RESUMO

A brown tumor, or osteoclastoma, is a nonneoplastic bony lesion associated with hyperparathyroidism and directly related to increased levels of parathyroid hormone. These tumors result from excessive osteoclastic activity. This article presents 3 cases of brown tumor localized in facial bones. The lesions were the result of secondary hyperparathyroidism associated with chronic renal failure. The patients were two 42-year-old men and a 39-year-old woman. All patients had been treated systemically by hemodialysis for more than 10 years. This article highlights the importance of proper diagnosis and management of dental patients presenting with a brown tumor.

PURPOSE: Photodynamic therapy (PDT) appears to be an effective method for the in vitro and in vivo inactivation of Candida spp., but no clinical trials in this context have yet been conducted. The aim of this study was to compare the effect of oral miconazole gel to PDT combined with low-power laser (LPL) therapy in the treatment of denture stomatitis. MATERIALS AND METHODS: Forty participants with clinical and microbiological diagnoses of type II denture stomatitis were randomly allocated to two treatment groups (PDT and miconazole gel), each with 20 individuals. The PDT group was submitted to one session of methylene blue-mediated PDT plus two sessions of low-laser therapy twice a week for 15 days. The miconazole group was submitted to the drug four times a day for 15 days. RESULTS: Forty percent of the patients achieved clinical and microbiological resolution of denture stomatitis after methylene blue-mediated photodynamic inactivation followed by low-laser therapy. The cure rate associated with miconazole was 80% (p < 0.05). Fifteen days after the end of treatment, the recurrence rate was 25% in patients treated with PDT combined with LPL therapy and 12.5% in patients treated with miconazole. CONCLUSION: Miconazole gel provides better results than a protocol combining methylene blue-mediated PDT and LPL therapy in the treatment of type II denture stomatitis.

PURPOSE: The purpose of this study was to evaluate the profile of pediatric oral lesions in northeastern Brazil. METHODS: Biopsy records of oral lesions were retrieved from patients aged zero to 18 years who were treated over 18 years in two university hospitals in northeastern Brazil. Routine pathological sections were re-examined and classified into six groups: (1) odontogenic and nonodontogenic cysts; (2) odontogenic tumors; (3) salivary gland pathology; (4) nonodontogenic tumors and tumor-like lesions; (5) bone pathology; and (6) miscellaneous. RESULTS: From a total of 4,690 oral biopsies, 564 (12 percent) were from children and adolescents. The largest number of cases was salivary gland pathology, followed by nonodontogenic tumors and tumor-like lesions. The most common oral biopsies were mucocele, pericoronal dental follicle, pyogenic granuloma, dentigerous cyst, and odontoma. Only three cases of malignant tumors were observed. Our results were similar to most studies. CONCLUSIONS: Although malignant tumors comprised a small portion of the pediatric oral biopsies, the possibility of their occurrence in children and adolescents should not be overlooked.

Introdução: Próteses mucossuportadas são consideradas facilitadoras, em potencial, da estomatite protética (EP), condição caracterizada pelo aspecto eritematoso, difuso ou pontilhado da mucosa palatina sob a base das próteses. A etiologia da doença é multifatorial, embora a infecção por Candida seja uma causa bastante comum. Objetivo: Relacionar a EP com a presença de Candida, identificar as espécies de Candida mais prevalentes, a partir do meio CHROMagar Candida(r), e caracterizar o perfil de sensibilidade das colônias ao miconazol e à terapia fotodinâmica (TFD). Material e Método: A amostra foi constituída por 45 usuários de prótese total, sendo 30 com diagnóstico clínico de EP e 15 sem a doença. Realizou-se raspagem com swab da mucosa palatina e das próteses, e as amostras foram semeadas em CHROMagar Candida(r), para identificação de espécies de C. albicans, C. krusei e C. tropicalis. Após incubação e leitura das placas, as colônias foram reisoladas em Ágar Sabouraud, para caracterização da sensibilidade ao miconazol e à TFD. Resultado: Espécies de Candida estiveram presentes em 53,33% das amostras dos pacientes com EP e em 6,67% das amostras dos pacientes sem EP, considerando-se o total de amostras de mucosa (p=0,008) e próteses (p=0,001). As espécies mais prevalentes foram C. albicans (36,67% e 53,33%), seguida de C. tropicalis (13,33% e 16,67%) e C. krusei (13,33% e 6,67%), em mucosa e prótese, respectivamente. As amostras mostraram maior sensibilidade ao miconazol que à TFD (p<0,0001). Conclusão: Existiu uma associação entre a presença de Candida e EP, sendo C. albicans a espécie mais prevalente. Miconazol proveu melhores resultados na eliminação in vitro de colônias de Candida quando comparado à TFD. .


