RESUMO

Background: MicroRNA (miRNA) have the potential to be non-invasive and attractive biomarkers for a vast number of diseases and clinical conditions; however, a reliable analysis of miRNA expression in blood samples meets a number of methodological challenges. In this report, we presented and discussed, specifically, the principles and limitations of miRNA purification and analysis in blood plasma samples collected from the left atrium during an ablation procedure on patients with atrial fibrillation (AF). Materials and Methods: Consecutive patients hospitalized in the First Department of Cardiology for pulmonary vein ablation were included in this study (11 with diagnosed paroxysmal AF, 14 with persistent AF, and 5 without AF hospitalized for left-sided WPW ablation-control group). Whole blood samples were collected from the left atrium after transseptal puncture during the ablation procedure of AF patients. Analysis of the set of miRNA molecules was performed in blood plasma samples using the MIHS-113ZF-12 kit and miScript microRNA PCR Array Human Cardiovascular Disease. Results: The miRNS concentrations were in the following ranges: paroxysmal AF: 7-23.1 ng/µL; persistent AF: 4.9-66.8 ng/µL; controls: 6.3-10.6 ng/µL. The low A260/280 ratio indicated the protein contamination and the low A260/A230 absorbance ratio suggested the contamination by hydrocarbons. Spectrophotometric measurements also indicated low concentration of nucleic acids (<10 ng/µL). Further steps of analysis revealed that the concentration of cDNA after the Real-Time PCR (using the PAXgene RNA Blood kit) reaction was higher (148.8 ng/µL vs. 68.4 ng/µL) and the obtained absorbance ratios (A260/A280 = 2.24 and A260/A230 = 2.23) indicated adequate RNA purity. Conclusions: Although developments in miRNA sequencing and isolation technology have improved, detection of plasma-based miRNA, low RNA content, and sequencing bias introduced during library preparation remain challenging in patients with AF. The measurement of the quantity and quality of the RNA obtained is crucial for the interpretation of an efficient RNA isolation.

RESUMO

The aim of the present study is to examine beliefs about hypnosis in a Polish sample, with a focus on possible misconceptions. The research included groups with different backgrounds regarding hypnosis: 1st- and 5th-year psychology students, psychotherapists using hypnosis, people interested in paranormal phenomena, and people from the general population. The results demonstrated mild misconceptions about hypnosis and significant differences in beliefs about hypnosis between the aforementioned groups. Additionally, within a Polish sample, psychotherapists using hypnosis and 5th-year students' views were most similar in their current scientific knowledge about hypnosis, while the groups targeting individuals unexposed to hypnosis and followers of paranormal beliefs showed significantly more misconceptions.

RESUMO

The paper investigates the illusion of control by proxy in games of chance - an attempt to exert control by assigning it to others who are perceived as more capable, communable or luckier. Following up on research by Wohl & Enzle, who showed participants' preference to ask lucky others to play a lottery instead of doing it themselves, we included proxies with positive and negative qualities in the domains of agency and communion, as well good and bad luck. In three experiments (total N = 249) we tested participants' choices between these proxies and a random number generator in a task consisting of obtaining lottery numbers. We obtained consistent preventative illusions of control (i.e. avoidance of proxies with strictly negative qualities, as well as proxies with positive communion but negative agency), however we observed indifference between proxies with positive qualities and random number generators.

RESUMO

AIMS: The study aims to investigate how trust in science, conspiratorial thinking, and religiosity affected people's declared willingness to vaccinate against COVID-19 at the onset of the vaccination program in Poland, their actual vaccination, and the consistency between intention and vaccination. METHODS: In a longitudinal design, a representative sample of 918 members of the Polish general population was polled at the beginning of the vaccination program (February 2021) and polled again after 6 months of mass vaccination (August 2021). We measured the willingness to vaccinate, actual vaccination after 6 months, and individual variables-trust in science, conspiratorial thinking and religiosity. RESULTS: The actual vaccination rate was higher than the declared intent, especially in the initially undecided and unwilling groups. Higher Trust in science and lower Conspiratorial Thinking were associated with declared intent to vaccinate and actual vaccination, while Religiosity was not clearly associated with vaccination. CONCLUSIONS: Declared willingness to vaccinate is not an effective indicator of actual vaccination. Trust in science and Conspiratorial thinking are important factors associated with vaccine hesitancy. There may be a possibility to influence those unwilling to vaccinate and that are undecided to eventually get vaccinated.

