboratory Standards Institute [CLSI]) y al EDef 7.3.1 (European Committee on Antifungal Susceptibility Testing [EUCAST]). Estudiamos 67 cepas de Candida de distintas muestras clí nicas y un panel de 9 cepas resistentes a fluconazol y equinocandinas. El mayor porcentaje de concordancia interlaboratorio se observó con anfotericina B (96,8%) y el menor porcen taje con voriconazol (77,2%). La caspofungina mostró un 5,8% de discrepancias muy mayores con el método de referencia del CLSI. Con el del EUCAST, el itraconazol, el posaconazol y la anidulafungina mostraron porcentajes de discrepancias mayores: el 17,6, el 18,1 y el 19,6%, respectivamente. El panel Sensititre YeastOne™ es una alternativa confiable y fácil de usar, que permite detectar especies de Candida resistentes a los antifúngicos, con algunas limitaciones para las equinocandinas. Los resultados son equiparables a los de los métodos de referencia.

Según la Organización Mundial dela Salud (OMS), entre el 60 y 90% de la población infantil presenta lesiones cariosas concavitación. Las patologías pulpares son consecuencia de la evolución de la caries o traumatismo dental, manifestándose con dolor, inflamación o infección, que obliga a los pacientesa acudir de forma urgente a la consulta odontológica con cuadros de pulpitis reversible, irreversible o necrosis pulpar. Dependiendo dela gravedad de la patología, esta puede intervenirse mediante terapias curativas y cuando ha alcanzado un nivel muy avanzado, laúnica opción es la exodoncia, dejando secuelas a corto, mediano y largo plazo en el niño. Objetivo: Analizar las diferentes patologías pulpares en molares de ciduos de pacientes infantiles entre 5 y 9 años que acuden a la clínica de Odontopediatría de la Facultad de Odontología de la Universidad Nacional Autónoma de Honduras (UNAH) durante 2016-2018.Pacientes y métodos: Estudio descriptivo, retrospectivo y cuantitativo.Se recolectaron historias clínicas de niños entre 5 y 9 años que acudieron entre 2014 -2016 con una muestra de 310 expedientes de un universo de1605. Resultados: Predominaron las patologías pulpares en el género masculino (54.2%). La caries dental fue la etiología más registrada (77.34%),predominó la pulpitis reversible (9.3%), el órgano dentario más afectado, en el sistema de nomenclatura FDI, (Federation Dentaire Internationale), fue el primer molar deciduo inferior izquierdo (7,4). El tratamiento más realizado fue pulpotomía (15.2%). Conclusión: en la población infantil la caries dental no tratada evolucionó en su mayoría apulpitis reversible...(AU)

This paper aims to describe the construction and operation of a respiration chamber of the head-box type for methane (CH4) measurements in bovines. The system consists of (1) a head box with a stainless steel frame and acrylic walls, floor, and ceiling; (2) a stainless steel feeder; (3) an automatic drinking water bowl; (4) a hood made from reinforced canvas; (5) an infrared (IR) CH4 gas analyzer, a mass flow generator, a data-acquisition system; and (6) a steel metabolic box. Six assays were conducted to determine the pure CH4 recovery rate of the whole system in order to validate it and comply with standards of chamber operation. The gravimetrical method was used for the recovery test and the recovery rate obtained was 1.04 ± 0.05. Once the system was calibrated, measurements of CH4 were conducted using eight animals consisting of four Holstein cows with a live weight of 593.8 ± 51 kg and an average milk yield of 23.3 ± 1.8 kg d-1 and four heifers with a live weight of 339 ± 28 kg. The CH4 production values were 687 ± 123 and 248 ± 40 L CH4 d-1 for cows and heifers, respectively. The CH4 yield was 19.7 ± 3.4 g and 17.1 ± 3.4 g CH4 kg-1 of dry matter consumed for cows and heifers, respectively. These results are consistent with those reported in the literature.