Introduction: Denture stomatitis (DS) is a common oral mucosal lesion in complete denture users. It is usually seen as limited to the area beneath an upper denture, characterized by focal or diffuse erythema of the denture-supporting tissues. The etiology of this lesion is multifactorial, although Candida infection is a common cause of disease. Objective: To relate the DS with the presence of Candida, to identify the most prevalent species of Candida using CHROMagar Candida(r) medium and to verify the susceptibility of the isolates to miconazole and photodynamic therapy (PDT). Material and Method: The sample was constituted of 45 complete denture users, 30 patients with clinical DS and 15 without disease. Palatal mucosa and dentures were frictioned with sterilized swabs and sown in CHROMagar Candida(r) medium to identify species of C. albicans, C. tropicalis and C. krusei. After incubation and observation of the plaques, the yeasts were reisolated in Sabouraud Agar medium for verification of species susceptibility to miconazole and PDT. Result: Candida species were found in 53,33% of the patients with DS and in 6,67% of the patients without DS, considering all of the palatal mucosa (p=0,008) and denture (p=0,001). The most prevalent species were C. albicans (36,67% and 53,33%), followed by C. tropicalis (13,33% and 16,67%) and C. krusei (13,33 and 6,67%), on mucosal and denture, respectively. The samples were more sensitive to miconazole than PDT (p<0,0001). Conclusion: There was an association between the presence of Candida and DS, and C. albicans the most prevalent species. Miconazole provided better results in the elimination of in vitro colonies of Candida compared with PDT. .

CONTEXT: Ameloblastomas are benign tumors of the jaws with locally invasive capacity. AIM: The aim of this study was to review 112 cases of ameloblastoma seen over an 18-year period (1992-2009) at the Pernambuco Dental School, University of Pernambuco and at Federal University of Sergipe, in the northeast region of Brazil. MATERIALS AND METHODS: THE FOLLOWING DATA WERE SELECTED FOR ANALYSIS: age, gender, race, site distribution, radiographic appearance, association with an impacted tooth, size, presence of symptoms, clinicopathologic subtypes and recurrence. SETTINGS AND DESIGN: In this retrospective study, Pearson's χ(2) test and t-test were employed. The critical level of significance was set at P < 0.05. RESULTS: The mean age of the patients at presentation was 35.1 ± 16.8 years with a slight female preference. The peak prevalence was in the 11- to 20-year age group and declined with increasing age. Total 75 patients were black and 37 were white, for a 2:1 black: white ratio. The location of the ameloblastomas showed a marked predominance in the mandible (84.8%) and 69% of the cases presented with a multilocular radiographic appearance. The tumor was associated with an embedded tooth in 14 cases (12.7%): nine unilocular and five multilocular ameloblastomas. The maximum radiological extension of the lesions on panoramic radiographs was 0.5-20 cm (mean ± SD: 5.2 ± 3.3 cm) and most cases were symptom-free (75.9%). Solid/multicystic ameloblastoma was the most common clinicopathologic subtype. There was an association between the clinicopathologic subtypes and radiographic appearance (P < 0.001). Recurrence was observed in 13.3% of cases. CONCLUSION: We propose that racial factors may have strong influence on the incidence of ameloblastomas in the northeast region of Brazil, since most people have African descent. Data related to gender, location, radiographic appearance, size, symptoms, clinicopathologic subtypes and recurrence were similar to previous studies conducted in various parts of the world.