RESUMO

INTRODUCTION: Cryptogenic stroke may be associated with a patent foramen ovale (PFO). Both cardiovascular risk factors and transcatheter closure of PFO may have an impact on the risk of recurrent cerebrovascular events. OBJECTIVES: The aim of the study was to assess the occurrence and risk factors of recurrent cerebrovascular events (rCVE) in patients with a history of cryptogenic stroke or transient ischemic attack (TIA) and PFO. PATIENTS AND METHODS: Overall, 392 patients (median [interquartile range, IQR] age 39.5 [30-49] years, 64.3% women, 35.7% men) with a history of cryptogenic stroke / TIA and confirmed PFO underwent a longterm followup with a median (IQR) of 51.5 (35-65) months. The primary end point was defined as rCVE including stroke and TIA. RESULTS: During the followup, 17 patients with a history of cryptogenic stroke / TIA and confirmed PFO (4.3%, 11 women, 6 men) developed rCVE. In a multivariable analysis, the Risk of Paradoxical Embolism (RoPE) score was associated with a lower risk of rCVE (odds ratio [OR], 0.61 per 1 point; 95% CI, 0.45-0.84; P = 0.002). The transcatheter closure of PFO did not have a significant impact on rCVE in the study population (P = 0.19). CONCLUSIONS: The occurrence of rCVE in the patients with cryptogenic stroke / TIA and PFO reached 4.3% regardless of a high percentage of patients who underwent the PFO closure. RoPE score was associated with a lower risk of rCVE in the study population.

RESUMO

AIMS: The aim of the study was to investigate the relation between declared intention to get/not get vaccinated against COVID-19, prior to the start of the global vaccination program, and actual vaccine uptake. Moreover, reasons for getting vaccinated or rejecting it were measured along with declared intent and behavior. METHODS: Within a longitudinal design, a representative sample of 918 Polish people was surveyed in February 2021 and August 2021. In February 2021, participants were asked about their intention to get vaccinated against COVID-19 and the reasons behind it. In August 2021, the same group was asked about having been vaccinated, along with the reasons. RESULTS: A significant pro-vaccine shift from declared intent to behavior was observed, with many participants turning away from being anti-vaccine or undecided and getting vaccinated. Significant correlations with attitudes toward general mandatory vaccination of children were found. Increased support for anti-vaccine arguments was seen over time in the unvaccinated sample, and decreased support for pro-vaccine arguments was seen in the vaccinated sample. Several key arguments for and against vaccination were identified. CONCLUSIONS: Declared attitude toward COVID-19 vaccination is not fully consistent with vaccination behavior. Pro-vaccine changes in attitudes of previously anti-vaccine and undecided individuals indicate that these groups may be influenced to potentially accept the COVID-19 vaccination over time.

RESUMO

This study evaluated the frequency and risk factors for surgery dissatisfaction in patients undergoing lumbar or cervical surgery for degenerative spinal conditions. Based on the Patient Satisfaction Index (PSI) at 6 months after surgery, we divided patients into two groups: a satisfied and a dissatisfied group. We evaluated the association between patient dissatisfaction and five categories of variables:1) sociodemographic; 2) preoperative pain and disability [pain duration, level of surgery, previous spinal surgeries, pain scores as measured by the Short Form McGill Pain Questionnaire (SF-MPQ), numerical rating of average pain (NRS), disability as measured by the Oswestry Disability Index (ODI)]; 3) preoperative psychological status [depression, anxiety, and overall distress as measured by the Hospital Anxiety and Depression Scale (HADS), life satisfaction as measured by the Satisfaction With Life Scale (SWLS), and surgery expectations (SE) as measured by a Likert scale]; 4) postoperative improvements in pain and disability [improvements in SF-MPQ, improvement in ODI] and 5) postoperative psychological status [HADS, SWLS]. Results showed that 17.8% patients were dissatisfied with surgery. In the multivariate logistic analysis, more negative surgery expectations, smaller improvement in ODI scores, and a greater postoperative overall distress were significant risk factors associated with patient dissatisfaction with surgery.