PURPOSE: To report on a case of Fusarium solani subretinal abscess in a patient with acute myeloid leukemia treated with an allogenic bone marrow transplant. METHODS: A 47-year-old male with a history of acute myeloid leukemia with intermediate cytogenetic risk was admitted in our hospital. The disease relapsed after two cycles of chemotherapy. He was then treated with an allogenic bone marrow transplant, with busulfan, cyclophosphamide, and thymoglobulin. One week after the procedure, a sepsis of unknown origin in neutropenia occurred. Blood cultures and sputum were negative for bacteria and fungi. At the eighth week after the procedure, the patient had acute vision loss of the right eye. Funduscopy in the right eye revealed an inferior temporal yellowish white elevated lesion of approximately 10 disk areas and superficial perifoveal and perilesional hemorrhages. RESULTS: Vitrectomy was performed and samples from the vitreous and the subretinal abscess material were sent for analysis. Vitreous and subretinal specimens grew colonies of a fungus morphologically consistent with F. solani. CONCLUSION: Fusarium solani should be included in the differential diagnosis of subretinal abscesses.

ABSTRACT: The aim of the present research was to evaluate the effect of Pithecellobium dulce, Tagetes erecta and Cosmos bipinnatus on methane emission, milk yield and dry matter intake in dairy cattle. A 4×4 Latin square experimental design was employed, using four multiparous Holstein cows of 553±72.4kg body weight, at mid lactation and average milk yield of 17.3±3kg/day. The experiment lasted 92 days, divided into four experimental periods of 23 days each. All cows had free access to maize and alfalfa silage in a 50:50 proportion, 4kg of concentrate/day and ad libitum access to water. Treatments consisted in supplementation of 0.5kg/day of the experimental plants, with one control treatment without supplementation. Each cow received one of each treatment in turn during one of the four periods. The C. bipinnatus reduced methane production by 16% (P<0,05) in comparison with the control diet. Milk production, milk composition and dry matter intake were not affected (p>0 0.05) by the use of C. bipinnatus or any other plant species. Supplementation at low doses of C. bipinnatus showed a reduction in ruminal methane production in dairy cows.


RESUMO: O objetivo deste estudo é avaliar o efeito do Pithecellobium dulce, Tagetes erecta e o Cosmos bipinnatus na emissão do metano, a produção de leite e de ingestão diária de matéria seca pelo gado. Um desenho experimental do quadrado latino 4x4 foi usado, com quatro vacas da raça Holandesa, multíparas, de 553±72,4kg, no seu segundo terço da lactação e rendimento médio de leitedo de 17,3±3kg/dia. O experimento durou 92 dias e foi dividido em quatro períodos de 23 dias cada um. Das vacas tinham acesso livre a milho e silagem de alfafa, numa proporção de 50:50, com 4kg de concentrado por dia, com acesso a água ad libitum. Os tratamentos consistiram na ingestão de 0,5kg por dia, com as plantas experimentais; as vacas no tratamento de controle não receberam nenhuma planta, porém, elas receberamum tratamento, em cada um dos quatro períodos. O C. bipinnatus reduziu significativamente a produção de metano em 16% (P<0,05), em comparação com a dieta controle. A produção de leite, sua composição e o consumo de matéria seca não foram afetados (P>0,05) pelo uso de C.bipinnatus ou outras espécies de plantas. A ingestão com doses baixas de C. bipinnatus, que é uma planta arbustiva com um centéudo moderado de taninos, mostrou ter potencial para reduzir a produção de metano ruminal em vacas leiteiras.

PURPOSE: To report on a case of Histoplasma capsulatum endogenous endophthalmitis in an immunocompetent patient. METHODS: A 30-year-old patient was admitted with floaters and vision impairment of 1 month's duration. He had a history of adrenal insufficiency, together with nasal, septum, and soft palate lesions of 3 months; duration. Culture results from specimens of these lesions were positive for H capsulatum. He was human immunodeficiency virus negative and there was no evidence of immunodepression or history of immunosuppression. Fundus examination revealed multiple fluffy balls with a string of pearls appearance, 2+ vitreous haze, multiple foci of retinochoroiditis inferiorly in the peripheral retina, and a 6-disk area lesion of retinochoroiditis at the superotemporal periphery. Due to poor response to oral itraconazole, a vitrectomy was performed with an intraocular injection of amphotericin B 5 µg/0.1 mL and removal for a vitreous specimen for culture of bacteria and fungi. RESULTS: Vitreous specimen culture of the yeast at 28°C grew a white filamentous fungus colony, which was again cultured in a brain heart infusion agar medium, where it developed hyaline septate hyphae with microconidia and circular macroconidia with double wall, which was stained with a lactophenol dye at microscopic examination. The macroscopic morphology was consistent with H capsulatum. CONCLUSIONS: Although endogenous H capsulatum endophthalmitis is a rare entity, it should be considered as a possible etiology even in apparently immunocompetent hosts, especially in patients with history of disseminated disease.