CONTEXT: Focal fibrous hyperplasia, also known as irritation or traumatic fibroma, is a reactive, inflammatory hyperplastic lesion of the connective tissue. AIM: The aim of this study is to perform a retrospective study of a focal fibrous hyperplasia of 18 years. MATERIALS AND METHODS: We retrospectively reviewed 193 cases of focal fibrous hyperplasia of the oral cavity from the medical and histological reports of the Department of Oral Pathology, Pernambuco University, Brazil, during the period between January 1992 and December 2009. SETTINGS AND DESIGN: Data with regard to age, gender, location, size of the lesion (equal to or less than 1 cm, between 1 and 2 cm and greater than 2 cm), pain, history of trauma, treatment, length of follow-up (from diagnosis to release or last review) and recurrence, were collected. RESULTS: The most commonly affected site was the buccal mucosa (n = 119, 61.7%). Almost two-thirds of the cases were concentrated from the second to the fifth decade of life. Females were more affected than men and a history of trauma was related by 90.7% of the patients. Two recurrences were notified (1.0%). CONCLUSION: Further studies are needed on the distribution of the lesion in different ethnic and geographical populations. The influence of sex hormones on the development of focal fibrous hyperplasia must be clarified.

RESUMO

The oral cancer in Brazil is considered an important public health issue. Despite almost a century of combat disease, with oncological assistance programs and prevention campaigns based public policies, the incidence of oral cancer in several regions is still high. The major goal of cancer prevention and control is a reduction in both the incidence of the disease and the associated morbidity and mortality, as well as improved life for cancer patients and their families. Low-income and disadvantage groups are generally more exposed to avoidable risk factors such as tobacco and alcohol consumption. These groups have less access to the health services and education that would empower them to make decisions to protect and improve their own health. Thus, cancer control programs are the most effective instrument to bridge the gap between knowledge and practice and achieve this goal. The authors report some key historical facts concerning the fight against oral cancer in Brazil, in addition to addressing the epidemiological aspects of disease.


O câncer de boca no Brasil é considerado um importante problema de saúde pública. Apesar de quase um século de combate à doença, com políticas públicas baseadas em programas de assistência oncológica e campanhas de prevenção, ainda é alta a incidência do câncer de boca em diversas regiões do país. O objetivo principal da prevenção e controle do câncer é a redução na incidência da doença, como também, da morbidade e mortalidade associadas, além da melhoria da qualidade de vida para pacientes e suas famílias. Populações com baixa renda e grupos desfavorecidos são geralmente mais expostos a fatores de risco evitáveis, como o tabaco e o consumo de álcool. Estes grupos têm menos acesso aos serviços de saúde e educação que lhes permitam tomar decisões para proteger e melhorar a sua própria saúde. Assim, os programas de controle do câncer são o instrumento mais eficaz para diminuir a distância entre conhecimento e prática e alcançar esta meta. Os autores relatam alguns dos principais fatos históricos relativos à luta contra o câncer de boca no Brasil, além de abordar aspectos epidemiológicos da doença. .

Introdução : A pesquisa epidemiológica de lesões maxilofaciais em determinada região estabelece as necessidades populacionais e orienta os profissionais da saúde na definição de ações preventivas e tratamento adequado. Objetivo: Analisar os laudos histopatológicos de lesões maxilofaciais do período de 1996 a 2011 do Laboratório de Patologia Oral do Departamento de Odontologia da Universidade Federal de Sergipe (UFS), Brasil. Material e método: Estudo retrospectivo das biópsias realizadas de 1996 a 2011, recuperando-se os dados referentes ao gênero e à idade dos pacientes, à localização das lesões e ao diagnóstico histopatológico. As lesões foram agrupadas em: neoplasias benignas, lesões potencialmente malignas, neoplasias malignas, lesões inflamatórias, lesões odontogênicas, lesões ósseas, lesões de glândulas salivares e anomalias de desenvolvimento. Resultado: Foram analisados 762 laudos, havendo maior prevalência das lesões inflamatórias (n=205, 26,9%). O diagnóstico mais comum entre as neoplasias benignas foi a lesão periférica de células gigantes (n=15); entre as lesões potencialmente malignas, foi a displasia epitelial (n=80), e entre as neoplasias malignas, foi o carcinoma de células escamosas (n=29). Dentre as lesões inflamatórias, a lesão mais prevalente foi a hiperplasia fibrosa inflamatória (n=74). O granuloma periapical (n=62) foi a lesão mais comum dentre as lesões odontogênicas. A lesão mais prevalente dentre as lesões ósseas foi o fibroma ossificante central (n=08); entre as lesões de glândulas salivares, foi o fenômeno de retenção de muco (n=64), e entre as anomalias de desenvolvimento, foi a mácula melanocítica ...