RESUMO

BACKGROUND: Post-pulmonary embolism (PE) syndrome occurs in up to 50% of PE patients. The pathophysiology of this syndrome is obscure. OBJECTIVE: We investigated whether enhanced oxidative stress and prothrombotic state may be involved in post-PE syndrome. METHODS: We studied 101 normotensive noncancer PE patients (aged 56.5 ± 13.9 years) on admission, after 5-7 days and after a 3-month anticoagulation, mostly with rivaroxaban. A marker of oxidative stress, 8-isoprostane, endogenous thrombin potential, fibrinolysis proteins, clot lysis time (CLT) and fibrin clot permeability (Ks ), along with PE biomarkers, were determined. RESULTS: Patients who developed the post-PE syndrome (n = 31, 30.7%) had at baseline 77.6% higher N-terminal brain natriuretic propeptide and 46.8% higher growth differentiation factor 15, along with 14.1% longer CLT associated with 34.4% higher plasminogen activator inhibitor-1 as compared to subjects without post-PE syndrome (all P < .05). After 5-7 days, only hypofibrinolysis was noted in post-PE syndrome patients. When measured at 3 months, prolonged CLT and reduced Ks were observed in post-PE syndrome patients, accompanied by 23.8% higher growth differentiation factor 15 and 35.8% higher plasminogen activator inhibitor-1 (all P < .05). 8-isoprostane levels ≥108 pg/ml (odds ratio=4.36; 95% confidence interval 1.63-12.27) and growth differentiation factor 15 ≥ 1529 pg/ml (odds ratio=3.89; 95% confidence interval 1.29-12.16) measured at 3 months were associated with higher risk of developing post-PE syndrome. CONCLUSIONS: Enhanced oxidative stress and prothrombotic fibrin clot properties could be involved in the pathogenesis of the post-PE syndrome. Elevated growth differentiation factor 15 assessed at 3 months might be a new biomarker of this syndrome.

RESUMO

Vaccine rejection is a problem severely impacting the global society, especially considering the COVID-19 outbreak. The need to understand the psychological mechanisms underlying the active involvement of the pro-vaccine and anti-vaccine movements is therefore very important both from a theoretical and practical perspective. This paper investigates the group identities of people with positive and negative attitudes towards vaccination, and their attitudes toward general science. A targeted sample study of 192 pro-vaccine and 156 anti-vaccine group members showed that the group identity of pro-vaccine individuals is higher than of anti-vaccine individuals. and that both pro-vaccine and anti-vaccine individuals had a positive attitude toward science. Results are discussed in context of the heterogeneity of motivations causing vaccine rejection and the relation between active involvement in online discussion and group identity.

RESUMO

BACKGROUND: The research focused on the relationships between attitudes towards vaccination and the trust placed in different sources of information (science, experts and the information available on the Internet) before and during COVID-19. METHOD: A longitudinal design was applied with the first measurement in February 2018 (N = 1039). The second measurement (N = 400) was carried out in December 2020 to test if the pandemic influenced the trust in different sources of information. RESULTS: The final analyses carried out on final sample of 400 participants showed that there has been no change in trust in the Internet as a source of knowledge about health during the pandemic. However, the trust in science, physicians, subjective health knowledge, as well as the attitude towards the vaccination has declined. Regression analysis also showed that changes in the level of trust in physicians and science were associated with analogous (in the same direction) changes in attitudes toward vaccination. The study was also focused on the trust in different sources of health knowledge as possible predictors of willingness to be vaccinated against SARS-nCoV-2. However, it appeared that the selected predictors explained a small part of the variance. This suggests that attitudes toward the new COVID vaccines may have different sources than attitudes toward vaccines that have been known to the public for a long time.

RESUMO

BACKGROUND: Prothrombotic fibrin clot properties are associated with higher early mortality risk in acute pulmonary embolism (PE) patients. It is unknown whether different types of PE are associated with particular clot characteristics. METHODS: We assessed 126 normotensive, noncancer acute PE patients (median age: 59 [48-70] years; 52.4% males), who were categorized into central versus peripheral PE with or without concomitant deep vein thrombosis (DVT). Plasma fibrin clot permeability (K s), clot lysis time (CLT), thrombin generation, platelet-derived markers, and fibrinolytic parameters were measured on admission. Plasma fibrin clot morphology was assessed by scanning electron microscopy (SEM). RESULTS: Patients with central PE (n = 76; 60.3%) compared with peripheral PE (n = 50; 39.7%) had 17.8% higher K s and 14.3% shortened CLT (both p < 0.01 after adjustment for potential confounders including fibrinogen), with no differences between segmental and subsegmental PE. SEM analysis demonstrated larger fibrin fiber diameter and pore size in central PE compared with peripheral PE (both p < 0.01). For isolated PE, there was 23.3% higher K s in central PE than in peripheral PE (n = 24; 19%) with no differences in other variables. Central PE combined with DVT (n = 45; 35.7%), as compared with central isolated PE (n = 31; 24.6%), was associated with shortened CLT (all p < 0.05). CONCLUSION: Our findings suggest that looser fibrin networks composed of thicker fibers with increased susceptibility to lysis characterize patients with central PE, suggesting that fibrin clot phenotype affects the size of thrombi occluding the pulmonary arteries, highlighting the role of fibrin structures in thrombus formation and stability.

RESUMO

Venous thromboembolism is associated with formation of denser fibrin clots resistant to lysis. We investigated whether prothrombotic plasma clot properties are associated with the severity of acute pulmonary embolism (PE). We enrolled 126 normotensive acute PE patients (aged 58 ± 14 years) and 25 age- and sex-matched healthy controls. Plasma fibrin clot permeability (Ks), clot lysis time (CLT), endogenous thrombin potential (ETP), plasminogen activator inhibitor-1 (PAI-1), and citrullinated histone H3 (citH3) were evaluated on admission. PE patients compared to controls had 370% higher citH3 levels, 41% higher ETP, 16.5% reduced Ks, and 25.6% prolonged CLT. Patients with intermediate-high (n = 29) and intermediate-low (n = 77) PE mortality risk had reduced Ks and prolonged CLT, increased PAI-1 and ETP as compared to low-risk PE (n = 20) patients. Prolonged CLT was predicted by PAI-1 and citH3, while low Ks by C-reactive protein. During a 12-month follow-up 9 (7.1%) patients who had 24% higher ETP, 45% higher citH3 levels, and 18% prolonged CLT at baseline died. High ETP combined with elevated citH3 levels and prolonged CLT was associated with eightfold increased risk of PE-related death. Prothrombotic fibrin clot properties and enhanced neutrophil extracellular traps formation are associated with higher early mortality risk in acute PE patients, which suggests a prognostic role of these biomarkers.

RESUMO

INTRODUCTION: Despite progress in medical and interventional treatment of acute myocardial infarction (AMI) resulting in low in-hospital mortality, the post-discharge prognosis in MI survivors is still unacceptable. The Managed Care in Acute Myocardial Infarction (MC-AMI, KOS-zawal) is a program introduced by Poland's National Health Fund aiming at comprehensive care for patients with AMI to improve prognosis. It includes acute intervention, complex revascularization, cardiac rehabilitation (CR), scheduled outpatient follow-up, and prevention of sudden cardiac death. The aim of the study was to assess the effect of MC-AMI on major adverse cardiovascular events (MACE) in 3-month follow-up. MATERIAL AND METHODS: In this single-center, retrospective observational study we enrolled 1211 patients, and compared them to 1130 subjects in the control group. After 1 : 1 propensity score matching two groups of 529 subjects each were compared. Cox regression was performed to assess the effect of MC-AMI and other variables on MACE. RESULTS: MC-AMI participation is related to reduced MACE rate by 45% in a 3-month observation. Multivariable Cox regression analysis revealed MC-AMI participation to be inversely associated with the occurrence MACE at 3 months (HR = 0.476, 95% CI: 0.283-0.799, p < 0.005). Also, older age, male sex (HR = 2.0), history of unstable angina (HR = 3.15), peripheral artery disease (HR = 2.17), peri-MI atrial fibrillation (HR = 1.87) and diabetes (HR = 1.5) were significantly associated with MACE. CONCLUSIONS: Participation in MC-AMI - the first comprehensive in-hospital and post-discharge care for AMI patients - improves prognosis and is related to a MACE rate reduction by 45% as soon as in 3 months.

RESUMO

The influence of grammatical gender on cognitive processes is an important issue in contemporary psycholinguistics and language psychology, particularly in research concerning the relations between grammar and semantics. The extent of this effect is dependent on a given language's gender system and its grammatical specifics. The aim of the presented research was to investigate grammatical gender effects in Polish - a Slavic language with three singular and two plural grammatical genders. In Experiment 1, triadic similarity judgments were used, and it turned out that the grammatical gender of nouns influenced perceived similarity of words in case of animals, but not inanimate objects or abstract concepts. In Experiment 2 we used a modified Implicit Association Test; results suggest that grammatical gender seems to be of implicit nature, as grammatical gender consistency influenced reaction times and the number of classification errors. In Experiment 3 participants assigned male and female voices to animals and inanimate objects, which were presented either as words or as pictures. Grammatical gender effects occurred for both animate and inanimate objects and were similar for verbal and visual stimuli. It turned out that in the Polish language the influence of grammatical gender may occur on the lexicosemantic level and the conceptual level, and concerns both animate and inanimate objects. Results are discussed in context of the similarity and gender and the sex and gender hypotheses.