Introduction: Epidemiological survey of maxillofacial lesions in a specific geographic region establishes the real needs of this population and guides health professionals in defining preventive actions and appropriate treatment. Objective: To analyze the histopathological diagnosis of maxillofacial lesions issued in the period 1996-2011 managed by the Laboratory of Oral Pathology, Department of Dentistry, Federal University of Sergipe, Brazil. Material and method: A retrospective study of biopsies removed from 1996-2011 was conducted, recovering data related to gender and age of the patients, location of the lesions and histopathological diagnosis. The lesions were grouped into eight diagnostic categories: benign neoplasms, potentially malignant lesions, malignant neoplasms, inflammatory lesions, odontogenic lesions, bone lesions, salivary gland lesions and malformations and developmental anomalies. Result: 762 reports were analyzed, with a higher prevalence of inflammatory lesions (n = 205, 26.9 %). The most common diagnosis among benign neoplasms was the peripheral giant cell granuloma (n=15), among potentially malignant lesions was the epithelial dysplasia (n=80), and in the malignant neoplasm group the most common lesion was the squamous cells carcinoma. (n=29). Analyzing the group of inflammatory lesions, we observed that the prevalent lesion was inflammatory fibrous hyperplasia (n=74). Periapical granuloma was the lesion that appeared most commonly among odontogenic lesions. The prevalent lesion among bone lesions was central ossifying fibroma (n=08), among salivary gland lesions was mucous retention (n=64), in the group of developmental anomalies the prevalent lesion was melanocytic macule (n=04). Conclusion: The findings of this survey highlight the importance in developing treatment plans and educational measures to prevent and reduce patients' exposure to risk factors. .

Due to the frequent presence of inflammation in cases of carcinoma and its use as a parameter for the assessment of tumor aggressiveness, the role of inflammation in oral carcinogenesis was investigated. This was performed by evaluating the expression of cellular markers, cytokines and nuclear transcription factors that identify the cells that participate in the antitumor defense in cases of oral epithelial dysplasia (OED) and oral squamous cell carcinoma (OSCC). A semi-quantitative immunohistochemical analysis was performed for the transcription factors cluster of differentiation 8 (CD8), forkhead box P3 (FOXP3), transforming growth factor (TGF)-ß, tumor necrosis factor (TNF)-α and nuclear factor κ-light chain enhancer of activated B-cells (NF-κB), in cases of OED and OSCC. CD8, TGF-ß, TNF-α and NF-κB participated in the processes of tumor transformation and progression. The presence of inflammatory infiltrate in cases of OED favors the transformation and invasion process when stromal TNF-α and NF-kB are overexpressed, as NF-kB activated by TNF-α during inflammation predisposes the lesion to transformation, functioning as a link between inflammation and cancer. The control of these inflammatory mediators may prevent malignant transformation in the oral cavity.

Introdução: Uma infecção crônica nos tecidos periapicais pode desencadear uma lesão periapical em função da estimulação e da proliferação dos restos epiteliais de Malassez. Ao estudo microscópico, é possível detectar a presença de fendas de cristais de colesterol no interior de cistos e granulomas. De acordo com alguns estudos, a origem do colesterol no interior de lesões no periápice pode ser, parcialmente, por causa da condensação e da cristalização de colesterol dentro da parede do cisto. Outros estudos afirmam que a presença de colesterol pode ser derivada de produtos metabólicos locais ou da circulação vascular. Objetivo: O presente estudo teve por objetivo analisar a relação entre as fendas de cristais de colesterol encontradas nas lesões periapicais e os níveis de colesterol sérico. Material e Método: Estudo observacional de natureza descritiva, que incluiu 70 pacientes portadores de lesões periapicais, de acordo com exame radiográfico, atendidos no Departamento de Odontologia da Universidade Federal de Sergipe. Esses pacientes foram submetidos à exodontia da unidade dentária acometida e à remoção da lesão periapical, sendo esta peça encaminhada para exame anatomopatológico. Na mesma consulta, era solicitado um lipidograma para avaliação das taxas de colesterol sérico dos pacientes. Resultado: Fendas de cristais de colesterol estavam presentes apenas em cistos grandes (maiores que 1,5 cm de diâmetro, aproximadamente) e, portanto, mais antigos. Conclusão: A presença de cristais de colesterol é rara nas lesões periapicais. O estudo sugere uma relação entre a presença dos cristais de colesterol e o índice de colesterol sérico elevado, em lesões grandes e antigas.