RESUMO

BACKGROUND: Despite progress in the treatment of acute myocardial infarction (AMI), long-term prognosis in MI survivors remains a challenge. The Managed Care in Acute Myocardial Infarction (MC-AMI, KOS-zawal) is the first program of a comprehensive, supervised care for patients with AMI to improve long-term prognosis. It includes acute intervention, complex revascularization, cardiac rehabilitation (CR), outpatient follow-up, and prevention of SCD. Our aim was to assess the relation between participation in MC-AMI and major adverse cardiovascular and cerebrovascular events (MACCE) in 12-month follow-up. METHODS AND RESULTS: In this single-center, retrospective analysis we compared 719 patients participating in MC-AMI and compared them to 1130 subjects in the control group. After propensity score matching, two groups of 529 subjects each were compared. MC-AMI was related with MACCE reduction by 40% in a 12-month observation. Participants of MC-AMI had a higher adherence to cardiac rehabilitation (98 vs. 14%), higher rate of scheduled revascularisation (coronary artery bypass grafting: 9.8% vs. 4.9%, p ≪ 0.001; elective percutaneous coronary intervention: 3.0% vs 2.1%, p ≪ 0.05) and ICD implantation (2.8% vs. 0.6%, p ≪ 0.05) compared to control. Multivariable Cox regression analysis revealed MC-AMI to be inversely associated with the occurrence of MACCE (HR = 0.500, 95% Cl 0.349-0.718, p ≪ 0.001). Besides, older age, diabetes mellitus, hyperlipidemia, prior PAD, previous UA, and lower LVEF were significantly associated with the primary endpoint. CONCLUSIONS: MC-AMI is the first program of comprehensive care for AMI patients. MC-AMI improves prognosis by increasing the rate of patients undergoing CR, complete revascularization and ICD implantation, thus reducing MACCE.

RESUMO

When talking about absence, we may express it in a negative statement (using explicit negation e.g. I was not) or in a positive statement (using implicit negation e.g. I wished I were). Previous research has shown that explicitly negated statements may cause false recall-negated items may paradoxically be remembered as present. The current study compares false recall caused by implicit and explicit negation. Participants listened to a recording in which some objects were mentioned as present, some as absent, and some not mentioned at all. The absence of objects was expressed using explicit or implicit negation. Participants' recall of the recording was measured either five minutes or one week after exposure to the recording. Results indicate that implicit and explicit negation lead to a nearly identical false recall of negated items. However, items not mentioned in the recording (i.e. neither mentioned nor negated) were more often recognized as present by participants exposed to implicit, rather than explicit negation. We postulate that false recall of negated items could be explained by participants remembering the item itself, but forgetting the context in which it was present (an affirmative or a negative statement), hence objects would be recalled as present just because they were spoken of.

RESUMO

AIMS: The proarrhythmic effect of fibroblast growth factor 23 (FGF23) was observed in patients with end stage kidney disease (ESKD). However, there is no data on the role of FGF23 and soluble Klotho (sKlotho) in the pathogenesis of atrial fibrillation (AF) beyond ESKD. The aim of the study was to assess the peripheral vein and left atrial (LA) serum levels of FGF23 and sKlotho along with calcium-phosphates parameters in patients with AF undergoing percutaneous radiofrequency pulmonary vein isolation (PVI). METHODS AND RESULTS: Sixty-nine consecutive patients (mean age: 55.8 ±â€¯9.7 years, F/M: 26/43, CHA2DS2-Vasc: 1.7 ±â€¯1.1) with paroxysmal/persistent AF undergoing PVI were included into the study. Blood samples were taken during PVI - baseline from the peripheral vein, then from the LA immediately after a septal puncture. RESULTS: There were significant differences in the concentrations of peripheral and LA serum sKlotho, intact FGF23 (iFGF23), calcium and phosphates; peripheral FGF23, calcium and phosphates levels were significantly higher, and sKlotho levels were significantly lower than the LA concentrations. Serum sKlotho levels correlated with the CHADS2-VASc score (r = 0.254, p = 0.034) and glucose level (r = 0.300, p = 0.005). Serum sKlotho gradient (LA - peripheral vein) correlated with the baseline AF burden in the Holter monitoring (r = -0.389, p = 0.003). PVI efficacy was confirmed in 52 (75%) patients. There was a significant difference in the iFGF23 gradient between patients with AF and without AF (80.3 vs. -47.6 pg/ml, p = 0.009) in the six-month follow-up. A receiver operating characteristic (ROC) analysis revealed that an iFGF23 gradient >28.7 pg/ml (AUC = 0.742, p = 0.002) was a predictor for AF recurrence. CONCLUSIONS: There is a gradient between the LA and peripheral vein in the markers of calcium-phosphate metabolism in patients undergoing PVI. Lower sKlotho and higher iFGF23 serum levels are associated with episodes of AF. Serum iFGF23 gradient is a potent predictor for the recurrence of AF.