Introduction: A chronic infection in periapical tissue can trigger a periapical lesion due to stimulation and proliferation of epithelial rests of Malassez. On microscopic study, it is possible to detect the presence of clefts of cholesterol crystals within cysts and granulomas. According to some studies, the source of cholesterol inside the periapice lesions may be partially due to the condensation and crystallization of cholesterol in the cyst wall. Yet another part, can be derived metabolic products local or vascular circulation. Objective: This study aimed to analyze the relationship between the clefts of cholesterol crystals found in periapical lesions and cholesterol levels in the blood circulation. Material and Method: An observational study of a descriptive nature that included 70 patients with periapical lesions (according to radiographic examination) at the Department of Dentistry, Federal University of Sergipe. These patients underwent extraction of the affected tooth unit and removal of the periapical lesion, and this number sent for pathological examination. At the same query was asked a lipid profile to assess rates of serum cholesterol of patients. Result: Clefts of cholesterol crystals were present only in cysts (larger than 1.5 cm in diameter, approximately), and therefore older. Conclusion: The presence of cholesterol crystals is rare in periapical lesions. The study suggests a relationship between the presence of cholesterol crystals and the rate of cholesterol serum levels in large and old injuries.

Mucoceles are the most common lesions of the minor salivary glands and typically appear as a fluctuant, bluish, nontender, submucosal swelling with a normal overlying mucosa. Mucoceles of the glands of Blandin-Nuhn (in the anterior portion of the ventral surface of the tongue) have been considered to be uncommon. This article reports an unusual case of a large extravasation mucocele involving the ventral surface of the tongue, which appeared after a lingual frenectomy.

To prevent rejection of kidney transplants, patients must be kept in immunosuppressive therapy for a long time, which includes the use of drugs such as cyclosporine, azathioprine, cyclophosphamide, and prednisone. The action of these drugs reduces the general immune response of transplant patients and thus increases their susceptibility to infections. Moreover, these drugs increase the potential of developing lesions. Therefore, oral hygiene in kidney transplant recipients contributes to maintenance of the transplanted organ and its function. Thus, an investigation of oral lesions could be counted as a notable work. The aim of this study was to investigate oral lesions in a group of 21 kidney transplant patients under immunosuppressive therapy attended during a 1-year period in the Nephrology Department of the Federal University of Sergipe, Brazil. Data related to sex, age, etiology of renal disease, types of renal transplant, time elapsed after transplantation, immunosuppressive treatment, use of concomitant agents, and presence of oral lesions were obtained. All patients received a kidney transplant from a living donor, and the mean posttransplantation follow-up time was 31.6 months; 71.5% used triple immunosuppressive therapy with cyclosporine A, azathioprine, and prednisone. Ten patients were also treated with calcium-channel blockers. Of the 21 transplant patients, 17 (81%) presented oral lesions. Gingival overgrowth was the most common alteration, followed by candidiasis and superficial ulcers. One case of spindle cell carcinoma of the lower lip was observed. Oral cavity can harbor a variety of manifestations related to renal transplantation under immunosuppressive therapy.

This study investigated the immunodetection of PTCH in epithelial components of dental follicles associated with impacted third molars without radiographic signs of pathosis. One hundred and five specimens of dental follicles associated with impacted third molars with incomplete rhizogenesis (between Nolla's stage 6 and 9) were surgically removed from 56 patients. Epithelial cell proliferation was determined by using immunohistochemical labeling. Statistical analysis was performed using Fisher exact test and a level of significance of 5 percent. Of the 105 dental follicles collected, 3 were PTCH-positive. The specimens with squamous metaplasia and epithelial hyperplasia had higher rates of positivity for PTCH, as well as those with active remnants of odontogenic epithelium. This study suggests that the odontogenic cells of the dental follicle might be proliferating during the rhizogenesis, while the squamous metaplasia and hyperplasia of the epithelial lining and proliferative odontogenic epithelial rests show the differentiation potential of dental follicles.


Se investigó la inmunodetección de PTCH en los componentes epiteliales de los folículos dentales asociados a terceros molares retenidos sin signos radiográficos y morfológicos de patología. Fueron quirúrgicamente extraídos de 56 pacientes 105 muestras de folículos dentales asociadas a terceros molares retenidos con rizogénesis incompleta (entre el estadio de Nolla 6 y 9). La proliferación de células epiteliales se deteminó mediante inmunohistoquímica. El análisis estadístico se realizó mediante la prueba exacta de Fisher. De los 105 folículos dentales recogidos, 3 fueron PTCH-positivos. Las muestras con metaplasia escamosa e hiperplasia epitelial tuvieron mayores tasas de positividad para PTCH, así como aquellos con los restos de proliferación del epitélio odontogénico. En conclusión, este estudio sugiere que las células odontogénicas del folículo dental podrían estar proliferando durante la rizogénesis, mientras que la metaplasia escamosa e hiperplasia del epitelio y de restos epiteliales odontogénicos en proliferación muestran el potencial de diferenciación de los folículos dentales.

RESUMO

Kaposi's sarcoma is an angioproliferative tumour rarely found in the oral cavity. We present the 3rd case of iatrogenic gingival Kaposi's sarcoma reported in the English-language literature which developed in a young patient 5 years after a renal transplant and discuss their histological features and differential diagnosis.

BACKGROUND: Actinic cheilitis (AC) is a potentially malignant disorder of the lip caused by chronic exposure to solar radiation. Despite being a relatively common lesion in tropical countries, the data related to AC prevalence in Brazil are sparse, particularly in regions with high levels of ultraviolet radiation. OBJECTIVE: To estimate AC prevalence among farmers in a semiarid northeastern Brazil. METHODS: Cross-sectional study was carried out involving a representative sample (n = 240) of the farm workers. Sociodemographic, clinical, and histopathological variables were analyzed. RESULTS: AC prevalence was 16.7% [95% confidence interval (CI): 12.0% to 21.4%]. The disease was more prevalent in males [prevalence ratio (PR) = 2.72; 95% CI: 1.52-4.90], fair-skinned farmers (PR = 2.07; 95% CI: 1.00-4.30), age over 50 years (PR = 2.91; 95% CI: 1.40-6.04), daily exposure time more than eight hours (PR = 1.84; 95% CI: 1.03-3.27), and among smokers/ex-smokers (PR = 2.17; 95% CI: 1.22-3.87). Mantel-Haenszel test showed a positive relationship between cumulative exposure time and development of AC (P < 0.001). Two cases were diagnosed as microinvasive squamous cell carcinoma. CONCLUSIONS: Our results suggest that farmers in this semiarid area have a high risk of lip cancer and indicate the need for health public programs in the early detection of AC and education to teach the population about precautions to be taken during sun exposure.

RESUMO

O presente estudo objetivou conhecer a percepção dos cirurgiões-dentistas em relação ao Sistema Único de Saúde e ao Programa Saúde da Família no município de Nossa Senhora do Socorro, Sergipe. Foram aplicados 41 questionários contendo perguntas objetivas e subjetivas, com análise dos resultados desenvolvida por meio da abordagem qualiquantitativa. Os resultados demonstraram que a maioria dos cirurgiões-dentistas enfatiza a importância da capacitação de recursos humanos como forma de integração e efetividade da Equipe de Saúde Bucal no Sistema Único de Saúde e no Programa Saúde da Família. Concluiu-se que os profissionais, os gestores e as entidades públicas devem buscar promover a interação entre avanço do conhecimento, progresso tecnológico, mercado de trabalho, estrutura de serviços, políticas públicas, agenda social e perfil das demandas, contribuindo, dessa maneira, na resolução do paradoxo entre formação profissional, com ênfase no domínio cognitivo/instrumental e mudanças nas tendências atuais das políticas de saúde.


This study examines perceptions of dentists in relation to the Health System and the Family Health Program in the city of Nossa Senhora do Socorro, Sergipe. Questionnaires containing 41 questions with objective and subjective questions were utilized and the analysis of the results were developed through the qualitativequantitative approach. The results showed that most dentists emphasizes the importance of training human resources as a means of integration and effectiveness of the Oral Health Team in the National Health System and the Family Health Program. It was concluded that professionals, managers and public authorities should seek to promote the interaction between scientific knowledge, technological progress, labor market structure, services, public policy, social agenda and profile of demand, contributing, in this way, with the resolution of paradox between training with emphasis on the cognitive / instrumental and changes in current trends of health policies.
